Současný stav: Výzkumné studie prokázaly, že nejenom rozdíly v pohlaví mají vliv na zdravotní stav, ale též etnická příslušnost a společenská třída. Imigranti jsou zranitelnou skupinou, která je ve vyšším riziku řady onemocnění ve srovnání s majoritou. Sociální determinanty zdraví jsou významné při hledání příčin sociálních nerovností ve zdraví (Bates et al., 2009; Lindström et al., 2001). Cíl: Hlavním záměrem výzkumu bylo zmapování využití zdravotní péče imigranty. Metodika: Kvantitativní metoda byla realizována pomocí dotazování. Výzkumné šetření probíhalo v období leden až červenec 2012. Bylo kontaktováno 236 imigrantů ve dvou regionech (Jihočeský kraj a hlavní město Praha). Byli osloveni legálně usazení imigranti, ve věkové kategorii 18-65 let, vietnamské, mongolské a ukrajinské národnosti. Výsledky: Čerpání zdravotní péče imigranty bylo ovlivněno typem zdravotního pojištění, délkou pobytu, pohlavím a znalostí českého jazyka. Imigranti, kteří mohli vstoupit do systému veřejného zdravotního pojištění, čerpali zdravotní péči častěji než imigranti s komerčním zdravotním pojištěním. Závěry: Mezi opatření, vedoucí k odstranění sociálních nerovností a zlepšení situace imigrantů v ČR patří zvyšování jazykových znalostí imigrantů a umožnění všem legálně usazeným imigrantům přístup k veřejnému zdravotnímu pojištění., Background: Research shows that health state is influenced not only by gender but also by class and ethnic membership. Immigrants form a vulnerable group in comparison to the majority, more susceptible to a wide range of diseases. Social determinants of health play an important role in search for reasons for social inequality in health (Bates et al., 2009; Limdström et al., 2001). Goal: The main aim of the research was to map use of healthcare by immigrants. Methodology: The quantitative method used were interviews. The survey was carried out in the period from January to July 2012. 246 immigrants were interviewed from two regions (South Bohemian Region and the capital Prague). They were legally settled immigrants in the age group 18-65, of Vietnamese, Mongolian and Ukrainian nationality. Results: The factors influencing use of healthcare by immigrants are the type of health insurance, length of residence, sex and knowledge of Czech language. We studied 52 patients with epilepsy with the average age of 36.2±14.7 years old. Of them, 38 patients had idiopathic epilepsy, 14 patients had symptomatic epilepsy. Our study has shown that epilepsy is accompanied with increased levels of autoantibodies to NF-200, GFAP, S100, MBP, DNA, GABA and dopamine receptors, testifying to the important role of autoimmune disturbances in the pathogenesis of epilepsy. More severe attacks are accompanied by worsening of neuroimmune dysregulation. Objective: To prevent the possible functional complications after gastrectomy, to improve the quality of life and the process of early rehabilitation. Material and Methods: The study included 165 patients with gastric cancer of stage III (T3N12M0), which, depending on the method of recovery divided into 2 groups: Group 1 n = 80 patients who, after an extensive gastrectomy had reservoir formed, II group n = 85 patients who had gastrectomy performed without forming a reservoir. Results: There were no incidence of reflux esophagitis and dumping syndrome observed in the main group, while in the control group, these complications were respectively 61.2% and 25.8%. Weight gain in the main group was observed in 83.7% of patients, in the control group in 4.7%. Assessment of physical activity on the Karnovsky scale in the study group was 85.0%, while 62.0% in the control group. Within 3 months, physical rehabilitation was observed in 90.0% of patients of the main group, while in the control group it was 25.7%. Threeyear survival rate in the main group was 71.4+0.4%, while in the control group, 57.2+0.3% (P> 0.05). Conclusions: Creation of a stomach replacing reservoir after gastrectomy is adequate way in ridding patients from possible