INTRODUCTION: Leptin is an adipokine which has a direct relationship to obesity. Our aim was to measure this hormone in pregnant women at three months intervals throughout their pregnancies to determine the serum value of those who developed preeclampsia. MATERIAL AND METHODS: We followed 19 women (median age 24.8 +/- 5.7 years) with pre-gestational Body Mass Index (BMI) less than 25 kg/m2, 21 (median age 26.1 +/- 4.6 years) with BMI higher than 25 kg/m2 and 16 (median age 30.9 +/- 5.8 years) with Gestational Diabetes Mellitus (GDM) (median age 30.9 +/- 5.8 years), recruited in the 1st trimester of pregnancy. Serum levels of leptin were measured with radioimmunoassay (RIA) technique. RESULTS: In the first trimester of pregnancy leptin levels showed statistically significant differences between normal weight and overweight-obese women (p < 0.001), diabetic women (p < 0.05) and the subgroup of preeclamptic women (p < 0.001). For those women with PGBMI > or = 40 kg/m2 and leptin > or = 40 ng/ml in the second trimester, the Odds Ratio (OR) to develop preeclampsia was of 47.95% CI (4.1-527.2). Analyzing leptin values with ROC curves, the greatest area under the curve (AUC) was for leptin in the second trimester (0.773, CI: 0.634-0.911). CONCLUSION: Women with morbid obesity (BMI > or = 40 kg/m2) had significantly higher levels of serum leptin (p < 0.01) and a value of 40 ng/ml of this hormone seems to be predictive of developing preeclampsia in this group of patients. and H. Karcinom jícnu je onemocnění s 90% mortalitou, ale část pacientů s lokálně pokročilým onemocněním může dlouhodobě přežívat díky multidisciplinární léčbě. Dokladem je tato kazuistika 40letého muže s dlaždicovým karcinomem středního jícnu prorůstajícím do dýchacích cest ve stadiu T4N1M0. Prvním léčebným zákrokem bylo zavedení Dumon Y-stentu do dýchacích cest jako urgentní zábrana sufokace. V dalším postupu byla použita konkomitantní chemoradioterapie, s dávkou záření 66Gy/33 frakcí/47 dní normofrakcionací. Chemoterapie v kombinaci cisplatina a fluorouracil byla podávána v celkové délce 6 cyklů, s ozářením během 2. a 3. cyklu. Na CT po léčbě byla zjištěna částečná regrese nádoru, endoskopické biopsie z jícnu i průdušek byly negativní. V prvním roce po chemoradioterapii bylo nutno řešit těsnou stenózu jícnu s příznaky až úplné afagie, což se podařilo zvládnout dilatacemi. V dalším období stav nemocného komplikovaly endobronchiální granulace řešené lokálním ošetřením laserem a jejich mechanickým snesením. Opakované biopsie ukazovaly na lokální progresi tumoru endobronchiálně, takže byla indikována paliativní chemoterapie docetaxelem, která byla podávána po dobu 1 roku. Během tohoto období opakované endobronchiální biopsie neprokázaly karcinom, CT hrudníku se normalizovalo, stav pacienta se zlepšil. Posléze byl odstraněn Y-stent 25 měsíců po zavedení a ukončena chemoterapie. Při kontrole 3 roky po diagnóze byl pacient bez známek nádoru., Esophageal cancer is characterized by 90 % mortality. Nevertheless, the part of the patients suffering from locally advanced disease can survive for a long time thanks to the multidisciplinar treatment. This fact can be documenetd by this case report of 40 year old man with squamous cell carcinoma of medial oesophagus intergrowing to respiratory tract, stage T4N1M0. At the start of the treatment Dumon Y stent was introduced into airways to avoid suffocation. Then, the concurrent chemoradiotherapy was used, with the radiation dose of 66Gy/33 fractions/47 days. Chemotherapy using combinationof cisplatin and fluorouracil was administered in the total length of 6 cycles, with the radiation applied during the second and third cycle. After the treatment, CT revealed the partial regression of the tumor, while endoscopic biopsies from oesophagus and bronchial tubes being negative for malignancy. During the first year after the chemoradiotherapy the tight stenosis of the oesophagus with the total aphagia was to be solved using the repeated dilatations. During the following period, the illness course was complicated by endobronchial granulations, treated by the local laser treatment and mechanical ablation, as well. Repeated biopsies shown the local endobronchial progression of the tumor, so that the paliative chemotherapy with the docetaxel was applicated, lasting one-year. During this period, repeated endobronchial biopsies did not proved a malignancy, CT finding was normalized and the patient´s status was improved. Finally, the Y stent was removed 25 months after insertion, and the chemotherapy was ended. We report the case of a 22-year old male, a self-confessed recreational drug user who developed cardiogenic shock because of severe destruction of the aortic valve by rapidly progressive aortic valve endocarditis. The disease progression was acute; in a matter of days, the clinical manifestations were