Úvod: Dynamická scintigrafie jícnu (DSJ) je funkční vyšetřovací metoda, která analyzuje tranzit bolusu radiofarmaka (RF) jícnem do žaludku. Hodnocení motility jícnu je přínosné po vyloučení organické příčiny onemocnění v diagnostice řady patologických stavů u dospělých i dětí. Nejčastější indikací je podezření na poruchu jícnové peristaltiky a funkce dolního jícnového svěrače (LES) u jícnové i mimojícnové manifestace refluxní choroby jícnu (RCHJ), méně často u osob se systémovým onemocněním pojiva, diabetiků apod. Metodika: 3 – 6hodinové lačnění, 48 hodin před vyšetřením vysadit prokinetika. Poloha vleže na zádech, detektor kamery nad hrudníkem. Laborant (sestra) provede nácvik polknutí příslušného objemu roztoku RF – dospělí 10 ml, děti 5 ml. Poté pacient bolus RF (20–50 MBq 99mTc-MAA) polkne a zahajujeme dynamickou studii: 1. grupa – 240 snímků á 120 ms, 2. grupa – 120 snímků á 15 s. Vyznačujeme oblasti zájmu (ROI) nad jednotlivými třetinami jícnu a žaludkem. Po sestrojení křivek hodnotíme v rámci první fáze studie tranzitní čas jícnem (MTT), přítomnost antiperistaltiky či retence RF v jícnu. Z druhé fáze vyšetření hodnotíme známky gastroezofageálního refluxu (GER). Výsledky: Uvádíme příklady patologických nálezů antiperistaltiky, prodlouženého MTT, retence v jícnu a známek GER u nemocných s RCHJ, bronchiálním astmatem, chronickým kašlem, stavem po fundoplikaci pro RCHJ, sklerodermií a achalázií. Závěr: DSJ je jednoduchá, funkční, neinvazivní vyšetřovací technika s nízkou radiační zátěží, která je vysoce citlivá v detekci poruch jícnové motility. Po vyloučení organicity onemocnění podává doplňující informaci o funkci jícnu. Vyšetření se osvědčilo také u dětí. Úvod: Dynamické metody kvantitativního testování senzitivity (Quantitative Sensory Testing; QST) jsou relativně novou nadstavbovou aplikací QST umožňující objektivizovat funkci centrálních modulačních mechanizmů algické percepce, jejichž individuální nastavení může přispívat k rozvoji chronických bolestivých stavů. Testování těchto mechanizmů umožňuje např. predikci rozvoje pooperační bolesti. K nejčastěji hodnoceným dynamickým QST metodám patří tzv. podmíněná modulace bolesti (Conditioned Pain Modulation; CPM) a časová sumace percepce bolestivých podnětů (Temporal Summation; TS). Soubor a metodika: Vyšetření dQST (dynamic methods of Quantitative Sensory Testing) metod s využitím termoalgických podnětů a modulace bolestivým chladem bylo provedeno u 77 zdravých dobrovolníků (47 žen testovaných v ovulační fázi menstruačního cyklu, 30 mužů, věk 19–73 let). Hodnocen byl také vliv věku a pohlaví na výsledky obou dQST testů. Výsledky: Uspokojivá funkce podmíněné modulace bolesti byla prokázána u 84 % testovaných dobrovolníků. Časová sumace bolestivých podnětů byla zachycena u 92, resp. 98 % jedinců v závislosti na použité teplotě podnětů. Nefunkčnost mechanizmů u zbylé části souboru byla ve většině případů podmíněna nesprávně nastavenými technickými parametry testu. U žen testovaných v ovulační fázi menstruačního cyklu jsme prokázali signifikantně vyšší efektivitu CPM mechanizmu ve srovnání s muži. Funkce TS se mezi oběma pohlavími významně nelišila. Významný vliv věku na efektivitu CPM ani TS mechanizmu nebyl ve sledovaném souboru prokázán. Závěr: Provedená studie ověřila použitelnost zvolených algoritmů dynamického testování senzitivity a prokázala antinociceptivní nastavení žen v ovulační fázi menstruačního cyklu ve srovnání s muži, zatímco významný vliv věku na funkci centrálních modulačních mechanizmů algické percepce nebyl zachycen., Background: So called “dynamic quantitative sensory methods” (dQST) represent relatively new QST application which enables to objectify certain mechanisms of pain processing, e.g. central integration or descending control. Their dysfunction may contribute to the development of chronic painful conditions. dQST methods proved the potential to predict the development of chronic postoperative pain. Conditioned pain modulation (CPM) and temporal summation (TS) are the most commonly used dQST tests. Patients and methods: The CPM and TS testing was performed in a group of 77 healthy volunteers (47 women tested in the ovulatory phase of the menstrual cycle, 30 men, age range 19–73) using thermal pain stimuli. Efficiency of CPM and TS mechanism including age and gender influence was evaluated. Results: Efficient CPM was found in 84% of healthy volunteers. In remaining 16%, the inefficiency was mainly due to low intensity of testing or modulatory stimuli. Based on the temperature used, efficient TS mechanism was found