Karcinom prsu je nejčastější nádorové onemocnění u žen. Každoročně postihuje více než 1 000 000 žen na celém světě. Z toho zhruba 11 % případů rakoviny prsu lze připisovat konzumaci alkoholu u žen. Cílem tohoto článku je vytvořit představu o podílu nákladů na léčbu karcinomu prsu, který lze přiřadit konzumaci alkoholu v ČR v roce 2007. Zjištěná výsledná částka, kterou je třeba vynaložit na léčbu pacientek pravidelně konzumujících alkohol, je 39 mil. Kč., Breast cancer is the most common cancer in women. More than 60 neuropeptides that inhibit juvenile hormone synthesis by the corpora allata have been isolated from the brains of various insect species. Most of them are characterized by a common C-terminal pentapeptide sequence Y/FXFGL/I/V (allatostatin A family, allatostatin superfamily). Besides the allatostatin A family, allatostatic neuropeptides belonging to other two peptide families (W2W9)-allatostatins or allatostatin B family; lepidopteran allatostatin) were reported. Four peptides with allatostatic activity were isolated from brains of the Mediterranean field cricket, Gryllus bimaculatus. Three of them (Grb-AST A3: AGMYSFGL-NH2; Grb-AST A4: SRPFGFGL-NH2; Grb-AST A5: GPDHRFAFGL-NH2) belong to the wide-spread family of Y/FXFGL/I-amide peptides, the fourth (Grb-AST B5: AWDQLRPGW-NH2) is a member of the W2W9 - amide family of neuropeptides. All of these peptides are potent inhibitors of juvenile hormone (JH) biosynthesis by cricket corpora allata in vitro, causing 50% inhibition of JH biosynthesis at 0.4-3 × 10-8 M. The two peptides Grb-AST A5 and Grb-AST B5 have virtually the same potency and efficacy in inhibiting JH biosynthesis in vitro. No synergistic effect of the two peptide families with respect to the inhibition of JH biosynthesis could be observed. Peptides of both families decrease the accumulation of methylfarnesoate, the direct precursor of JH, within CA that have been incubated in farnesol-rich medium. Allozyme polymorphism was investigated in adult males of the stenotopic peat bog (tyrphobiontic) noctuid moth, Coenophila subrosea Stephens, from three isolated peat bog localities in Austria (Styria) and Czech Republic (South and North Bohemia). Of the eighteen enzyme loci examined, twelve were polymorphic and six monomorphic. Significant deviations of genotype frequencies from Hardy-Weinberg equilibrium were observed at about one third of polymorphic loci within the populations. The average heterozygosities for the populations from three geographically distinct localities ranged from 0.192 to 0.245, and 61% of the loci were polymorphic. The FST mean value of 0.0675 was higher than that found in most other Lepidoptera. The genetic distances based on allozyme heterozygosity ranged from 0.019 to 0.051, with the population from South Bohemia being the most distant. The genetic distances and FST values do not reflect the geographic distances between the populations. Morphometric analysis revealed a difference between the Austrian Purgschachen Moor and Bohemian populations. Úvod: Alveolární echinokokóza je vzácné a závažné zoonotické parazitární onemocnění. Kazuistika: Autoři demonstrují případ mladého nemocného s postižením jater, bránice a plic touto formou infekce. Diagnóza echinokokové infekce byla stanovena na základě anamnézy, klinické symptomatologie v kombinaci s USG, CT, MRI a sérologickými metodami. Byla provedena radikální bloková resekce 7. jaterního segmentu, dolního plicního laloku vpravo a bránice. Definitivní stanovení diagnózy alveolární echinokokózy bylo provedeno histopatologicky a pomocí PCR metody ze vzorku resekované tkáně. Závěr: Nemocný je 8 měsíců po operaci bez potíž, trvale dispenzarizován s long-life terapií albendazolem., Introduction: Alveolar echinococcosis is a life-threatening zoonotic parasitic disease. Its incidence is rare. In some cases, the correct and timely diagnosis can be difficult. Case report: The authors present the case of a young patient with liver, diaphragm and lung involvement. The suspicion of echinococcus infection was made on the basis of medical history, clinical symptoms, and a combination of ultrasonography, computed tomography, magnetic resonance imaging tests and serological methods. The patient underwent multimodal treatment with albendazole and en-bloc resection of the liver, lung and diaphragm. The definitive diagnosis of alveolar echinococcosis was determined from samples of the resected tissues using histopathology and polymerase chain reaction methods. The patient has been followed regularly and is on life-long treatment with albendazole. Alzheimerova choroba (ACH) je nejčastější forma demence. Jedná se o degenerativní, nevyléčitelné, terminální onemocnění. Postihuje až 75 % nemocných na celém světě postižených demencí. Prevalence tohoto onemocnění je vysoká, etiologie stále není známá. Je sledována řada rizikových faktorů. Pilotní studie projektu Epidemiologie a genetika Alzheimerovy choroby s 334 případy a 102 kontrolami z psychiatrických léčeben přináší následující výsledky z dotazníkového šetření a stěru z bukální sliznice. Dotazník se skládá z otázek zaměřených na základní identifikační údaje, vzdělání, životní styl, vaskulární rizikové faktory, zájmy, zaměstnání, výskyt nemoci v rodině, výsledky Minimental State Examination (MMSE). Dále jsou uvedeny výsledky analýzy polymorfizmů genu pro angiotenzinkonvertázu (ACE). Ve výsledcích je naznačeno, že u pacientů s ACH se v anamnéze častěji vyskytuje kardiovaskulární onemocnění a úraz hlavy. Výsledky analýz vztahu polymorfizmu I/D genu pro ACE k Alzheimerově chorobě odpovídají dosud publikovaným výsledkům, tzn., že genotypy II a ID mají vztah k riziku ACH, i když zatím nebyl zjištěn statisticky významný rozdíl ve sledovaných skupinách respondentů., Alzheimer's disease (AD) is the most common form of dementia. It is a degenerative incurable terminal