complications. Cieľ: Cieľom práce bolo validovať definujúce charakteristiky ošetrovateľskej diagnózy Deficit objemu telových tekutín vybraným súborom slovenských sestier – expertov, teda zistiť, ktoré definujúce charakteristiky sú sestrami povaţované za hlavné a ktoré za vedľajšie. Metodika: Na hodnotenie definujúcich charakteristík ošetrovateľskej diagnózy Deficit objemu telových tekutín bol zvolený Fehringov model validity diagnostického obsahu (DCV model). Definujúce charakteristiky hodnotili sestry, ktoré získali minimálne 4 body podľa modifikovaných Fehringových kritérií. Súbor tvorilo 116 slovenských sestier – expertov. Definujúce charakteristiky boli sestrami hodnotené na Likertovej škále od 1 do 5 (1 – ţiadna významnosť, 2 – malá významnosť, 3 – stredná významnosť, 4 – veľká významnosť, 5 – najväčšia významnosť). Zber údajov prebiehal v období máj aţ október 2010. Výsledky: Za hlavné definujúce charakteristiky bolo súborom slovenských sestier – expertov zo 17 charakteristík označených 6 charakteristík: zníţený turgor koţe, zníţená diuréza, suché sliznice, zníţené napätie jazyka, zníţená venózna náplň, zníţený krvný tlak. Za vedľajšie definujúce charakteristiky boli označené nasledujúce charakteristiky: zvýšená merná hmotnosť moču, suchá pokoţka, zvýšený hematokrit, slabosť, smäd, oslabený pulz, zníţený objem pulzu (nitkovitý pulz), zvýšená pulzová frekvencia, zmeny v psychickom stave, zvýšená telesná teplota. Charakteristika „náhla strata hmotnosti (s výnimkou tretieho priestoru)“ dosiahla váţené skóre menej ako 0,5. Záver: Súbor slovenských sestier za významné znaky pre určenie ošetrovateľskej diagnózy Deficit objemu telových tekutín povaţuje 6 znakov., Aim: The aim of this study is to validate the defining characteristics of a nursing diagnosis of Deficient Fluid Volume by considering a selected sample of Slovak nurse – experts. In other words, to ascertain which defining characteristics are rated by these nurses as major or minor. Methods: Fehring’s Diagnostic Content Validation model (DCV model) was used for validation of this nursing diagnosis. Slovak nurses who met the modified criteria (4 points) recommended by Fehring comprised the sample of experts - a total of 116 Slovak nurses – experts. Defining characteristics were rated by the nurses using Likert’s 5-point scale (1 – not at all characteristic, 2 – very little characteristic, 3 – somewhat characteristic, 4 – considerably characteristic, 5 – very characteristic). Data collection occurred between May and October 2010. Results: Out of 17 defining characteristics of a nursing diagnosis of Deficient Fluid Volume, the Slovak nurses’ sample classified six cues as major: decreased skin turgor, decreased urine output, dry mucous membranes, decreased tongue turgor, decreased venous filling, and decreased blood pressure. The following cues were classified as minor defining characteristics: increased urine concentration, dry skin, elevated hematocrit, weakness, thirst, decreased pulse volume and pressure, increased pulse rate, changes in mental status, increased body temperature. The weighted score of the defining characteristic „sudden weight loss (except in the third space) was less than 0.5. Cíl: Zjistit vyuţívání ošetřovatelských intervencí na chirurgických pracovištích neintenzivní péče a chirurgických jednotkách intenzivní péče. Porovnat uţití intervencí mezi uvedenými typy pracovišť a porovnat získané výsledky se závěry výzkumného týmu NIC. Metodika: Informace byly získány kvantitativní výzkumnou metodou za vyuţití dotazníku vlastní konstrukce. Dotazník byl sestaven s vyuţitím Nursing Intervention Classification (NIC) a obsahoval názvy i definice 101 vybraných intervencí klasifikace NIC. Sestry se prostřednictvím dotazníku u intervencí vyjadřovaly k četnosti uţití v chirurgické klinické ošetřovatelské praxi na jejich pracovišti. Pro induktivní analýzu byl pouţit Pearsonův chí–kvadrát test na hladině významnosti α = 0,05. Výsledky: Šetření se zúčastnilo 11 zdravotnických zařízení, a