Úvod: Monoklonální gamapatie nejistého významu (MGUS) je bezpříznakový a potenciálně maligní stav charakterizovaný nezhoubnou klonální proliferací plazmatických buněk secernujících monoklonální imunoglobulin (MIG, M-protein) detekovatelný v séru a/nebo v moči při absenci zhoubného lymfoproliferativního stavu. Je známo, že část jedinců s MGUS transformuje v některou z maligních forem monoklonálních gamapatií. Mezi známé faktory určující míru rizika progrese patří zejména kvantita, typ M-proteinu a poměr volných lehkých řetězců imunoglobulinu (VLŘ) umožňující účelnou stratifikaci jedinců s MGUS. Někteří autoři pokládají i za možný faktor progrese přítomnou supresi hladin polyklonálních imunoglobulinů. Cíl: Cílem studie bylo provedení pilotní analýzy hladin párů těžkých/lehkých řetězců imunoglobulinu (HLC) v souboru jedinců s MGUS a porovnání výsledků s hladinami M-proteinu vyšetřených gelovou elektroforézou. Dále pak provedení porovnání hladin polyklonálních imunoglobulinů a hladin alternativního páru HLC s cílem ověření stupně imunitní parézy v závislosti na rizikovosti MGUS. Metody: Analyzovaná sestava zahrnovala 148 vzorků sér od jedinců s MGUS (102 IgG, 28 IgA, 18 IgM typu), kteří byli v závislosti na hladinách, typu M-proteinu a hodnotě indexu VLŘ stratifikováni do 4 rizikových skupin (nízké, nízké-střední, vysoké-střední a vysoké riziko transformace). Pro stanovení hladin VLŘ, hladin polyklonálních imunoglobulinů a hladin HLC bylo využito platformy turbidimetru SPA Plus. Statistická analýza byla provedena pomocí Spearmanovy korelační analýzy a Mann-Whitneyeho U testu s Bonferroniho korekcí. Výsledky: Při porovnání hladin HLC byla v souboru IgG MGUS zjištěna silná korelace s hladinami M-proteinu jak v případě IgGκ (r = 0,698, p = < 0,0001), tak v případě IgGλ (r = 0,847, p = < 0,0001), součet obou párů izotypu IgG velmi silně koreloval s celkovými hladinami IgG v séru (r = 0,893, p = < 0,0001). Obdobné výsledky byly získány u MGUS s IgA izotypem MIG. V případě izotypového páru IgAκ byla zjištěna středně silná korelace s hladinami M-proteinu (r = 0,601, p = 0,018), v případě IgAλ pak silná korelace (r = 0,733, p = 0,004), přičemž součet obou HLC párů vykazoval velmi silnou korelaci s hladinami celkového IgA ( r = 0,941, p = < 0,0001). Při analýze jedinců s IgM typem byly taktéž zjištěny velmi silné korelace hladin IgMκ (r = 0,904, p = 0,001) resp. IgMλ (r = 0,783, p = 0,013) a hladin M-proteinu. Nejsilnější korelace byly opět zjištěny mezi součtem obou izotypových párů a celkových hladin IgM imunoglobulinu v séru (r = 0,967, p = < 0,0001). Při porovnání míry suprese hladin polyklonálních imunoglobulinů u jednotlivých rizikových tříd v souboru IgG MGUS byly zjištěny signifikantně vyšší hladiny IgA imunoglobulinu u jedinců s nízkým resp. nízkým středním rizikem ve srovnání se skupinou s vysokým středním rizikem (p = 0,0001 resp. p = 0,047). Při analýze skupiny MGUS s IgA izotypem byly zjištěny významně vyšší hladiny IgM imunoglobulinu u skupiny s nízkým středním rizikem nežli u jedinců s vysokým středním resp. vysokým rizikem (p = 0,035 resp. p = 0,017). V souboru IgG MGUS bylo rovněž provedeno porovnání hladin alternativních izotypových párů mezi jednotlivými rizikovými skupinami. U jedinců s dominantní sekrecí IgGκ byly zjištěny signifikantně vyšší hladiny alternativního páru IgGλ ve skupině nízkého a nízkého středního rizika nežli ve skupině vysokého středního rizika (p = 0,003 resp. p = 0,006). V případě dominantní sekrece IgGλ byly zjištěny významně vyšší hladiny IgGκ u jedinců s nízkým resp. nízkým středním rizikem nežli ve skupině s vysokým středním rizikem (p = 0,0006 resp. p = 0,009). Závěr: Analýza potvrdila velmi těsnou korelaci mezi hladinami M-proteinu a stanovením HLC, která se jeví jako pomocná ev. alternativní metoda stanovení M-proteinu s výhodou u atypických výsledků elektroforézy (zejména u IgA izotypu) či v případě velmi