Motivation. Our previous study showed differences in the atherosclerosis phenotype between Lithuanian and Swedish men that could be influenced by complementary factors, namely oxidation processes and/or oxidative stress. The goal of this study was to evaluate the mainstream biological pathways inducing and maintaining the atherosclerotic process by analyzing genetic biomarkers particularly in inflammatory and metabolic pathways where the main focus is laid on the oxidation process. Methods. There were 32 families recruited for the study and clinical as well as biochemical analyses were performed. For genetic analysis 150 SNPs in 89 genes were selected in order to construct a microarray based on Arrayed Primer Extension (APEX) genotyping technology. Genotyping was carried out in 28 families and transmission disequilibrium test (TDT), siblingTDT (STDT), and combined analysis were performed. Results. Clinical and biochemical analysis revealed that probands with premature CAD were more likely to have diabetes mellitus, arterial hypertension, dyslipidemia and were male with high body mass index. Genetic analysis showed six SNPs statistically significantly associated with the atherosclerosis phenotype in the candidate genes ITGA2, IL1B, ALOX5A, OR13G1, MMP9 and NFKB1. These genes belong to different biological pathways: trombocyte adhesion and vessel damage, inflammation response, cholesterol and lypoxygenase metabolic pathway and nutrition. Conclusions. Cíl: Cílem výzkumu bylo zjistit důležitost spirituálních potřeb pacientů v paliativní péči a jejich saturaci na onkologické klinice FN Ostrava. Metodika: Výzkumný soubor tvořilo 93 pacientů, u nichž byla ukončena onkologická léčba s kurativním záměrem a vzhledem k negativní prognóze nemoci jim byla poskytována paliativní péče na onkologické klinice FN Ostrava. Do souboru byli zařazeni pacienti s Karnofského skóre KI ≤ 60 %. Ke sběru dat byly použity otázky z domény spirituální potřeby dotazníku pro měření potřeb PNAP (Patient Needs Assessment in Palliative care) a dotazník EORTC QoLC-30. Pro hodnocení rozdílu v důležitosti a saturaci potřeb dle jednotlivých faktorů byl použit párový t-test a t-test pro dva výběry. Výsledky: V oblasti spirituálních potřeb považovali respondenti nejčastěji za důležité „nenáboženské“ duchovní potřeby a to „být naplněn klidem“ (95 %), „mít smysl života“ (92 %) a „mít pozitivní náhled na život a naději“ (91 %). Pacienti byli nejčastěji nespokojeni s nemožností účasti na bohoslužbě (44 %, 56 % u druhého měření). Spirituální potřeby považovali signifikantně častěji za důležité pacienti starší 65 let a pacienti bez náboženského vyznání (p<0,05). V hodnocení saturace duchovních potřeb nebyl zjištěn statisticky významný rozdíl. Závěr: Identifikace pro pacienta důležitých spirituálních potřeb by se měla stát součástí ošetřovatelské péče., Aim: The aim of the research was to find the importance of the patient´s spiritual needs in the palliative care and their saturation at the Clinic of Oncology at University hospital Ostrava. Methods: The research file consisted of 93 patients whose oncological treatment with curative intention was finished and according to the negative prognosis of illness the palliative care was given to these patients at the Clinic of oncology at University hospital Ostrava. Patients registered in this file were registered the Karnofsky score KI ≤ 60 %. The questions from domain of spiritual needs assessment for measurement of needs PNAP - Patient Needs Assessment in Palliative care were used for the data collection. To evaluate the differences in the importance and saturation according to the needs of individual factors used paired t-test and t-test for two samples. Results: In relation to spiritual needs the respondents found as most important the „no-religion“ spiritual needs and the fact „to be peaceful“ (95%), „to have the meaning of life“ (92%) and to have the positive view of life and hope (91%) most often. The patients were dissatisfied