- Primární nádory mozku jsou heterogenním onemocněním. Nejzávažnější diagnózu představuje glioblastoma multiforme, který tvoří 75 % z high grade gliomů. Jedná se o agresivní nádory s omezenými terapeutickými možnostmi a špatnou prognózou. Medián přežití od stanovení diagnózy je bez léčby 4,6 měsíce, s léčbou kolem 12 měsíců. Zlatým standardem terapie je v dnešní době radikální operace, chemoradioterapie a následně adjuvantní chemoterapie temozolomidem. V naší práci jsme se zaměřili na přínos přidání temozolomidu ke standardní radioterapii v porovnání s jeho toxicitou., Primary brain tumors are a heterogeneous disease. The most important is glioblastoma multiforme, which constitutes 75 % of the high grade gliomas. These are aggressive tumors with limited therapeutic options and poor prognosis. Median survival from diagnosis is 4,6 months without treatment in the management of around 12 months. The gold standard therapy today is radical surgery, chemoradiotherapy followed by adjuvant chemotherapy with temozolomide. - INTRODUCTION: Annually more than 27,000 persons die of cancer in the Czech Republic and the overall incidence of malignancies is still increasing. These data shows the need for affordable and good follow-up care especially for patients without any cancer treatment due to irreversible progression of tumor. Currently the outpatient palliative cancer care gets more into the forefront. Prerequisite for a well working outpatient palliative care is cooperation with general practitioners and home health care agencies. The purpose of the so called program of palliative cancer care is to guide a patient in palliative cancer care and to improve the cooperation among health care providers. METHODS: During the period from January 2008 to October 2010 we evaluated in patient without any oncology treatment due to irreversible progression of tumor. RESULTS: In palliative outpatient clinic we treated 446 patients, 119 of them received home care services with average length of 27.8 days. 77 patients died at home, 51 in health facilities and 41 in inpatient hospice care. CONCLUSION: We present pilot study focusing on outpatient palliative cancer care which shows the real benefit from early indication of palliative cancer care. - Natalizumab (monoklonální protilátka) je v současné době indikován jako lék druhé volby u relabující-remitující formy roztroušené sklerózy (RR-RS). Závažným a potenciálně fatálním nežádoucím účinkem této léčby je progresivní multifokální leukoencefalopatie (PML), která patří k vzácným oportunním infekcím mozku. Je vyvolána JC virem (JCV). Dochází k progredujícím zánětům bílé hmoty v mnoho četných lokalizacích mozku. Vysoké riziko vzniku PML zahrnuje pozitivitu JCV v krvi, předchozí imunosupresivní léčbu a délku léčby natalizumabem více než 2 roky. Jedním z nejdůležitějších diagnostických metod je MRI, neboť může určit postižení mozku ve velmi raných fázích nemoci. Prezentujeme nálezy 47leté pacientky s RR-RS, která byla pro progresi choroby poslední 3 roky léčena natalizumabem s velmi dobrým klinickým efektem. V posledním roce léčby se u pacientky vyskytl epileptický záchvat, v pozdějších fázích pak rychle probíhající deteriorace kognitivních funkcí a těžká kvadruparéza. V likvoru byl pozitivní nález kopií JCV, na MR byla diagnostikována splývavá ložiska frontálně, charakteristická pro PML. I přes okamžité vysazení léčby natalizumabem a následné zaléčení opakovanými plazmaferézami onemocnění PML progredovalo a pacientka zemřela za 2 měsíce od definitivního stanovení diagnózy. PML je závažná komplikace léčby roztroušené sklerózy natalizumabem. Důležité je včasné rozpoznání příznaků této nemoci. Proto je nezbytné sledovat tyto pacienty podrobně klinicky a včas vyšetřit MR (zejména flair), aby se po stanovení diagnózy a zaléčení nemoci předešlo fatálním komplikacím., Natalizumab (monoclonal antibody) is currently indicated as a second line treatment for relapse-remitting multiple sclerosis (RR-MS). Serious and potential fatal side effect of this treatment is progressive multifocal leukoencefalopathy (PML), which belongs to rare brain infections caused by JC virus (JCV). There is a progressive inflammation of the brain white matter in multiple localizations. A high risk of developing PML includes serum JCV positivity, previous immunosuppressive treatment and duration of natalizumab treatment of more than two years. Magnetic resonance is one of the most important diagnostic methods because it can identify brain damage in the early stage of the disease. We present 47-years-old female with RR-MS which has been successfully treated by natalizumab for three years with very good clinical outcome. In last year of treatment, patient experienced a seizure and in later stages progressive deterioration