Užívání hormonální antikoncepce je považováno za rizikový faktor vzniku hluboké žilní trombózy a plicní embolie. Od zavedení hormonální antikoncepce v 60. letech 20. století byl u jejích uživatelek pozorován častější výskyt těchto komplikací. Nejnovější studie pak ukázaly dvakrát vyšší riziko vzniku žilní trombózy při užívání kombinovaných preparátů obsahujících progestiny třetí generace (desogestrel, gestoden, norgestimate) ve srovnávání s progestiny druhé generace (levonorgestrel a norgestrel), ačkoli progestiny třetí generace neprokázaly žádný klinicky významný benefit. Přičemž riziko vzniku hluboké žilní trombózy je ještě vyšší u neperorálních forem hormonální antikoncepce (vaginální kroužek, transdermální náplast)., Since the early 1960s, it has been well documented that combined hormonal contraceptives increase the risk of venous thrombosis. Second and third generation oral contraceptives (OCs) differ in their progestin component. Third generation OCs contain desogestrel, gestodene or norgestimate while second generation OCs contain norgestrel or levonorgestrel. In spite of the fact that the third generation OCs have not shown any clinically significant benefit over the second generation OC, multiple studies and two meta‑analyses show that the third generation OC containing desogestrel are associated with approximately doubled risk of venous thrombosis. Cíl:Cílem naší studie bylo kvantitativně vyhodnotit poruchy písma u pacientů s Parkinsonovou nemoci (PN) ve srovnání s věkově a pohlavím vázanými zdravými kontrolami (ZK) pomocí digitalizačního tabletu. Soubor a metoda: Prospektivně jsme zařadili 40 pacientů s PN (průměrný věk 68,6 ? 11,36 let, délka trvání nemoci 8,02 ? 4,79 let) a 40 věkem a pohlavím vázaných ZK (průměrný věk 62,55 ? 11,22 let). Všichni jedinci byli praváci bez přítomnosti deprese či demence. Každý subjekt podstoupil sedm cvičení pro vyšetření písma a kresbu Archimédovy spirály a elips s pomocí digitalizačního tabletu. Byly hodnoceny rychlostní parametry mikrografie a kresby při pohybu pera po tabletu i nad tabletem. Pro statistickou analýzu dat jsme použili Mann‑Whitneyho U test a Spearmanovy korelace s korekcí na opakovaná měření (Benjamini‑Hochbergova metoda). Výsledky: U PN ve srovnání se ZK jsme při psaní na tabletu zjistili statisticky významné snížení v parametrech: okamžitá rychlost, okamžité zrychlení, okamžitá změna zrychlení v čase. Změny se zvýrazňovaly s délkou psaného segmentu. Ještě významnější byly rozdíly mezi oběma skupinami při hodnocení pohybu pera nad tabletem, tj. před vlastním zahájením psaní, při přípravě na pohyb. Zaznamenali jsme pokles sledovaných hodnot až o 20 % ve srovnání se ZK. Závěr: U pacientů s PN jsme prokázali specifické změny nejen při vlastním psaní, ale i ve fázi přípravy na psaní, které lze kvantifikovat pomocí digitalizačního tabletu. Výsledky studie mohou mít přímý klinický dopad: umožní nám studovat mikrografii jako možný časný klinický marker rozvoje PN., Aim: The aim of this study was to assess micrographia in patients with Parkinson’s disease (PD) as compared to healthy controls (HC) using a digitizing tablet. Methods: We included 40 PD (mean 68.6 ? 11.36 years, duration of illness 8.02 ? 4.79 years) and 40 age- and sex-matched HC (mean 62.55 ? 11. 22 years). All subjects were right-handed, without the presence of depression or dementia. Each subject underwent seven exercises for writing and drawing of Archimedes spiral and ellipses using a digitizing tablet. The speed parameters of micrographia and drawing during the movement of a pen in the air and on the tablet were evaluated. The Mann-Whitney U test, Spearman correlation and Benjamini-Hochbergs method were used for statistical data analysis. Results: A statistically significant reduction in parameters of velocity, acceleration, and jerk was found when comparing both groups during writing. Changes were more pronounced with increased length of the written segment. The differences between the two groups were more pronounced when the in-air movements were assessed, i.e. during movement preparation. The values decreased up to 20% compared to HC. Conclusion: PD-specific changes assessed with a