Hodgkinův lymfom (HL) je jedním z nejlépe léčitelných nádorových onemocnění. Moderními léčebnými strategiemi je vyléčeno 85–95 % pacientů. Do popředí začaly vystupovat pozdní následky léčby, především kardiotoxicita a sekundární malignity, na které 15 let po skončené terapii umírá více pacientů než na HL. Cílem výzkumu je najít takovou terapii, která by zajistila maximální kontrolu tumoru při minimalizaci akutní a dlouhodobé toxicity. German Hodgkin Study Group (GHSG) prokázala ve studii HD10 pro počáteční stadia HL možnost redukce chemoterapie i radioterapie, bez snížení účinnosti léčby. Studie pro pokročilá stadia HD9 prokázala výborné výsledky chemoterapie BEACOPP eskalovaný oproti konvenční terapii ABVD, ale s vyšší akutní a dlouhodobou toxicitou. Cílem dalších studií GHSG pro pokročilá stadia (HD12, HD15 a nyní aktivní studie HD18) je snížit intenzitu chemoterapie a posoudit, zda je nutná následná radioterapie. Pozitivní výsledky přináší hodnocení studie HD15, kde je testován význam FDG – PET při indikaci radioterapie. Současná studie HD18 sleduje pomocí FDG – PET i časnou odpověď tumoru po 2 cyklech chemoterapie. Další naděje jsou vkládány do nových biologických léků, které cílí přímo na receptory maligních buněk (monoklonální protilátky, inhibitory)., Hodgkin lymphoma (HL) is one of the best curable malignant diseases. Modern therapeutic strategies can cure 85–95 % of patients. Late effects have become increasingly important, especially cardiotoxicity and second tumors, that cause more deaths than HL 15 years after completion of treatment. The goal of the research is to find the therapy that maximizes tumor control and minimizes acute and longterm toxicity. German Hodgkin Study Group (GHSG) proved the possibility to reduce chemotherapy and radiotherapy without reducing the effectiveness of treatment in the HD10 study for early stages of HL. HD9 study for advanced stages demonstrated excellent results of BEACOPP escalated chemotherapy compared to the conventional therapy ABVD, but BEACOPP escalated induced higher acute and long-term toxicity. The goal of subsequent GHSG studies for advanced stages (HD12, HD15 and ongoing active HD18 study) is to reduce the intensity of chemotherapy and to evaluate the significance of subsequent radiotherapy. The HD15 study tests the significance of FDG-PET for indication of radiotherapy. Current HD18 study tests by FDG-PET the early response of tumor after 2 cycles of chemotherapy. Cíl: Minireview podává přehled aktuálního stavu měření 25-hydroxyvitaminu D v krevním séru. Metoda: Hodnocení analytické kvality měření 25-hydroxyvitaminu v programech externího hodnocení kvality. Wound healing is a complex physiological process important for tissue homeostasis. An acute injury initiates massive cell migration, proliferation and differentiation, synthesis of extracellular matrix components, scar formation and remodelling. Blood flow and tissue oxygenation are parts of the complex regulation of healing. Higher organisms utilize molecular oxygen as a terminal oxidant. This way of gaining energy for vital processes such as healing leads to the production of a number of oxygen compounds that may have a defensive or informatory role. V letech 1991 až 2009 bylo v České republice ohlášeno 1 038 případů profesních nádorových onemocnění. Dlouhodobý trend byl sestupný, v 95 % byli postiženi muži. Střední věk při zjištění diagnózy zhoubného novotvaru byl 67 let. Převažoval profesní zhoubný novotvar průdušek a plic před difúzním mezoteliomem pleury a peritonea, následovaly nádory močového měchýře a kůže. Jako příčina zhoubných novotvarů dominovalo ionizující záření, 77 % osob postižených profesními nádory pracovalo při těžbě nebo úpravě uranové rudy. Autoři se zabývají zásadami diagnostiky i příčinami malého počtu uznaných nádorových onemocnění za nemoci z povolání. Autoři upozorňují, že seznam nemocí z povolání byl od července 2011 rozšířen o další zhoubné novotvary a je i širší možnost je uznat za profesní novotvary, které jsou způsobeny chemickými karcinogeny., The authors have undertaken the analysis of all cases of occupational malignant neoplasmas which were reported in the National Register of Occupational Diseases in the Czech Republic. In the years 1991–2009, 1038 occupationally-related tumour diseases have been notified. The trend was decreasing, males were affected in 95 %. The mean age of persons at diagnosing malignant neoplasma was 67 years. The occupational malignant neoplasm of bronchi and lunge prevaleid, followed by the diffuse mesothelioma of pleura or peritoneum, neoplasms of an urinary bladder and skin. Ionizing radiation dominated as the cause of malignant tumours in 77 % affected persons worked at mining and manufacturing uranium ore. The authors deal with current review attitudes and opinions of experts on principles of diagnostics of occupational malignant tumours. In the conclusions the authors analyze the cause of decreasing tendency in the number of notified occupational diseases.
