Základní ideou dekompresivní kraniektomie (DK) je chirurgické řešení konfliktu vzrůstajícího nitrolebečního obsahu s pevným objemem kostní schránky. Zvětšený nitrolebeční obsah je většinou představován edémem mozku při nejrůznějších patologických stavech. Přestože DK zažívá v posledních letech renesanci a objevily se důkazy o její účinnosti, její použití není dosud paušálně akceptováno. Nejčastější indikací DK je kraniocerebrální poranění. Důkazy o její efektivitě jsou však limitované. Jsou očekávána data o účinnosti sekundární DK u nemocných s refrakterní nitrolebeční hypertenzí po selhání konzervativní léčby. Provedení bifrontální sekundární dekomprese ještě před vyčerpáním konzervativní léčby nepřináší nemocným s difuzním poraněním benefit. Postavení primární DK u akutního subdurálního hematomu je testováno v randomizované studii. Nejsilnější důkazy o efektivitě DK jsou u nemocných s maligním hemisferálním infarktem, přesto frekvence operace stále neodpovídá incidenci maligního hemisferálního infarktu. DK by měla být zvážena u nemocných s trombózou mozkových splavů a refrakterní nitrolebeční hypertenzí, kteří mohou z operace profitovat i při oboustranné mydriáze. DK s evakuací nebo bez evakuace hematomu může snížit mortalitu nemocných s expanzivním supratentoriálním intracerebrálním krvácením, kteří jsou v bezvědomí nebo mají refrakterní nitrolebeční hypertenzi. Dekomprese může přinést benefit selektované podskupině nemocných se subarachnoidálním krvácením, její použití však není v současných doporučeních specifikováno., The fundamental idea behind decompressive craniectomy (DC) is a surgical solution to the conflict of an increasing intracranial content within the fixed-volume intracranial cavity. The expanding intracranial content is caused by brain edema caused by a variety of pathologies. In spite of DC renaissance over the recent years and some evidence of its effectiveness, its use is not yet generally accepted. Traumatic brain injury is by far the most frequent indication for DC. However, the evidence of its effectiveness is limited. Data on the role of secondary decompression in patients with refractory intracranial hypertension after conservative treatment failure are expected to be released soon. Bifrontal early DC is not superior to medical management in patients with diffuse traumatic injury. Primary decompression in patients with acute subdural haematoma has been investigated in a randomized trial. The most conclusive evidence is in patients with malignant middle cerebral artery infarction. In spite of this, the surgery is still underutilized and the frequency of its use does not correspond with the incidence of malignant infarction. When decompression is performed within 48 hours of stroke onset in patients younger than 60 years, it reduces mortality and improves functional outcome. DC is also a lifesaving procedure in patients over 60 years of age for whom it improves chances of survival without total dependency. Decompression should be considered in patients with cerebral venous thrombosis that causes intractable intracranial hypertension. Furthermore, patients with bilateral mydriasis can also profit from the procedure. DC with or without hematoma evacuation might reduce mortality in patients with large supratentorial intracerebral hemorrhage who are in coma or have refractory intracranial hypertension. Even though the DC can be effective in selected subgroups of patients with subarachnoid hemorrhage, current guidelines, do not specify the role of decompression in these patients. Infekce kůže a měkkých tkání (IKTM) zahrnují široké spektrum zánětlivých onemocnění postihujících kůži, podkoží, fascie a svaly. V posledním období pozorujeme nárůst množství pacientů ošetřených pro tyto diagnózy na chirurgických ambulancích, ale zvyšují se i počty pacientů hospitalizovaných v nemocnici se závažnými formami IKMT. Smrtnost nejzávažnějších forem je v literatuře uváděna až 90 %. Příčinou je podcenění počáteční symptomatologie a z ní pramenící pozdní zahájení radikální léčby až ve fázi septického šoku. Text tvoří mezioborový přehled základních principů diagnostiky a léčby IKTM, který odráží současné poznatky opřené o medicínu důkazů., Skin and Soft Tissue Infections (SSTIs) encompass a wide spectrum of inflammatory diseases of the skin, subcutis, fascia and muscles. Recent national trends indicate increasing rates of patients treated for these diagnoses at outpatient surgical department, but also of patients hospitalized with severe forms of SSTIs. Up to 90% fatality rates are reported in the literature for the most severe forms, resulting from underestimation of initial symptoms and therefore delayed onset of radical therapy only the phase of septic shock. Úvod: Inhibitory protonové pumpy (PPI) jsou léky často