Carnosine is a dipeptide formed of the amino acids β-alanine and histidine. Only a limited number of studies have examined the effects of carnosine on sympathetic nerve activation and anxiety. The present study was undertaken to determine the dose-related effects of carnosine on anxiety in the elevated T-maze test (ETM) with electrodermal activity (EDA). Carnosine was injected in three groups of rats with doses of 10 (low dose), 100 (medium dose) and 1000 (high dose) mg/kg i.p. Physiological saline was injected in the sham group. The anxiety scores of the rats were measured with ETM 20 minutes after injection. Then, SCL was measured. The decreased number of entries into the open arm (NEOA), the percentage of time spent in the open arm (% TSOA) and higher EDA [shown by skin conductance level (SCL)] indicate higher anxiety. The NEOA and % TSOA were lower in the high-dose group than in the other groups. SCL was lower in the medium-dose carnosine group than in the high-dose carnosine and sham groups. SCL was higher in the high-dose group than in the medium-dose and sham groups. Our results suggest that high-dose carnosine produced anxiety-like effects as assessed in the SCL and ETM. Inzulinovou rezistenci (IR) definujeme jako stav, při němž normální koncentrace volného plazmatického inzulinu vyvolává sníženou odpověď organizmu. V užším slova smyslu IR chápeme jako poruchu účinku inzulinu v cílové struktuře, která může vzniknout na kterékoli úrovni inzulinové signalizační kaskády. V klinických podmínkách ji obvykle definujeme jako poruchu účinku inzulinu v metabolizmu glukózy, i když platí, že porucha se může se týkat různých účinků inzulinu a různých buněčných struktur. Charakteristickým rysem IR svázané s metabolickým syndromem či diabetem 2. typu je defektní signalizace, která postihuje PI3-kinázovou větev inzulinové signalizační kaskády. Další účinky inzulinu, které závisí na signalizaci vedoucí přes Ras komplex a MAP-kinázu, nemusí být postiženy. Vlivem kompenzatorní hyperinzulinemie mohou být dokonce zvýšeny. Článek shrnuje některé novější poznatky týkající struktury a regulací inzulinové signalizační kaskády a rozebírá vybrané primární a sekundární příčiny IR, které zahrnují faktory genetické a epigenetické, regulační roli mikroRNA a metabolické, humorální a imunologické faktory. Detailní poznání příčin IR nabízí možnosti její racionální léčby. Ta se v současné době opírá o léčbu odstranitelných příčin IR, tj. důslednou kompenzaci diabetu, redukci hmotnosti, režimová opatření (dieta, fyzická aktivita), přehodnocení potřeby léčby kortikoidy, léčbu přidružených onemocnění, a případně podání metforminu či pioglitazonu. In the narrower sense we understand IR as the impairment of insulin action in the target structure which may arise at any level of the insulin signalling cascade. In the clinical conditions we usually define it as the impairment of insulin action in glucose metabolism, although it is true that the impairment may concern different effects of insulin and different cell structures. The characteristic feature of IR linked to the metabolic syndrome or Type 2 diabetes is defective signalling which affects PI3-kinase branch of insulin signalling cascade. Other insulin actions depending on the signalling through the Ras complex and MAP-kinase, may not be affected. Due to compensatory hyperinsulinemia they may be even increased. The article summarizes some recent findings regarding the structure and regulation of insulin signalling cascade and analyses selected primary and secondary causes of IR which include genetic and epigenetic factors, the microRNA regulation role, metabolic, humoral and immunological factors. The detailed knowledge of the causes of IR opens possibilities of its rational treatment. This is currently based on the treatment of curable causes of IR, i.e. consistent compensation of diabetes, weight reduction, regimen arrangements (diet, physical activity), re-assessment of the need to use corticosteroids in therapy, treatment of coexisting conditions and possibly administration of metformin or pioglitazone. Cíl: Cílem studie bylo zhodnotit přínos elektrofyziologického vyšetření v diferenciální diagnostice lumbální spinální stenózy (LS) a diabetické polyneuropatie (DPN). Soubor a metodika: Elektrofyziologické vyšetření bylo provedeno u 68 pacientů s klinicky symptomatickou LS, u 28 pacientů s DPN a 32 zdravých dobrovolníků. Výsledky: Vyšetřené elektrofyziologické parametry z horních končetin (latence F vlny n. ulnaris a amplituda senzitivního neurogramu n. radialis), všechny hodnocené latence z dolních končetin (F vlny n. tibialis, H reflexu m. soleus, spinální latence MEP) a amplituda senzitivního neurogramu n. suralis signifikantně odlišovaly pacienty s LS a DPN. ROC analýza nicméně prokázala pouze dva elektrofyziologické parametry jako efektivní nezávislé faktory pro odlišení pacientů s LS a DPN: latenci ulnarisové F vlny (optimální diskriminační hodnota je 24,2 ms, senzitivita 82,7 % a specificita 63,9 %) a amplitudu senzitivního neurogramu n. radialis (optimální diskriminační hodnota je 10,5 ?V, senzitivita 75,5 % a specificita 58,2 %). Vícerozměrná diskriminační analýza poskytla kanonické skóre s nejsilnější prediktivní hodnotou. Závěry: Elektrofyziologické vyšetření z horních končetin je užitečné v diferenciální diagnostice LS a DPN, nicméně nejlepší diskriminační efekt vykazuje kanonické skóre, které zahrnuje několik elektrofyziologických parametrů. Methods: Electrophysiological parameters were examined in 68 patients with clinically symptomatic LS, in 28 patients with DPN, and in a group of 32 healthy volunteers. Results: Electrophysiological parameters evaluated from the upper extremities (F-wave latency of the ulnar nerve and radial SNAP amplitude), all the latencies measured in the lower extremities (tibial