digitizing tablet were demonstrated not only during writing but also during preparation for writing. Předpoklad: Studie IMProved Reduction of Outcomes: Vytorin Efficacy International Trial (IMPROVE-IT) hodnotila potenciální prospěch na snížení velkých kardiovaskulárních (KV) příhod při přidání ezetimibu vs placebo k léčbě 40 mg simvastatinu u nemocných s akutním koronárním syndromem a nízkou hladinou LDL-cholesterolu ≤ 125 mg/dl. Metodika: Randomizovaná dvojitě slepá studie u nemocných po akutním koronárním syndromu s nízkou hladinou cholesterolu. Ve větvi se simvastatinem v monoterapii byl cílový LDL-cholesterol < 70 mg/dl, druhá větev byla kombinace simavastatin + ezetimib. Předpokladem bylo, že ezetimib sníží hladinu LDL-cholesterolu o dalších 15 mg/ml s efektem léčby kolem 8–9 %. Primární smíšený cíl byl KV úmrtí, nefatální infarkt myokardu (MI), rehospitalizace pro nestabilní anginu pectoris (NAP) a koronární revaskularizace po 30 dnech. Cílový počet příhod byl 5 250. Výsledky: Bylo zařazeno 18 144 nemocných s ST elevacemi IM (STEMI, n = 5 192) nebo non-ST elevacemi IM či NAP (NAP/nonSTEMI, n = 12 952) od října roku 2005 do července roku 2010. Primární cíl se vyskytl u 2 742 nemocných (34,7 %) na léčbě simvastatinem v monoterapii a u 2 572 nemocných (32,7 %; p = 0,016) léčených kombinací. Pacienti léčení kombinací simvastatin + ezetimib vs pacienti léčení simvastatinem + placebo měli o 6,4 % nižší kombinovaný cíl kardiovaskulární úmrtí, nefatální infarkt myokardu, nefatální cévní mozková příhoda, hospitalizace pro nestabilní anginu pectoris a revaskularizace po 30 dni. Srdeční infarkty byly sníženy o 13 %, nefatální CMP o 20 %. Úmrtí z kardiovaskulární příčiny bylo v obou skupinách stejné. Průměrná doba sledování byla 6 let, nejdelší 8,5 roku. Průměrně 2 pacienti ze 100 předešli kardiovaskulární příhodě za 7 let (Number Needed to Treat – NNT) 50/7 let. Závěr: Studie prokázala jasný prospěch z kombinační léčby simvastatinem a ezetimbem u nemocných po akutním koronárním syndromu a nízkým LDL-cholesterolem., Background: The IMProved Reduction of Outcomes: Vytorin Efficacy International Trial (IMPROVE-IT) is evaluating the potential benefit for reduction in major cardiovascular (CV) events from the addition of ezetimibe versus placebo to 40 mg/d of simvastatin therapy in patients who present with acute coronary syndromes and have low-density lipoprotein cholesterol (LDL-C) ≤ 125 mg/dl. Methods: Randomized double blind clinical trial in patients with acute coronary syndrome and low cholesterol level. The simvastatin monotherapy arm’s LDL-C target was < 70 mg/dl, the comparison arm was simvastatin + ezetimibe. Ezetimibe was assumed to further lower LDL-C by 15 mg/dl and produce an estimated ~ 8 % to 9 % treatment effect. The primary composite end point was CV death, nonfatal myocardial infarction (MI), nonfatal stroke, rehospitalization for unstable angina (UA), and coronary revascularization (≥ 30 days postrandomization). The targeted number of events was 5,250. Results: 18,144 patients were enroled with either ST segment elevation MI (STEMI, n = 5,192) or UA/non-ST segment elevation MI (UA/NSTEMI, n = 12,952) from October 2005 to July 2010. Primary endpoint occured in 2 742 patients (34.7 %) treated with simvastatin in monotherapy and in 2 572 patients (32.7 %) (p = 0.016) treated with combination. Compared to patients with coronary heart disease given the drug simvastatin plus a placebo, those given both simvastatin and the non-statin drug, ezetimibe, had a 6.4 % lower combined risk of subsequent heart attack, stroke, cardiovascular death, rehospitalization for unstable angina and procedures to restore blood flow to the heart. Heart attacks alone were reduced by 13 %, and non-fatal stroke by 20 %. Deaths from cardiovascular disease were statistically the same in both groups. Patients were followed an average of approximately six years, and some as long as 8.5 years. Erytrocytární aloimunizace (izoimunizace, hemolytické onemocnění plodu a novorozence – HON) se v současnosti díky rutinní imunoprofylaxi anti-D imunoglobulinem vyskytuje sporadicky. Výskyt atypických aloimunizací non-RhD-antigeny zůstává neměnný, poněvadž