of cognitive function and severe quadriplegia. Positive JCV was found in cerebrospinal fluid. MR presented diffuse hyperintensive lesions located in frontal lobes. Despite immediate discontinuation of natalizumab treatment and subsequent daily plasma-exchange PML disease progressed and patient died within two months of the final diagnosis. PML is a serious complication of natalizumab treatment in multiple sclerosis. It is important to recognize early signs and symptoms of this disease. - Periprosthetic fractures are the third most common reason for revision total hip arthroplasty. Surgical treatment of periprosthetic fractures belongs to the most difficult procedures due to the extensive surgery, elderly polymorbid patients and the high frequency of other complications. The aim of this study was to evaluate the results of operatively treated periprosthetic femoral fractures after total hip arthroplasty. We evaluated 47 periprosthetic fractures in 40 patients (18 men and 22 women) operated on between January 2004 and December 2010. The mean follow-up period was 27 months (within a range of 12-45 months). For the clinical evaluation, we used modified Merle d'Aubigné scoring system. In group of Vancouver A fractures, 3 patients were treated with a mean score of 15.7 points (good result). We recorded a mean score of 14.2 points (fair result) in 6 patients with Vancouver B1 fractures, 12.4 points (fair result) in 24 patients with Vancouver B2 fractures and 12.7 points (fair result) in 7 patients with Vancouver B3 fractures. In group of Vancouver C fractures, we found a mean score of 16.2 points (good result) in 7 patients. Therapeutic algorithm based on the Vancouver classification system is, in our opinion, satisfactory. Accurate differentiation of B1 and B2 type of fractures is essential. Preoperative radiographic images may not be reliable. If in doubt, checking the stability of the prosthesis fixation during surgery should be performed. and M. Korbel, P. Sponer, T. Kucera, E. - Cíl: Stanovit, jak často je SPECT/CT potřebné ke specifikaci abnormálního nálezu na planárním WB kostním skenu, zda je užitečné porovnat scintigrafický nález s předcházejícím vyšetřením, v kolika případech SPECT/CT určí definitivní diagnózu a kolik ložisek abnormální aktivity nacházíme na kostním skenu u benigních a maligních afekcí. Pacienti a metoda: Retrospektivně jsme prostudovali 471 WB skenů u 450 pacientů s karcinomem prsu, plic a prostaty, kteří v roce 2008 podstoupili kostní scintigrafii. SPECT/CT nebo CT jsme použili v 11,9 % vyšetření u 12,4 % pacientů. Opakovaná scintigrafie (319 pacientů po kostní scintigrafii a 36 pacientů po SPECT/CT) sloužila jako referenční vyšetření. Výsledky: Srovnání s předchozí scintigrafií bylo užitečné u pacientů s karcinomem prsu před indikací SPECT/CT. S použitím SPECT/CT jsme mohli specifikovat nejisté scintigrafické léze u 90,9 % vyšetření. Pro detekci kostních metastáz dosáhlo CT ve SPECT/CT vyšetření senzitivitu 72,8 %, specificitu 96 %, pozitivní a negativní prediktivní hodnotu 88,9 %. V pravděpodobnosti rozvoje kostních metastáz do 2 let od vyšetření mezi pacienty s jedním suspektním ložiskem ve skeletu a pacienty bez suspektního ložiska jsme nezaznamenali signifikantní rozdíl (p = 0,4272), ovšem riziko vzrůstalo, jestliže jsme nalezli suspektních ložisek více (p ≤ 0,0001). Závěr: SPECT/CT zvyšuje specificitu kostní scintigrafie. Po porovnání scintigrafického nálezu s předchozím vyšetřením se zdá rozumné uvažovat o použití SPECT/CT v 10-20 % vyšetření. - Účel studie: Retrospektivně jsme zpracovali výsledky léčby mozkových nádorů stereotaktickou radioterapií a radiochirurgií. Použité metody: Pacienti podstoupili plánovací CT a MR vyšetření. Poté bylo provedeno plánování cílového objemu a ozařovací techniky v BrainLab systému a pacienti byli ozářeni na X-noži Varian s vícelamelovým kolimátorem BrainLab. Radiochirurgie byla aplikována jednorázově s fixací ve stereotaktickém rámu. Pacienti, kteří podstoupili frakcionovanou stereotaktickou radioterapii, byli fixováni pomocí speciální stereotaktické masky. Statistické zpracování bylo provedeno ve spolupráci s Ústavem matematiky a statistiky PřF MU Brno. Výsledky: Celkem bylo ozářeno 101 pacientů s primárními mozkovými tumory. Nejčastěji se jednalo o meningeomy a high-grade gliomy. Medián sledování byl 22,4 měsíců. Pacienti, kteří podstoupili radiochirurgii, měli medián dávky 18 Gy. Pacienti, ozařovaní frakcionovaně, měli medián dávky 25 Gy. Toxicita léčby byla nízká. Stabilizace onemocnění byla zaznamenána u 68 % pacientů, parciální remisi dosáhlo 10 % a jeden pacient měl kompletní remisi. Závěr: Výsledky naší léčby jsou srovnatelné s publikovanými studiemi. Stereotaktické ozařovací metody mají své důležité místo v léčbě primárních tumorů