life-threatening necessitating urgent aortic valve replacement surgery. Cultivation revealed Streptococcus viridans as the microbial agent. Subsequent recovery with antibiotic treatment was without complication. This case report shows that immediately performed transoesophageal echocardiography and early consultation with a cardiac surgeon has fundamental importance in diagnosis and management of acute infective endocarditis in haemodynamically instable patients., Malkia S. Cíl. Zhodnotit možnosti hybridního zobrazení a frekvenci používaných výkonů v detekci okultního muskuloskeletálního pyogenního zánětu jako příčiny sepse. Metoda. Retrospektivně byly zhodnocena hybridní zobrazení indikovaná během 3 let k detekci příčiny septického stavu nejasného původu, vyšetření byla vyhledána mezi všemi provedenými 18F-FDG-PET/CT a SPECT/CT. Výsledky. Bylo provedeno celkem 27 vyšetření 18F-FDG-PET/CT z indikace sepse nejasného původu, kdy v šesti případech byla nalezena spondylodiscitida, pyogenní artritida ve třech případech, svalový absces ve dvou případech a infekce endoprotézy ve čtyřech případech. Muskuloskeletální příčina sepse tvořila 55,5 % případů. Bylo provedeno celkem 16 vyšetření SPECT/CT, kdy v deseti případech byly použity techneciem značené fragmenty myších protilátek a v šesti případech leukocyty značené 99mTc-HMPAO. Ve dvou případech byla prokázána akutní spondylodiscitida, v jednom případě chronická spondylitida a v jednom případě mnohočetné abscesy svalů původem v infekci endoprotézy. Muskuloskeletální příčina tvořila 25 % nálezů. Závěr. Za dobu současného používání PET/CT a SPECT/CT v detekci okultních infekcí tvoří větší díl vyšetření pomocí 18F-FDG-PET/CT, neboť nabízí vícečetné výhody před zobrazeními leukocytárními., Aim. To asses a role and frequency of indications of hybrid imaging methods in the detection of occult pyogenous musculoskeletal inflammation as a cause of the septic state of unknown origin. Method. The retrospective evaluation of the hybrid imaging indicated due a sepsis of unknown origin during three years was performed. All procedures of 18F-FDG-PET/CT and the SPECT/CT using labeled leucocytes were enrolled into the assessment. Results. 27 procedures of 18F-FDG-PET/ CT were performed due to the searching of the cause in cases of the sepsis of unknown origin; the spondylodiscitis was found in 6 cases, pyogenous arthritis in 3 cases, muscular abscesses in 2 cases and 4 cases of pyogenous infection of implanted joint prosthesis. Muskuloskeletal inflammations formed 55,5% of all findings. 16 procedures of SPECT/CT were performed, 10 using 99mTc-labeled antigranulocyte antibodies, and 6 using 99mTc-HMPAO labeled leukocytes. In 2 cases, the acute spondylodiscitis was found, one case of chronic spon-dylitis and one case of soft tissue abscesses due to the infection of endoprosthesis were found. Muskuloskeletal inflammations formed 25% of all findings. Conclusion. Cílem této pilotní studie bylo srovnat výsledky ze dvou používaných nepřímých metod pro stanovení maximální spotřeby kyslíku (odhadu VO2max) a zhodnotit jejich využitelnost pro posuzování fyzické zdatnosti v rámci primárně preventivního vyšetření. K tomuto srovnání jsme použili Cooperův 12minutový běžecký test (CT 12) na běhátku a stupňovaný zátěžový test na bicyklovém ergometru (BE) do maxima. Naše výsledky ukázaly signifikantně nižší hodnoty odhadu VO2max z CT 12 na běhátku oproti stupňovanému zátěžovému testu na BE do maxima napříč různými skupinami populace. V celkovém souboru byly tyto hodnoty v průměru o 8,6 % (p<0,001) nižší. Rozdíly v odhadech VO2max se lišily svojí mírou v jednotlivých skupinách podle trénovanosti a převažujícího typu pohybové aktivity; největší rozdíly mezi oběma testy byly u cyklistů (-12,0 %), studentů medicíny (-10,9 %) a "nevyhraněných" (-10,4 %) a u těchto skupin dosáhly rovněž statistické významnosti (p<0,01). Podstatně nižší rozdíly byly naopak u běžců (-4,6 %, statisticky nevýznamné) a ve skupině triatlonistů prakticky žádné rozdíly nebyly. Výsledky a pozorování z tohoto pilotního testování naznačují, že CT 12 na běhátku je oproti stupňovanému testu na BE do maxima podstatně náchylnější k vlivům jako trénovanost, převládající typ pohybové aktivity, motivace apod. Po celkovém zhodnocení výsledků se nám pro vyšetření fyzické zdatnosti v rámci primárně preventivního vyšetření u běžné populace s průměrnou, ale i mírně nadprůměrnou fyzickou zdatností jako vhodnější jeví stupňovaný zátěžový test na BE do maxima., The aim of this pilot study was to compare the results of two commonly used indirect methods of assessing maximum oxygen uptake (VO2max estimation) and to assess the applicability of these two methods for physical fitness assessment of an individual as part of primary preventive examination. For this comparison we selected the Cooper run/walk test (CT 12) on a treadmill