with impossibility of their presence at church service (44%, 56% in case of second measurement) most often. The patients older than 65 years and the non-religious patients (p<0.05) found the spiritual needs as important more often. Statistically the significant difference was not known in the evaluation of the spiritual needs saturation. Cíl: Cílem studie bylo zjistit spokojenost pacientů s poskytovanou ošetřovatelskou péčí na vybraných odděleních dvou nemocnic Moravskoslezského kraje. Porovnat spokojenost pacientů podle zdravotnického zařízení a oddělení, kde byli hospitalizováni. Zjistit, zda existuje souvislost mezi demografi ckými a klinickými charakteristikami pacientů a jejich spokojeností s ošetřovatelskou péčí. Metodika: Zkoumaný soubor deskriptivní korelační studie tvořilo 507 pacientů hospitalizovaných v období od února do března 2011 na 13 lůžkových odděleních v nemocnicích Karviná-Ráj a Orlová. Ke sběru dat byl použit standardizovaný dotazník Patient Satisfaction Scale (PSS), který měří spokojenost pacientů v jedenácti položkách třech oblastí – technické/racionální aspekty, podávání informací a oblast interakce a podpory. Výsledky: Pacienti byli nejvíce spokojeni s technickými aspekty ošetřovatelské péče a nejméně s poskytováním informací. Demografi cké charakteristiky pacientů na jejich spokojenost zásadní vliv neměly. Pouze v jedné nemocnici (NsP Karviná) byli muži ve všech faktorech spokojenější než ženy a naopak nejmenší spokojenost udávali pacienti, kteří popisovali svůj zdravotní stav jako uspokojivý. V NsP Orlová udávali vyšší spokojenost s podporou a interakcí pacienti hospitalizovaní delší dobu. Závěr: Výsledky studie neprokázaly statisticky významné rozdíly v hodnocení spokojenosti pacientů mezi nemocnicemi ani mezi sledovanými odděleními., Aims: The aim of this study was to determine patient satisfaction with nursing care provided in selected wards of two hospitals of Moravian-Silesian region, to determine whether there is a connection between demographic and clinical characteristics of patients and their satisfaction and to compare patient satisfaction according to health facility and department, where they were hospitalized. Methods: The sample of descriptive correlation study consisted of 507 patients hospitalized in the period from February to March 2011 on 13 inpatient wards in hospitals Karvina-Raj and Orlova. Data were collected using a standardized questionnaire Patient Satisfaction Scale (PSS), which measures patient satisfaction in eleven items of three areas – technical/rational aspects, provision of information and area of interaction and support. Results: Patients were most satisfi ed with the technical aspects of nursing care and least with provision of information. Demographic characteristics of the patients did not have a major impact on their satisfaction. Only in one hospital (NsP Karvina) were men in all factors happier than women and on the contrary the smallest satisfaction reported patients who described their health status as satisfactory. In NsP Orlova they reported higher satisfaction with the support and interaction patients hospitalized longer period of time. The aim of the study was to establish the frequency of hypovitaminosis D in children with type 1 diabetes mellitus (T1D), its influence on biochemical and densitometric parameters and the relation to diabetic nephropathy. 58 children with T1D at the age 9–19 years were enrolled to the study. Vitamin D concentration less than 30 ng/ml was considered as insufficient. 