užívané pro široké spektrum onemocnění horního trávicího traktu. U mnoha pacientů je léčba potřebná chronicky. Ačkoli je dlouhodobé podávání považováno za bezpečné, je v posledních letech věnována široká pozornost možnému zvýšenému výskytu fraktur u uživatelů inhibitorů protonové pumpy, což se promítá i do informací poskytovaných v příbalové dokumentaci léčiv. Cílem práce je poskytnutí přehledu o aktuálních poznatcích v této oblasti a analýza podkladů, na jejichž základě je informace o zvýšeném riziku založena. Výsledky: Základní informace o zvýšeném riziku fraktur u osob užívajících inhibitory protonové pumpy pochází z retrospektivních epidemiologických studií. Dalším zdrojem jsou studie, u nichž byl jako vedlejší cíl sledován vliv inhibitorů protonové pumpy na efekt léčby osteoporózy pomocí bisfosfonátů. Přímá pozorování efektu inhibitorů protonové pumpy na vstřebávání vápníku a vlivu na kosti při hodnocení denzitometrií jsou ojedinělá. Závěr: Jednoznačné určení rizika ovlivnění kostí dle dosavadních pozorování není možné, zvláště při významné polymorbiditě a polypragmazii řady uživatelů inhibitorů protonové pumpy. Žádná práce dosud nehodnotila výskyt rizikových událostí v závislosti na indikaci pro dlouhodobou léčbu. Rutinní sledování kostních parametrů při léčbě PPI není doporučeno. Základem bezpečnosti je správná indikace a omezení nadbytečné preskripce léků., Introduction: The proton pump inhibitors are commonly used drugs for the treatment of the digestive disorders. Many patients require long-term maintenance therapy. The prolonged acid inhibition is considered to be safe, but recently, the attention has been paid to the possible effects on the bone metabolism and the higher incidence of the fractures in patients using the PPIs. The aim of this work is to give a complex overview on the topic and analyse the source of information about the increased risk of the fractures. Results: Several epidemiological studies describe the incidence of the fractures in the patients with PPIs. Further articles, mainly describing the effect of the bisphosphonate therapy for the osteoporosis, also track the PPI effects. The studies dealing with the articles on densitometry targeting on the effect of the PPIs on the bone metabolism are rare. Conclusion: It is not possible to unequivocally determinate the risks of the long term PPI therapy on bone based on the existing studies due to the heterogenous populations, multimorbidity of the patients and the concurrent medication. Untill now, no study has evaluated the incidence of the risk events based on the indication of the long-term therapy. Routine monitoring of the bone parameters during the PPI therapy is not recommended. OBJECTIVE: The aim of the present study was to investigate the effect of allergic rhinitis on the success of the operation in chronic otitis surgery by using score for allergic rhinitis (SFAR). MATERIALS AND METHODS: In the present study; 121 patients, who underwent type 1 tympanoplasty were examined retrospectively. SFAR of all patients were recorded. The graft success rates of 26 patients with allergic rhinitis (AR) and 95 patients with no allergic rhinitis group (NAR) were compared. RESULTS: While the graft success rate in NAR group was 89.5%, this rate was 80.8% in the AR group. However, the difference between groups was not statistically significant (p = 0.311). CONCLUSION: These findings suggest that allergic rhinitis decreases the graft success rate of the pathologies occurring in eustachian tube, middle ear and mastoid although statistically significant difference wasn't found. Poúrazový hydrocefalus způsobený poruchami cirkulace mozkomíšního moku často komplikuje klinický průběh a proces léčby po kraniocerebrálním poranění. Hydrocefalus po poranění míchy je popisován pouze výjimečně. Sdělení popisuje dvě kazuistiky nemocných s kompletní lézí krční míchy, u kterých v poúrazovém období došlo k rozvoji hydrocefalu. Rozbor obou případů a přehled literárních dat potvrzují rozhodující úlohu extraspinálních faktorů pro rozvoj hydrocefalu po míšním poranění. I když k rozvoji hydrocefalu po míšním poranění dochází výjimečně, tato možnost by měla být zvážena při pozdním zhoršení klinického stavu nemocného po poranění krční míchy, zvláště při přítomnosti anatomických anomálií mokového prostoru a poúrazovém subarachnoidálním krvácení., Posttraumatic hydrocephalus caused by cerebrospinal fluid circulation disturbances frequently complicates the clinical course and treatment after craniocerebral injury. Hydrocephalus complicating spinal cord injury is only exceptionally reported. The paper presents two cases of complete cervical spinal cord injury with subsequent development of hydrocephalus. The analysis of both cases and literature data confirmed the dominant role