Alzheimerova choroba (ACH) je nejčastější forma demence. Jedná se o degenerativní, nevyléčitelné, terminální onemocnění. Postihuje až 75 % nemocných na celém světě postižených demencí. Prevalence tohoto onemocnění je vysoká, etiologie stále není známá. Je sledována řada rizikových faktorů. Pilotní studie projektu Epidemiologie a genetika Alzheimerovy choroby s 334 případy a 102 kontrolami z psychiatrických léčeben přináší následující výsledky z dotazníkového šetření a stěru z bukální sliznice. Výsledky analýz vztahu polymorfizmu I/D genu pro ACE k Alzheimerově chorobě odpovídají dosud publikovaným výsledkům, tzn., že genotypy II a ID mají vztah k riziku ACH, i když zatím nebyl zjištěn statisticky významný rozdíl ve sledovaných skupinách respondentů., Alzheimer's disease (AD) is the most common form of dementia. It is a degenerative incurable terminal disease. AD affects a high percentage (75%) of persons with dementia all over the world. Prevalence is very high. Etiology is still unknown. Numerous etiological factors of AD have already been discovered. Pilot evaluation of 334 cases and 102 controls from mental hospitals in project Epidemiology and Genetics of Alzheimer's Disease provided following the results from questionaires and buccal mucosa smears. The questionnaires contain questions about identification, education, lifestyle, vascular risk factors, hobbies, occupation, incidence of the disease in the family, results of Minimental State Examination (MMSE).Results of the analysis of the gene of I/D polymorphism of ACE are presented. Results suggest that persons with AD have often a cardiovascular disease and head injury in their case history. The results of analysis of the relationship between I/D polymorphism of ACE gene and Alzheimer's diseases in accordance with results published until now, even if the statistical difference between AD patients and controls is not statistically significant., Jana Povová, Hana Tomášková, Omar Šerý, Petr Ambroz, Kateřina Vařechová, Vladimír Janout, and Literatura
Prenatální kurzy navštěvuje ale celkově malý počet žen, nejméně ženy s nižšími stupni vzdělání, pro které ale paradoxně kurzy přinášejí vyšší benefit., Healthy lifestyle resulting from well-informed parental knowledge is a condition of both reproductive and general health. Results of current studies show that both pre-conceptual and prenatal unhealthy lifestyles of mothers/parents play an important role in the etiology of different health malfunctions and abnormalities in ontogeny. These malfunctions and abnormalities can be manifested either immediately or with a long latency. The knowledge and habits of women can be significantly influenced by midwives in prenatal courses or, optimally, in a pre-conception advisory capacity. The goal of our work was to ascertain the current situation of pregnanacy and maternity preparedness and associated determining factors. A questionnaire survey and evaluation of chosen anthropometrical parameters were used. The study revealed that existing pregnancy and maternity preparation is insufficient, with associated inadequacies or risks in behaviour. In most cases the results correlated with the educational capacity of mothers and were better in cases where the subjects had prepared for their pregnancy and maternity. Prenatal courses are attended by a relatively low number of women and are least attended by women with lower education levels, for whom such courses would yield greater benefits., Radka Wilhelmová, Drahoslava Hrubá, and Literatura
Retrospektivní dotazníkové šetření, jehož cílem bylo prokázat, že kouření matek a kouření v domácnostech sledovaných dětí má signifikantní vliv na zvýšení celkové nemocnosti, respiračních infekcí a zánětů středního ucha u dětí do tří let věku, probíhalo v Brně a na jižním Slovensku. Zahrnovalo 120 matek (60 z ČR a 60 z SR) tříletých dětí za spoluúčasti příslušných registrujících praktických pediatrů. Ve sledovaném souboru jsme zaznamenali nejnižší celkovou nemocnost i počet respiračních onemocnění v nekuřáckých domácnostech. Nejvyšší celková nemocnost i počet onemocnění byl v rodinách matek kuřaček, kdy v domácnosti nikdo jiný nekouřil. Studie je v souladu se zahraničními i českými studiemi, které popisují zvýšení nemocnosti a respiračních onemocnění v souvislosti s expozicí tabákovému kouři u dětí do tří let věku., The retrospective data collection whose aim was to prove the effects of smoking associated with the increase of overall illness rate, respiratory infections and the inflammation of the middle ear in children up to the age of 3 living with smoking mothers and in smoking households was carried out in Brno, Czech Republic (CR) and in Southern Slovakia (SR). The study involved 120 mothers of 3 year old children (60 from the CR and 60 from the SR) and the participating practicing pediatricians. The lowest overall illness rate and the lowest number of respiratory diseases was found in the no-smoking families within the monitored group. The highest overall illness rate and the highest number of diseases was encountered in the families of smoking mothers with nobody else smoking in the family. The study is in accordance with foreign and Czech studies that describe an increase in the illness rate and respiratory diseases related to exposure to tobacco smoke in children up to 3 years of age., Ildikó Nagy, Martin Sebera, and Literatura
Statisticky významné rozdíly mezi skupinami s a bez SO byly zjištěny u celého souboru pacientek v méně častém výskytu viróz u pacientek se SO (bez viróz 70,3 % vs 48,3 % bez SO). Klíčová slova: karcinom prsu – onkologická léčba – sociální opora – socioekonomický status, The objective of the study was to investigate the relationship between social support, socioeconomic determinants and oncological treatment progress in patients with complications due to the toxicity of antitumor therapy, viral diseases during 6 months following surgery, as well as postoperative complications. The study sample consisted of 139 female patients with breast cancer (age median: 65 years), and social support (SS) and socioeconomic status (SES) were evaluated based on a structured interview. SES was estimated based on income and education classification. The analyses performed included information from patients’ documentation, including data on the occurrence of general therapy complications, postoperative complications, and the occurrence of infectious viral diseases within the first 6 months following surgery. The level of statistical significance of the differences within category-specific variables between the analysed groups was evaluated with Fisher’s exact test. The differences in statistical significance between groups with and without SS were found within the entire group of patients, while viral diseases occurred less in patients with SS (no viral diseases 70.3% vs 48.3% without SS). Patients with SS and below 60 years of age experienced a lower occurrence of postoperative complications (56.7% vs 0%), as well as generally lower numbers of complications resulting from oncological treatment (more than 4 complications occurred in 47.1% of patients with SS vs 100% of patients without SS). Patients with SS in the Stage II–III experienced a lower occurrence of viral diseases (63.2% no virosis vs 28.6%) and a lower occurrence of complications (35.5% more than four complications vs 73.3%). A statistically significant relationship (P = 0.015) was determined between the education of patients and the number of complications in patients in Stage II–III: more than four complications occurred in patients with basic and vocational education (55.9%), compared to patients with secondary and higher education (32.6%). An increasing trend was determined for the relationship between SES and the occurrence of complications due to oncological treatment (P = 0.06) within the group of patients with Stage II–III breast cancer. We have found evidence supporting the idea of a relationship between social support and oncological treatment for older patients across clinical stages and for the relationship between SES and the occurrence of complications for older women in Stage II–III. Social support is a factor which is of great importance for patient health, while higher socioeconomic status and education play a significant role, too.