nízkých koncentrací M-proteinu s výhodou výpočtu indexu Igκ/Igλ. Odhalená souvislost míry imunosuprese v závislosti na rizikovosti MGUS a zejména jasně patrný přínos stanovení alternativního HLC páru přináší další aspekt k pochopení souvislosti biologie, chování a případné maligní evoluce MGUS s výhodou získání dobře měřitelného parametru. Využití platformy SPA Plus se jeví jako rychlé, efektivní, s vysokou výpovědní hodnotu a v neposlední řadě i s příznivými ekonomickými aspekty, Introduction: Monoclonal gammopathy of undetermined significance (MGUS) indicates an asymptomatic and potentially malignant state characterised by benign clonal proliferation of plasma cells secerning (discharging) monoclonal immunoglobulin (MIG, M-protein) detectable in serum and/or urine in the absence of malignant lymphocyte proliferation. It has been observed that in some patients, MGUS transforms into one of the malignant forms of monoclonal gammopathy. The wellknown factors determining the risk degree of progression include in particular the quantity, M-protein type and the ratio of immunoglobulin free light chains (FLC) allowing the efficient stratification of MGUS patients. In addition, several authors consider the present suppression of polyclonal immunoglobulin levels to be a potential factor of progression. Aim: The study aimed at conducting a pilot analysis of levels of immunoglobulin heavy/light chains pairs (HLC) in a group of MGUS patients and at comparing the results with the M-protein levels detected by gel electrophoresis. Furthermore, the polyclonal immunoglobulin levels and the levels of HLC alternative pairs were to be compared with a view to verify the degree of immune paresis depending on the MGUS risk degree. Methods: The analysed set comprised 148 serum samples of MGUS patients (102 IgG, 28 IgA, 18 IgM type) who were stratified into 4 risk groups (low, low-intermediate, high-intermediate, and high risk of transformation) according to the levels, M-protein type and FLC index values. FLC levels, polyclonal immunoglobulin levels, and HLC levels were determined by means of the SPA Plus turbidimeter platform. In the statistical analysis, Spearmans rank correlation and Mann-Whitney U Test with Bonferroni correction were applied. Results: When comparing the HLC levels, the IgG MGUS group revealed strong correlation with M-protein levels both in the case of IgGκ (r = 0.698, p = < 0.0001), and in the case of IgGλ (r = 0.847, p = < 0.0001), the summation of both pairs of the IgG isotype correlated considerably with the total levels of IgG in serum (r = 0.893, p = < 0.0001). Similar results were acquired in MGUS with the IgA isotype of MIG. In case of the IgAκ isotype pair, medium-strong correlation with M-protein levels was detected (r = 0.601, p = 0.018), and strong correlation in case of the IgAλ (r = 0.733, p = 0.004), whereas the summation of both HLC pairs showed very strong correlation with levels of the total IgA (r = 0.941, p = < 0.0001). Comparison of the suppression degree of polyclonal immunoglobulin levels in individual risk classes in the IgG MGUS group brought significantly higher levels of IgA immunoglobulin in patients with low or low-intermediate risk as opposed to the group with high-intermediate risk (p = 0.0001 and p = 0.047 respectively). When analysing the MGUS group with IgA isotype, notably higher levels of IgM immunoglobulin were evident in the group with low-intermediate risk than in patients with high-intermediate or high risk (p = 0.035 and p = 0.017 respectively). In addition, the levels of alternative isotype pairs were compared among the individual risk groups of the IgG MGUS group. Patients with dominant IgGκ secretion showed significantly higher levels of the IgGλ alternative pair in the group of low and low-intermediate risk than in the group of high-intermediate risk (p = 0.003 and p = 0.006 respectively). In case of dominant