A hybrid approach to elephant trunk technique for treatment of thoracic aortic aneurysms combines a conventional surgical and endovascular therapy. Compared to surgery alone, there is a presumption that mortality and morbidity is reduced. We present a case report of a 42-year-old man with a giant aneurysm of the entire thoracic aorta, significant aortic and tricuspid regurgitation and ventricular septum defect. Hereditární hemoragická teleangiektázie neboli morbus Rendu‑Osler‑Weber je fibrovaskulární dysplazie s autozomálně dominantní dědičností charakteristická přítomností teleangiektázií a arteriovenózních malformací kůže, sliznic a vnitřních orgánů. Plicní arteriovenózní malformace jako zdroj septických embolů mohou zapříčinit vznik mozkového abscesu, který tak bývá prvním a často jediným projevem heriditární hemoragické teleangiektázie. V práci jsou prezentovány tři kazuistiky pacientů trpících touto chorobou, kde prvním a závažným příznakem byl vzniklý mozkový absces. Protože se jedná o relativně vzácné onemocnění, nebývá vždy zvažováno v rámci diferenciální diagnostiky a tím pádem ani diagnostikováno. U každého pacienta s mozkovým abscesem je nezbytné na toto onemocnění pomýšlet a v případě podezření je nutné jej řádně vyšetřit. Při verifikované plicní arteriovenózní malformaci je třeba ji ošetřit a zabránit tak vzniku recidivujících mozkových abscesů. Péče o tyto pacienty je komplexní a multioborová., Hereditary hemorrhagic telangiectasia or Rendu‑Osler‑Weber disease is an autosomal‑dominant disorder that results in fibrovascular dysplasia and is characterized by telangiectases and arteriovenous malformation of the skin, mucosa and viscera. Pulmonary arteriovenous malformations as a source of septic emboli may give rise to brain abscess that so often is the first and only clinical manifestation of hereditary hemorrhagic telangiectasia. Three cases of patients suffering from this disease with brain abscess as the first and serious symptom are presented. Since this is a relatively rare disease, it is not always considered in the differential diagnosis and thus not diagnosed. This disease has to be considered in patients with brain abscess and, in case of a suspicion, it must be properly examined. When pulmonary arteriovenous malformation is verified, it has to be treated in order to prevent recurrent brain abscesses. Care of these patients should be comprehensive and multidisciplinary. Dyskinetické extrapyramidové syndromy mohou vzácně vést k akutnímu zhoršení do život ohrožujícího stavu, který si vyžádá hospitalizaci na jednotce intenzivní péče, hlubokou analgosedaci a umělou plicní ventilaci. V případě neúspěšnosti symptomatické farmakologické léčby lze uvažovat o stereotaktickém neurochirurgickém výkonu (pallidotomii nebo hluboké mozkové stimulaci). V příspěvku uvádíme naše dosavadní zkušenosti se třemi pacienty, kteří v akutní fázi dyskinetického syndromu takový výkon podstoupili. U dvou dětských pacientů (12 a 6 let) pro status dystonicus s geneticky verifikovanou hereditární generalizovanou dystonií (DYT‑6 a DYT‑1) byla provedena hluboká mozková stimulace vnitřního pallida a u jedné pacientky (21 let) s Huntingtonovou nemocí byla provedena pallidotomie pro akutní zhoršení generalizované chorey s rozvojem balizmu. Přes počáteční kritický stav došlo u všech tří pacientů v odstupu 1–6 týdnů od výkonu k významnému potlačení mimovolných pohybů. Stereotaktická neurochirurgická léčba by proto měla být zvažována jako metoda volby v terapii akutních, život ohrožujících hyperkinetických stavů, kde farmakologická terapie selhala., Dyskinetic movement disorders may rarely progress to an acute life-threatening condition requiring admission to an intensive care unit, deep sedation and mechanical ventilation. Stereotactic neurosurgery (pallidotomy or deep brain stimulation) should be considered in all patients who did not respond to pharmacological treatment. We report three patients who underwent stereotactic neurosurgery due to acute dyskinetic syndrome. Two boys (12 and 6 years) with generalized hereditary dystonia (DYT-6 and DYT-1) developed status dystonicus and were treated with deep brain stimulation of the globus pallidus interna. The third patient was a 