Toxická epidermální nekrolýza je vzácné, akutní, život ohrožující onemocnění projevující se rozsáhlým odlučováním epidermis a slizničních povrchů. Jde o závažnou nežádoucí reakci na podávané léky, nejčastěji antibiotika, antikonvulziva či nesteroidní antirevmatika. Jako první ji popsal skotský dermatolog Alan Lyell v roce 1956, proto je též nazývána Lyellův syndrom. Mortalita tohoto onemocnění je vysoká, proto je klíčová včasná diagnostika a neodkladné zahájení adekvátní péče o pacienta. Jedná se o vzácné onemocnění s velmi malou incidencí a závažnou prognózou, což značně ztěžuje provedení velkých randomizovaných klinických studií. Je tedy obtížné hodnotit efektivitu jednotlivých terapeutických možností. Jako nejperspektivnější se prozatím jeví nitrožilní podávání imunoglobulinů se zřejmým zlepšením klinického stavu pacienta, dobrou tolerancí a minimálními vedlejšími nežádoucími účinky. Nové experimentální postupy se snaží hledat cestu terapie cíleným ovlivněním granulyzinu jakožto hlavního cytotoxického mediátoru. It is a serious adverse response to administered drugs, mostly antibiotics, anticonvulsants or NSAIDs. First described by Scottish dermatologist Alan Lyell in 1956, it is also known as Lyell’s syndrome. Mortality of the disease is high, therefore its early diagnosis is crucial and immediate initiation of appropriate patient care necessary. It is a rare disease with a very low incidence and serious prognosis, which is a considerable hindrance to undertaking large randomized clinical studies. It is therefore difficult to evaluate the effectiveness of various therapeutic options. As the most promising so far appears the administration of intravenous immunoglobulins, apparently reaching improvement in the clinical condition of the patient, with a good tolerances and minimal side adverse effects. New experimental techniques endeavour to seek a way to the therapy through targeted influencing of granulysin as the major cytotoxic mediator. BACKGROUND: Intussusception of the large bowel in adults is a very rare pathological condition. However, it has its clinical importance because intussusception is very often associated with an intraluminal lesion. CASE REPORT: We report two cases of the large bowel intussusception, ileocolic and colorectal. Both intussusceptions were associated with a malignant tumor. However, the clinical presentation was different. One of the intussusceptions was of non-transient character, while the second one resolved spontaneously before operation. Both patients underwent surgery and malignant tumors were found and removed. RESULTS: The purpose of the article is to draw attention to intussusception and emphasize that intussusception, either transient or non-transient, should be further examined. Cíl práce: V kazuistice prezentujeme raritní nález primárního plazmablastického lymfomu penisu, který klinicky imitoval karcinom glans penis. Jedná se o druhý publikovaný případ tohoto typu lymfomu penisu ve světě. Kazuistika: Pacient, narozen 1929, byl odeslán na naše pracoviště ošetřujícím urologem pro nově zjištěný exofytický tumor glans penis, indikovali jsme parciální amputaci penisu. Histologické vyšetření amputátu prokázalo primární plazmablastický lymfom. Následná restagingová vyšetření neprokázala systémové onemocnění, pacient je nyní 22 měsíců po výkonu bez známek onemocnění a zůstává v hematologické dispenzární péči. Závěr: Primární plazmablastický lymfom penisu představuje extrémně raritní diagnózu, ve světové literatuře je popsán pouze jediný případ. Jedná se o agresivní typ lymfomu, který obvykle vede v krátké době ke smrti pacienta. Ačkoli léčba těchto nádorů je převážně založena na chemoterapii, u našeho pacienta vedl radikální operační výkon k úplnému vyléčení., We report a case of an elderly man who has presented with tumorous affection involving the uncircumcised penile glans and prepuce. Primary clinical examination gave strong suspicion for primary carcinoma of penis, therefore a suprapubic cystostomy and partial amputation of penis were performed. Histopatological examination revealed uncommon diagnosis of plasmablastic lymphoma involving glans penis and prepuce. The vast majority of penile tumours are squamous cell carcinomas, primary malignant lymphoma of penis is extremely rare. As staging results were negative, no adjuvant systemic chemotherapy was neccesary. Sometimes intraorbital foreign bodies lead to unexpected findings. A 16-year old boy was referred due to ocular surface trauma. A conjunctival laceration