Nádory ledvin se vyskytují v každém věku. Existují věkově specifické rozdíly nejen v histologickém typu nádoru, ale i epidemiologii, etiologii, patogenezi, biologických studiích a klinickém chování. Tyto věkově specifické rozdíly jsou rovněž patrné u stejného typu nádoru a ovlivňují prognózu a přežívání pacientů. Otázkou zůstává, jestli jsou současně platná doporučení léčebné strategie skutečně vhodná pro všechny věkové skupiny., Renal tumors occur accross the age. The age specific differences are described not only on histology type of tumor, but also on epidemiology, ethiology, pathogenesis, biology studies and clinical behavior. These age specific differences are noticed also in the same type of the tumor and have an effect on prognosis and survival of the patients. Neuroendokrinní nádory představují heterogenní skupinu nádorů s velmi odlišnými vlastnostmi. Od správné diagnózy typu nádoru se odvíjí i specifická léčba. V současné době k armamentáriu v terapii těchto nádorů patří nejen výkony chirurgické, lokální léčba jaterních metastáz a ve vybraných případech chemoterapie, ale zaznamenáváme výrazné pokroky i v cílené biologické léčbě, včetně antiproliferativního efektu somatostatinových analog, TKI a M-TOR inhibitorů. Značný pokrok učinila i léčba otevřenými zářiči navázanými na analoga somatostatinu., Neuroendocrine tumors are a heterogeneous group of tumors with very different properties. A correct diagnosis of tumor type leads to specific treatment. Current treatment options for these tumors include not only surgery, local therapy for hepatic metastases and, in selected cases, chemotherapy, but significant advances have been achieved in targeted biological therapy, including the antiproliferative effect of somatostatin analogs, TKIs and mTOR inhibitors. Článek se zabývá masivním vymíráním tří druhů supů ( Gyps bengalensis , Gyps indicus , Gyps tenuirostris ) v jižní Asii. Drastický pokles početnosti supích populací začal v de vadesátých letech minulého století v d ůsledku zavedení nesteroidního protizánětlivého léčiva s n ázvem diclofenac do v eterinární medicíny. V č lánku je popsán mechanismus toxického účinku diclofenacu a t aké jsou zde vysvětleny příči- ny kontaminace supí potravy tímto přípravkem. Speciální pozornost je pak věnována diskusi možných následků vymírání supů jako je zvýšení početnosti populací zdivočelých psů, zvýšené riziko onemocnění lidí vzteklinou a vliv na pohřební rituály členů etnika Pársů., The article deals with massive mortality in three vulture species ( Gyps bengalensis , Gyps indicus , Gyps tenuirostris ) on the South Asian subcontinent, which started in the nineties of the last century. Accidental exposure of scavengers to diclofenac, a non-steroidal anti-inflammatory drug (NSAID), via their food chain is considered to be a scientifically proven reason for the decline of vulture population in India, Pakistan and Nepal. The mechanism of diclofenac toxicity is explained and the sources of diclofenac in vulture food resources are discussed. Z 213 179 nových onemocnění karcinomy štítné žlázy (TC) ve světě v roce 2008 bylo podle údajů Globocan (IARC) 46,1 % případů ve vyspělých a 53,9 % v méně vyspělých zemích, 33 599 v EU a 934 v ČR; z 35 383 zemřelých na TC bylo 27,3 % ve vyspělých a 72,7 % v méně vyspělých zemích, 3 581 v EU a 81 v ČR. V roce 2030 mohou očekávaná nová onemocnění dosáhnout ve světě více než 310 tisíc TC, s nárůstem o 17,1 % u mužů a 7,1 % u žen ve více rozvinutých, o 58,7 % u mužů a 56,5 % u žen v méně rozvinutých zemích, respektive o 14,1 % a 7,2 % v EU, o 20,4 % a 7,9 % v ČR. Celosvětově byl u žen 3,3krát vyšší počet nových případů a 2,2krát vyšší počet zemřelých než u mužů. Z 184 zemí dosáhli muži i ženy ČR v roce 2008 hrubou incidencí TC 14. nejvyšší hodnoty, z 20 zemí Evropy 9. a 7. hodnoty. Ve věku 65 a více let bylo v roce 2008 ve světě 21,9 % nových případů TC u mužů a 18,5 % u žen, respektive 30,6 % a 26,4 % v ČR, s očekávaným nárůstem o 97 tisíc případů ve světě a 100 případů v ČR v roce 2030. Varující počty TC a ostatních novotvarů by měly podpořit osobní odpovědnost, směřující ke snížení očekávaných statistických údajů., Of the worldwide 213 179 new cases of thyroid cancer (TC) based in Globocan 2008 (IARC) are estimated 46.1 % cases in more