V listopadu roku 2014 se konal v Paříži mítink evropských expertů v kardiovaskulárních (KV) onemocněních a lipidech, na kterém se diskutoval závažný problém preventivní kardiologie – reziduální vaskulární riziko dané aterogenní dyslipidemií (AD), tj. zvýšenými triglyceridy a sníženým HDL-cholesterolem. Experti přijali na základě diskuse konsenzus o vlivu AD na KV riziko a o dosud existujících důkazech o snížení KV rizika kombinovanou léčbou staninem a fenofibrátem. Aterogenní dyslpidemie by měla být sekundárním cílem léčby dyslipidemií, protože se podílí na makrovaskulárním i mikrovaskulárním reziduálním riziku. Sekundárním cílem léčby je non-HDL-cholesterol jako nejlevnější a nejdostupnější marker reziduálního vaskulárního rizika. Kombinovaná terapie statinem a fibrátem je bezpečná, dobře tolerovaná, snižuje aterogenitu plazmy a výskyt KV příhod u pacientů s AD a vysokým KV rizikem, diabetem 2. typu nebo metabolickým syndromem. Klíčová slova: aterogenní dyslipidemie – fenofibrát – kardiovaskulární riziko – kombinovaná terapie – statiny – triglyceridy, A meeting of European experts in cardiovascular (CV) disease and lipids was convened in Paris, November 2014, where an important problem of preventive cardiology – residual vascular risk done by atherogenic dyslipidemia (AD) – was discussed. On the basis of discussion have the experts summarised a consensus concerning AD, its CV risk and up to date evidence of combined therapy with statin and fenofibrate on CV risk. Atherogenic dyslipidemia should be the secondary aim of dyslipidemia treatment, because it is a reason of residual vascular risk. Non-HDL-cholesterol should be the secondary target of AD treatment, which is a most unexpensive and easiest marker of residual vascular risk. Combined therapy with statin and fenofibrate is safe, well tolerated and reduces plasma atherogenity and CV events in patients with AD and high CV risk, type 2 diabetes or metabolic syndrome. Hlavní příčiny morbidity a mortality u polycythemie vera (PV) a esenciální trombocytemie (ET) jsou trombóza a krvácení, progrese do myelofibrózy a přechod do akutní leukemie. Myelosupresivní léčba, především pak hydroxyurea, sice může četnost cévních komplikací omezit, ale u tohoto přípravku existují jisté obavy stran zvýšeného výskytu leukemické transformace. Léčba těchto poruch proto představuje náročný problém a abychom se vyhnuli neadekvátní expozici cytotoxickým léčivům na straně jedné či suboptimální léčbě na straně druhé, měli bychom dodržovat „risk-oriented” léčebnou strategii. Mezi známé rizikové faktory kardiovaskulárních příhod patří vyšší věk a předchozí trombóza, zatímco vliv nových biologických faktorů včetně leukocytózy a mutačního statutu, popř. mutační zátěže JAK2V617F je předmětem bádání. Nízkorizikoví pacienti s PV by měli být léčeni pouze flebotomií a aspirinem, zatímco vysoce rizikoví pacienti by měli rovněž podstoupit cytotoxickou léčbu. Co se týká léčby ET, u nízkorizikových pacientů jednoznačné indikace pro intervenci neexistují, nicméně vysoce rizikoví pacienti by měli podstoupit chemoterapii. U vybraných pacientů včetně pacientů s rezistencí nebo intolerancí na hydroxyureu se mohou uplatnit i jiné léčebné možnosti, jako např. interferon alfa či anagrelid., Major causes of morbidity and mortality in polycythemia vera (PV) and essential thrombocythemia (ET) are represented by thrombosis and bleeding, progression to myelofibrosis and transformation to acute leukemia. Myelosuppressive therapy, preferentially with hydroxyurea, can reduce the rate of vascular complications, but there is some concern about an increased rate of leukemic transformation with this agent. Therefore, the management of these disorders poses a significant challenge, and a risk-oriented therapeutic approach should be followed to avoid inappropriate exposure to cytotoxic drugs on one side or suboptimal treatment on the other. Established risk factors for cardiovascular events are represented by older age and previous thrombosis, while the impact of novel biological factors, including leukocytosis and JAK2V617F mutational status and/or mutational burden, is under investigation. Low-risk PV patients should be managed only with phlebotomy and aspirin, while high-risk patients should also receive cytotoxic therapy. Regarding the management of ET, there is