There are various complications of prosthetic valvular surgeries. Among them, leaflet perforation should be emphasized and brought to mind when there have been eccentric valvular regurgitation. OBJECTIVE: The aim of the present study was to investigate the effect of allergic rhinitis on the success of the operation in chronic otitis surgery by using score for allergic rhinitis (SFAR). MATERIALS AND METHODS: In the present study; 121 patients, who underwent type 1 tympanoplasty were examined retrospectively. SFAR of all patients were recorded. The graft success rates of 26 patients with allergic rhinitis (AR) and 95 patients with no allergic rhinitis group (NAR) were compared. RESULTS: While the graft success rate in NAR group was 89.5%, this rate was 80.8% in the AR group. However, the difference between groups was not statistically significant (p = 0.311). CONCLUSION: These findings suggest that allergic rhinitis decreases the graft success rate of the pathologies occurring in eustachian tube, middle ear and mastoid although statistically significant difference wasn't found. Úvod: Kardiovaskulární rehabilitace je v současné době standardní součástí léčby nemocných po akutní koronární příhodě. Intervenční tréninkový kardiovaskulární rehabilitační program je součástí II. fáze kardiovaskulární rehabilitace, která je klíčovým bodem v celém sekundárně‑preventivním procesu u nemocných s ischemickou chorobou srdeční. Dochází zde k hemodynamické adaptaci pacienta na běžnou fyzickou zátěž, k postupnému zvyšování aerobní kapacity a k osvojování si principů pravidelného aerobně‑odporového tréninku. Cíl: Předložená práce se zaměřuje na posouzení vlivu modifikovaného aerobně‑odporového tréninku na kardiorespirační ukazatele u nemocných po akutní koronární příhodě. Soubor a metodika: Do studie bylo zařazeno 106 pacientů (85 % mužů) průměrného věku 60,4 ? 10,9 let s ejekční frakcí levé komory 57,4 ? 7,2 %. Jednalo se o pacienty po akutním koronárním syndromu. Doba od vzniku akutní koronární příhody do zahájení tréninkového programu byla 35 ? 8 dnů, u nemocných po aortokoronárním bypassu 50 ? 16 dnů. Všichni pacienti podstoupili dvouměsíční aerobně‑odporový trénink s frekvencí 3× týdně. Tréninková jednotka trvala 100 min (z toho 60 min vlastní aerobní trénink). Výsledky: Absolvování intervenčního tréninkového programu vedlo k signifikantnímu nárůstu pracovní tolerance (1,8 ? 0,3 vs 2,0 ? 0,4 W/kg; p < 0,001) a vrcholové spotřeby kyslíku (22,8 ? 4,5 vs 25,9 ? 5,5 pVO2; p < 0,001), taktéž byl zaznamenán nesignifikantní pokles klidových hodnot tepové frekvence a systolického a diastolického krevního tlaku. Závěr: Modifikovaný intervenční tréninkový program vede ke zlepšení aerobní kapacity, která je jedním z významných prognostických ukazatelů u nemocných po akutní koronární příhodě., Introduction: Cardiovascular rehabilitation is currently part of standard therapy in patients after acute coronary syndrome. The interventional cardiovascular rehabilitation training program is a part of the second rehabilitation phase, which is a key point in all secondary‑preventive processes in patients with coronary artery disease. Patients are hemodynamically adapted to normal physical exercise, their aerobic capacity is gradually increased, and they learn about the principles of regular aerobic‑resistance training. Design: This study is focused on assessing the impact of modified aerobic‑resistance training on cardiorespiratory parameters in patients after an acute coronary event. Methods: The study included 106 patients (85% men), mean age 60.4 ? 10.9 years, with a left ventricular ejection fraction 57.4 ? 7.2%. The time form acute coronary event to the start of the training program was 35 ? 8 days; in patients after coronary artery bypass graft this was 50 ? 16 days. All patients completed a 2-month aerobic‑resistance training program with 3 sessions a week. A training session lasted 100 min (including 60 min of aerobic training). Results: The completion of the intervention training program led to a significant increase in work tolerance (1.8 ? 0.3 vs. 2.0 ? 0.4 W/kg; p < 0.001) and peak oxygen consumption (22.8 ? 4.5 vs. 25.9 ? 