was detected at the level of the left caruncle with associated left exotropia, reduced adduction as well as a preretinal hemorrhage along the nasal periphery of the fundus. A blow-out fracture of the medial orbital wall was suspected and a CT scan of the orbits was scheduled which revealed the presence of a large intraorbital foreign body. The removal of the intraorbital foreign body (which proved to be a bullet) and precautionary laser photocoagulation along the nasal periphery of the left eye were performed. Variant branching pattern of the cords of brachial plexus coupled with erroneous communications has been an area of concern for surgeons opting to explore this region. Anaesthetic blocks and surgical approaches are the highlights of these interventions, where a keen familiarization of the anatomy of this region is mandatory. The present case description reports a unilateral variant branching pattern of the posterior cord coexistent with a neural communication between lateral and medial cords in an adult male cadaver. This intercordal neural communication between lateral and medial cords was oriented obliquely and measured 2.2 cm in length. Furthermore, the posterior cord revealed a variant branching pattern. It branched into three upper subscapular nerves and a common trunk for the thoracodorsal and lower subscapular nerves. The lowest of the three upper subscapular nerves gave a communicating twig to the thoracodorsal nerve. Úraz podtlakem v bazénu je skupina specifických raritních poranĕní s velkým rizikem fatálních důsledků. Častĕjší obĕtí s častĕji vážnějšími důsledky jsou zejména malé dĕti. Při tomto druhu poranĕní dochází ve vĕtší či menší míře k tonutí postiženého. Případ 19-letého pacienta zachyceného v oblasti hýždí nezajištěným otvorem sací drenáže zajímavĕ ilustruje nĕkteré zvláštnosti této problematiky. Preventivní opatření jsou v současnosti již známá, problém bývá vĕtšinou s jejich důslednou aplikací. K fatálním příčinám patří kromĕ utonutí, hypovolemický šok z náhlé redistribuce tekutin třetího prostoru a eviscerace s devastací vnitřních orgánů. Lokalizované často bizardní a velké otoky a sufúze lze v naprosté vĕtšinĕ případů řešit konzervativnĕ. Pro prevenci je klíčové rozšíření informace o existenci těchto poranĕní do obecného povědomí. Pro jejich správný management je pak důležité prohlubování povědomí a sdílení zkušeností s řešením těchto raritních poranĕní v odborné veřejnosti., Swimming pool suction injuries are unique and rare with a substantive risk of fatal consequences. Little children under the age of 8 are the most frequent victims with serious injuries. Drownings of different seriousness are also a usual part of accidents. The case of a 19 year old man trapped in the gluteal area by a unsecured suction drainage hole illustrates the uniqueness of this problem in an interesting way. Prophylactic arrangements are well known but the problem is with their strict application. Fatal causes excluding drowning include hypovolemic shock from the sudden redistribution of intersticial fluid and blood and also the evisceration of the bowel and other abdominal organs. Localised often bizarre and large swellings and sufusions can be treated nonoperatively in the vast majority of cases. For the prevention of these injuries it is important to inform the public and increase their awareness to these injuries. „Upside-down stomach syndrome“ je sporadicky se vyskytující typ velké paraezofageální hiátové hernie vyžadující při známkách uskřinutí neodkladné chirurgické řešení. Autoři článku prezentují kazuistiku 53letého nemocného s volvulem žaludku při „upside-down stomach syndromu“. Chirurgický výkon byl komplikován poraněním abdominální části jícnu, které bylo úspěšně léčeno mimo jiné použitím samoexpandibilního metalického stentu. Přes závažný klinický pooperační průběh s nutností revize dutiny břišní, zavedení jícnového stentu, provedení torakotomie pro absces mediastina a sekundárního hojení laparotomie byl pacient po padesáti dvou dnech propuštěn do domácího léčení v uspokojivém stavu, plně realimentován a rehabilitován, se zhojenou perforací jícnu i laparotomií., Upside-down stomach syndrome is a rare type of a large paraoesophageal hiatal hernia, which requires an immediate surgical treatment in case of incarceration. The authors present a case report of a 53-year-old male patient with gastric volvulus related to the upside-down stomach syndrome. Surgical treatment was complicated by an injury to distal oesophagus, which was successfully