Politika dlouhodobé péče o starší věkové skupiny chronicky nemocných a nesoběstačných osob je poměrně často ve zjednodušených modelech dávána do souvislosti s budoucími hrozbami pro veřejný sektor a veřejné finance. Navrhovaným „nezbytným“ řešením jsou pak obvykle změny v organizaci a financování dlouhodobé péče v rámci tržně orientovaných reforem, ovšem se zásadními důsledky vzniku nerovností v dostupnosti a ve zdraví, nekoordinovaným procesem poskytování služeb, v nichž dochází k výrazné asymetrii mezi příjemci a poskytovateli služeb. Předmětem práce je tržně orientovaný přístup případu věcného návrhu zákona o dlouhodobé péči v ČR v roce 2011, porovnávaný s rozdílným vývojem diskurzu této oblasti v EU/EC-OECD-WHO a také od vývoje poznání/výzkumu. Cílem práce je kritická reflexe příčin tak odlišného zjednodušujícího vývoje v ČR s využitím metody diskurzivní analýzy. The proposed solution is then formulated in changes of the organization and financing of LTC as a part of market-oriented reforms, without the evaluation of major consequences in terms of inequalities in access and in health. The subject of the work is marketoriented approach of the bill on LTC in the Czech Republic in 2011, in contrast to the discourse of LTC policy options in the EU / EC-OECD-WHO and the development of knowledge / research. The article is focused on the key problems of long-term care provided to disabled seniors above 65 years of their age and disabled children in the Czech Republic from informal carers' point of view. The research is based on 25 semi-structured interviews with informal carers. The results show a serious shortage of (social and health care) services generally as well as for informal carers alone, shortage of useful information and medical devices. In some cases, insufficient coordinative and informative role of general practitioners and paediatricians is also a problem. Narůstající počet závislých osob seniorského věku si vyžaduje adekvátní reakci ze strany sociálně zdravotní péče o tyto osoby. Jde zejména o hledání optimálního nastavení spolupráce mezi formální a neformální péčí, a dále o rozvíjení vhodných forem péče - terénních, ambulantních i pobytových služeb a služeb zdravotních a sociálních tak, aby na sebe navazovaly a dokázaly pružně uspokojovat aktuální potřeby seniorů a jejich rodinných pečujících. Článek se zaměřil na situaci v oblasti sociálně-zdravotních služeb určených pro seniorskou populaci v hlavním městě Praha (HMP) ve srovnání s dalšími re - giony ČR a identifikoval nejvýznamnější neuralgické body současného systému dlouhodobé sociálně-zdravotní péče v HMP. Využívá kvantitativní i kvalitativní metody, na jejichž základě byla podrobně analyzována existující nabídka sociálně-zdravotních služeb v HMP a byla reflektována očima zúčastněných aktérů, tj. uživatelů služeb a jejich rodinných příslušníků, poskytovatelů a zadavatelů sociálních a sociálně zdravotních služeb. Výsledky kvantitativního i kvalitativního výzkumu ukazují na závažné deficity jak v nabídce těchto služeb, tak v jejich vzájemné propojenosti., The increasing number of dependent seniors requires an adequate response from long-term care system for these citizens. This mean a search for setting of cooperation between formal and informal care, and need for appropriate forms of long-term care (field care, outpatient care and residential services) There is an increasing need for health and social services to relate mutually and cooperate flexibly, so that they would appropriately meet the current needs of seniors and their family caregivers. The article is focused on long-term care for the senior population in the capital Prague in comparison with other Czech regions and identifies the most neuralgic points of long- term care in Prague. It uses both qualitative and quantitative methods to analyse in detail supply of long-term care services in Prague from the point of view of main stakeholders. Cílem práce je identifikace příčin rozdílů v tvorbě a realizaci českých veřejných a sociálních politik, zaměřených na problém bezdomovectví v porovnání s přístupy v USA, Austrálii a vybraných zemích EU, včetně samotné EU. Jsou použity metody srovnávací analýzy politických agend, dokumentů a dosahovaných výsledků v kombinaci s kritickou diskurzivní analýzou. Mezi hlavními příčinami dlouhodobě rozdílného přístupu ČR k bezdomovectví figurují izolace aktivit státu od nezávislého výzkumu, chybějící