An unusual combination of median nerve's variations has been encountered in a male cadaver during routine educational dissection. In particular, the median nerve was formed by five roots; three roots originated from the lateral cord of the brachial plexus joined individually the median nerve's medial root. The latter (fourth) root was united with the lateral (fifth) root of the median nerve forming the median nerve distally in the upper arm and not the axilla as usually. In addition, the median nerve was situated medial to the brachial artery. BACKGROUND: Occurrence of gastric metastasis as the first symptom of breast carcinoma with a long period of latency before presentation of the primary breast carcinoma is rare. CASE REPORT: A patient with gastric metastasis as the first symptom of lobular breast carcinoma, treated by neoadjuvant preoperative chemoradiotherapy and total gastrectomy, with complete local control. Fourteen months after presentation of the gastric metastasis a primary lobular breast carcinoma was discovered, treated by radiotherapy, chemotherapy and hormonal treatment with complete local response. Twenty-three months after diagnosis of breast cancer multiple colorectal metastases from the breast cancer occurred, which were treated by chemotherapy and hormonal treatment. Eighty-six months after diagnosis of gastric metastasis the patient died due to progression of cancer. CONCLUSIONS: Metastases to gastrointestinal or gynaecological tracts are more likely in invasive lobular carcinoma than invasive ductal cancer. The pathologist should determine whether or not they check estrogen and progesterone receptor status not simply by signet ring cell morphology but also by consideration of clinic-pathological correlation of the patient, such as the presence of a past history of breast cancer, or the colorectal localization of poorly differentiated carcinoma, which may occur less frequently than in the stomach. and D. Endometrióza znamená přítomnost tkáně endometria mimo dutinu děložní. Hlubokou infiltrující endometriózu (deep infiltrating endometriosis – DIE) představují rektovaginální léze, infiltrující onemocnění střev, močového měchýře, močovodu a DIE se vzácně objevuje i v dalších lokalizacích. Negynekologické komplikace DIE rozdělujeme do dvou hlavních kategorií: samotné onemocnění zahrnuje pánevní a břišní bolest, střevní striktury, opakující se krvácení z konečníku, dyschezii, chronickou anémii, dysurii, hematurii, stenózu ureterů, ztrátu ledvinných funkcí a komplikace léčby, ty jsou charakterizovány vznikem fistul, dysfunkcí močového měchýře a opakujícími se záněty urinárního traktu. DIE přináší riziko pro vznik malignit zasažených orgánů. DIE může být léčena konzervativně, nebo chirurgicky. Předkládaná práce je přehledem gastrointestinálních a urologických komplikací DIE a předkládá také kazuistiku pacientky centra pro léčbu endometriózy v nemocnici ve Znojmě (ČR)., Endometriosis is the presence of endometrial tissue outside the endometrial cavity. Deep infiltrating endometriosis (DIE) includes rectovaginal lesions as well as infiltrative disease to the bowel, urinary bladder, ureter and other rare locations. Non gynecological complications of DIE can be divided into two major categories: complications of the disease itself which include pelvic and abdominal pain, bowel stricture, cyclic rectal bleeding, dyschezia, chronic anemia, dysuria, hematuria, ureteral stenosis as well as loss of kidney function and complications of the treatment which include fistula formation, bladder void dysfunction and recurrent urinary tract infections. DIE has potential for malignant transformation in the affected organ. DIE may be managed conservatively or surgically. However definitive treatment is surgical. Hypertrofická kardiomyopatie (HCM) je nejčastější geneticky podmíněné srdeční onemocnění s prevalencí 1 : 500. Ve familiárních případech je dědičnost autozomálně dominantní s neúplnou penetrancí a různou expresivitou, může ovšem vznikat i de novo mutacemi. Nesmírná heterogenita projevů i prognózy HCM případ od případu značně komplikuje klinický management, a proto toto onemocnění stále často přináší mnoho otázek nejen pro lékaře prvního kontaktu, ale i pro kardiology. Stanovení rizika náhlé srdeční smrti u pacientů s HCM je integrální součástí klinického managementu a doporučení pro stratifikaci se neustále vyvíjí. V této kazuistice prezentujeme případ familiární HCM u dvou bratrů nesoucích stejnou mutaci s velmi rozdílným klinickým nálezem a průběhem., Hypertrophic cardiomyopathy (HCM) is the most common genetic cardiac disease with a prevalence of 1 : 500. In familial cases the inheritance is autosomal dominant with non‑complete penetrance and variable expression; however, it can also be caused by de novo mutations. The heterogeneity of both its presentation and prognosis from case to case largely complicates clinical management, and therefore the disease often represents a dilemma for primary care clinicians as well as cardiologists. An estimation of sudden cardiac death risk is an integral part of clinical management and the stratification guidelines are continuously developing. Aneuryzma břišní aorty (AAA) je závažné onemocnění s vyšší prevalencí v pokročilém věku. Výdutě šíře nad 5,5 cm jsou indikovány k operačnímu nebo endovaskulárnímu řešení. Zřídka se dnes pro velké riziko jejich ruptury setkáváme s rozsáhlými výdutěmi. Autoři na dvou předložených kazuistikách představují širokou diferenciální diagnostiku granulomatózních procesů hepatobiliárního traktu. Izolovaný granulomatózní zánět v této oblasti nacházíme velmi zřídka. Diferenciální diagnostika těchto procesů zasahuje od infekční etiologie až po systémová onemocnění či chemickou iritaci. Granulomy často imitují maligní tumory v jaterní krajině, a chirurg tak může být lékařem, který dané onemocnění odhalí jako první při resekčním výkonu či cholecystektomii., The authors present two case reports to demonstrate wide differential diagnosis of granulomatous disorders of the hepatobiliary tract. Pro hemodialyzované pacienty, u kterých byl diagnostikován diferencovaný karcinom štítné žlázy, může mít zásadní význam léčba 131I. Diagnóza nádorového onemocnění tyto pacienty vyřazuje z registru čekatelů pro transplantaci ledviny do doby, než bude prokázána 3-5letá remise. Jelikož je 131I vylučován z těla převážně ledvinami, je efektivní poločas 131I u pacientů s renálním selháním prodloužen. To je důvod, proč by měla být redukována podávaná léčebná aktivita, aby došlo ke snížení radiační zátěže především pro krvetvornou kostní dřeň. Prezentujeme kazuistiku 48letého muže v terminálním renálním selhání, léčeného 131I pro papilární karcinom štítné žlázy. Terapeutické aktivity jsme snížili o 30 %. Hemodialýza byla prováděna po podání léčebné aktivity 131I na lůžkové části kliniky nukleární medicíny. 131I je dialyzovatelný a po každé dialýze klesla zbytková aktivita o 33-67 %. Radiační zátěž personálu hemodialyzačního centra byla minimální. Vnitřní břišní hernie jsou méně častou chirurgickou diagnózou, mnohdy zjištěnou jako náhodný peroperační nález. Ileózní stavy na podkladě těchto hernií jsou pak raritními případy. V našem článku prezentujeme případ 69leté zdravé ženy, operované na základě chybně interpretovaného CT nálezu pro suspektní volvulus žaludku, kde pravou příčinou byla peroperačně zjištěná herniace pravé části kolon do bursy omentalis s dilatací céka a známkami nekrotizace stěny střevní., Internal abdominal hernias present an infrequent surgical diagnosis and are usually encountered accidentally during surgery. Heterotopická mezenteriálna osifikácia (HMO) je zriedkavé ochorenie podmieňujúce patologickú tvorbu kostného tkaniva v dutine brušnej po opakovaných operačných traumách. Autori prezentujú neobvyklý prípad vzniku mezenteriálnej osifikácie u 30ročného muža po operácii pre ileózny stav na podklade tenkočrevnej inkarcerácie. Po prepustení do domáceho liečenia musel byť znovu hospitalizovaný a reoperovaný. V priebehu 6 týždňov bol ešte 3krát reoperovaný pre tenkočrevný ileózny stav a výraznú produkciu z entero-kutánnej fistuly. Napriek intenzívnej parenterálnej hyperalimentácii sa stav pacienta nezlepšil, metabolický rozvrat sa stal pacientovi osudovým. Patofyziológia mechanizmu formovania kostného tkaniva v mäkkých tkanivách nie je známa. The authors present an unusual case of mesenteric ossification formation in a 30 year-old man after surgery due to status ileosus on the basis of intestinal incarceration. After being released into home treatment the patient had to be hospitalized again and reoperated. During the next six weeks, the patient was reoperated three more times due to intestinal status ileosus and significant leaking of the entero-cutaneous fistula. Despite intensive parenteral hyperalimentation the patient‘s condition did not improve