and an incremental maximum test on a bicycle ergometer (BE). Our results showed significantly lower VO2max estimations gained from CT 12 on a treadmill in comparison to incremental maximum testing on a BE in the population groups examined. In total the results were lower by about 8.6% on average (p<0.001). The differences varied in the groups according to how trained the subjects were and according to the prevailing type of their physical activity. The highest differences between the two tests appeared in the group of cyclists (-12.0%), medical students (-10.9%) and in the group of subjects without a preferred type of physical activity. In these groups the differences were also statistically significant (p<0.01). They were less substantial in the group of runners (-4.6%, statistically insignificant), and in the group of triathletes there were practically no differences. This suggests that a CT 12 on a treadmill is much more susceptible to influences such as training experience, prevailing type of physical activity and motivation compared with an incremental maximum test on a BE. Regulatory T cells (Treg) are a specialized subpopulation of T cells that act to suppress inadequate immune response. Psoriasis is recognized as a T -cell driven immune-mediated systemic inflammatory disease with skin manifestation. Effective therapeutical approach to treat psoriasis is Goeckerman therapy (GT ). The aim of this study was to compare the number of Treg in the peripheral blood of 27 psoriatic patients and 19 controls and to evaluate the influence of GT on Treg population in peripheral blood of patients with psoriasis. There was no significant difference in the relative number of Treg cells in the peripheral blood of healthy blood donors and patients with psoriasis before initiation of GT (P = 0.2668). In contrary, the relative number of Treg cells in peripheral blood of patients with psoriasis after GT was significantly higher than those found in healthy blood donors (P = 0.0019). Moreover, the relative number of Treg is significantly increased in psoriatic patients after Goeckerman therapy compared to the pre-treatment level (P = 0.0042). BACKGROUND: The presence of several risk factors (genetic and non-genetic) has greater impact on the risk of premature coronary artery disease (CAD) than single risk factor. OBJECTIVE: The aim of the study was to establish possible relations between genotypes and alleles of 677C>T polymorphism of MTHFR gene and some traditional risk factors e.g. elevated levels of lipid parameters and smoking in development of premature CAD. METHODS: The groups comprised 152 patients with angiographically documented premature CAD (aged 42.9 +/- 5.5) and 121 age-matched blood donors (aged 42.3 +/- 6.5) were studied. The MTHFR 677C>T polymorphism was genotyped with Polymerase Chain Reaction-Restriction Fragment Length Polymorphism (PCR-RFLP) method. RESULTS: Patients with TT genotype who simultaneously smoked had increased risk of premature CAD compared to non-smoking cases with CC genotype (OR = 24.62). We also found that individuals with TT genotype and elevated LDL-cholesterol (LDL-chol.) level had significantly higher risk of CAD (OR = 9.92) than individuals with normal LDL-chol. level and CC genotype. CONCLUSIONS: The present study shows that simultaneous presence of MTHFR TT genotype and smoking or elevated levels of LDL-chol. influences the risk of premature CAD. Cíl. Korelace ultrazvukového vyšetření polohy disku temporomandibulárního kloubu s vyšetřením na magnetické rezonanci. Metoda. Celkem bylo vyšetřeno 95 pacientů, 112 čelistních kloubů, soubor zahrnoval 10 mužů, 85 žen, věkové rozhraní 14-74 let, průměrný věk 37 let. Vyšetření na ultrazvuku probíhalo na přístroji Philips iu22 lineární 17 MHz sondou ve dvou fázích - klidové a dynamické - při otevírání úst. Na magnetické rezonanci byli pacienti vyšetření na přístroji Philips Achieva 1,5 T s povrchovou cívkou a mikroskopickou cívkou, každé vyšetření zahrnovalo sekvenci s postupným otevíráním úst. Výsledky. Souhlas mezi ultrazvukovým vyšetřením a magnetickou rezonancí při ventrální poloze byl 96%, z toho byla diagnostikována repozice disku v 94%, u pacientů bez repozice byla senzitivita nižší - 65%., Aim. The aim of this study was the correlation of an ultrasound examination of the disk of the temporomandibular joint with the magnetic rezonance examination. Method. In total has been examined 95 patients, 112 mandibular joints, file include 10 men, 85 women, age interface 14-74 years old, the average age of 37 years. The patients were examined using Philips iu22 with linear 17 MHz probe with dynamic phase - with opening the mouth. On the MRI, the patients were examined using Philips Achieva 1.5 T with the surface coil and microscopic coil, each examination included a sequence with the gradual opening of the mouth. Results.