není známa profylaxe a ani prevence nezajišťuje stoprocentní účinnost. Pokud se imunoprofylaxe anti-D imunoglobulinem neprovede nebo selže, hrozí vznik hemolytického onemocnění plodu a novorozence. Každý případ je potom unikátní (specifita a typ protilátky, blokující protilátky v séru matky, antigenní výbava erytrocytů plodu, gestační stáří při začátku onemocnění atd). Pokroky v genové diagnostice, imunohematologii a dopplerovské ultrasonografii významně snižují frekvenci nutných invazivních zákroků v diagnostice a léčbě senzibilizovaného těhotenství. Intraumbilikální přísně intravenózní transfuze pod ultrasonografickou kontrolou je první metodou volby léčby případů s těžkou anémií plodu i při rozvinutém hydropsu. Konzultace a léčba se soustřeďují do center s perfektní porodnickou a neonatologickou péčí a nepřetržitou operační pohotovostí. The occurrence of atypical alloimmunizations related to non-RhD-antigens remains unchanged, as there is no prophylaxis known and even the prevention does not provide one hundred percent efficiency. When anti-D immunoglobulin immunoprophylaxis is not undertaken or fails, the threat of hemolytic disease of the fetus and newborn arises. Every case is then unique (specificity and type of antibody, blocking antibodies in maternal serum, antigenic make-up of fetal erythrocytes, gestational age at the disease onset etc). The advancements in genetic diagnostics, immunohematology and Doppler ultrasonography significantly reduce the frequency of necessary invasive interventions in the diagnostics and treatment of sensibilized pregnancy. Intraumbilical strictly intravenous transfusion under ultrasound guidance is the first-line method of therapy for cases of severe fetal anemia as well as advanced hydrops. Consultations and treatment are provided in the centres for comprehensive obstetrical and neonatal care with continuous intervention preparedness. Infekce močových cest patří u dospělých k nejčastějším bakteriálním infekcím, v naprosté většině je způsobena Escherichia coli. Přestože jsou k dispozici doporučené postupy k diagnostice a terapii uroinfektů, neexistuje jednotný přístup založený na důkazech. Diagnóza je často postavena klinicky. U predisponovaných jedinců mohou infekce močových cest vést k postižení horních cest močových nebo až k urosepsi. Základem léčby je antibiotická terapie, k dispozici jsou i profylaktické postupy a alternativní strategie. Závažný problém představuje rezistence na antibiotika. Ke zpřesnění diagnostiky IMC je zapotřebí dalších studií. V tomto přehledném článku diskutujeme prevalenci, diagnostiku včetně jejích úskalí, léčbu a prevenci infekcí močových cest a asymptomatické bakteriurie u dospělých., Urinary tract infections are one of the most frequent clinical bacterial infections in adults. Escherichia coli is the organism that causes urinary tract infections in most patients. Consensus guidelines have been published to assist clinicians with diagnosis and treatment of urinary tract infection; however, a single evidence‑based approach to diagnosis of urinary tract infection does not exist. There is no gold-standard definition of urinary tract infection that clinicians agree upon. In a subgroup of individuals with coexisting morbid conditions, complicated urinary tract infections can lead to upper tract infections or urosepsis. Although the initial treatment is antimicrobial therapy, use of different prophylactic regimens and alternative strategies are available to reduce exposure to antibiotics. Future studies improving the diagnostic accuracy of urinary tract infections are needed. Cíl: Zjistit průměrnou incidenci infekční endokarditidy (IE) v okrese Tábor za sledované období a srovnat získané údaje s dříve publikovanou celorepublikovou studií. Metodika: Retrospektivní monocentrická observační studie sledující výskyt a charakteristiky infekční endokarditidy v definované spádové oblasti za roky 2009–2013. Zařazeni byli pouze pacienti s prokázanou a léčenou infekční endokarditidou a pacienti, u nichž byla diagnóza stanovena až pitvou. Výsledky: Ve sledovaném období bylo zachyceno celkem 39 případů infekční endokarditidy, což odpovídá průměrné