F-wave, soleus H-reflex and spinal latency of MEP), and the sural SNAP amplitude contributed significantly to distinguishing the LS from DPN patients. ROC analysis, however, disclosed only two electrophysiological parameters as effective in independent discrimination between the LS and DPN patient groups: ulnar F-wave latency (cut-off point at 24.2 ms, sensitivity 82.7% and specificity 63.9%); and radial SNAP amplitude (cut-off point at 10.5 ?V, sensitivity 75.5%, specificity 58.2%). Multivariate discrimination provided a canonical score with the most powerful predictive value of all. Conclusions: Electrophysiological examination evaluated from the upper extremities proved very useful in the differential diagnostics of LS and DPN, but the most powerful predictive value was obtained by using a canonical score involving several electrophysiological parameters., and B. Přítomnost kardiovaskulárního onemocnění a zejména ischemické choroby srdeční u pacientů s diabetes mellitus 1. typu vede ke zkrácení očekávané délky života. Hyperglykemie, která zvyšuje oxidativní stres, je považována za klíčový faktor v jejich etiopatogenezi. Kalcifikace v koronárním řečišti, přítomnost srdeční autonomní neuropatie a diabetické nefropatie představuje stavy, které jsou významně asociovány s manifestací ischemické choroby srdeční. Rovněž hypoglykemické stavy významně zvyšují kardiovaskulární riziko. Na podkladě výsledků klinických studií existují důkazy o tom, že dobrá kompenzace diabetes mellitus 1. typu od doby její manifestace snižuje významně výskyt a progresi makrovaskulárních komplikací, včetně ischemické choroby srdeční. Korekce hypertenze a poruchy lipidového metabolizmu je rovněž spojena s příznivým účinkem., The presence of cardiovascular disease, and especially coronary artery disease, in patients with type 1 diabetes mellitus impairs life expectancy. Hyperglycaemia, which increases oxidative stress, is the key pathophysiological factor. Coronary artery calcifications, the presence of cardiac autonomic neuropathy and diabetic nephropathy are significantly associated with coronary artery disease manifestations. Likewise, the hypoglycaemic episodes significantly increase the cardiovascular risk. Based on the clinical trial outcomes, there is evidence showing that good compensation of type 1 diabetes from the time of its manifestation can significantly reduce the occurrence and progression of macrovascular complications, including coronary artery disease. Diabetes mellitus 2. typu léčíme dnes postupným přidáváním antidiabetik od monoterapie k dvojkombinacím, trojkombinacím i vícekombinacím léků. Metformin zůstává zatím lékem první volby, pokud je snášen a pokud není kontraindikován. V dalším kroku máme širokou možnost volby nejméně 5 skupin léků. Nové algoritmy používané ve Spojených státech zavádějí určitou preferenci inkretinové léčby (gliptiny a inkretinová analoga) a nové skupiny gliflozinů před tradičními postupy. Indikaci antidiabetik tak čekají v nejbližších letech buď velké změny nebo minimálně diskuse o preferenci jednotlivých lékových skupin včetně skupin v diabetologii nových. Metformin still remains the first-line medicine, if well tolerated and not contraindicated. In the next step, we have a wide choice of 5 groups of medicines at least. The new algorithms used in the U.S.A. introduce some degree of preference for the incretin therapy (gliptins and incretin analogues) and the new groups of gliflozins over the standard treatments. Klasickým důsledkem léčby antidiabetiky včetně inzuiínu byl vzestup hmotnosti, který prohlubuje inzulinovou rezistenci a vede k dalšímu zvyšování dávek léků. Tak vzniká nepříjemný cyklus. Jeho prvním prolomením bylo podání antidiabetik váhově neutrálních. Další možností bylo podání antiobezitik, kterých však dnes není mnoho k dispozici. Ve Spojených státech jsou však již 2 roky užívána nová antiobezitika. Nejvýznamnější změnou v léčbě obézních diabetiků 2. typu je podání antidiabetik, která snižují hmotnost. To jsou již několik let inkretinová analoga, u nichž je nepříjemné injekční podání. Aplikace se však zjednodušuje a přechází se na preparáty aplikované v delším časovém intervalu. Ještě větší změnou v léčbě je podání gliflozinů, z nichž první dva dapagliflozin a empagliflozin přicházejí nyní téměř současně do České republiky. Tato skupina léků má velmi komplexní metabolický efekt a především výrazně redukuje hmotnost, a to dokonce i u pacientů léčených inzulinem. V letošním roce se tak možnosti léčby obézních diabetiků 2. typu výrazně rozšířily. Klíčová slova: antiobezitika – diabetes mellitus 2. typu – glifloziny – inkretinová analoga, Classical consequence of treatment with anti-diabetic agents, including insulin, is weight gain which enhances insulin resistance and leads to a further increase in drug dose. This creates an unwanted cycle. Administration of weight neutral anti-diabetic drugs has broken this scenario. Another option is to use anti-obesity agents, but actually many of them are not available. However, in the United States, two new drugs have been used in past two years. The most important change in the treatment of obese type 2 diabetes patients is administration of anti-diabetic agents that reduce weight. These agents are well-known incretin analogs characterized by their unpleasant injecting administration. However, application becomes easier and the drugs are administered for the866 longer time period. Even greater change in treatment is administration of gliflozins. Dapagliflozin and empagliflozin enter almost simultaneously to the Czech Republic. The new class of drugs has very complex metabolic effect and in particular significantly reduces the weight even in patients treated with insulin. The treatment options have significantly expanded for obese type 2 diabetes patient in course of this year.