37 children (63.79%) had vitamin D level under 30 ng/ml, from these 19 subjects (32.7%) had vitamin D level under 20 ng/ml and 2 subjects (3.44%) under 10 ng/ml. Children with vitamin D deficiency had significantly lower magnesium concentration and lower Z score of lumbar spine (−1.34 ± 1.24 vs. −0.30 ± 1.21, p = 0.01) compared to diabetics with sufficient vitamin D concentration. No significant difference was found in parameters calcium, phosphorus or glycosylated hemoglobin. Patients with diabetic nephropathy (n = 18) showed no significant difference in vitamin D, glycosylated hemoglobin or Z score of lumbar spine compared to the patients without nephropathy (n = 40). Subjects with nephropathy had significantly longer diabetes duration, significantly higher cholesterol and triacylglycerol concentration. Moderní lékařské diagnostické metody umožňují zjištění vážného postižení plodu. Přesto dochází k narozením těžce malformovaných jedinců, neschopných bezprostředně nebo do budoucna uvědoměle využívat faktického stavu své biologické existence. Prezentuje se názor, že u novorozenců s takto vážnými malformacemi by mělo být přípustné upustit od úsilí o záchranu života i při naději na přežití, jsou-li malformace tak závažné, že vylučují do budoucna možnost smysluplné a uvědomělé lidské existence. Mělo by však jít o mimořádnou situaci, které je třeba předcházet především zabezpečením takové úrovně prenatální péče a zdravotnické výchovy, aby se prakticky možnost donošení a porodu těžce malformovaného jedince vyloučila., Modern medical diagnostic methods allow the detection of serious birth defects. Yet there are born severely malformed individuals who are unable (immediately or in the future) to use consciously actual state of their biological existence. There is opinion that it should be possible to abandon the effort to save lives (even when there is a chance of survival) when the malformations of the neonates are so severe that they exclude the future possibility for meaningful and conscious human existence. Pro klasifikaci prekanceróz vulvy se používá termín vulvární intraepiteliální neoplázie (VIN). Histologicky lze rozlišit dva základní typy VIN: VIN obvyklého typu (u-VIN) a VIN diferencovaného typu (d-VIN). Rozsah a intenzita dysplastických změn se v případě u-VIN vyjadřuje třístupňovým gradingem (u-VIN I, II a III). Prekancerózní potenciál d-VIN odpovídá vždy karcinomu in situ, proto se u této diagnostické jednotky grading neprovádí. Etiologicky je u-VIN spojena s infekcí vysoce rizikovými typy lidského papillomaviru (HPV), zatímco d-VIN je HPV negativní a rozvíjí se v terénu chronických vulvárních dermatóz typu lichen sclerosus a lichen simplex chronicus. u-VIN má tendenci k multifokalitě a k dlouhodobému recidivujícímu průběhu před transformací v invazivní dlaždicobuněčný karcinom. d-VIN se naopak vyskytuje spíše solitárně a má sklon k rychlé stromální invazi. u-VIN typicky postihuje ženy premenopauzálního věku s vyšší incidencí ostatních prekancerózních lézí dolního ženského pohlavního systému včetně perinea a anu. Pacientky s d-VIN bývají postmenopauzální ženy a není u nich pozorována asociace s výše zmíněnými dyspláziemi ostatních anatomických lokalizací. Nízká diagnostická reproducibilita kategorie u-VIN I, pochyby o prekancerózním potenciálu u-VIN I a problematické rozlišení u-VIN II a u-VIN III vedly k revizi současného diagnostického schématu. Podstatou změn je zrušení gradingu u-VIN se sloučením kategorií u-VIN II a u-VIN III a dále zrušení u-VIN I jako diagnostické jednotky. V revidované klasifikaci prekanceróz vulvy proto termín u-VIN reprezentuje high grade dysplastické léze asociované s HPV (dříve u-VIN II a u-VIN III) a d-VIN i nadále zůstává označením pro HPV negativní high grade prekancerózu., Classification of vulvar precancerous lesions is based on the concept of vulvar intraepithelial neoplasia (VIN). Two basic types of VIN are recognized histologically: VIN of the usual type (u-VIN) and VIN of the differentiated type (d-VIN). The three grade evaluation system is used to assess the intesity of dysplastic changes in u-VIN (u-VIN I, II and III). The precancerous potential of d-VIN corresponds to carcinoma in situ and therefore grading is not applied. u-VIN is etiologically linked to HPV infection, whereas d-VIN is HPV negative and emerges in the environment of chronic vulvar dermatoses (lichen sclerosus and lichen simplex chronicus). u-VIN has a tendency to