Extramedulárne (EM) postihnutie pri akútnej myeloblastovej leukémii (AML) je relatívne vzácne a vyskytuje sa najčastejšie v 2 formách – ako myeloidný sarkóm (MS) alebo leukemia cutis (LC). Doposiaľ nebol presvedčivo objasnený molekulový mechanizmus EM postihnutia, je však pravdepodobné, že podiel chemokínových receptorov, adhezívnych molekúl a ich vzájomných interakcií, ktoré kontrolujú migráciu leukemických buniek do periférnych tkanív, je kľúčový. Vznik izolovaného EM relapsu (IEMR) po alogénnej transplantácii krvotvorných kmeňových buniek (ASCT) predstavuje menej častý klinický prejav s nepriaznivou prognózou, najmä v kombinácii s relapsom systémovým. Je pravdepodobne dôsledkom nepomeru medzi GvL efektom v kostnej dreni (KD) a periférnych tkanivách. Jeho liečba nie je jednoznačne definovaná. Predložená kazuistika ukazuje rýchly EM relaps v oboch prsných žľazách u pacientky s akútnou leukémiou so zmiešaným fenotypom (MPAL) po ASCT. Bioptická verifikácia pod ultrazvukovou kontrolou potvrdila prítomnosť leukemickej infiltrácie. Cieľom práce je ukázať menej obvyklý prípad IEMR s dôrazom na komplexnú diagnostiku a veľmi dobrú liečebnú odpoveď po ukončení imunosupresie., Extramedullary (EM) manifestation of acute myeloid leukemia (AML) is relatively rare, it occurs frequently in 2 forms: myeloid sarcoma (MS) or leukemia cutis (LC). The molecular mechanism of EM involvement has not been clearly explained yet, but it is obvious that homing of leukemic cells to peripheral tissues is controlled by chemokine receptors, adhesion molecules and their interactions. The isolated EM relapse (IEMR) after allogeneic haematopoietic stem cell transplantation (ASCT) is an uncommon clinical manifestation with an unfavourable prognosis, especially in case of systemic relapse. It is probably the result of GvL imbalance in the bone marrow and peripheral tissues. The treatment of IEMR is not clearly defined. We present a case report of the rapid EM involvement in both breasts in patient with acute leukemia (mixed phenotype) (MPAL) after ASCT. Verification biopsy under ultrasound-guidance confirmed the presence of leukemic infiltration. Úvod: Infekčná endokarditída u pacienta po transplantácii obličky je závažná infekčná komplikácia, ktorá zvyšuje riziko straty štepu ako aj mortalitu pacientov. Najdôležitejším predisponujúcim faktorom je imunosupresívna liečba – predovšetkým indukčná imunosupresia. Materiál a popis prípadu: V transplantačnom centre Martin podstúpilo v období 12 rokov transplantáciu obličky 250 pacientov. Z tohto súboru pacientov bolo 5 pacientov (2 %) po náhrade chlopne. Prezentujeme prípad pacienta po transplantácii obličky s rozvojom endokarditídy bioprotézy aortálnej chlopne mesiac po úspešnej transplantácii obličky. Diagnostika endokarditídy štandardnými postupmi (transtorakálne echokardiografické vyšetrenie, transezofageálne echokardiografické vyšetrenie, hemokultúry) bola neúspešná. Endokarditídu sme diagnostikovali raritne až pomocou PET-CT vyšetrenia s následnou zmenou antibiotickej liečby a úspešným zvládnutím tejto potransplantačnej komplikácie. Záver: Endokarditída po transplantácii obličky je vážna komplikácia, ktorá výrazne zhoršuje mortalitu príjemcov. Riziko rozvoja infekčnej endokarditídy po transplantácii zvyšuje aj indukcia, a to predovšetkým antitymocytárny globulín. Diagnostika iba pomocou PET-CT vyšetrenia je raritná, ale v prípade nášho popísaného prípadu, zásadne zmenila prístup k pacientovi a viedla k úspešnej liečbe. Kľúčové slová: endokarditída – indukcia – PET-CT – transplantácia obličky, Introduction: Infective endocarditis in a patient after kidney transplantation is a serious infective complication which increases the risk of loss of the graft and also the mortality of patients. The most important predisposing factor is the immunosuppressive therapy – mainly induction immunosuppression. Material and case description: 250 patients underwent kidney transplantation throughout the period of 12 years in the Transplant Center Martin. This set of patients included 5 patients (2 %) after heart valve replacement. We present the case of a patient after kidney transplantation with development of endocarditis of the bioprosthesis of the aortic valve one month after successful kidney transplantation. Diagnostics of endocarditis by standard procedures (examination by transthoracic echocardiogram, transesophageal echocardiography, hemocultures) was unsuccessful. We rarely diagnosed endocarditis only by PET-CT examination with a consequent change of the antibiotic treatment and successful managing of this post-transplant complication. Conclusion: Endocarditis after kidney transplantation is a serious complication which significantly worsens the mortality of patients. The risk of development of infective endocarditis after transplantation is also increased by induction, mainly by antithymocyte globulin. Diagnostics only by PET-CT examination is rare; however, in this case it fundamentally changed the approach to the patient and led to a successful treatment. Epilepsia je chronické ochorenie, ktoré je často sprevádzané psychiatrickou komorbiditou. Najčastejšou z nich je depresia, ktorá je sprevádzaná subsyndromálnymi, atypickými a chronifikovanými príznakmi depresie. Incidencia depresie je u pacientov s epilepsiou vyššia ako u iných neurologických alebo chronických ochorení. Psychopatológia spojená s epilepsiou obťažuje pacienta častokrát viac ako samotné ochorenie. Cieľom práce je prezentácia a analýza kazuistiky 37-ročného muža s diagnózou epilepsie so sprievodnou psychiatrickou symptomatikou, ktorá znížila kvalitu jeho života oveľa viac ako samotné neurologické ochorenie. V kazuistike popisujeme klinické symptómy neuropsychiatrického ochorenia – epilepsie ako aj životné udalosti, ktoré sú u každého pacienta jedinečné a majú závažný vplyv na priebeh a liečbu epilepsie., Epilepsy is a chronic disease that is often accompanied by psychiatric comorbidity. The most frequent of them is depression accompanied by subsyndromic, chronic and atypical symptoms of depression. The incidence of depression in patients with epilepsy is higher than in other neurological or chronic diseases. Psychopathology associated with epilepsy often bothers the patient more than the disease itself. The aim of this work is to present and analyze a case report of a 37‑year‑old man diagnosed with epilepsy and associated psychiatric symptomatology that reduced his quality of life much more than the neurological disease itself. In this case report, we describe clinical symptoms of neuropsychiatric disease – epilepsy as well as life events that are unique to each patient and have a significant impact on the course and treatment of epilepsy.