IgGλ secretion, considerably higher IgGκ levels were detected in patients with low or low-intermediate risk than in the group of high-intermediate risk (p = 0.0006 and p = 0.009 respectively). Conclusion: The analysis confirmed a very close correlation between M-protein levels and HLC determination which appears to be an auxiliary or alternative method of M-protein determination with an advantage in case of atypical electrophoresis results (in particular in IgA isotypes) or by very low M-protein concentrations with the benefit of Igκ/Igλ index calculation. The discovered connection between the degree of immune suppression depending on the MGUS risk level and especially the clearly visible benefit of determining the alternative HLC pairs contributes with another aspect to understanding the links between the biology, behaviour, and the potential malignant evolution of MGUS with the advantage of obtaining a well-measurable parameter. Kazuistika pojednává o třicetileté pacientce, která byla opakovaně hospitalizována pro levostrannou ileofemorální trombózu. Pacientka byla indikována k lokální farmakologické trombolýze. V průběhu trombolýzy bylo však vyjádřeno podezření na možnost komprese cév zvenčí. Následovala další vyšetření, která odhalila měkkotkáňovou nebolestivou formaci na mediální straně stehna jako příčinu na léčbu rezistentní trombózy. Rezistence se nacházela v těsném kontaktu s nervově-cévním svazkem na stehně. Dle histologie se jednalo o bifázický synoviální sarkom. V době diagnózy byly již přítomny vzdálené plicní metastázy I přes intenzivní neoadjuvantní léčbu dochází k progresi zejména plicního postižení. Chirurgická léčba není toho času indikována. Pacientka umírá 6 měsíců od stanovení diagnózy., Case report deals with a 30-year-old female patient, who was repeatedly admitted to hospital with left sided ileofemoral thrombosis. The patient was scheduled to local farmacological thrombolysis. During the thrombolysis a suspicion of compression of the vessels from outside was expressed. A further examination revealed an extensive soft-tissue non-painful mass on the medial side of the thigh as a cause of the thrombosis, which was not responding to the treatment. The lesion was in close contact with the femoral neurovascular bundle. The histological examination proved biphasic synovial sarcoma. At the time of diagnosis, distant lung metastases were already present. Despite intensive neoadjuvant therapy the disease, especially the lung affection progressed. Mozkové metastázy jsou nejčastějším nádorovým postižením mozku u dospělých pacientů. Dle publikovaných dat se vyskytují u 10–20 % pacientů se solidními nádory. Ačkoliv moderní metody protinádorové léčby přinesly v posledních dvou dekádách prodloužení přežití pacientů se solidními tumory, v případě mozkových metastáz se udává průměrná doba přežití do šesti měsíců od stanovení diagnózy. Cíl. Shrnout dostupné informace a analyzovat zobrazené nálezy na námi zachycených běžných i méně běžných případech tarzální koalice. Budeme se zabývat primárními i sekundárními nálezy při vlastním zobrazení i korelací s literaturou, udávanými histologickými nálezy, zobrazovacími metodami a protokoly, diferenciální diagnózou a krátkou informací o klinickém nálezu a léčbě. Metody. Vyšetřili jsme osm pacientů s různými typy tarzální koalice - s koalicí talokalkaneální mediální i dorzální, s koalicí kalkaneonavikulární, s oboustrannou talokalkaneální koalicí i s koalicí talokalkaneokuboidní, která byla součástí komplexnější dysplazie. Nálezy jsou analyzovány především na CT a MR, jsou doplněny RTG snímky. Výsledky. V závislosti na typu koalice a zvolené projekci, jsou známky na RTG snímku buď patognostické (u šikmé projekce a kalkaneonavikulární koalice), nebo diskrétní a nepřímé (u ostatních projekcí a ostatních typů koalice). CT a MR zobrazí tuto vadu při vhodně zvolených rovinách zobrazení a sekvencích