multifocality and prolonged recurrent progression to the invasive squamous cell carcinoma. On the other hand, d-VIN represents rather a solitary lesion with a propensity to the rapid stromal invasion. u-VIN usually affects premenopausal women with a higher incidence of other precancerous lesions of the lower female genital tract including the perineum and anal area. Patients diagnosed with d-VIN are of postmenopausal age without any association with aforementioned dysplastic lesions of other anatomic locations. The low diagnostic reproducibility of the u-VIN I category, doubts about the precancerous potential of u-VIN I and the problematic distinction between u-VIN II and u-VIN III resulted in the modification of the current terminology. The grading of u-VIN was abandoned, u-VIN II and u-VIN III categories were merged and u-VIN I was removed from the classification scheme. The revised u-VIN category therefore represents high grade dysplastic lesions associated with HPV infection (former u-VIN II and u-VIN III categories) and the term d-VIN is still reserved for the HPV negative high grade vulvar precancerosis., and P. Studie se snaţí zodpovědět otázku, jak vnímají, proţívají a hodnotí svou kvalitu ţivota ţeny v období mateřství. Nepouţívá však běţný dotazník, jehoţ znění je stejné pro všechny ţeny a je tedy zaměřen na neexistující „průměrnou ţenu“. Naopak vyuţívá individualizovaného přístupu, kdy ţena můţe sama nadefinovat a poté zhodnotit ty oblasti ţivota a ţivotní cíle, na nichţ právě jí záleţí. Cíl: 1. vyzkoušet metodu SEIQoL (Schedule for the Evaluation of Individual Quality of Life) v mateřství, 2. identifikovat spektrum jejich ţivotních cílů a míru jejich naplňování, 3. stanovit kvalitu jejich ţivota, 4. zjistit vztah mezi uváděnou kvalitou ţivota a volbou ţivotních cílů. Metodika: Metoda SEIQoL v úpravě J. Křivohlavého (2001), statistická analýza dat pomocí programu NCSS 8. Výsledky: U souboru 43 ţen respondentky uvedly celkem 16 ţivotních cílů. Subjektivně udávaná celková kvalita ţivota je poněkud vyšší (81,4), neţ kvalita vypočítaná u dílčích cílů (76,2). Obě stupnice spolu středně korelují (r = 0,513), rozdíl je na hranici statistické významnosti (p = 0,0862). Dále se u obou porovnávaly roz díly mezi vzděláním ţen, počtem dětí a vedením porodu. Závěr: V našem zkoumaném souboru byly ţeny v období mateřství s kvalitou ţivota spokojené. Nejfrekventovanější ţivotní cíle pro ně byly rodina, zdraví, zaměstnání, přátelé a finanční zajištění. Zajímavé zjištění bylo, ţe nebyly shledány statisticky významné rozdíly v kvalitě ţivota dle počtu dětí, vzdělání ţen a vedením porodu., The study is trying to answer the question of how women in their motherhood perceive, experience and evaluate their quality of life. Unlike a standard questionnaire which is the same for all women, an individualized approach was used, with women identifying and then evaluating their own most important areas and goals of life. Aim: (1) to test the Schedule for the Evaluation of Individual Quality of Life (SEIQoL) instrument in motherhood, (2) to identify the spectrum of their life goals and degree of their realization, (3) to determine the quality of their life, and (4) to determine the relationship between the stated quality of life and choice of life goals. Methods: The SEIQoL instrument adapted by Křivohlavý (2001); statistical analysis of data using the NCSS 8 software. Results: A group of 43 respondents stated a total of 16 life goals. The overall subjective quality of life is slightly higher (81.4) than the quality calculated from sub-goals (76.2). Both scales are moderately correlated (r = 0.513), the difference is on the edge of statistical significance (p = 0.0862). Differences in women’s education, number of children and management of delivery were also compared. Conclusion: In our sample, women were satisfied with the quality of life in their motherhood. The most frequent life goals were family, health, job, friends and financial security.