of non-spinal factors in the development of hydrocephalus after spinal cord injury. Despite the exceptional occurrence of hydrocephalus after spinal cord injury, this diagnosis should be considered in cases of delayed deterioration of a patient with cervical spinal cord injury, particularly if cerebrospinal fluid space abnormalities and posttraumatic subarachnoid haemorrhage are present., and J. Rozměry kónusu – délku, tloušťku a obvod – je třeba uvádět podle doporučení Mezinárodní federace cervikální patologie a kolposkopie z roku 2011. V klinickém souboru 27 konizací pro carcinoma in situ byla hodnocena velikost kónusu, fragmentace vzorku, výskyt termických artefaktů, zánětlivých změn a dosah změn do okrajů. Jsou diskutovány požadavky na adekvátní provedení výkonu. Velikost kónusu je vždy individuální a je nutno ji řídit podle typu transformační zóny, velikosti a tvaru léze, velikosti čípku, histologického typu léze a eventuálně reprodukčních plánů., Dimensions of cone – length, thickness and circumference – have to be presented according recommendations of International Federation for Cervical Pathology and Colposcopy from the year 2011. In a clinical series of 27 conizations due to carcinoma in situ the size of cone, fragmentation of the specimen, thermic artefacts, inflammatory changes and range to margins were evaluated. Requirements for adequate performing of the procedure are discussed. Cíl: Posoudit, zda a jak se liší hodnocení nálezů degenerativních změn bederní páteře na magnetické rezonanci (MR) mezi zkušenými radiology, radiologickými rezidenty a lékaři jiných odborností zabývajícími se touto problematikou. Metodika: Devět hodnotitelů ze tří skupin: zkušení radiologové (ZR), radiologičtí rezidenti (RR) a neradiologové (NR) posoudilo nezávisle MR vyšetření bederní páteře u 55 pacientů. Hodnotitelé posuzovali ve všech segmentech bederní páteře přítomnost a stupeň postižení sedmi parametrů: stav ploténky, protruze či hernie ploténky změny signálu obratlových těl, posun obratlů, dorzální osteofyty artrotické změny na intervertebrálních kloubech a šíři ligamenta flava. Statistické hodnocení míry shody mezi jednotlivými hodnotiteli MR nálezů bylo provedeno na jednotlivých segmentech bederní páteře pomocí Fleissova kappa; pro srovnání hodnocení stupně závažnosti MR nálezu byl použit Friedmanův test a mnohonásobné párové porovnání Nemenyiho post hoc testem. Výsledky: Míra shody v celkovém hodnocení MR nálezu na jednotlivých segmentech bederní páteře se pohybovala v rozmezí hodnot kappa od 0,32-0,80, nejčastěji v rozmezí 0,5-0,7, byla tedy průměrná až nadprůměrná. Při statistickém testování shody v hodnocení stupně závažnosti MR nálezu jsme mezi jednotlivými hodnotiteli prokázali statisticky signifikantní rozdíl (p < 0, 0001). V každé skupině hodnotitelů (ZR, RR, NR) byl vždy jeden, který se v hodnocení "stupně závažnosti" MR nálezu statisticky významně lišil od ostatních dvou. Rozdíly byly nejmenší mezi zkušenými radiology. Závěr: Prokázali jsme statisticky signifikantní rozdíly mezi hodnoceními MR nálezů mezi jednotlivými lékaři i uvnitř jednotlivých skupin. Mezi zkušenými radiology byly tyto rozdíly nejmenší., Aim: To assess differences in the evaluation of degenerative changes of the lumbar spine MRI among experienced radiologists, radiology residents and other specialists. Method: Nine evaluators from three groups: experienced radiologists (ER), radiology residents (RR) and non-radiologists (NR) independently assessed MRI examinations of the lumbar spine of 55 patients. All segments of the lumbar spine were evaluated for presence and degree of severity of changes in seven parameters: disc height, disc protrusion or herniation, vertebral body signal changes, spondylolisthesis, dorsal osteophytes, arthritic changes on the intervertebral joints and thickness of ligamenta flava. Statistical evaluation of the degree of agreement among multiple observers was made in individual segments of the lumbar spine by Fleiss kappa; the degree of severity of MRI findings was assessed by Friedman test and pairwise multiple comparisons by Nemenyi post hoc test. Results: Degree of agreement in the overall MRI comparison in individual segments of the lumbar spine ranged from 0.32 to 0.80 kappa values, most frequently from 0.5 to 0.7 - it was moderate or substantial. Testing of the degree of severity of MRI findings showed a statistically significant differences (p < 0.0001) among individual evaluators. In each group of evaluators (ER, RR, NR) was always one evaluator who differed significantly from the other two. These differences were smallest among experienced radiologists. Conclusion: We found statistically significant differences in the assessment of MRI findings among individual evaluators and also within groups of evaluators.