no clear indication for intervention in low-risk patients, while high-risk patients should be managed with chemotherapy. Tato práce pojednává o některých klíčových aspektech diagnostických kritérií pro MPN podle klasifikace Světové zdravotnické organizace (WHO) z roku 2008, přičemž většina poznámek se omezuje na onemocnění polycythaemia vera (PV). Při analýze hlavních kritérií poznamenáváme, že existují pacienti s PV, jejichž erytrocytóza může být dokumentována pouze měřením objemu erytrocytární masy a nikoli měřením koncentrace hemoglobinu nebo hematokritu. Probíráme reprodukovatelnost histopatologie pro diagnostiku různých stadií MPN, existenci normálních hladin EPO u pacientů s PV, specifitu růstu endogenních erytroidních kolonií (EEC) pro diagnózu PV a naše současné znalosti týkající se cytogenetiky a molekulární biologie PV a jiných MPN a jejich patofyziologie. Rovněž analyzujeme různé prognostické ukazatele morbidity (trombóza) a mortality u PV – vysokou hladinu hematokritu, leukocytózu a procentuální zastoupení buněk s mutací JAK2V617F. V případě esenciální trombocytemie (ET) komentujeme rozlišení pravé ET a “časné PMF”. V závěru stručně shrnujeme obecně přijímaná doporučení European LeukemiaNet (ELN) pro léčbu Ph-negativní MPN na základě nové stratifikace rizika, na důkazech založené riziko kancerogeneze u hydroxyurey a racionální doporučení pro výběr konkrétních léků., In this paper, we discuss some critical parts of the World Health Organization (WHO) 2008 criteria for classification of MPNs classification, with most of the remarks limited to polycythemia vera (PV). When analyzing the major criteria, we note that there are PV patients whose erythrocytosis can only be documented by red cell mass measurements and not by hemoglobin concentration or hematocrit measurements. We discuss the reproducibility of histopathology for the diagnosis of different stages of MPNs, the existence of normal EPO levels in PV patients, the specificity of Endogenous erythroid colony (EEC) growth for PV diagnosis and our current knowledge of the cytogenetics and molecular biology of PV and other MPNs and their pathophysiology. We also analyze various prognostic morbidity (thrombosis) and mortality markers in PV – high hematocrit level, leukocytosis and percentage of cells with the JAK2V617F mutation. In essential thrombocythemia (ET), we comment on the distinction of true ET and “early PMF”. Pankreatoduodenektomie (PD) je výkonem volby v léčbě resekabilních periampulárních a pankreatických karcinomů a u některých pacientů s chronickou pankreatitidou. PD je jedním z nejnáročnějších GI/ HPB chirurgických výkonů, který zahrnuje resekci několika orgánů a rekonstrukci s několika anastomózami. Přestože mortalita se snížila na méně než 5 %, morbidita je neustále vysoká. Pacienti podstupující PD jsou většinou starší s přidruženými komorbiditami– všeobecné komplikace po takto náročném výkonu, např. ranné, hrudní, kardiální a venózní trombembolické jsou časté. Hlavní peroperační morbiditou PD je krvácení, které může být z vícero míst, jako například z lůžka žlučníku, choledochálních žil, gastrokolického trunku, pankreatoduodenálních žil, jejunálních žil, uncinátních žil a z resekované plochy pankreatu. Při preparaci ductus choledochus může dojít k poranění aberantní arteria hepatica dextra (z arteria mesenterica superior). Pankreatický únik je definován jako přítomnost tekutiny bohaté na amylázu (>3x sérová amyláza) ve drénech po dobu delší než 3 dny. Rozděluje se na typy A (bez klinického dopadu), B (perzistující, infikovaná) a C (systémová sepse, reoperace). Vyšetřením volby je počítačová tomografie; lokalizovaná kolekce se může perkutánně zadrénovat. Velký únik si vyžaduje revizi s provedením laváže a drenáže; neměla by se pokoušet korekce či předělání anastomózy. Měkký pankreas a nedilatovaný vývod jsou dva nejdůležitější rizikové faktory pro únik z pankreatické anastomózy; role oktreotidu v prevenci úniku je diskutabilní. Předoperační drenáž žlučových cest použitím endoskopicky zavedeného stentu může snížit krvácivé komplikace regulací koagulopatie, ale zvyšuje riziko infekčních komplikací. Pooperační krvácení může být časné nebo pozdní a intraluminální nebo intraabdominální. Časné intraabdominální krvácení má chirurgickou příčinu – buď jde o uvolněnou ligaturu např. z