nepochybně. Mezi nejvýznamnější nálezy patří zúžená nebo přemostěná kloubní štěrbina, nerovná kortikální kost, edém, osteoskleróza a změna tvaru kostí. Časté jsou sekundární degenerativní změny okolních kloubů. Závěr. Povědomí o známkách této vady umožňuje její rozpoznání i přes její málo frekventní výskyt, a to i v případech, kdy na ni klinický lékař nevysloví podezření. Na CT a MR je třeba při vyšetřování hlezna rutinně zachycovat i oblast subtalárních kloubů a tarzu., Aim. To summarize and display the imaging findings in patients with different types of both frequent and less frequent types of tarsal coalition, to discuss the imaging protocols, differential diagnoses and give brief information on clinical findings and treatment. Method. The authors display CT, MRI and X-ray findings in 8 patients with the main types of coalition (talocalcaneal, calcaneonavicular), with a bilateral talocalcaneal coalition and in a patient with a complex synostosis. Results. The encountered findings were: narrowed or absent joint space, thinned irregular cortical line, adjacent oedema and sclerosis and an unusual shape of the involved bones. Secondary degenerative changes in the adjacent joints were often encountered. Depending on the type of coalition and on the projection, this diagnosis presents with X-ray findings, which are either prominent (in calcaneonavicular coalition and an oblique X-ray) or subtle and indirect (all other cases). In MRI and CT the diagnosis is much more obvious. The pathognomonic signs as well as secondary degenerative signs will be pointed out together with the appropriate imaging protocols. A differential diagnosis will be discussed. The findings of the authors will be correlated with the literature. Conclusion. It is necessary to be aware of the findings in tarsal coalition, in order to establish the diagnosis even in cases unsuspected by the clinitian. Cíl studie: Vyhodnotit přínos interleukinu-6 u těžké akutní pankreatitidy pro odlišení sterilní nekrózy (SN) a nekrózy infikované (IN). Typ studie: Retrospektivní deskriptivní observační studie série případů. Metody: Ve vlastní práci bylo zahrnuto 59 pacientů hospitalizovaných s těžkou akutní pankreatitidou (Atlantská klasifikace 1992) na chirurgickém oddělení Nemocnice Písek v průběhu sedmi let. Pravidelně byly vyšetřovány C-reaktivní protein (CRP) a interleukin-6 (IL-6), přičemž dostatečná data byla u 42 pacientů: 14 s IN (průkaz mikrobiologicky či dle CT), 28 se SN. Statistické vyhodnocení pomocí Fisherova testu a chí-kvadrát testu. Výsledky: Byla nalezena statisticky významná diskriminační schopnost denních průměrů IL-6 5. a 6., resp. 7. a 8. den hospitalizace mezi IN a SN (p=0,0014 pro den 5 a 6, resp. p=0,0009 pro den 7 a 8). Rozvoj IN reálně nastával nejčastěji mezi 5. a 9. dnem (mikrobiologický průkaz agens či nepochybné CT známky infekce). CRP tuto schopnost diskriminace neprokázalo, jeho hladiny byly od 2. do 8. dne setrvale výrazně zvýšeny. Charakteristický byl průběh koncentrací IL-6, kdy vstupně byla přítomna výrazná elevace nezávislá na pozdějším rozvoji IN a již od druhého dne hospitalizace došlo k výraznému poklesu. K dalšímu vzestupu IL-6 došlo pouze u pacientů s rozvojem IN. Pro cut-off (diskriminace IN a SN) IL-6 (platný od 5. dne hospitalizace) 100 ng/l bylo p<0,005, pro 150 ng/l p<0,01. Závěr: Interleukin-6 v naší retrospektivní studii prokázal statisticky významnou schopnost diskriminovat infikovanou a sterilní nekrózu u těžké akutní pankreatitidy. Překročení cut-off IL-6 od 5. dne hospitalizace svědčilo pro přítomnost infikované nekrózy. Cut-off 100 ng/l diskriminoval obdobně (p<0,005, senzitivita 100 %, specificita 92 %) jako cut-off 150 ng/l (p<0,01, senzitivita 87 %, specificita 100 %)., Objective: Our retrospective study aimed to evaluate the contribution of interleukin-6 in severe acute pancreatitis. Mortality of severe acute pancreatitis ranges from 