Úvod: Frekvencia pankreatických metastáz sa pohybuje od 2 % do 5 % všetkých malígnych tumorov pankreasu. Pankreas je elektívnym miestom metastáz renálneho karcinómu a všeobecne sa vyskytujú v siedmej dekáde a sú obyčajne asymptomatické. Kazuistika: V kazuistike prezentujeme prípad 58 ročnej ženy s multifokálnym a izolovaným postihnutím pankreasu metastázami renálneho karcinómu, ktorý sme riešili totálnou duodenopankreatektómiou. Záver: V selektovaných prípadoch považujeme resekčný výkon na pankrease pri metastázach renálneho karcinómu za indikovaný a zlepšujúci celkové prežívanie pacientov. Pancreatic metastases from renal cell carcinoma are frequently the only metastatic site and they typically occur in the seventh decade of life. Autoři referují o vzácném případu traumatické totální avulze očního bulbu s optickým nervem v úrovni chiazmatu. Kazuistické sdělení retrospektivně popisuje průběh ročního sledování mladé ženy, která byla zraněna při hasičských závodech. Avulze bulbu a optiku, kombinovaná s poraněním chiazmatu, rezultovala v hemianopický defekt druhostranného zorného pole. Při traumatu vzniklo subarachnoidální krvácení do oblasti cisteren chiazmatu, jež spontánně ustoupilo. Nedošlo k rozvoji vaskulárních spazmů mozku. Celková kortikoterapie a neuroprotekce neměla vliv na průběh onemocnění. Během sledovací doby objektivně došlo k descendentní atrofii korespondujících axonů a retinálních gangliových buněk. V současné době neexistuje efektivní terapie poranění v oblasti chiazmatu., The authors report a rare case of complete traumatic avulsion of the eyeball with the optic nerve at the level of chiasm. The case report retrospectively describes a year-long monitoring of a young woman who was injured in fire-fighter races. Avulsion of the globe and optic nerve combined with injured chiasm resulted in contralateral hemianopic visual field defect. The trauma led to subarachnoid hemorrhage in cisterns of chiasm, which spontaneously resolved. There was no development of vascular spasms in the brain. Systemic corticosteroid therapy and neuroprotection did not affect the course of the disease. During the observation period, descending atrophy of the corresponding axons and retinal ganglion cells developed. There is currently no effective therapy of chiasm injuries., and L. Hejsek, V. Toxická epidermální nekrolýza je vzácné, akutní, život ohrožující onemocnění projevující se rozsáhlým odlučováním epidermis a slizničních povrchů. Jde o závažnou nežádoucí reakci na podávané léky, nejčastěji antibiotika, antikonvulziva či nesteroidní antirevmatika. Jako první ji popsal skotský dermatolog Alan Lyell v roce 1956, proto je též nazývána Lyellův syndrom. Mortalita tohoto onemocnění je vysoká, proto je klíčová včasná diagnostika a neodkladné zahájení adekvátní péče o pacienta. Jedná se o vzácné onemocnění s velmi malou incidencí a závažnou prognózou, což značně ztěžuje provedení velkých randomizovaných klinických studií. Je tedy obtížné hodnotit efektivitu jednotlivých terapeutických možností. Jako nejperspektivnější se prozatím jeví nitrožilní podávání imunoglobulinů se zřejmým zlepšením klinického stavu pacienta, dobrou tolerancí a minimálními vedlejšími nežádoucími účinky. Nové experimentální postupy se snaží hledat cestu terapie cíleným ovlivněním granulyzinu jakožto hlavního cytotoxického mediátoru. It is a serious adverse response to administered drugs, mostly antibiotics, anticonvulsants or NSAIDs. First described by Scottish dermatologist Alan Lyell in 1956, it is also known as Lyell’s syndrome. Mortality of the disease is high, therefore its early diagnosis is crucial and immediate initiation of appropriate patient care necessary. It is a rare disease with a very low incidence and serious prognosis, which is a considerable hindrance to undertaking large randomized clinical studies. It is therefore difficult to evaluate the effectiveness of various therapeutic options. As the most promising so far appears the administration of intravenous immunoglobulins, apparently reaching improvement in the clinical condition of the patient, with a good tolerances and minimal side adverse effects. New experimental techniques endeavour to seek a way to the therapy through targeted influencing of granulysin as the major cytotoxic mediator. BACKGROUND: Intussusception of the large bowel in adults is a very rare pathological condition. However, it has its clinical importance because intussusception is very often associated with an intraluminal lesion. CASE REPORT: We report two cases of the large bowel intussusception, ileocolic and colorectal. Both intussusceptions were associated with a malignant tumor. However, the clinical