INTRODUCTION: Cellulitis remains a very serious disease even today. Mortality, which varied between 10-40%, has been reduced owing to the standard securing of airway patency and use of an appropriate surgical treatment approach. MATERIALS AND METHODS: A total of 195 patients were hospitalised for cellulitis at the University Hospital in Hradec Králové during 2007-2011. The following parameters were evaluated: age, gender, dependence of incidence of the disease on the season of the year, frequency of attacks of the particular areas and their clinical characteristics, aetiology of the inflammation, types of patient complaints, prevalence of current systemic diseases, results of microbiological and selected laboratory analyses, socio-economic status of the patients, and duration of patient stay at the hospital. Statistical analysis was performed by using Pearson's correlation coefficient, the statistical significance level was p < 0.05. RESULTS: The mean age of the patients was 39.8 years. The group of 195 patients included 108 (55%) males and 87 (45%) females. The mean time between the first symptoms of the disease and admission to the Department was 5 days. From among the 195 patients, 116 (59.5%) were working persons, 79 (40.5%) were non-working (children, students, unemployed persons, women on maternity leave, retired people). The odontogenic origin of the disease was verified in 173 (88.7%) patients. In total, 65 (33.3%) patients had no coinciding complicating systemic disease, 22 (11.3%) patients had diabetes mellitus. The most frequent symptom of cellulitis was painful swelling, found in 194 (99.5%) patients, followed by jaw contracture, found in 153 (78.5%) patients. CONCLUSION: The results are largely very similar to those of previous studies performed in other countries, except that we found no correlation between the prevalence of cellulitis and the socio-economic status, nor have we confirmed Klebsiella pneumoniae sp. as the cause of cellulitis in patients with diabetes mellitus. and H. Úvod: Biliární ileus představuje 1−4 % mechanických obstrukcí gastrointestinálního traktu, ale u pacientů nad 65 let již 25 % nestrangulačních mechanických ileů tenkého střeva. Objemný kámen žlučníku obvykle vytvoří tlakovou nekrózu, protlačí se vzniklou biliodigestivní píštělí a obturuje gastrointestinální trakt. Kazuistiky: Mezi daty 1. 1. 2000 a 31. 1. 2015 bylo na Chirurgické klinice FN v Plzni operováno 35 pacientů pro poruchy pasáže s nálezem biliárního ileu. Průměrný věk pacientů v době operace byl 79 let (medián 78 let, min. 58 let, max. 92 let). Aerobilie na rentgenovém nativním snímku břicha byla přítomna z 27 pořízených snímků v 7 případech (26 %). Předoperačně byla provedena dvakrát kolonoskopie a devětkrát ezofagogastroduodenoskopie. Z operačních výkonů byla 21x provedena extrakce kamene z enterotomie, třikrát extrakce z gastrotomie, sedmkrát resekce tenkých kliček, jedenkrát Hartmannova resekce, jedenkrát resekce ileoascendentoanastomózy a jedenkrát extrakce konkrementu s následnou duodenojejunální anastomózou a s cholecystektomií v jedné době. Jedenkrát byla provedena ileotransverzoanastomóza. Pooperační 30denní mortalita byla 14,3 %. Pooperační morbidita dle klasifikace Clavien-Dindo činila u I. stupně 22,8 %, u II. stupně 5,7 %, u III. stupně 11,5 % a u IV. stupně 11,5 %. K recidivě biliárního ileu došlo u 8,8 % pacientů. Primární ošetření píštěle a cholecystektomie byla provedena v jednom případě. Závěr: Rozhodnutí o ošetření cholecystoenterální píštěle a cholecystektomii v první době bude vždy záležet na celkovém stavu pacienta a místním nálezu v oblasti žlučníku. Při ponechání píštěle je nutné počítat s rizikem recidivy ileu. Při operaci je nutné též zrevidovat celé střevo k vyloučení přítomnosti dalších konkrementů a snížit tak riziko časné recidivy. However, the rate of non-strangulated small bowel obstructions reaches as much as 25% in patients over 65 years of age. Usually, a pressure necrosis is created by a large gallstone that passes through the developed biliodigestive fistula, subsequently obturating the gastrointestinal tract. Case reports: 35 patients underwent a surgical procedure at the Department of Surgery, Teaching Hospital Pilsen for biliary ileus from January 1, 2000 to January 31, 2015. Mean age was 79 years (median 78 years; min. 58 years; max. 92 years). Aerobilia was visible in abdominal X-ray scans in 7 