zákon o bezdomovcích, jednostranná tržní orientace transformace společnosti v neoliberálním kontextu a rozpory s lidskými právy. Ke koncepční změně orientace české veřejné a sociální politiky na výzkum příčin bezdomovectví a na cíle integrace bezdomovců a prevence bezdomovectví dochází až v roce 2013 v rámci přímého vlivu strategie Evropa 2020. V porovnání se srovnávanými zeměmi však jde v ČR o časový posun o 20 let. Do této doby byla věnována v ČR pozornost pouze poskytování sociálních a zdravotních služeb bezdomovcům, přičemž v důsledku sociálních a ekonomických změn dochází ke kontinuálnímu zhoršování strukturálních faktorů chudoby a sociálního vyloučení včetně bezdomovectví., The aim of this work is to identify the causes of existing differences in the development and implementation of Czech public and social policy, focusing on the problem of homelessness in comparison with the approaches in the USA, Australia and selected EU countries, including the EU itself. There are used the methods of comparative analysis of political agendas, documents and achieved results of evaluated policies in combination with critical discourse analysis. Cíl: Cílem výzkumu bylo zjistit vztah reminiscenčních aktivit (RA) a depresivity u seniorů žijících v institucionální péči. Sledována byla míra depresivity u seniorů starších 65 let. Byl sledován rozdíl mezi skupinou experimentální a kontrolní po 50 - ti seniorech. Metodika: Pro sběr dat a vymezení depresivity respondentů byl použit screeningový nástroj Škála deprese pro geriatrické pacienty (GDS). Studiem zdravotní dokumentace byla sbírána data, jako je věk, výsledky kognitivního testu MMSE a data z oblasti psychofarmakologické medikace, jelikož je to významný faktor ovlivňující míru depresivity. Hodnocení depresivity proběhlo ve dvou měřeních. U experimentální skupiny proběhla měření před a po absolvování strukturovaných RA. U kontrolní skupiny byla měření provedena ve stejném časovém období, jako ve skupině experimentální, ale již bez RA. Výsledky: Prevalence depresivity v celém souboru respondentů byla 43 %. Depresivita v experimentální skupině byla prokázána u 20 % respondentů. Ve stejném časovém období u skupiny kontrolní byla zjištěna depresivita u 23 % respondentů. Po absolvování RA klesla depresivita u experimentální skupiny o 4 %. Ve zkoumaném souboru bylo statisticky potvrzeno, že RA snižují míru depresivity u seniorů v institucionální péči. Přítomnost depresivní symptomatologie po absolvování RA u experimentální skupiny byla nižší než u skupiny kontrolní. Závěr: Deprese je vážným a život ohrožujícím onemocněním, které se podílí na snížení kvality života seniorů a jejich rodin. Správná diagnostika a léčba deprese vede ke zlepšení kvality života seniorů a jejich rodin. RA jsou významnou psychosociální nefarmakologickou intervencí, která může výrazně přispět ke zlepšení kvality života., Aim: The aim of this research was to determine the relationship between reminiscence activities (RAs) and depression in seniors living in institutional care. The rate of depression was studied in seniors over 65 years of age. Differences between experimental and control groups, each comprising 50 individuals, were assessed. Methods: The Geriatric Depression Scale was used for data collection and identification of depression in the participants. Medical records were used to ascertain their age, Mini Mental State Examination results and psychopharmacological drug use, a significant factor affecting the rate of depression. Depression was evaluated by two measurements. In the experimental group, the measurements were carried out before and after completion of structured RAs. In the control group, the measurements were carried out at the same times but no RAs were provided. Results: The overall prevalence of depression in all participants was 43%. In the experimental and control groups, the rates were 20% and 23%, respectively. After completion of RAs, the rate decreased by 4% in the experimental group. RAs were confirmed to lower the rates of depression in seniors in institutional care. Depressive symptoms were less frequent after RAs in the experimental group than in the controls. Conclusion: Depression is a serious and lifethreatening disease contributing to impaired quality of life of both seniors and their families. Correct diagnosis and treatment of depression lead to improved quality of life of seniors and their families.