developed and 53.9 % in less developed regions, 33 599 in the European Union and 934 in the Czech Republic in 2008; of the worldwide 35 383 deaths there were 27.3 % in more developed and 72.7 % in less developed regions, 3,581 in the European Union and 81 in the Czech Republic. In 2030 the expected new cases can reach more than 310 thousands of TC in the world, increased by 17.1 % in males and 7.1 % in females in more developed, by 58.7 % in males and 56.5 % in females in less developed regions, respectively by 14.1 % and 7.2 % in the European Union, by 20.4 % and 7.9 % in the Czech Republic. The worldwide new cases of TC were 3.3 times higher and deaths 2.2 times higher in females than in males. The Czech males and females reached 14th highest values by crude rate of TC among 184 countries, respectively 9th and 7th among 20 European countries in 2008. The worldwide new cases of TC aged 65 years and over were 21.9 % in males and 18.5 % in females in 2008, respectively 30.6 % and 26.4 % in the Czech Republic with expected increasing by 97 thousands cases in the world and 100 cases in the Czech Republic in 2030. Paraneoplastická limbická encefalitida je vzácné onemocnění náležící do skupiny paraneoplastických neurologických syndromů. V diagnostice hraje zásadní roli zobrazení mozku magnetickou rezonancí. Předkládáme kazuistiku 57letého muže, u něhož vyšetření magnetickou rezonancí jistě přispělo ke stanovení této diagnózy. Další významnou součástí diagnostického postupu je průkaz specifických, takzvaných antineuronálních autoprotilátek Prognóza choroby nebývá dobrá, je závislá především na možnostech suprese základního tumorózního onemocnění., Paraneoplastic limbic encephalitis is a rare disorder which belongs to a group of paraneoplastic neurologic syndromes. The diagnostic role of magnetic resonance imaging (MRI) is of utmost importance. We report a case of a 57-year-old man in whom MRI significantly contributed to establishing of proper diagnosis. Other important diagnostic test includes identification of specific, so called antineural auto antibodies. CT s vysokým rozlíšením (HRCT) sa stalo metódou voľby v zobrazovaní a diagnostike ochorení stredného ucha a prevodovej poruchy sluchu. Pomocou HRCT dokážeme detailne popísať anatómiu spánkovej kosti, patológiu stredného ucha (otitis média chronica, cholesteatóm a ich komplikácie) a traumatické zmeny. Magnetická rezonancia má naopak vysoké rozlíšenie mäkkých tkanív, preto ju využívame k diferenciálnej diagnostike mäkkotkanivovej masy stredného ucha zobrazenej na CT, k odlíšeniu reziduálneho ochorenia alebo recidívy v teréne pooperačných zmien a k zobrazeniu intrakraniálnych komplikácií akútnej a chronickej otitídy CT a MR sú komplementárnymi zobrazovacími metódami, pomáhajú stanoviť správnu diagnózu, ktorá je pre klinika dôležitá z hľadiska stratégie liečby. V publikácii uvádzame spektrum patológií stredného ucha, s ktorými sa pri zobrazení spánkovej kosti môžeme stretnúť a na ktoré treba myslieť., High resolution computed tomography (HRCT) is a modality of choice in diagnostic imaging of middle ear pathology and conductive hearing loss. It enables to image a detail anatomy of temporal bone, pathology of middle ear (e.g. chronic otitis média, cholesteatoma and their complications) and traumatic changes in this area. On the other hand, magnetic resonance imaging (MRI) enables a high resolution of soft tissue structures. It is used in imaging of soft tissue masses in middle ear cavity previously diagnosed on CT, differentiating residual disease or recurrence in postoperative ear, and in imaging of intracranial complications of acute and chronic otitis. CT and MRI are complementary imaging modalities in helping the clinicians to determine the proper strategy for patient treatment. Alzheimerova choroba (ACH) se řadí mezi závažná neurodegenerativní onemocnění provázená oxidačním stresem. Produkty radikálových reakcí mohou difundovat z primárních míst a být detekovány v cerebrospinální tekutině (CSF) či krvi. Tyto produkty představují potenciální biochemické markery pro diagnostiku ACH. Nejvíce pozornosti je zaměřeno na analýzu CSF, protože odráží patologické změny v mozkové tkáni u ACH. V CSF byly potvrzeny zvýšené hladiny oxidačních produktů lipidů i proteinů. Hladiny