5.5 pVO2; p < 0.001). Other results observed were a non‑significant decrease in resting heart rate, systolic and diastolic blood pressure values. Cíl: Na kohortě pacientů s asymptomatickou stenózou vnitřní karotidy podstupujících karotickou endarterektomii otestovat protokol vyšetření hemodynamiky mozku pomocí magnetické rezonance a zjišťovat rozdíl v kognitivní kondici pomocí neuropsychologické testové baterie RBANS (Repeatable Battery for the Assessment of Neuropsychological Status). Metody: Standardní karotická endarterektomie pro asymptomatickou stenózu vnitřní karotidy v intravenózní celkové anestezii se selektivním užitím shuntu. Měření průtoku velkými cévami Willisova okruhu pomocí magnetickorezonační angiografie s fázovým kontrastem před operací a tři měsíce po operaci, a to paralelně s psychologickým vyšetřením testovou baterií RBANS. Výsledky: U pěti žen a 12 mužů s nekomplikovanou karotickou endarterektomií pro asymptomatickou stenózu vnitřní karotidy bylo zaznamenáno signifikantní zlepšení v oblasti kognitivního výkonu v celkovém skóru neuropsychologické baterie (p = 0,05) a indexech týkajících se bezprostřední paměti (p = 0,04) a řeči (p = 0,01). Následné korelace lokální změny hemodynamiky a změn v kognitivním výkonu z magnetickorezonančních dat získaných od 12 pacientů byly signifikantní pro přední mozkovou tepnu a index bezprostřední paměti (p = 0,01), a pro střední mozkovou tepnu a indexy řeči (p = 0,02) a vizuoprostorové orientace (p = 0,02). Závěr: V pilotní studii byl na malé skupině pacientů s klinicky i radiologicky asymptomatickou stenózou vnitřní karotidy poprvé v naší literatuře otestován protokol vyšetření průtoku krve (ml/min) na všech velkých cévách Willisova okruhu a srovnány změny hemodynamiky před karotickou endarterektomií a tři měsíce po ní. V úrovni kognitivního výkonu bylo prokázáno zlepšení, u kterého byl v dílčích testech neuropsychologické baterie RBANS identifikován vztah s hodnotami průtoků v povodí přední nebo střední mozkové tepny. Větší studie s delším sledováním výsledků je proveditelná., Aim: To test a brain haemodynamics examination protocol using magnetic resonance imaging and to investigate the differences in cognitive performance with a neuropsychology test battery RBANS (Repeatable Battery for the Assessment of Neuropsychological Status) on a cohort of patients with an asymptomatic carotid stenosis undergoing carotid endarterectomy. Methods: Standard carotid endarterectomy in asymptomatic carotid stenosis under general intravenous anaestehesia with selective use of a shunt. Flow measurement in major vessels of the Willis circle using magnetic resonance angiography with phase contrast before and three months after the operation and concurrently with RBANS psychological examination testing battery. Results: Significant cognitive performance improvement in neuropsychological test battery total score and in short-term memory (p = 0.04) and speech (p = 0.01) indexes was seen in five women and 12 men undergoing uncomplicated carotid endarterectomy. Subsequent correlations of local haemodynamic change and cognitive performance change in 12 patients were significant for the anterior cerebral artery and immediate memory index (p = 0.01), and for the medial cerebral artery and speech indeces (p = 0.02) and visuospatial orientation (p = 0.02). Conclusion: In a pilot study, a flow (ml/min) examination protocol of all major Willis circle arteries was tested and haemodynamic changes before the operation and in three months follow-up after carotid endarterectomy were compared on a small group of patients with radiologically and clinically asymptomatic carotid stenosis for the first time in Czech literature. Improvement in cognitive performance was demonstrated and an association with anterior or medial cerebral artery flow values was identified in individual RBANS tests. A larger study with a longer follow-up is feasible. Karcinom penisu patří mezi řídce se vyskytující onemocnění, avšak s významnými psychosociálními následky. Postihuje muže převážně vyššího věku s maximem výskytu v šesté dekádě života. Etiologické a epidemiologické faktory jeho vzniku jsou poměrně dobře známy. Významnou roli při vzniku nádoru hraje infekce HPV, zejména typy HPV 16 a 18. Vzhledem k nízké incidenci karcinomu penisu kolem 1/100 000 osob jsou na většině urologických a onkologických pracovišť malé zkušenosti s jeho terapií. Neexistují žádné prospektivní randomizované studie o jeho léčbě. Literární soubory o chirurgické a další onkologické léčbě, jako je radioterapie a chemoterapie, se týkají obvykle jen několika desítek pacientů léčených během dlouhého období. Časná chirurgická léčba lokálního nálezu a případného uzlinového postižení hraje podstatnou roli ve vyléčení a prognóze pacienta. Nechirurgická léčba karcinomu penisu je v českém písemnictví málo publikována. V přehledovém článku je zdůrazněna nutnost přesné diagnózy a stagingu pro stratifi kaci pacientů do skupin ke vhodné léčbě podle rizika a prognostických faktorů. Dále je uveden pohled na současné možnosti a roli radioterapie a chemoterapie v léčbě karcinomu penisu. Stručně jsou také uvedeny vlastní zkušenosti s léčbou 25 pacientů ozářených pomocí HDR brachyterapie., Penile cancer is a rare tumour, however it has serious psychological and social consequences. The incidence of penile cancer