10-20% (for sterile necrosis, SN) to 20-85% (infected necrosis, IN). Resolution of SN and IN is clinically very important, but often difficult and therefore they are still searching for new laboratory markers for their differentiation. Design: Retrospective descriptive observational study of cases series. Methods: 59 patients hospitalized with severe acute pancreatitis (Atlanta classification 1992) in the surgery department of Hospital Písek in the 2000-2006 were included. Regularly they were investigated by C-reactive protein (CRP) and interleukin- 6 (IL-6), sufficient data was in 42 patients: 14 with IN (identified microbiologically or according to computed tomography, CT), 28 with SN. Statistical analysis was performed using Fisher‘s test and chi-square test. Results: There was found statistically significant discriminatory ability of daily averages of IL-6 5th and 6th + 7th and 8th day of hospitalization between the IN and SN (p = 0.0014 for day 5 and 6, respectively. p = 0.0009 for day 7 and 8). Developing IN realistically set in the most between 5th and 9th day (needle biopsy with identification of microbiological agents or CT signs of infection). The ability of CRP to discriminate SN and IN was not found, the levels were between 2nd to 8th days increased steadily. The run of the concentrations of IL-6 was characteristic, where at the admission there was present significant elevation irrespective of subsequent development of IN and from the second day of hospitalization it decreased significantly. Further increase in IL-6 was present only in patients with the development of IN. For the cut-off (discrimination of SN and IN) for IL-6 (valid from 5th of hospitalization) 100 ng/L was P<0.005, for 150 ng/L P<0.01. Conclusion: Interleukin-6 in our retrospective study showed a statistically significant ability to discriminate between infected and sterile necrosis in severe acute pancreatitis. Autoři ve svém příspěvku uvádějí první výsledky šetření, které bylo zaměřeno na zjištění hmotností školních tašek chlapců a dívek 1. a 2. tříd základních škol v olomouckém regionu. Celkem byla zjištěna hmotnost u 502 školních tašek, změřeno 502 dětí (263 dívek a 239 chlapců). Z výsledků měření je zřejmé, že hmotnost tašky se zvyšuje s postupem žáků do vyšší třídy. Největší rozdíl mezi minimální a maximální hodnotou hmotnosti tašky byl zjištěn u 8letých dívek, činil 8,9 kg. Výsledky ukázaly, že procentuální podíl hmotnosti tašky doporučeným 10 % tělesné hmotnosti dítěte odpovídal pouze u 17,5 % dětí. 82,5 % dětí pak nosí aktovky těžké (10–20 %), přičemž 18,3 % dětí nosí aktovky o hmotnosti nad 20 % své tělesné hmotnosti, což už lze považovat za extrémně těžké. Nepřiměřená hmotnost školní tašky pak může ovlivňovat držení těla a výskyt bolesti zad., Effects of the weights of school bags carried by pupils from the first two classes of primary schools in the Olomouc region (502 schoolbags belonging to 263 girls and 239 boys) on the pupils’ posture were examined. The weight of the schoolbags of pupils from the 2nd year class was found to be higher than of pupils in the first year. The greatest difference between minimum and maximum weights (8.9 kilograms) was detected in the schoolbags of 8 year old girls. Only 17.5% of children carried bags that were within the recommended 10% of the child’s body weight. Introduction: We studied influence of mud-bath on bone status in male Wistar rats with subchronic arthritis. Methods: Arthritis was induced by 2 subplantar injections of Freund’s adjuvans with heat-killed Streptoccocus pyogenes into paw. Groups: intact (int) on chippings; (con) arthritis on chippings; (san38) arthritis on hot sand; (mu38) arthritis on hot mud; (mu21) arthritis on mild mud. Bone mineral density (BMD, g/cm2) was measured by dual energy X-ray absorptiometry and femurs were tested biomechanically. Bone markers osteocalcin (OC), PINP and CTX were analysed