gastroduodenální arterie, nebo jedné z žil, nebo pankreatoduodenálního lůžka; měla by se provést revize a ošetření krvácení pokud je závažné. Časné intraluminální krvácení bývá z jedné z anastomóz- nejčastěji pankreatické. UGIE vylučuje krvácení z gastroduodenální anastomózy (pokud se vyskytne, lze řešit endoskopicky). Krvácení z pankreatického pahýlu se řeší reoperací − snesením přední vrstvy anastomózy nebo provedením jejunotomie. Pozdní intraabdominální krvácení bývá z pseudoaneuryzmatu tepny, obvykle gastroduodenální, způsobeného erozí abscesem sekundárně z úniku; léčbou volby je angioembolizace. Pozdní intraluminální krvácení bývá způsobené stresovým vředem při systémové sepsi, většinou sekundárně po úniku a intraabdominální sepsi. Opožděné vyprazdňování žaludku a akutní pankreatitida jsou dalšími častými komplikacemi. Reoperace po PD jsou četné, nejčastější příčinou je krvácení, poté únik; mortalita po revizi je vysoká. Mezi dlouhodobé komplikace patří stenóza anastomózy (PJ/ HJ), endokrinní a exokrinní nedostatečnost, neschopnost přibrat na váze a špatná kvalita života. Klíčová slova: pankreatoduodenektomie – komplikace, Pancreato-duodenectomy (PD) is the procedure of choice for management of resectable periampullary and pancreatic cancers and some patients with chronic pancreatitis. PD is one of the most major GI/ HPB surgical procedures performed involving resection of multiple organs and reconstruction with multiple anastomoses. While mortality of PD has been brought down to less than 5% morbidity still remains high. Patients undergoing PD are usually elderly with comorbidities – general complications of a major operation e.g. wound, chest, cardiac and venous thrombo-embolism, are common. The major intra-operative morbidity of PD is bleeding which can be from multiple sites viz. gall bladder bed, choledochal veins, gastro-colic trunk, pancreato-duodenal veins, jejunal veins, uncinate veins and cut surface of pancreas. An aberrant right hepatic artery (from the superior mesenteric artery) can be injured while dissecting the common bile duct. Pancreatic leak is defined as presence of amylase rich (>3x serum amylase) fluid in drain beyond 3 days. It is graded as A (no clinical impact), B (persists, infected) or C (systemic sepsis, reoperation). Investigation of choice is computed tomography; a localized collection can be drained percutaneously. Major leak requires reexploration at which lavage and drainage should be performed; no attempt should be made to repair or redo the anastomosis. Soft pancreas and undilated duct are the two most important risk factors for pancreatic anastomotic leak; role of octreotide to prevent the leak is debatable. Preoperative biliary drainage in the form of endoscopic stenting may reduce bleeding complications by controlling coagulopathy but increases the risk of infective complications. Postoperative bleed can be early or delayed and intra-luminal or intra-abdominal. Early intra-abdominal bleed is surgical – either a slipped ligature e.g. of the gastro-duodenal artery or one of the veins or from the pancreato-duodenal bed; reexploration should be done to control it if it is severe. Early intra-luminal bleed is from one of the anastomoses – commonest being the pancreatic. UGIE rules out bleed from the gastro/duodenal anastomosis (which if present, can be controlled endoscopically). Control of pancreatic stump bleed requires reoperation - taking down anterior layer of the anastomosis or a jejunotomy. Delayed intra-abdominal bleed is from a pseudo-aneurysm of an artery, usually gastro-duodenal caused by erosion by an abscess secondary to a leak; treatment of choice is angio-embolization. Delayed intra-luminal bleed is because of stress ulcers caused by systemic sepsis usually secondary to a leak and intra-abdominal sepsis. Other common complications include delayed gastric emptying and acute pancreatitis. Reoperations after PD are frequent – commonest cause is bleed, followed by leak; mortality of reexploration is high. Long term complications include anastomotic (PJ/ HJ) stricture, endocrine and exocrine insufficiency and inability to regain weight and poor quality of life. Concha bullosa (CB) is among the most common anatomic variations of sinonasal anatomy. Although usually asymptomatic, CB can occasionally cause nasal obstruction