21-years-old woman with Huntington’s disease who underwent a pallidotomy due to acute worsening of generalized chorea with ballistic movements. Despite the initial critical state of all three patients, their abnormal movements significantly subsided within six weeks after the neurosurgery. The stereotactic neurosurgical procedure should be considered as the method of choice in patients with acute life-threatening hyperkinetic movement disorders that failed to improve with pharmacological treatment. Cíl: Cílem studie je představit dlouhodobé výsledky chirurgické léčby supratentoriálních kavernózních malformací mozku na Neurochirurgické klinice 1. LF UK a ÚVN‑VFN. <p align="">Soubor a metodika: Retrospektivně jsme do studie zařadili pacienty se supratentoriálním, kortiko‑subkortikálně uloženým kavernomem léčené v letech 2000–2012. Hodnotili jsme epidemiologická data pacientů, radiologický nález, klinický obraz onemocnění a operační výsledky včetně komplikací. <p align="">Výsledky: Iniciálním příznakem byl epileptický záchvat (u 49 % kavernomů), cefalea (22 %), fokální neurologický deficit (19 %) a 15 % bylo náhodně nalezených. Známky recentního krvácení na MR byly přítomny u 27 % pacientů. Operovali jsme celkem 145 pacientů s 158 kavernomy. Dvacet pět lézí bylo léčeno konzervativně. K chirurgické komplikaci došlo v 8 %. U dvou pacientů (1,4 %) se trvale zhoršil stav v přímé souvislosti s operací. Pooperační výskyt epileptického záchvatu byl signifikantně vyšší ve skupině s rannou infekcí či pooperačním hematomem (p < 0,05). <p align="">Závěr: Mikrochirurgická resekce hemisferálního kavernomu je relativně bezpečná operace s minimální morbiditou a mortalitou. Pooperační hematom nebo ranná infekce může mít epileptogenní potenciál., Aim: The aim of the study is to present surgical outcome of treatment of supratentorial cavernous malformation of the brain at the Department of Neurosurgery, Charles University and the Central Military Hospital in Prague. <p align="">Material and methods: We retrospectively enrolled patients diagnosed between 2000 and 2012 with supratentorial, cortico‑subcortically located cavernoma. We analysed epidemiological and radiological data, clinical presentation and surgical results including complications. <p align="">Results: Initial symptoms included epileptic seizure (49%), headache (22%) and focal neurological deficit (19%); 15% of cavernomas were found incidentally. Radiological signs of recent haemorrhage on MR scans were found in 27% patients. We performed surgery in 145 patients with 158 cavernous malformations. Twenty five lesions were treated conservatively. Surgical complications occurred in 8% of patients. One patient died and one had permanent neurological deficit attributable to surgery. Postoperative seizure rate was significantly higher in a group with wound infection or postoperative hematoma (p < 0.05). <p align="">Conclusion: Microsurgical resection of lobar cavernoma is relatively safe procedure with minimal morbidity and mortality. Prezentujeme případ mladé ženy, jež prodělala v dospělém věku operaci vrozené cysty pravého hepatiku, kterou vzhledem k těsnému kontaktu s velkými cévami nebylo možné kompletně resekovat. Byla provedena parciální resekce a cysto-jejuno-anastomóza (CJA) kdrenáži žluči. Pacientka byla 4 roky bez obtíží. Posléze se dostavily bolesti břicha, subikterus a laboratorní známky cholestázy Kvůli těmto potížím a opakovaným cholangoitidám prodělala řadu vyšetření a intervencí, které měly zajistit ozřejmění příčiny obstrukce a obnovení odtoku žluči. Použité zobrazovací metody neprokázaly jednoznačnou příčinu obtíží, proto bylo přistoupeno k operační revizi, při níž byl peroperační ultrasonografií prokázán tumor v oblasti CJA. Nález byl vzhledem k lokalizaci inoperabilní. Pacientka měla následně zjištěnu diseminaci malignity v játrech i v uzlinách a zemřela 5 měsíců od zjištění maligní diagnózy na hepatorenální selhání. Tento poměrně raritní případ dokládá zvýšené riziko maligního zvratu vrozených malformací žlučových cest především v případě jejich diagnózy a léčby v dospělém věku. V dostupné literatuře existuje jen málo dokumentovaných případů metachronního karcinomu po resekci žlučových cest., We present a case of