incidenci 7,8 případů na 100 000 obyvatel ročně. Medián věku pacientů v době diagnózy byl 67 let. V souboru nemocných výrazně převažovali muži, kteří byli postiženi 3krát častěji než ženy. Přibližně stejně četně byla postižena mitrální (36 %) a aortální (33 %) chlopeň a elektroda kardiostimulačního systému (31 %). Ve srovnání s referenční prací bylo zachyceno více infekcí u pacientů s kardiostimulátory. Dvě třetiny infekcí byly způsobeny stafylokoky, což je proti pilotní studii výrazný nárůst. Naopak mortalita se podstatně neměnila. Závěr: Charakteristiky infekční endokarditidy se vyvíjejí, hospitalizační mortalita pacientů s IE je vysoká, kolem 30 %. Samo onemocnění IE je z hlediska roční úmrtnosti špatným prognostickým faktorem. Klíčová slova: incidence – infekční endokarditida, Aim: To find out an average incidence of infective endocarditis (IE) in Tabor district in a monitored period of time and compare the obtained data with previously published national study. Methods: Retrospective monocentric observational study monitoring occurence and characteristics of infective endocarditis in a defined region in years 2009–2013. Only patients with proved and treated infective endocarditis and patients whose diagnosis was revealed by autopsy were involved. Results: In the monitored time 39 cases of infective endocarditis occured, which means that the average incidence was 7.8 cases on 100 000 people per year. Median age in the time of diagnosis was 67. The group of patients was strongly dominated by men, who were affected three times more often than women. Almost the same rates of infective endocarditis were localized on mitral (36 %) and aortic (33 %) valve and electrode of a pacemaker (31 %). In comparison with the reference study there were more pacemaker-related infections detected. Two thirds of infections were caused by staphylococci. That means a prominent growth when compared to the pilot study. On the contrary, there was no significant change in mortality. Conclusion: Characteristic features of infective endocarditis are evolving, patients’ in-hospital mortality rate is high, about 30 %. Tento přehledový článek vychází z kurzu prezentovaného na kongresu Americké urologické asociace v červnu 2014, který byl zaměřen na problematiku intersticiální cystitidy a syndromu bolestí v močovém měchýři. Autoři uvádějí nejnovější úpravy v taxonomii a názvosloví těchto syndromů včetně aktuálních definic a nabízejí pohled na bolestivé syndromy z gynekologické i urologické perspektivy. Článek je doplněn přehlednými diagramy a algoritmy uznávaných mezinárodních společností, které mohou sloužit jako vodítko při diagnostice a léčbě těchto komplexních syndromů., This review is based on the course presented on AUU meeting held in July 2014 focusing on the conditions known as interstitial cystitis and bladder pain syndrome. The authors are introducing last updates in taxonomy and nomenclature of these syndromes, including current definitions and offering an overview of painful syndromes from both gynaecologic and urologic perspective. PURPOSE: A variety of anti-glaucomatous shunt designs have been proposed so far. This study evaluates the feasibility of a novel shunt design, the intra-scleral parathalamus implant (IPI), in a rabbit eye model. METHODS: Ten healthy albino rabbits were included. Measurements of the IOP, using TonoPenXL, as well as ocular rigidity (OR) and aqueous outflow facility (AO), using a previously described methodology, were performed in both eyes of each animal. Subsequently, the IPI was implanted at the left eye of all animals whereas the right eye served as control. Measurements of IOP were repeated at weekly intervals for 2 months postoperatively whereas measurements of OR and AO were repeated at the 1st and 2nd postoperative week. RESULTS: The IOP decreased significantly whereas the AO increased significantly postoperatively at the operated eyes. A significant postoperative decrease in OR was also recorded at the operated eyes. Respective differences at the fellow eyes were statistically not significant. Two animals developed postoperative complications and were excluded from follow-up. CONCLUSIONS: Results