vitaminů jsou snížené, nicméně existují i práce, které nenašly rozdíly oproti kontrolám. Některé studie se zaměřily na detekci produktů oxidačního stresu v krvi u ACH, ale výsledky nejsou konzistentní. Část prací ukazuje na nárůst oxidačních produktů a snížené hladiny antioxidantů v plazmě. Nicméně jiné práce tyto výsledky nepotvrdily. Z hlediska hledání diagnostického biomarkeru pro ACH má význam se zaměřit na specifické produkty peroxidace lipidů, tzv. lipofuscinoidní pigmenty (LFP), v erytrocytech. Na základě fluorescenčních analýz LFP je možné najít specifický produkt v krvi u ACH., Alzheimer´s disease (AD) is a serious neurodegenerative disorder accompanied by oxidative stress. Products of free radical reactions diffuse from primary sites and can be detected in cerebrospinal fluid (CSF) and blood. Such products represent potential biochemical markers for diagnosis of AD. Most studies are focused on CSF since its composition reflects pathological changes in the brain in AD. Increased levels of lipid and protein oxidative products have been found in CSF. Levels of vitamins were reduced in CSF. However, there are studies which show no difference between AD and controls. There is research on free radical products in blood in AD but results are not consistent. Several studies show increased oxidative products and reduced antioxidant in plasma. Nevertheless, others did not confirm it. Considering the investigation of a diagnostic biomarker for AD, specific end-products of lipid peroxidation, so called lipofuscin-like pigments (LFP), in erythrocytes represent an important possibility. Mojmíra Vlašína z p rvního čísla časopisu Anthropologia integra nabízí čtenářům lákavou představu eko- logicky šetrné technologie pěstování kulturních plodin, nevyžadující aplikace pesticidů či minerálních hnojiv, energeticky minimálně náročné a p řitom poskytující více potravin než třeba obilné pole. Z p ohledu současné vědy o r ostlinách je ovšem možnost realizace takto efektivních permakultur pro dostatečnou výživu rostoucí lidské populace čirou mystifikací. Text polemiky nejen dokládá nereálnost tvrzení zmíněného článku, ale také nabízí jinou, vědecky podloženou alternativu moderního, vysoce produktivního a p řitom ekologicky šetrného zemědělství, vy- cházejícího z v yužití nejrůznějších typů geneticky modifikovaných (GM) plodin. Její přínos a d e facto budoucí nezbytnost pro světový boj proti hladu a c hudobě ilustruje s p omocí „ammanovských tezí“, shrnujících závěry nedávných jednání jednak vatikánské Pontifikální Akademie, jednak předních německých vědeckých společností., The contribution of Mojmír Vlašín entitled Permaculture? and published in the first issue of Anthropologia Integra offered to its readers attractive view on the ecologically friendly technology of the crop production, supposed to supply growing human population with the sufficient food amount. As postulated, in permaculture praxis no mineral fertilization, no pesticide application would be necessary to produce more edible biomass then conventional cereal field is able to provide. Unfortunately, from the point of view of recent plant biology the realization of similar world permaculture appears to be pure mystification. The only modern alternative to make existing conventional agrotechnologies both more effective and also ecologically friendly represents the application of various GM crops. Only a few cases of pneumocystis pneumonia (PCP) in Cushing’s syndrome have been published in the literature so far. In the majority of these patients, the pneumonia occurred after reduction of the hypercortisolism with medicamentous treatment. We report two cases of PCP during conservative treatment of hypercortisolism. We describe clinical, imaging and laboratory findings in two patients and review published cases of pneumocystits pneumonia in Cushing’s syndrome. A 60-year-old woman and 20-year-old man with Cushing’s syndrome due to ectopic ACTH syndrome were treated at our department. Both developed pneumocystis pneumonia early after treatment with ketoconazole and ethomidate bromide had been introduced and the levels of cortisol rapidly decreased. PCP prophylaxis in patients with high cortisolemia should be started before treatment of hypercortisolism in current practice.