bylo vyhodnoceno 255 dotazníků vyplněných sestrami, které pracovaly na chirurgických pracovištích neintenzivní péče, a 45 dotazníků vyplněných sestrami, které pracovaly na chirurgických ošetřovatelských jednotkách poskytujících intenzivní péči. Padesát intervencí bylo vyhodnoceno jako stejně často vyuţívané na pracovištích neintenzivní péče i jednotkách intenzivní péče, 49 intervencí jako častěji vyuţívaných na chirurgických jednotkách intenzivní péče a 2 intervence byly vyhodnoceny jako častěji uţívané na chirurgických pracovištích neintenzivní péče. Závěr: V šetření jsme zjistili rozdílné uváděné četnosti uţití intervencí na chirurgických odděleních neintenzivní péče a pracovištích intenzivní péče. Z výsledků je patrné, ţe u nejvyššího počtu testovaných intervencí jsme neshledali rozdíl v četnosti uţití mezi pracovišti intenzivní a neintenzivní péče. V porovnání s výsledky šetření, které uskutečnil výzkumný tým NIC, jsme však zjistili, ţe sestry často vykonávají na chirurgických odděleních neintenzivní péče intervence, které byly výzkumným týmem NIC vyhodnoceny jako více uţívané na jednotkách intenzivní péče., Aim: Determine the frequency use of nursing interventions to non-intensive surgical departments and surgical intensive care unit (ICU). Compare the frequency of use of interventions between these departments and compare the results with the findings of the research team NIC. Methods: Informations obtained quantitative research method using a questionnaire. The questionnaire contained 101 names and definitions of selected nursing interventions classification (NIC). Nurses in these interventions reflect the frequency of their use in surgical clinical nursing practice. For inductive analysis was used Pearson's chi-square test at a significance level of α = 0.05. Results: The survey was attended by 11 hospitalis and were evaluated used 255 questionnaires completed by general nurses who worked at non-intensive surgical departments and 45 questionnaires completed general nurses who worked in intensive surgical departments. It was found 50 interventions that as often used on non-intensive department and intensive department, 49 interventions that are more ofen used in surgical intensive care and 2 interventions that are more ofen used in non-intensive surgical departments. Conclusion: In the survey, we have found different declarations frequency use of interventions between non-intensive surgical department and intensive care units. The results show that the highest number of tested interventions were no differences in frequency of use between the intensive and non-intensive care. Adolescent hallux valgus (HV) is a progressive deformity of adolescent age consisting of metatarsus primus varus and hallux valgus. It has a high recurrence rate after conventional surgical correction. Ten feet in nine patients (two males, seven females) were treated surgically with the Peterson Newman bunion procedure, with a minimum follow-up of one year. During the final follow-up all these patients had no complaints of pain, joint stiffness or limping. Even though the patients had some mild loss of range of movements at the MTP joints 4–6° compared to preoperative value, it did not cause any functional impairment and all were satisfied with the final outcome. The double ostetomy for treatment of hallux valgus is technically precise procedure, provides excellent correction and stability and has low rate of recurrence of deformity. Lidské papilomaviry jsou v centru pozornosti jako původci mnoha benigních i maligních onemocnění ženského genitálu. V posledních desetiletích byl učiněn významný pokrok ve výzkumu v oblasti virové genetiky, etiopatogeneze, prevence a léčby papilomavirových infekcí. Výsledkem jsou informace, které umožnily zavedení nových metod v prevenci a diagnostice cervikálního karcinomu. Cíl: Cílem práce bylo posoudit, jak všeobecné sestry pečují o své zdraví. Zda se zabývají vlastní prevencí, zmapovat výskyt zdravotních problémů, jejich řešení a