cases (26%) of 27 acquired images. Preoperatively, colonoscopy was done 2 times and esophagogastroduodenoscopy 9 times. Regarding surgical procedures, enterolithotomy was done in 21 cases, extraction of the gallstone from gastrotomy was done 3 times, small bowel resection 7 times, Hartmann resection 1 time, resection of ileoascendentoanastomosis 1 time, and extraction of the gallstone from duodenotomy with duodenojejunoanastomosis and cholecystectomy was done in 1 case. Ileotransversoanastomosis was performed in 1 case. Postoperative 30-day mortality was 14.3%. Postoperative morbidity according Clavien-Dindo was 22.8% for grade 1, 5.7% for grade 2, 11.5% for grade 3, and 11.5% for grade 4. A recurrence was observed in 8.8%. Primary treatment of the fistula and cholecystectomy were done in 1 case. Conclusion: The indication of primary biliodigestive fistula treatment always depends on the general condition of the patient and on the local finding in the gallbladder area. Residual fistula increases the risk of reccurence, but primary treatment is associated with higher mortality. Terapie akutní fáze ischemické cévní mozkové příhody (CMP) je zaměřena jednak bezprostředně na omezení rozsahu poškození pomocí co možná nejčasněji zahájené reperfuzní strategie a dále nastavení léčby zajišťující farmakologickou sekundární prevenci recidivy onemocnění. Klíčový předpoklad správné a efektivní sekundární prevence představuje včasné stanovení příčiny ischemického iktu, protože riziko recidivy iktu je nejvyšší v prvních týdnech po prodělané CMP. Nedílnou součástí managementu pacienta s ischemickým iktem je tedy vyloučení potenciálních kardiálních a cévních zdrojů embolizace do mozkové cirkulace pomocí zobrazovacích metod. Tyto embolizační zdroje jsou kategorizovány na vysoce rizikové stavy a stavy spojené se středním nebo nejasným rizikem., Treatment of acute phase of ischemic stroke is aimed to limit the amount of damage using reperfusion strategy and subsequently administration of treatment providing pharmacological secondary prevention of recurrence. The early aetiology diagnosis of stroke is crucial for effective secondary prevention as the highest risk of recurrence is within few weeks after stroke. Exclusion of cardiac and vascular source of embolism by imaging methods is an important part in management of patients with ischemic stroke. Úvod: Naše výsledky byly zhodnoceny za účelem posouzení bezpečnosti a efektivity endoskopické endonazální techniky v léčbě kraniofaryngeomů. <p align="">Materiál a metody: Od roku 2008 bylo endoskopicky endonazálně operováno14 pacientů s typickým supraselárním extraventrikulárním kraniofaryngeomem. Soubor se skládá z 11 mužů a 3 žen (věk 17–60 roků, průměrný věk 38 let). Jedenáct pacientů mělo předoperační deficit zorného pole v rozsahu od malého deficitu ve vnějším kvadrantu až k slepotě jednoho oka a těžkému deficitu druhého oka. Byla použita endoskopická endonazální technika čtyř rukou s peroperační 3,0 T MR. <p align="">Výsledky: V sedmi případech bylo dosaženo radikální resekce. Ve čtyřech případech byla provedena resekce subtotální, ve dvou případech resekce parciální. V jednom případě se jednalo o vypuštění kraniofaryngeomové cysty. V resekci po iMR bylo pokračováno ve třech případech. K úpravě zorného pole došlo v pěti případech. Pooperační diabetes insipidus se nově objevil ve třech případech. Reoperace pro únik mozkomíšního moku byla nezbytná ve třech případech. <p align="">Závěr: Endoskopické techniky v léčbě kraniofaryngeomu jsou bezpečné. Nicméně jsou spojeny s vyšším výskytem pooperačního úniku mozkomíšního moku ve srovnání s transkraniálními přístupy., Introduction: We reviewed our results in order to evaluate safety of endoscopic endonasal technique in the treatment of craniopharyngiomas. <p align="">Material and methods: Since 2008, endoscopic endonasal approach was used in 14 patients with typical suprasellar extraventricular craniopharyngioma. This cohort consisted of 11 males and three females (age 17 to 60 years, average age 38 years). Eleven patients had preoperative visual field deficit ranging from small deficit in the outer quadrant to blindness on one eye and a severe deficit of the second eye. Endoscopic endonasal four hands technique was used with intraoperative 3.0 T MRI. <p align="">Results: In seven cases, the radical resection was achieved. Subtotal resection was performed in four