in bone. Results: BMD of right femur decreased vs. left in san38 (p = 0.030) and mu38 (p = 0.047). Fracture load of right/left femur (N) decreased in experimental groups, significantly in san38 (p = 0.05). Fracture threshold of neck decreased in right vs. left in experimental groups, but significantly in san38 (p = 0.05). OC decreased in mu38 vs. con (1.84 ± 0.14/2.62 ± 0.23). PINP decreased in int vs. san38 (p = 0.005) and mu21 (p < 0.001). CTX decreased in int vs. mu38 (p = 0.006) and mu21 (p = 0.005). Conclusion: The hot bath appears indifferent in relation to osteoporosis, while cold mud-bath shows good effect on bone metabolism. Regulatory T cells (Treg) are a specialized subpopulation of T cells that act to suppress inadequate immune response. Psoriasis is recognized as a T -cell driven immune-mediated systemic inflammatory disease with skin manifestation. Effective therapeutical approach to treat psoriasis is Goeckerman therapy (GT ). The aim of this study was to compare the number of Treg in the peripheral blood of 27 psoriatic patients and 19 controls and to evaluate the influence of GT on Treg population in peripheral blood of patients with psoriasis. There was no significant difference in the relative number of Treg cells in the peripheral blood of healthy blood donors and patients with psoriasis before initiation of GT (P = 0.2668). In contrary, the relative number of Treg cells in peripheral blood of patients with psoriasis after GT was significantly higher than those found in healthy blood donors (P = 0.0019). Moreover, the relative number of Treg is significantly increased in psoriatic patients after Goeckerman therapy compared to the pre-treatment level (P = 0.0042). Cíl práce: Lipoperoxidace je dávána do příčinné souvislosti s rozvojem nejdůležitějších rizikových faktorů kardiovaskulárních onemocnění. Lipoperoxidační procesy jsou důsledkem reakcí lipidů s volnými radikály bez ohledu na jejich původ. Vztah mezi triacylglyceroly (TAG) a volnými radikály (VR) není zcela jasný. Může se dokonce stát, že hypertriacylglycerolémie je doprovázena nízkými koncentracemi VR a naopak. Jedno z možných vysvětlení spočívá ve vztahu lipoperoxidace a složení mastných kyselin v molekulách TAG. Cílem této studie bylo analyzovat vztah složení mastných kyselin v TAG a produkci VR u pacientů trpících diabetes mellitus 2. typu. Typ studie: observační s kontrolní skupinou Materiál a metody: Studie zahrnovala 18 normotriacylglycerolemických (NTG) jedinců (M/F 12/6) a 24 hypertriacylglycerolemických (HTG) jedinců (M/F 16/8). Pacienti v obou skupinách byli diabetici 2. typu. Analýzu mastných kyselin v triacylglycerolech plazmy jsme provedli kapilární plynovou chromatografií a koncentrace volných radikálů byly změřeny přímou spektrofotometrií. Statistické analýzy (parametrické i neparametrické) byly provedeny pomocí statistického software STATISTICA pro Windows. Výsledky: Ve skupině HTG jsme pozorovali nižší zastoupení kyseliny myristové (14:0) (1.62 [1.23-2.07] vs. 1.90 [1.72-2.79], p = 0.022, [medián (1.-3. kvartil]) a vyšší poměr alfa-linolenové kyseliny (18:3n-3) (1.51 [1.21-1.86] vs 1.16 [1.02-1.42], p = 0.021). Poměr kyselin arachidonové/eikosapentaenové byl ve skupině HTG nižší (5.54 [3.62-7.98] vs 7.25 [5.98-15.12], p = 0.027). Když jsme HTG skupinu dále rozdělili podle koncentrací VR na tercily, prokázali jsme nižší hodnoty poměru kyseliny arachidonová/eikosapentaenová ve druhém tercilu (4.05 [2.66-4.67] vs 7.64 [5.83-11.00], 1. tercil, a 6.21 [3.20-8.31], 3. tercil p = 0.019). V spojených HTG+NTG skupinách jsme zaznamenali negativní korelaci mezi MA a TAG (r = -0.3153, p = 0.045). V triaclylglycerolech jsme prokázali pozitivní korelace VR a kyseliny stearové (p < 0.001) spolu s negativní korelací některých n-6 vícenenasycených mastných kyselin (18:3n-6, p < 0.05, 22:5n-6, p < 0.01). Závěr: Zastoupení mastných kyselin v molekulách TAG je spojeno s