odhalit postoj všeobecných sester k rizikovým faktorům ohrožujících jejich zdraví. Metodika: Metodou průzkumu bylo dotazníkové šetření provedené v období listopad 2011 - leden 2012 v Bílovecké nemocnici a. s. a v Městské nemocnici v Odrách. Nestandardizovaný dotazník obsahoval otázky zaměřené na prevenci onemocnění, výskyt zdravotních problémů a postoje všeobecných sester k rizikovým faktorům ohrožující jejich zdraví. Výzkumný soubor tvořil 100 všeobecných sester. Výsledky: Všeobecné sestry navštěvují praktického lékaře jen při subjektivních potížích, nedodržují léčebný režim u běžných respiračních onemocnění, dané onemocnění přecházejí, nevystavují si pracovní neschopnost k doléčení, zdravotní problémy konzultují na pracovišti s lékařem nebo si zvolí léčbu sami. Dlouhodobým zdravotním problémem u všeobecných sester je bolestivost pohybového aparátu, pouze čtvrtina pravidelně sportuje a upevňuje si zdraví. Pracovní výkon sester je závislý na kvalitě odpočinku a na celkové fyzické a psychické kondici. Téměř všechny všeobecné sestry se cítí fyzicky i psychicky vyčerpané, polovina sester uvažovala o změně povolání z důvodu vysoké psychické zátěže a nespokojenosti s platem. Zdravotničtí pracovníci vědí, jakým způsobem ohrožují faktory návykové a faktory životního stylu délku a kvalitu života, přesto třetina sester pravidelně kouří a trpí nadváhou. Závěr: Povolání všeobecné sestry je velmi náročné a společensky nedoceněné. Všeobecné sestry by měli využít své kvalifikace a více se angažovat v oblasti prevence, poradenství v otázkách ochrany a upevňování zdraví., Aim: One of objectives of this work is to assess how the nurses take care of their own health and whether the nurses deal with their own prevention. Also, to do the mapping of prevalence of health problems, their solution and to detect the attitude of nurses to the risk factors that may be life threatening and have an impact to their own health. Methods: The survey method supporting this work was done by questionnaire in Bílovec Hospital a.s. and Municipal hospital in Odry in period November 2011 - January 2012. Unstandardized questionnaire included questions regarding the prevention of diseases, incidents of health problems and the attitude of nurses to the threatening factors influencing their health. The experimental group was made of 100 nurses. Results: The nurses attend their doctors only in case of subjective symptoms, they do not follow the treatment regime in case of common respiratory health problems, they often pass the illness as well as they do not want to be issued the sick leave to finish the treatment properly. Often, they discuss their health problems in the work place or they decide to cure themselves. The long-term health disorder among nurses is musculoskeletal pain due to the fact that only quarter of them strengthens their health by doing a sport activity regularly. Work performance of the nurses is dependent on the quality of their relaxation and overall physical and mental condition. Almost all nurses feel physically and mentally exhausted, half of them thought of career change due to the high mental stress and pay dissatisfaction. All healthcare professionals are aware of how the addictive and lifestyle factors threaten their life, yet one third of the nurses smokes regularly and is overweight. Conclusion: The profession of general nurse is very challenging and socially undervalued. The article presents results of peculiarities of the hepatobiliary system in pregnant women with preeclampsia. It is set in the pregnant with preeclampsia of mild severity the frequency and combination of two and more complaints was reliably lower comparing to the frequency and combination of complaints in the pregnant with preeclampsia of mean severity. It must be mentioned that the received laboratory data positively correlated with the clinical manifestations of hepatobiliary dysfunction and the level of severity of preeclampsia that makes the further scientific researches necessary in future.