imply that the implantation of the IPI is feasible in rabbit eyes with significant favorable effects on the IOP, AO and OR. and E. T. Detorakis, K. Loggaki, N. Karyotakis, H. Cíl: Cílem studie bylo zhodnotit přínos elektrofyziologického vyšetření v diferenciální diagnostice lumbální spinální stenózy (LS) a diabetické polyneuropatie (DPN). Soubor a metodika: Elektrofyziologické vyšetření bylo provedeno u 68 pacientů s klinicky symptomatickou LS, u 28 pacientů s DPN a 32 zdravých dobrovolníků. Výsledky: Vyšetřené elektrofyziologické parametry z horních končetin (latence F vlny n. ulnaris a amplituda senzitivního neurogramu n. radialis), všechny hodnocené latence z dolních končetin (F vlny n. tibialis, H reflexu m. soleus, spinální latence MEP) a amplituda senzitivního neurogramu n. suralis signifikantně odlišovaly pacienty s LS a DPN. ROC analýza nicméně prokázala pouze dva elektrofyziologické parametry jako efektivní nezávislé faktory pro odlišení pacientů s LS a DPN: latenci ulnarisové F vlny (optimální diskriminační hodnota je 24,2 ms, senzitivita 82,7 % a specificita 63,9 %) a amplitudu senzitivního neurogramu n. radialis (optimální diskriminační hodnota je 10,5 ?V, senzitivita 75,5 % a specificita 58,2 %). Vícerozměrná diskriminační analýza poskytla kanonické skóre s nejsilnější prediktivní hodnotou. Závěry: Elektrofyziologické vyšetření z horních končetin je užitečné v diferenciální diagnostice LS a DPN, nicméně nejlepší diskriminační efekt vykazuje kanonické skóre, které zahrnuje několik elektrofyziologických parametrů. Methods: Electrophysiological parameters were examined in 68 patients with clinically symptomatic LS, in 28 patients with DPN, and in a group of 32 healthy volunteers. Results: Electrophysiological parameters evaluated from the upper extremities (F-wave latency of the ulnar nerve and radial SNAP amplitude), all the latencies measured in the lower extremities (tibial F-wave, soleus H-reflex and spinal latency of MEP), and the sural SNAP amplitude contributed significantly to distinguishing the LS from DPN patients. ROC analysis, however, disclosed only two electrophysiological parameters as effective in independent discrimination between the LS and DPN patient groups: ulnar F-wave latency (cut-off point at 24.2 ms, sensitivity 82.7% and specificity 63.9%); and radial SNAP amplitude (cut-off point at 10.5 ?V, sensitivity 75.5%, specificity 58.2%). Multivariate discrimination provided a canonical score with the most powerful predictive value of all. Conclusions: Electrophysiological examination evaluated from the upper extremities proved very useful in the differential diagnostics of LS and DPN, but the most powerful predictive value was obtained by using a canonical score involving several electrophysiological parameters., and B. Příznaky dolních močových cest v důsledku neurogenní dysfunkce dolních cest močových (NDMC) patří mezi symptomy, které nejvíce obtěžují pacienty s roztroušenou sklerózou (RS). Pokud nejsou léčeny správně, mohou způsobit ireverzibilní změny dolních i horních močových cest. NDMC mohou také zhoršit průběh samotné RS a tím i celkového zdravotního postižení. Současné doporučení vytvořené ICS a EAU pro sledování pacientů s NDMC je koncipováno pouze obecně pro pacienty postižené jakýmkoli neurologickým postižením, ne však pro pacienty s RS. Naše klinické pozorování naznačovalo zhoršování NDMC u pacientů s RS v návaznosti na dynamickou progresi demyelinizačního onemocnění. Cílem prospektivní kontrolované studie bylo zhodnotit parametry urodynamického vyšetření u pacientů s RS v časové ose a vytvořit případně doporučení dispenzarizace pro klinickou praxi., Lower urinary tract symptoms due to neurogenic dysfunction of the lower urinary tract (NDLUT) belong to symptoms that most often bother patients with multiple sclerosis (MS). If not treated properly, can lead to irreversible changes of the lower and upper urinary tract. NDLUT may also worsen the course of MS and general health disability. Current recommendations generated by ICS and EAU for monitoring patients with NDLUT are generally designed for patients with any neurological disabilities, but not for patients with MS.