a young woman, who underwent surgery for congenital bile duct cyst in adulthood. Due to location of the cysts it was impossible to completely remove it. The cyst was partially resected and it was performed cyst-jejunal anastomosis (CJA) to drain of bile. After operation the patient was 4 years without any problems. After this interval appeared pain in the right upper quadrant and subicterus with laboratory evidence of cholestasis. The patient suffered from recurrent cholangitis due to which a number of diagnostic tests and interventions were performed. Used imaging metods did not demonstrate an unequivocal cause of problems, therefore subsequently surgical revision was performed and the in-traoperative ultrasonography demonstrated tumor in anastomosis. The finding was inoperable due to inappropriate location. In the following period liver and lymphnode metastases appeared and patient died five months after diagnosis of malignancy. This relatively rare case illustrates an increased risk of malignant transformation of congenital malformations of the biliary tract. Cysta choledochu je vzácné onemocnění, výrazně vyšší výskyt zaznamenáváme u asijské populace. Etiologie není zcela známa, avšak uvažuje se o spojení s anomáliemi v anatomii žlučových cest. Přestože je svojí povahou benigní, jedná se o prekancerózu s nebezpečím zvratu v karcinom žlučových cest. Jediným kurativním řešením je radikální chirurgické odstranění s rekonstrukcí žlučových cest. Autoři prezentují kazuistiku nemocné s cystou ductus hepatocholedochus typu I dle Todaniho klasifikace., Choledochal cyst is a rare disease with a considerably higher incidence found in the Asian population. Although its etiology is not completely known, the disease is believed to be associated with anomalies in the anatomy of the biliary tract. While being a benign unit, it is considered as a precancerosis with the risk of conversion to the biliary tract carcinoma. Denosumab is a human monoclonal antibody representing a novel therapy of osteoporosis. Contrary to always other antiosteoporotic drugs, it is not contraindicated in advanced chronic kidney disease, as its pharmacokinetic does not differ from patients with normal kidney function. However, published case reports in chronic kidney disease (CKD) patients stopped the therapy after single dose because of hypocalcemia. Cílem práce je ukázat v průběhu 18letého sledování kognitivní, emoční a behaviorální změny u pacientky s roztroušenou sklerózou mozkomíšní (RS). Předkládaná kazuistika dokumentuje maladaptaci na diagnózu RS a dlouhodobé maladaptivní zvládání, prolínání a vliv afektivních složek na inhibici kognitivních schopností a psychického stavu, vliv stresu na vznik „pseudoatak“ RS, psychosociální problematiku a kvalitu života. Diagnóza RS měla negativní vliv na pacientku a rodinu. Nepředvídatelný průběh ovlivňoval potíže v adaptaci a v plánování podpory. Vliv stresu souvisejícího s onemocněním a zároveň vedlejší negativní efekt léčby vedli k mnoha psychosociálním potížím, které snížily kvalitu života pacientky a měly negativní vliv na její spolupráci při léčbě a rehabilitaci. Součástí komprehensivní diagnostiky i léčby RS byla opakovaná neuropsychologická vyšetření a v závěru sledování, ve fázi chronicko‑progresivní RS, i kognitivně‑behaviorální terapie cílená na snížení zátěže způsobené chronicitou nemoci a zlepšení kvality života., The aim of the paper is to show cognitive, emotional and behavioural changes in a multiple sclerosis (MS) patient over eighteen years of observation. The diagnosis of MS was devastating for the patient herself and her family. Its unpredictable course complicated adjustment and created difficulties in planning for support. Effects of disease-related stress as well as a negative side-effect of treatment led to many psychosocial problems that impaired the patient’s quality of life and affected compliance with treatment and rehabilitation. Repeated neuropsychological assessment was a part of a comprehensive diagnostic process and treatment of MS and, at the end of observation, in chronic-progressive phase of MS, was complemented with cognitive-behavioural therapy aimed at reducing burden of disease and improving the quality of life., and A.