disease. AD affects a high percentage (75%) of persons with dementia all over the world. Prevalence is very high. Etiology is still unknown. Numerous etiological factors of AD have already been discovered. Pilot evaluation of 334 cases and 102 controls from mental hospitals in project Epidemiology and Genetics of Alzheimer's Disease provided following the results from questionaires and buccal mucosa smears. The questionnaires contain questions about identification, education, lifestyle, vascular risk factors, hobbies, occupation, incidence of the disease in the family, results of Minimental State Examination (MMSE).Results of the analysis of the gene of I/D polymorphism of ACE are presented. Results suggest that persons with AD have often a cardiovascular disease and head injury in their case history. Žilní tromboembolická nemoc (TEN), zahrnující žilní trombózu stejně jako plicní embolii, je časté a potencionálně fatální onemocnění. S uvedením nízkomolekulárních heparinů (LMWH) do praxe bylo prokázáno, že ambulantní léčba žilní trombózy je efektivní a bezpečná pro velkou část nemocných s TEN. Vzrůstající data o ambulantní léčbě LMWH v posledních letech ukazují, že až 50 % pacientů s klinicky stabilní plicní embolií může být léčeno doma. I přes tato fakta dosud neměla domácí léčba plicní embolie pevnou pozici v běžné praxi. Pokud bychom měli sumarizovat podmínky pro domácí léčbu, potom by bylo možné uvažovat o ambulantní léčbě u nemocných s nízkým rizikem dle pomocných kritérií, bez hemodynamické nestability (především bez šokového stavu), bez selhávání pravé komory, bez předchozího chronického srdečního či plicního onemocnění, bez závažných komorbidit (onemocnění gastrointestinálního traktu, onemocnění ledvin, krevní choroby, pokročilá nádorová onemocnění), s nízkým rizikem časné rekurence tromboembolické nemoci, bez jiných indikací pro hospitalizaci (bolest vyžadující parenterální analgetika, infekce apod), s nízkým rizikem krvácení a s garantovanou spoluprací nemocného a dobře organizovanou domácí péčí., Venous thromboembolic disease which includes both venous thrombosis and pulmonary embolism, is a frequent and potentially fatal disease. Based on the introduction of low-molecular-weight heparins (LMWH) into practice it has been proved that outpatient treatment of venous thrombosis is effective and safe for a large number of patients with VTE. The growing volume of data on LMWH outpatient treatment in recent years shows that up to 50 % of patients with clinically stable pulmonary embolism can be treated at home. In spite of these facts home treatment of pulmonary embolism has not been established as part of common practice as yet. If we were to summarize the conditions for home treatment, we would consider outpatient care for patients at low risk based on auxiliary criteria, free from hemodynamic instability (primarily without a shock state), free from right ventricular failure, prior chronic heart or lung disease, serious comorbidities (gastrointestinal tract disease, kidney disease, blood diseases, advanced cancers), at low risk of early thromboembolism recurrence, free from other indications for hospitalization (pain requiring parenteral analgesics, infections etc.), at low risk of bleeding and with guaranteed patient‘s cooperation and well-organized home care., and Radovan Malý, Jaroslav Malý
Clinical oncologists have been focusing their efforts on attempting to define risk groups of patients with unusual biological reactions to the recommended therapy regimens using molecular biology techniques. The aims of our study were: (i) to find a design and validate a method for fast and reliable analysis of the D1853N (5557G>A) genetic polymorphism in the ATM (ataxia-telangiectasia mutated) gene; (ii) to use side-directed mutagenesis to generate ATM 5557A-positive DNA (reference ATM5557A DNA); and (iii) to analyze a group of patients suffering from cervical carcinoma with adverse responses to radiotherapy. The 5557A variant was found in three of twenty women (15%). Our data show that the prevalence of the 5557A allelic variant in cervical cancer subjects with adverse responses after irradiation probably does not differ from the prevalence common in Caucasians. Je předložena první část plánovaného souboru malých prací, určených pro tento časopis. Práce se týká věcí, podle našeho názoru důležitých pro analytickou kvalitu v klinické laboratoři. Celý soubor prací se bude zabývat tématy metrologie (charakteru předmětů měření), statistikou, vnitřní kontrolou kvality, externím hodnocením kvality a kritickými postřehy v uvedených oblastech. Příspěvky jsou napsány s maximální snahou po jednoduchosti podle konceptu ASAP-„as simple as possible“. V prvním příspěvku byl aplikován tento přístup pro odvození požadavků na analytickou kvalitu z hodnot biologických variabilit: je diskutována situace při měření koncentrace Na v krevním séru. Analytická kvalita tohoto měření je akceptovatelná, pokud dlouhodobě sledované výsledky vykazují stabilitu v rozmezí 1 %. The articles touch, what we think, is important about analytical quality in the medical laboratory The articles will address topics such as metrology (philosophy of the measurand), statistics, internal quality control (IQC), external quality assessment (EQA), critiques to the discipline. All contributions follow the ASAP-concept, meaning As Simple As Possible. Here, we explore the „ASAP“ concept for analytical quality specifications derived from biological variation. The application of the concept results for the great majority of the mainstream serum-, plasma-, and blood-analytes in CVa values within boundaries of 0.4 – 15 % and for Delta-SE within boundaries of 0.2 – 10 %. The take-home message is: analytical quality in the medical laboratory must NOT be viewed in absolute terms („a CVa of 2 % is good“), BUT in relative terms.