cases. The resection was partial in two cases. In one case, a cyst was drained. Resection after iMRI was performed in three cases. Visual field deficit improved in five cases. Postoperative diabetes insipidus developed in three cases. Reoperation for CSF leakage was necessary in three cases. <p align="">Conclusion: Endoscopic technique in the treatment of craniopharyngioma is safe. Cíl. Retrospektivně analyzovat výsledky léčby plicních arteriovenózních zkratů (PAVZ) embolizací v prevenci paradoxní embolizace. Metoda. Do studie bylo zahrnuto 16 nemocných (7 mužů, 9 žen, průměrného věku 48,3 roků, v rozmezí 17-82 roků) s celkem 35 PAVZ. U šesti nemocných byly PAVZ jednoduché, u jednoho nemocného PAVZ diagnostikovaná na CT angiografii nebyla potvrzena. U devíti nemocných byly PAVZ vícečetné (2-11 zkratů). Zkraty byly u jedné třetiny v levé plíci, u jedné třetiny v pravé plíci a u třetiny nemocných oboustranně. Pouze u dvou nemocných s vícečetnými PAVZ byly zkraty uloženy v horních plicních polích, jinak vždy ve středních a dolních. U 12 nemocných byla prokázána hemoragická hereditární teleangiektázie (HHT). Bylo provedeno 18 embolizačních sezení, u dvou nemocných byla provedena pouze angiografie, z toho u jednoho nebyl zkrat prokázán a jeden nemocný byl léčen pneumonektomií. Výsledky. Všechny PAVZ byly na konci embolizace bez angiograficky průkazného průtoku. U dvou nemocných došlo k rekanalizaci zkratu, což se projevilo nárůstem dusnosti. Další nemocná původně léčená na jiném pracovišti měla emb olizovány dvě rekanalizované PAVZ. Jedna nemocná se sekundární epilepsií po prodělaném mozkovém abscesu náhle zemřela 3 měsíce po embolizaci zkratu, nejspíše v důsledku epileptického záchvatu. Jeden nemocný byl ztracen ze sledování. Ostatní nemocní jsou bez potíží. Závěr. Embolizace je primární léčebnou metodou při uzávěru PAVZ v prevenci paradoxní embolizace. Nemocní s HHT vyžadují trvalou péči., Aim. Results of the pulmonary arteriovenous shunts (PAVS) embolization therapy in prevention of paradoxical embolization are retrospectively analysed. Material and methods. The study included 16 patients (7 males, 9 females of average age 48.3 years, range 17-82 years) with 35 PAVS. The single shunt was present in 6 patients, a shunt diagnosed on CT angiography was not confirmed on angiogram in one patient. The multiple PAVS (from 2 to 11) were diagnosed in 9 patients. The PAVS were in the left lung in one third of patients, in the right lung in another third of patients, and on the both sides in the remaining third of patients. The PAVS were located in the upper lobes only in 2 patients, who both had multiple PAVS. The other PAVS were located in the middle and lower pulmonary fields. The hemorrhagic hereditary teleangiectasia (HHT) was confirmed in 12 patients. The authors performed 18 embolization sessions, in two patients only diagnostic angiogram was done, since the PAVS was not confirmed in one patients and one patient was treated by pneumonectomy. Results. All embolized PAVS were angiographically occluded in the end of the procedure. Recanalization of the PAVS occurred in 2 patients with dyspnoe. Another patient had reembolization of two PAVS after primary treatment in another hospital. One patient with epilepsy secondary to the brain abscess suddenly died 3 months after embolization, most probably due to epileptic seizure. One patient was lost for follow-up. Remaining patients have been doing well. Conclusion. Embolization is the primary method of the PAVS occlusion in prevention of paradoxical embolization. INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease. Gliomy limbického a paralimbického systému představují anatomicky a tím pádem i operačními přístupy unikátní skupinu mozkových nádorů. Mikrochirurgie v kombinaci s moderními perioperačními postupy zvyšuje šance na radikální odstranění těchto gliomů. Z hlediska lokalizace je dělíme do tří základních skupin: 1. gliomy inzuly, 2. amygdalohippokampálního komplexu a 3. cingula. V minimonografii se věnujeme anatomii i funkci limbického a paralimbického systému a v návaznosti na ni představujeme ucelený pohled na jednotlivé neurochirurgické přístupy k těmto oblastem. Maximální resekce za zachování funkční integrity mozku je předpokladem následné smysluplné onkologické léčby. Taktéž prezentujeme výsledky operací našeho pracoviště ve smyslu morbidity a radikality za posledních sedm let., Limbic