in 92% or 98% of healthy individuals. Women in the ovulatory phase of the menstrual cycle showed significantly more efficient CPM comparing to men. No sex differences of TS testing were found. Age had no significant effect on the CPM or TS mechanism in our group of healthy voluteers. Conclusions: Our study has verified the applicability of CPM and TS algorithms used. Women in ovulatory phase of the menstrual cycle show increased anti-nociceptive setting comparing to men. No significant effect of age on central modulation of pain perception has been found in pain-free healthy individuals. Still's disease is an inflammatory disorder of unknown etiology. First-line therapy is based on corticosteroids and non-steroidal anti-inflammatory drugs (NSAIDs) but the frequency of relapses and corticosteroid-induced adverse events are limiting factors. The efficacy of intravenous tocilizumab (TCZ) has been shown at a dose of 8 mg/kg but the corticosteroid-sparing effect of intravenous low-dose TCZ followed by subcutaneous (SC) injection in the course of the disease has been poorly investigated. We report the case of a 28-year old Caucasian woman presenting a relapse of Still's disease eleven months after diagnosis under treatment with 6 mg of methylprednisolone. TCZ at a dose of 4 mg/kg every 2 weeks was combined with 32 mg of methylprednisolone, followed by 162 mg SC every 3 weeks. Evolution was rapidly favourable with a decrease in corticosteroid doses. BACKGROUND: A few reports demonstrate the occurrence of egg allergy in adolescent and adult patients suffering from atopic dermatitis and the association of this allergy to other food and aeroallergens. AIMS AND OBJECTIVES: The aim of this study is to evaluate the occurrence of egg allergy in patients suffering from atopic dermatitis at the age 14 years and older and to evaluate the relationship between egg allergy or egg sensitisation and the sensitisation to dust, mites, feather, and animal dander. MATERIALS AND METHODS: Complete dermatological and allergological examination was performed. These parameters were examined: food allergy and food sensitisation to egg white and yolk, to mites, animal dander (mixture), feather and dust. The statistical evaluation of the relations among egg allergy, egg sensitisation and sensitisation to mites, animal dander (mixture), feather and dust was performed. Two hundred and eighty eight patients were included in the study (90 men, 198 women, with the average age 25.2). RESULTS AND CONCLUSION: Egg allergy was recorded in 5% and egg sensitisation in 20% of patients; sensitisation to dust is recorded more often in patients with positive results in sIgE for egg white and/or yolk. and J. INTRODUCTION: The splenium of the corpus callosum has a specific structure of blood supply with a tendency towards blood-brain barrier breakdown, intramyelinic edema, and damage due to hypoxia or toxins. Signs and symptoms of reversible syndrome of the splenium of the corpus callosum typically include disorientation, confusion, impaired consciousness, and epileptic seizures. CASE REPORT: A previously healthy 32-year-old man suffered from weakness, headache, and fever. Subsequently, he developed apathy, ataxia, and inability to walk, and therefore was admitted to the hospital. Cerebrospinal fluid showed protein elevation (0.9 g/l) and pleocytosis (232/1 ul). A brain MRI showed hyperintense lesions in the middle of the corpus callosum. The patient was treated with antibiotics, and subsequently, in combination with steroids. Two months later, the hyperintense lesions in the splenium and the basal ganglia had disappeared. Almost seven months since his hospitalization in the Department of Neurology, the patient has returned to his previous employment. He now does not exhibit any mental changes, an optic edema and urological problems have improved. In addition, he is now actively engaged in sports. CONCLUSION: We have described a case of a 32-year-old man with confusion, ataxia, and inability to stand and walk. The man developed a febrile meningeal syndrome and a hyperintense lesion of the splenium, which lasted for two months. Neurological changes, optic nerve edema, and urinary retention have resolved over the course of seven months. Úvod: Bolesti v pravém podbřišku jsou nejčastějším důvodem hospitalizace na chirurgických pracovištích obecně. Zároveň akutní apendicitida je nejčastěji se vyskytující náhlá příhoda břišní. Včasná indikace k apendektomii je jedinou metodou léčby, obvykle s nekomplikovaným průběhem a krátkou délkou hospitalizace. V