tvorbou VR. Nicméně do tohoto vztahu vstupují i jiné faktory., Objective: The lipoperoxidation plays a causal role in the development of most important risk factors of cardiovascular disease. The lipoperoxidation results from the reaction of lipid molecules with free radicals, regardless of their origin. The relationship between triacylglycerols (TAG) and the free radicals (FR) amount is not clear. Thus, it can turn up that the hypertriacylglyceridaemia is accompanied with the low values of FR, or vice versa. One possible explanation of this phenomenon is that the degree of lipoperoxidation of TAG is dependent on the composition of fatty acids (FA) in their molecule. Therefore, the aim of this study was to analyze FA composition in TAG and its relationship to FR production in type 2 diabetes mellitus. Design: observational study with internal control group Material and Methods: 18 normotriacylglycerolemic (NTG) individuals (12M/6F) and 24 hypertriacylglycerolemic (HTG) patients (16M/8F). Patients in both groups were persons, suffering from type 2 diabetes mellitus. Fatty acid analysis of plasma triacylglycerols was performed by gas chromatography and free radicals were determined by direct spectrophotometric method. Statistical analyses (both parametric and non-parametric) were performed with the statistical software STATISTICA for Windows. Results: We found lower content of myristic acid (14:0) (1.62 [1.23-2.07] vs. 1.90 [1.72-2.79], p = 0.022, [median (1st-3rd quartile)]) and higher content of alpha-linolenic acid (18:3n-3) (1.51 [1.21-1.86] vs. 1.16 [1.02-1.42], p = 0.021) in HTG group. The ratio of arachidonic/eicosapentaenoic acid was in HTG group lower (5.54 [3.62-7.98] vs. 7.25 [5.98-15.12], p = 0.027). When the HTG group was stratified into the tertiles by the FR concentrations, we observed lower values in the second tertile for ratios based on arachidonic acid (20:4n-6) and eicosapentaenoic acid (20:5n-3) (4.05 [2.66-4.67] vs. 7.64 [5.83-11.00], 1st tertile, and 6.21 [3.20-8.31], 3rd tertile, p = 0.019). In pooled groups, we proved negative correlation between MA and TAG (r = -0.3153, p = 0.045) and the link between FR and FA composition of TAG expressed as positive relationship of FR with content of stearic acid (p < 0.001) and negative correlation with some n-6 polyunsaturated fatty acids (18:3n-6, p < 0.05, 22:5n-6, p < 0.01). Conclusion: The composition of FA in TAG molecules has undoubtedly some relationship to the FR generation. However, the other factors seem to play a role, since the relationship is not straightforward within entire FR range., and Vecka M., Tvrzická E., Votruba M.
BACKGROUND: The presence of several risk factors (genetic and non-genetic) has greater impact on the risk of premature coronary artery disease (CAD) than single risk factor. OBJECTIVE: The aim of the study was to establish possible relations between genotypes and alleles of 677C>T polymorphism of MTHFR gene and some traditional risk factors e.g. elevated levels of lipid parameters and smoking in development of premature CAD. METHODS: The groups comprised 152 patients with angiographically documented premature CAD (aged 42.9 +/- 5.5) and 121 age-matched blood donors (aged 42.3 +/- 6.5) were studied. The MTHFR 677C>T polymorphism was genotyped with Polymerase Chain Reaction-Restriction Fragment Length Polymorphism (PCR-RFLP) method. RESULTS: Patients with TT genotype who simultaneously smoked had increased risk of premature CAD compared to non-smoking cases with CC genotype (OR = 24.62). We also found that individuals with TT genotype and elevated LDL-cholesterol (LDL-chol.) level had significantly higher risk of CAD (OR = 9.92) than individuals with normal LDL-chol. level and CC genotype. CONCLUSIONS: The present study shows that simultaneous presence of MTHFR TT genotype and smoking or elevated levels of LDL-chol. influences the risk of premature CAD.