treated using a self-expandable metallic stent among other methods. Despite the complicated postoperative course with a necessity of reoperation, insertion of an oesophageal stent, thoracotomy for a mediastinal abscess and secondary healing of the laparotomy, the patient was discharged in a good condition with healed oesophageal perforation and laparotomy after 52 days., and L. Epilepsia je chronické ochorenie, ktoré je často sprevádzané psychiatrickou komorbiditou. Najčastejšou z nich je depresia, ktorá je sprevádzaná subsyndromálnymi, atypickými a chronifikovanými príznakmi depresie. Incidencia depresie je u pacientov s epilepsiou vyššia ako u iných neurologických alebo chronických ochorení. Psychopatológia spojená s epilepsiou obťažuje pacienta častokrát viac ako samotné ochorenie. Cieľom práce je prezentácia a analýza kazuistiky 37-ročného muža s diagnózou epilepsie so sprievodnou psychiatrickou symptomatikou, ktorá znížila kvalitu jeho života oveľa viac ako samotné neurologické ochorenie. V kazuistike popisujeme klinické symptómy neuropsychiatrického ochorenia – epilepsie ako aj životné udalosti, ktoré sú u každého pacienta jedinečné a majú závažný vplyv na priebeh a liečbu epilepsie., Epilepsy is a chronic disease that is often accompanied by psychiatric comorbidity. The most frequent of them is depression accompanied by subsyndromic, chronic and atypical symptoms of depression. The incidence of depression in patients with epilepsy is higher than in other neurological or chronic diseases. Psychopathology associated with epilepsy often bothers the patient more than the disease itself. The aim of this work is to present and analyze a case report of a 37‑year‑old man diagnosed with epilepsy and associated psychiatric symptomatology that reduced his quality of life much more than the neurological disease itself. In this case report, we describe clinical symptoms of neuropsychiatric disease – epilepsy as well as life events that are unique to each patient and have a significant impact on the course and treatment of epilepsy. Úvod: Primární hyperparathyreóza je onemocnění způsobené nadměrnou sekrecí parathormonu z příštítných tělísek. Po stanovení diagnózy endokrinologem záleží úspěch operačního řešení zejména na zkušenosti chirurga. Zvláštní skupinu tvoří ektopická lokalizace patologicky zvětšeného příštítného tělíska v mediastinu, která vyžaduje modifikaci operačního přístupu. Při hlubokém uložení adenomu v mediastinu je indikována sternotomie či thorakotomie, anebo je lze nahradit metodou miniinvazivní – videothorakoskopickou thymektomií. Kazuistika: Předmětem sdělení je kazuistika pacienta, který měl normokalcemickou recidivující primární hyperparathyreózu a podstoupil miniinvazivní videoasistovanou thymektomii po scintigrafickém potvrzení uložení adenomu příštítného tělíska v mediastinu. Závěr: Odstranění adenomu příštítného tělíska z mediastina metodou videothorakoskopickou je metoda bezpečná. Tato metoda oproti sternotomiím zlepšuje pooperační průběh, zkracuje dobu hospitalizace a přináší větší komfort pro pacienta. Doporučujeme zvážit tuto videothorakoskopickou metodu v případě uložení patologicky zvětšeného příštítného tělíska v dolním a předním mediastinu., Introduction: Primary hyperparathyroidism is a disease caused by elevated secretion of parathyroid hormone from pathological parathyroid glands. After the diagnosis, the success of its surgical solution depends predominantly on surgical management and experience of the surgeons. A special group is formed by ectopic localizations of pathologically enlarged parathyroid glands in the mediastinum, which require a modified surgical approach. When the adenoma is deep in the mediastinum, sternotomy or thoracotomy is indicated; alternatively, a minimally invasive approach can be used – videothoracoscopic thymectomy. Case report: We present a case of a patient with normocalcemic recurrent primary hyperparathyroidism. This patient underwent a minimally invasive video-assisted thymectomy after scintigraphic confirmation of parathyroid adenoma in the mediastinum. Conclusion: The removal of parathyroid adenoma in the mediastinum using the videothoracoscopic method is safe. Compared to sternotomy, this method improves the postoperative period, reduces the length of stay and provides more comfort to patients. We recommend considering the videothoracoscopic method in cases where the pathologically enlarged parathyroid gland is localized in the inferior and anterior mediastinum., and P. Libánský, P.