and paralimbic gliomas represent a unique group of brain tumors both from an anatomic and surgical point of view. Microsurgery, together with modern perioperative procedures, enables a wider extent of resection of these gliomas. Based on localization, we divide gliomas into three basic groups: 1. insular, 2. amygdalohippocampal complex and 3. cingulum gliomas. In our review, we describe the anatomy and function of the limbic and paralimbic system and in relation to that, we present a description of the specific neurosurgical approaches to these regions. Maximal resection with brain function preservation represents the basis for subsequent oncologic therapy. We also present the surgical results of our department in the last seven years with regard to morbidity and radicality. Úvod: Správné zhodnocení jaterních pseudolézí pomocí multidetektorového CT či MR je velmi důležité z důvodu odlišení těchto benigních změn od primárních či sekundárních malignit v jaterním parenchymu. Metoda: Autoři provedli systémový přehled literatury v databázi PubMed s vyhledáváním slov játra, pseudoléze v angličtině a češtině od roku 2000 do roku 2014. Výsledky: Autoři předkládají přehled literatury. Dále autoři retrospektivně zhodnotili soubor pacientů léčených pro jaterní onemocnění ve FN Ostrava, kdy se setkali s touto abnormalitou celkem v 7 případech. U 3 pacientů byla provedena diagnostická laparoskopie s vizuální kontrolou léze, s upřesněním diagnostiky pomocí peroperační ultrasonografie (IOUS) a s excizí části ložiska pro následné histologické vyšetření. Histologický nález u všech 3 pacientů odpovídal steatotickému postižení jater. Další 2 pacienti byli operováni pro jiná ložiska metastazujícího kolorektálního karcinomu a revize suspektní léze byla provedena během operačního výkonu otevřenou cestou. Kromě vizuální kontroly a peroperačního IOUS byla provedena excize k histologickému vyšetření, které prokázalo známky velkokapénkové i malokapénkové steatózy a nepřítomnost maligních změn. Poslední dva pacienti jsou dále sledováni v hepatologické poradně a jsou v rámci dispenzarizace kontrolováni v 6měsíčních intervalech. Závěr: Autoři prezentují vlastní zkušenosti získané v rámci mezioborové spolupráce na multidisciplinárních konferencích a připojují literární údaje o této neobvyklé problematice. Správným vyhodnocením nálezů této variety zejména u onkologických pacientů je možno zabránit zbytečným biopsiím, duplicitním vyšetřením a diagnostickým laparoskopiím či explorativním laparotomiím., Introduction: Accurate detection of hepatic pseudolesions using multi-detector CT and MRI examinations is crucial for the differentiation of benign alterations from primary and secondary malignant lesions in hepatic parenchyma. Method: The authors conducted a systematic literature review in PubMed. “Liver“ and “pseudolesion“ were used as keywords in English and Czech, and papers/articles published from 2000 to 2014 were retrieved. Results: The authors presented a literature review. In addition, the authors performed a retrospective evaluation of a group of patients treated for liver disease at University Hospital Ostrava where this anomaly was encountered in 7 cases. In 3 of the patients, diagnostic laparoscopy was done, with visual examination of the lesion accompanied by intraoperative ultrasound exam (IOUS) and partial excision, to establish the diagnosis. Subsequent histological assessment of the specimens confirmed the diagnosis of a steatotic lesion in each of these 3 patients. Additional 2 of the 7 patients underwent liver surgery for concurrent metastatic lesions of colorectal cancer and an open-access revision of the suspected lesions was performed. Visual inspection and intraoperative ultrasound (IOUS) was followed by excisional biopsy. The histology revealed macro- and micro-vesicular steatosis and excluded malignant changes. The last 2 patients still continue to be followed–up regularly on a 6-month routine check-up basis at our hepatology unit. Conclusion: The authors presented their own experience gained through inter-disciplinary cooperation at Multidisciplinary conferences. A literature overview of this unusual subject is also included. Particularly in oncologic patients, correct interpretation of these pseudolesions may help to avoid unnecessary biopsies, further imaging examinations and diagnostic laparoscopies and/or explorative laparotomies., and P.
