This article presents results of using proton therapy after operation of patient with brain meningioma. After operation brain meningiomas is known to not rarely recur due to certain localization and growth of tumor into main cerebral vessels, nerves and other main brain structures. This creates certain difficulties in operative access and surgical intervention, i.e. tumor resection. Práce prezentuje prenatální diagnostiku, možnosti terapie a následnou péči u plodů a novorozenců s kongenitálním chylothoraxem na podkladě zhodnocení 14 případů na naší klinice v letech 1993 až 2012. Povšechný hydrops plodu byl popsán ve 4 případech (29 %), v 10 případech (71 %) byla provedena odlehčovací amniodrenáž a ve 3 případech (21 %) prenatální odlehčovací punkce hrudníku. Spontánně bylo porozeno 11 plodů (78 %), hrudní drenáž byla nezbytná v průměru 9 dnů po porodu. Mortalita dosáhla v souboru 29 % (4 případy). V článku popisujeme vyšetřovací algoritmy, terapeutické prenatální i postnatální postupy a je zdůrazněna nutnost mezioborové spolupráce., The work presents prenatal diagnosis, treatment and care options in fetuses and neonates with congenital chylothorax based on a detailed evaluation of 14 cases recorded in our department between years 1993 and 2012. Generalized fetal hydrops was described in 29% of cases, therapeutic amnio drainage performed in 71% and prenatal thoracentesis in 21% of cases. 78% of fetuses were delivered spontaneously; thoracic drainage was necessary nine days after birth in average. Mortality reached 29%. An eight-month-old male child was admitted with weakness and swelling in the feet. Paraparesis and bilateral lower extremity edema were present in the neurological examination. Thoracic MRI showed an intradural intramedullary mass 61 × 11 mm in size in the T5–T10 levels. Laminotomy between the T5–T10 vertebrae was performed. A mass with smooth borders was separated from most of the neural tissue. In the postoperative MRI, we observed a contrast enhancing area, considered a residual fragment, only 5 × 4 mm in size. Histopathological properties were compatible with the intermixed subtype of ganglioneuroblastoma. Only a limited number of thoracic cord Ganglioneuroblastoma reports have been previously published. Although very rare in children and young adults, ganglioneuroblastoma should be included in the differential diagnosis of thoracic cord tumors. It is difficult to obtain a preoperative diagnosis with clinical features and radiological investigations. Diagnosis depends on histopathological examinations. Curative treatment should be in the form of a complete resection of the tumor. Infantilní hemangiomy jsou nejčastějšími benigními vaskulárními nádory novorozeneckého a kojeneckého věku. Jedná se o nádory s velmi dobrou prognózou. Většina infantilních hemangiomů spontánně zcela regreduje v prvních deseti letech života. Jen deset procent infantilních hemangiomů vyžaduje nějakou terapii. V současné době je preferováno použití b-blokátoru propranololu. Ostatní typy benigních cévních nádorů je zapotřebí diferenciálně diagnosticky odlišit pro zahájení vhodné terapie a určení prognózy. Předložený článek shrnuje základní poznatky o hemangiomech u novorozenců. V závěru článku je zmíněna krátká kazuistika, kdy byla léčba rozsáhlého hemangiomu zahajována po konzultacích na našem pracovišti za kontroly kardiology., Infantile hemangiomas are the most common benign vascular tumors of neonatal and infancy. These are tumors with a good prognosis. Most infantile hemangiomas spontaneously regress completely in the first decade of life. Only ten percent of infantile hemangiomas require some therapy. Currently it is preferred to use b-blocker propranolol. Other types of benign vascular tumors are needed to distinguish the differential diagnosis for initiation of appropriate therapy and prognosis. The present article summarizes the main findings of hemangiomas in infants. Hymenální atrézie - potažmo její důsledky - je vzácnou příčinou bolestí v podbřišku u dospívajících dívek, snadno zaměnitelnou za chirurgickou etiologii obtíží. Gynekologické vyšetření dívek a žen před chirurgickou intervencí je nezbytnou součástí diagnostického algoritmu, v případě atrézie hymen tímto lze v některých případech předejít abdominálnímu operačnímu řešení obtíží. Tato kazuistika demonstruje překvapivý nález v dutině břišní u 15leté dívky s bolestmi břicha, primárně chirurgem operované pro diagnózu „akutní apendicitida“. We report the case of a 52year old man with severe acute pancreatitis. In this case report we discuss the undesirable effects of parenteral nutrition and the importance of nutritional support at patients with severe necrotizing pancreatitis. Severe acute pancreatitis is usually accompanied by systemic inflammatory response syndrome, which results in hypermetabolism with prominent protein catabolism. Providing nutrition to these patients is of paramount importance. An adequate nutritional support is crucial in patients with severe and complicated pancreatitis. Inversion of uterus is very rare and serious complication of childbirth. It is a situation with very high maternal morbidity and mortality, whitch can lead to developement of very serious periparthal bleeding, DIC, neurogenic and haematologic shock. Autoři předkládají kazuistiku „antibiomanie“ u 55leté pacientky s emočně nestabilní poruchou osobnosti a mentální anorexií v anamnéze, nyní dlouhodobě ambulantně léčenou pro úzkostnou a depresivní symptomatologii, související se závažnou somatickou nemocí manžela. Přesmyk do mánie nastal po druhé 500mg dávce klarithromycinu. Nicméně ten byl již třetím podaným antibiotikem v intervalu posledních čtyř měsíců. Manické symptomy dosáhly takové intenzity, že byla nutná nedobrovolná hospitalizace v psychiatrické nemocnici. Zde příznaky odezněly do týdne. Autoři nabízejí hypotézu patogeneze manického zvratu. Předpokládají, že přesmyk mohl nastat primárně v důsledku alterace střevního mikrobiomu antibiotiky. Další patogenetická cesta může vést buď přes přímé vagové dráždění, nebo přes zvýšenou propustnost střevní bariéry pro falešné neurotransmitery., The authors present a case of antibiomania in 55years woman with borderline personality, anorexia nervosa in personal history, and long lasting anxious and depressive symptomatology associated with husband´s serious illness. Switch to mania occured after two 500mg doses of clarithromycin. However, clarithromycin was the third antibiotic administered in the last four months. Manic symptoms were so severe that involuntary hospitalization was required. Signs of mania disappeared within a week on psychiatric ward.