and metabolic breakdown became fatal for the patient. Pathophysiology of the mechanism of bone tissue formation in soft tissues is unknown. V léčbě uveálních melanomů se v některých případech používá radiochirurgie CyberKnifem. Histologickými a imunohistologickými změnami v nádorové tkáni po terapii se zabývá poměrně málo autorů. V tomto sdělení uvádíme výsledky vyšetření dvou takových případů. U obou pacientů byla provedena enukleace bulbu pro bolestivý glaukom. U prvého pacienta šlo o recidivující melanom, který byl ošetřen gama nožem před deseti lety. Pro lehkou progresi ložiska melanomu bylo provedeno ošetření CyberKnifem celkovými dávkami 45Gy ve 3 frakcích. Po necelých pěti měsících po ozáření bulbu došlo k regresi recidivujícího nádoru z původní prominence 1,4 mm v ploše 6 mm × 0,6 mm na prominenci 0,3 mm v ploše 3 mm × 0,3 mm. Histologicky byla zaznamenána rezidua řídce celulárního ložiska regresivně změněných nádorových buněk a přítomnost reaktivních změn optického nervu. Druhý pacient byl měsíc po radioterapii CyberKnifem s následnou komplikovanou operací katarakty a s enukleací bulbu bez regrese – tedy zániku nádorových buněk. Velikost tumoru 11,1 mm × 15mm s prominencí 9,3mm přetrvávala i po ozáření dávkou 40Gy/1Fr. V histologickém vyšetření byly nádorové buňky bez patologických změn. V imunohistochemickém vyšetření jsme pozorovali u prvého pacienta vysokou difuzní expresi proteinu bcl-2 až do výše 75 % a méně než 1% pozitivitu ve vyšetření s proliferačním markerem Ki-67. U druhého případu byla situace odlišná, s výraznou až 75% pozitivní expresí bcl-2 proteinu v nádorových buňkách v povrchových vrstvách tumoru a s klesajícím počtem pozitivní exprese v hlubokých vrstvách nádoru, kde dosahovala maxima do 25 %. Proliferační aktivita s pozitivní jadernou expresí v Ki-67 vzrůstala od povrchu, kde se pohybovala v rozmezí 1–3 % do hloubky nádoru a k jeho bázi, kde dosahovala až 5 %. Naše pozorování ukazuje na výraznou redistribuci genetické aktivity uvnitř nádoru. Ta se projevuje „overexpresí“ bcl-2 proteinu a u druhého případu je patrná rozdílná pozitivita bcl-2 proteinu v různých vrstvách nádoru. Vztah tohoto fenomenu k radioterapii není dosud jasný., Radiosurgery with CyberKnife is used in the treatment of uveal melanomas in some cases. Histological and immunohistological changes in tumour tissue following treatment have been addressed by relatively few authors. This paper presents the results of investigations of two such cases. Enucleation of the eye was performed for painful glaucoma in both patients. Patient 1 had a recurrent melanoma treated with a Gamma Knife ten years ago. Due to slight progression in the melanoma site, treatment with CyberKnife was used with total doses of 45 Gy in 3 fractions. Some five months following eye irradiation, there was regression of the recurrent tumour from the original height of 1.4 mm in an area of 6 mm x 0.6 mm to that of 0.3 mm in an area of 3 mm x 0.3 mm. Histologically, residua of a sparsely cellular locus of regressively altered tumour cells and the presence of reactive changes in the optic nerve were noted. Patient 2 underwent radiotherapy with CyberKnife one month prior, with subsequent complicated cataract surgery and enucleation of the eye without regression, i.e. destruction of tumour cells. The size of the tumour of 11.1 mm x 15 mm with a height of 9.3 mm remained unchanged even after irradiation with a dose of 40 Gy/1 fr. No pathological changes in the tumour cells were seen on histological examination. In patient 1, immunohistochemistry showed high diffusion expression of the bcl-2 protein of up to 75 % and less than 1 % positivity on examination with the proliferation marker Ki-67. In patient 2, the situation was different, with significant, as high as 75 % positive expression of the bcl-2 protein in tumour cells in the superficial tumour layers and a decreasing rate of positive expression in the deep tumour layers where it reached no more than 25 %. The proliferation activity with positive nuclear expression of Ki-67 increased from the surface, where it ranged from 1 % to 3 %, to the depth of the tumour and toward its base, where it reached up to 5 %. Our observation suggests significant redistribution of genetic activity within the tumour. It is manifested by overexpression of the bcl-2 protein and, in patient 2, different bcl-2 protein positivity in the various tumour layers is apparent.