or headache. Obstructions within the mucociliary transport system can develop into a mucocele or mucopyocele. A 48-year-old female, with a history of progressive headache and nasal obstruction, was referred to our department. Paranasal sinus tomography revealed a nasal mass in the left nasal cavity resembling a mucopyocele in the middle turbinate. Under general anesthesia, the purulent material was aspirated, and the lateral part of the left turbinate was resected. Mucopyoceles are common within the paranasal sinuses, but uncommon with CB; thus, they should be considered in patients with a large hyperemic nasal mass. and K. Sari, Z. K. Gencer, Y. Kantekin
The authors demonstrate the possibility of improving surgical results by the reduction of perioperative bleeding in thoracic surgery associated with extended resection procedures. We focused on patients in whom the expected perioperative blood loss was greater than 500 ml. The first group consisted of patients with lung cancer stage III A after neoadjuvant chemotherapy had been indicated to extend the resection procedure. The second group consisted of patients with chest wall and mediastinum tumors of various etiologies. The third group consisted of patients with post-inflammatory thoracic complications in whom combined decortication and pleurectomy was necessary. Tuberkulózní postižení střev je v rozvinutých zemích vzácné onemocnění, které postihuje zejména imigranty a imunokompromitované pacienty. Klinický obraz může imitovat nitrobřišní infekce, nádorová onemocnění či Crohnovu chorobu. Diferenciální diagnostika Crohnovy choroby a břišní tuberkulózy může být pro klinika a patologa svízelná, v obou případech se jedná o chronický granulomatózní zánět s podobnou symptomatologií. Prezentujeme případ 53letého pacienta s perforační peritonitidou při tuberkulóze střeva, u kterého byla původně mylně diagnostikována Crohnova choroba. Průběh choroby, rozsah a povaha nálezu byly v každé fázi léčby příčinou kritických komplikací. S touto zkušeností předáváme i doporučení: břišní tuberkulózu zvažovat v diferenciálně diagnostické rozvaze každého chronického střevního onemocnění. Clinical presentation may mimic many other abdominal diseases such as intra-abdominal infections, tumors, and Crohn´s disease. Differential diagnosis of Crohn´s disease and intestinal tuberculosis poses a dilemma to clinicians and pathologists as both are chronic granulomatous disorders with similar clinical features. We report the case of a 53 year-old man presenting with perforated intestinal tuberculosis in which the initial diagnostic work-up suggested Crohn´s disease. The severity of the findings resulted in critical complications during the course of treatment. This case points out the need for awareness of intestinal tuberculosis in the differential diagnosis of chronic intestinal disease. Crohn's disease is a chronic immune-mediated intestinal inflammation targeted against a yet incompletely defined subset of commensal gut microbiota and occurs on the background of a genetic predisposition under the influence of environmental factors. Genome-wide association studies have identified about 70 genetic risk loci associated with Crohn's disease. The greatest risk for Crohn's disease represent polymorphisms affecting the CARD15 gene encoding nucleotide-binding oligomerization domain 2 (NOD2) which is an intracellular sensor for muramyl dipeptide, a peptidoglycan constituent of bacterial cell wall. The accumulated evidence suggests that gut microbiota represent an essential, perhaps a central factor in the induction and maintaining of Crohn's disease where dysregulation of normal co-evolved homeostatic relationships between intestinal microbiota and host mucosal immune system leads to intestinal inflammation. Taken together, these findings identify Crohn's disease as a syndrome of overlapping phenotypes that involves variable influences of genetic and environmental factors. A deeper understanding of different genetic abnormalities underlying Crohn's disease together with the identification of beneficial and harmful components of gut microbiota and their interactions are essential conditions for the categorization of Crohn's disease patients, which enable us to design more effective, preferably causative, individually tailored therapy. and L.