rámci diferenciálně-diagnostické rozvahy je nutné pátrat i po dalších symptomech, zejména dlouhodobých, při akutní exacerbaci často ustupujících do pozadí. Kazuistika: Prezentujeme případ 38leté pacientky, která byla přijata pro akutní bolesti břicha v pravém podbřišku. Klinický i laboratorní nález nasvědčoval pro typicky probíhající akutní apendicitidu, byla proto indikována k apendektomii. Při operaci byl nalezen hemorhagický výpotek v pánvi, na apendixu makroskopicky netypický nález pro akutní zánět. Překvapivý byl nález ztluštění stěny terminálního ilea a zvětšení pravých adnex. Nález si vyžádal rozšíření operačního výkonu na ileocekální resekci a pravostrannou adnexektomii. Definitivní diagnózu stanovilo histologické vyšetření potvrzující endometriózu apendixu, terminálního ilea a pravého ovaria. Dlouhodobou dispenzarizací nemocné byla následně zjištěna prekanceróza, resp. mikroadenokarcinom hrdla děložního. Závěr: Stanovení diagnózy endometriózy apendixu předoperačně je prakticky nemožné. V rámci diferenciální diagnostiky lze na ni pomýšlet u fertilních žen s chronickými gynekologickými obtížemi neobjasněné etiologie. Diagnostickou, resp. léčebnou metodou volby je laparoskopie, která dovolí přehlédnutí celé peritoneální dutiny, včetně malé pánve, provedení apendektomie a eventuálně excizi podezřelých ložisek z endometriózy. Definitivní stanovení diagnózy je doménou histopatologického vyšetření. Při průkazu endometriózy apendixu nebo jakékoli jiné lokalizace je nezbytné předání pacientky do péče gynekologa., Introduction: In general, pain in the lower right abdomen is the most frequent reason for hospital surgical admissions, acute appendicitis representing the most common cause of operation for acute abdomen. Timely appendectomy remains the only treatment in the early stages of inflammation and is usually uncomplicated, requiring only a short hospital stay. A differential diagnostic analysis necessitates a search for other, particularly long-term symptoms that might be driven to the background in cases of acute exacerbation. Case report: The case report presents a 38-year-old female patient who was admitted for lower right abdominal pain. Clinical examination and a blood test both suggested typical acute uncomplicated appendicitis, and therefore the patient underwent appendectomy. Haemorrhagic peritoneal fluid and nodularity of the appendix not typical for appendicitis was found. Oedema of the terminal ileum and a right adnexal tumour were a surprising finding. A more extensive surgical procedure involving ileocaecal resection and right-side adnexectomy was finally performed with regard to the intraoperative finding. The definitive diagnosis of appendiceal endometriosis, endometrial mass in the terminal intestine and ovarian endometriosis was established by histological evaluation. Long-term follow-up revealed microadenocarcinoma of cervix uteri. Conclusion: It is generally very difficult to confirm appendiceal endometriosis before operation, and revealing primary appendiceal endometriosis is virtually impossible. It is advisable to consider endometriosis in fertile women with chronic abdominal pain of unclear aetiology and gynaecological symptoms in their personal history. The best diagnostic and therapeutic method, respectively, is laparoscopy enabling exploration of the entire peritoneal cavity including the minor pelvis, and performing appendectomy as well as excision of suspicious endometrial lesions. Úvod: Naše výsledky byly zhodnoceny za účelem posouzení bezpečnosti a efektivity endoskopické endonazální techniky v léčbě kraniofaryngeomů. <p align="">Materiál a metody: Od roku 2008 bylo endoskopicky endonazálně operováno14 pacientů s typickým supraselárním extraventrikulárním kraniofaryngeomem. Soubor se skládá z 11 mužů a 3 žen (věk 17–60 roků, průměrný věk 38 let). Jedenáct pacientů mělo předoperační deficit zorného pole v rozsahu od malého deficitu ve vnějším kvadrantu až k slepotě jednoho oka a těžkému deficitu druhého oka. Byla použita endoskopická endonazální technika čtyř rukou s peroperační 3,0 T MR. <p align="">Výsledky: V sedmi případech bylo dosaženo radikální resekce. Ve čtyřech případech byla provedena resekce subtotální, ve dvou případech resekce parciální. V jednom případě se jednalo o vypuštění kraniofaryngeomové cysty. V resekci po iMR bylo pokračováno ve třech případech. K úpravě zorného pole došlo v pěti případech. Pooperační diabetes insipidus se nově objevil ve třech případech. Reoperace pro únik mozkomíšního moku byla nezbytná ve třech případech. <p align="">Závěr: Endoskopické techniky v léčbě kraniofaryngeomu jsou bezpečné. Nicméně jsou spojeny s vyšším výskytem pooperačního úniku mozkomíšního moku ve srovnání s transkraniálními přístupy., Introduction: We reviewed our results in order to evaluate safety of endoscopic endonasal technique in the treatment of craniopharyngiomas. <p align="">Material and methods: Since 2008, endoscopic endonasal approach was used in 14 patients with typical suprasellar extraventricular craniopharyngioma. This cohort consisted of 11 males and three females (age 17 to 60 years, average age 38 years). Eleven patients had preoperative visual field deficit ranging from small deficit in the outer quadrant to blindness on one eye and a severe deficit of the second eye. Endoscopic endonasal four hands technique was used with intraoperative 3.0 T MRI. <p align="">Results: In seven cases, the radical resection was achieved. Subtotal resection was performed in four cases. The resection was partial in two cases. In one case, a cyst was drained. Resection after iMRI was performed in three cases. Visual field deficit improved in five cases. Postoperative diabetes insipidus developed in three cases. Reoperation for CSF leakage was necessary in three cases. <p align="">Conclusion: Endoscopic technique in the treatment of craniopharyngioma is safe. Few studies concerning the occurrence of cow’s milk allergy with the use of double-blind, placebo controlled food challenge test in adolescents and adult patients suffering from atopic dermatitis exist. Aim: To evaluate the occurrence of cow’s milk allergy in adolescents and adults suffering from atopic dermatitis. Method: Altogether 179 persons suffering from atopic dermatitis were included in the study: 51 men and 128 women entered the study with the average age of 26.2 (s.d. 9.5 years). Complete dermatological and allergological examinations were performed. Results: The positive results in specificIgE and in skin prick tests were recorded in 12% of patients. According to the open exposure tests and double-blind, placebo controlled food challenge tests these patients are only sensitized to cow’s milk without clinical symptoms of allergy. Double-blind, placebo controlled food challenge test confirmed food allergy to cow milk only in one patient (worsening of atopic dermatitis), the oral allergy syndrome was observed in another one patient, occurrence of this allergy was altogether 1.1%. Extramedulárne (EM) postihnutie pri akútnej myeloblastovej leukémii (AML) je relatívne vzácne a vyskytuje sa najčastejšie v 2 formách – ako myeloidný sarkóm (MS) alebo leukemia cutis (LC). Doposiaľ nebol presvedčivo objasnený molekulový mechanizmus EM postihnutia, je však pravdepodobné, že podiel chemokínových receptorov, adhezívnych molekúl a ich vzájomných interakcií, ktoré kontrolujú migráciu leukemických buniek do periférnych tkanív, je kľúčový. Vznik izolovaného EM relapsu (IEMR) po alogénnej transplantácii krvotvorných kmeňových buniek (ASCT) predstavuje menej častý klinický prejav s nepriaznivou prognózou, najmä v kombinácii s relapsom systémovým. Je pravdepodobne dôsledkom nepomeru medzi GvL efektom v kostnej dreni (KD) a periférnych tkanivách. Jeho liečba nie je jednoznačne definovaná. Predložená kazuistika ukazuje rýchly EM relaps v oboch prsných žľazách u pacientky s akútnou leukémiou so zmiešaným fenotypom (MPAL) po ASCT. Bioptická verifikácia pod ultrazvukovou kontrolou potvrdila prítomnosť leukemickej infiltrácie. Cieľom práce je ukázať menej obvyklý prípad IEMR s dôrazom na komplexnú diagnostiku a veľmi dobrú liečebnú odpoveď po ukončení imunosupresie., Extramedullary (EM) manifestation of acute myeloid leukemia (AML) is relatively rare, it occurs frequently in 2 forms: myeloid sarcoma (MS) or leukemia cutis (LC). The molecular mechanism of EM involvement has not been clearly explained yet, but it is obvious that homing of leukemic cells to peripheral tissues is controlled by chemokine receptors, adhesion molecules and their interactions. The isolated EM relapse (IEMR) after allogeneic haematopoietic stem cell transplantation (ASCT) is an uncommon clinical manifestation with an unfavourable prognosis, especially in case of systemic relapse. It is probably the result of GvL imbalance in the bone marrow and peripheral tissues. The treatment of IEMR is not clearly defined. We present a case report of the rapid EM involvement in both breasts in patient with acute leukemia (mixed phenotype) (MPAL) after ASCT. Verification biopsy under ultrasound-guidance confirmed the presence of leukemic infiltration.