mozku., We analysed retrospectively our treatment of primary brain tumors using stereotactic radiotherapy and radiosurgery. Methods: Patients underwent CT and MR examinations. Target volume and technique were planned in BrainLab system. Patients were irradiated on X knife. Radiosurgery was apllied with fixation using stereotactic frame. Stereotactic mask was used in cases of stereotactic radiotherapy. Statistical processing was performed at Department of mathematics and statistics, Faculty of Science, Masaryk university Brno. Results: 101 paients were included in our study. In the most of cases there were diagnosis of meningeoma and high-grade glioma. Median follow up was 22,4 months. Median of dose of radiosurgery was 18 Gy. Patients, whose underwent stereotactic radiotherapy, were median of doses 25 Gy. Treatment toxicity was low. Stable disease was recorded in 68 % of cases, partial remission achieved in 10 % patients and one patient had the complete remission. Conclusion: Results of our study are comparable with results other publicated studies. - The aim of this study was to determine the prevalence of musculoskeletal problems among Czech dentists and to analyze the factors that affect these disorders. Information was gathered through questionnaire completed by 581 physicians. The questionnaire ascertained general information about physicians including their work habits and the characteristics of their work environment along with the occurrence of musculoskeletal problems as well as their intensity. In the past year the occurrence of at least mild difficulties associated with the locomotive system was reported by 96.9% of dentists surveyed (n = 557), with 66.3% of respondents (n = 381) reporting difficulties ofa moderate or major nature. Most respondents of both sexes indicated neck pain. A statistically significant correlation with the occurrence of musculoskeletal complaints of medium and major intensity was demonstrated for the following factors: sex, age, running a private practice, past injury or musculoskeletal diseases, and the perception of work as psychologically demanding. Musculoskeletal disorders in dentists in Czech Republic are relatively frequent and serious health problem. The causes of these diseases must be identified and appropriate preventive interventions undertaken that will contribute to a reduction in the incidence of these problems. and Z. Sustová, L. - Cardiac surgery is inseparably linked to the activation of innate immunity cells recognizing danger signals of both endogenous and exogenous origin via pattern recognition receptors such as TLR receptors. Therefore, we followed by flow cytometry TLR2 and TLR4 expression on blood monocytes and granulocytes of patients who underwent coronary artery bypass grafting using beating heart surgery (off-pump, n = 34), with use of standard cardiopulmonary bypass (CPB), (on-pump, n = 30), and miniinvasive CPB (mini on-pump, n = 25), respectively, before, during surgery, and up to 7th postoperative day. TLR2 and TLR4 expression both on monocytes and granulocytes was significantly diminished already at the end of CPB being highly significantly decreased at the end of surgery in all patients' groups. TLR2 and TLR4 expression reached preoperative value at the 1st postoperative day being significantly higher at the 3rd postoperative day. Using intracellular staining we found the peak of TLR2 and TLR4 expression inside of monocytes and granulocytes at the first postoperative day in a subgroup of on-pump patients. In conclusion, TLR2 and TLR4 expression is significantly modulated in patients undergoing coronary artery bypass grafting as a part of adaptive homeostatic mechanisms induced by major surgery. The very surgical trauma is responsible for TLR2 and TLR4 modulation. Surprisingly, cardiopulmonary bypass itself was little contributing to the modulation of TLR2 and TLR4 expression. and J. - The pilot study deals with the clinical results of the regenerative therapeutical method in intraosseous periodontal defects done by surgical augmentation procedure using a material of a new generation prepared on the basis of a bioactive glass. A group of 10 individuals were treated properly for chronic periodontitis. Some of treated infraalveolar periodontal lesions, both periodontal pockets and interradicular defects, persisted or recurred in posterior teeth. In order to eliminate them they were indicated for advanced periodontal surgery or regenerative surgical therapy stimulating healing processes of supportive tissues, mostly alveolar bone around treated teeth. Relevant clinical parameters, i. e. values of the pocket depth, gingival recession, and loss of attachment were evaluated before and after the periodontal surgery and compared with each other.