increases with reaching with maximum in the sixth decade of life. The aetiological and epidemiological penile cancer risk factors are well recognized. Among them human papilloma virus infection, namely types HPV 16 and 18 is an important. Due to its low incidence about 1/100 000, there is limited experience with penile cancer therapy in most of urology or oncology departments. Randomised prospective studies are not available. Reports about surgical and other oncological therapy describe small studies with patients treated during long period. Early surgical treatment of penile lesions, and if possible of nodal metastases, plays an essential role in curing the disease and its prognosis. The review article highlights the importance of precise diagnosis and staging for the stratifi cation of the patients and selection appropriate therapy according to risk and prognostic factors. The role of contemporary radiotherapy and chemotherapy in multimodal treatment of penile cancer is discussed. The pilot study deals with the clinical results of the regenerative therapeutical method in intraosseous periodontal defects done by surgical augmentation procedure using a material of a new generation prepared on the basis of a bioactive glass. A group of 10 individuals were treated properly for chronic periodontitis. Some of treated infraalveolar periodontal lesions, both periodontal pockets and interradicular defects, persisted or recurred in posterior teeth. In order to eliminate them they were indicated for advanced periodontal surgery or regenerative surgical therapy stimulating healing processes of supportive tissues, mostly alveolar bone around treated teeth. Relevant clinical parameters, i. e. values of the pocket depth, gingival recession, and loss of attachment were evaluated before and after the periodontal surgery and compared with each other. Cílem práce je zhodnotit význam stanovení hladiny TATI (tumour associated trypsin inhibitor) v moči pro neinvazivní detekci a odhad prognózy pacientů s tumory močového měchýře. Soubor, metodika: Do studie jsme zařadili 63 pacientů, z toho 30 s histologicky verifi kovaným tumorem měchýře. Střední doba sledování byla 23 měsíce. Pacienti byli rozděleni do jednotlivých skupin v závislosti na rozdílném stupni buněčné diferenciace, stupni nádorové invaze, počtu nádorů a jejich velikosti. Kontrolní skupinu tvořilo 33 pacientů s benigním onemocněním uropoetického traktu. Výsledky: Průměrná hladina TATI v moči u pacientů s tumorem močového měchýře byla 68,65 μg/l, zatímco v kontrolní skupině to bylo pouze 20,38 μg/l. Průměrná hladina TATI v moči u pacientů s G1 tumorem byla 15,76 μg/l, u G2 tumorů 86,3 μg/l a 33,3 μg/l u nádorů G3. Průměrná hladina TATI v moči u tumorů s TaT1 tumorem byla 73,3 μg/l, u infi ltrujících tumorů 40,51 μg/l. Průměrná hladina TATI v moči u pacientů se solitárním tumorem byla 60,28 μg/l a v případě mnohočetného postižení 94,42 μg/l, 26,3 μg/l u tumorů do 3 cm velikosti a 100,02 μg/l u tumorů větších než 3 cm. Průměrná hladina TATI v moči ve skupině pacientů, kteří recidivovali v průběhu sledování, byla 60,3 μg/l a 75,0 μg/l ve skupině bez recidivy onemocnění. Neprokázali jsme význam močové hladiny TATI pro odhad rizika recidivy onemocnění. Závěr: Hodnota TATI v našem souboru nekorelovala s přítomností tumoru močového měchýře. Ve skupině pacientů s tumorem měchýře nekorelovala hladina TATI s jeho velikostí, buněčnou diferenciací ani multiplicitou. Neprokázali jsme význam močové hladiny TATI pro odhad rizika recidivy onemocnění., Aim: The aim of this study was to assess the utility of TATI level in urine as a non-invasive method for detecting urinary bladder cancer and evaluating its prognosis. Materials and Methods: Sixty three patients were enrolled in the study (38 men, 25 women), with a mean age of 66.2 years, and an average follow-up time of 23 months. Patients were divided into groups according to tumour grade, stage, number of lesions, and their size. Thirty three patients with benign urological disease were included as controls. Results: The median TATI level in the group patients with tumours was 68.65 μg/l. In the control group it was 20.38 μg/l. The mean TATI level was 15.76 μg/l in the G1 group, 86.3 μg/l in G2 group, and 33.3 μg/l in G3 group. The mean TATI level in patients with a TaT1 tumour was 73.3 μg/l, and 40.51 μg/l in patients with an invasive carcinoma. The TATI level in solitary tumours was 60.28 μg/l, 94.42 μg/l in multiple tumours, 26.3 μg/l in tumours up to 3 cm, and 100.02 μg/l in larger tumours. The mean TATI level in the group of patients with bladder tumour recurrence during followup was 60.3 μg/l, 75.0 μg/l without recurrence, 12.0 μg/l in patients who progressed, and 55.7 μg/l in patients who did not progress.