presentation was different. One of the intussusceptions was of non-transient character, while the second one resolved spontaneously before operation. Both patients underwent surgery and malignant tumors were found and removed. RESULTS: The purpose of the article is to draw attention to intussusception and emphasize that intussusception, either transient or non-transient, should be further examined. Trombóza ledvinné žíly je vzácné onemocnění novorozeneckého věku. Jeho incidence je nízká, pohybuje se na úrovni 2,2 na 100 000 živě narozených dětí. Následky onemocnění jsou však časté a dosti závažné. Vzhledem k nízkému výskytu onemocnění nemáme v dostupné literatuře jednoznačná a jednotná terapeutická doporučení opírající se o randomizované studie. V tomto článku autoři předkládají svojí zkušenost s invazivní léčbou tohoto onemocnění u čtyřdenního novorozence., Renal vein thrombosis is a rare condition of neonatal age. Cíl práce: V kazuistice prezentujeme raritní nález primárního plazmablastického lymfomu penisu, který klinicky imitoval karcinom glans penis. Jedná se o druhý publikovaný případ tohoto typu lymfomu penisu ve světě. Kazuistika: Pacient, narozen 1929, byl odeslán na naše pracoviště ošetřujícím urologem pro nově zjištěný exofytický tumor glans penis, indikovali jsme parciální amputaci penisu. Histologické vyšetření amputátu prokázalo primární plazmablastický lymfom. Následná restagingová vyšetření neprokázala systémové onemocnění, pacient je nyní 22 měsíců po výkonu bez známek onemocnění a zůstává v hematologické dispenzární péči. Závěr: Primární plazmablastický lymfom penisu představuje extrémně raritní diagnózu, ve světové literatuře je popsán pouze jediný případ. Jedná se o agresivní typ lymfomu, který obvykle vede v krátké době ke smrti pacienta. Ačkoli léčba těchto nádorů je převážně založena na chemoterapii, u našeho pacienta vedl radikální operační výkon k úplnému vyléčení., We report a case of an elderly man who has presented with tumorous affection involving the uncircumcised penile glans and prepuce. Primary clinical examination gave strong suspicion for primary carcinoma of penis, therefore a suprapubic cystostomy and partial amputation of penis were performed. Histopatological examination revealed uncommon diagnosis of plasmablastic lymphoma involving glans penis and prepuce. The vast majority of penile tumours are squamous cell carcinomas, primary malignant lymphoma of penis is extremely rare. As staging results were negative, no adjuvant systemic chemotherapy was neccesary. Pacient s trombózou mozkových splavů a současným intracerebrálním hematomem parieto-okcipitálně byl neúspěšně léčen plnými dávkami intravenózní antikoagulace. Vzhledem k postupné progresi klinického stavu vedoucí ke ztrátě vědomí byla indikována endovaskulární intervence. Pro časté ucpávání aspiračního systému Penumbra byl originálně použit guiding katétr Neuron (současně se Separator 3D) zavedený přímo do trombu namísto Reperfusion katétru se současnou aspirací trombu. Tento postup byl bezpečný a úspěšný a může být použit u pacientů s rozsáhlým trombotickým postižením. A common endovascular aspiration technique was indicated because of the progressive severe deterioration, the level of consciousness had failed. As a novel approach, we used the 6F Neuron guiding catheter (together with The Separator 3D) inserted directly into the thrombus instead of the Reperfusion Catheter, for mechanical disruption of the thrombus mass together with continual aspiration. Sometimes intraorbital foreign bodies lead to unexpected findings. A 16-year old boy was referred due to ocular surface trauma. A conjunctival laceration was detected at the level of the left caruncle with associated left exotropia, reduced adduction as well as a preretinal hemorrhage along the nasal periphery of the fundus. A blow-out fracture of the medial orbital wall was suspected and a CT scan of the orbits was scheduled which revealed the presence of a large intraorbital foreign body. The removal of the intraorbital foreign body (which proved to be a bullet) and precautionary laser photocoagulation along the nasal periphery of the left eye were performed. Variant branching pattern of the cords of brachial plexus coupled with erroneous communications has been an area of concern for surgeons opting to explore this region. Anaesthetic blocks and surgical approaches are the highlights of these interventions, where a keen familiarization of the anatomy of this region is mandatory. The present case description reports a unilateral variant branching pattern of the posterior cord coexistent with a neural communication between lateral and medial cords in an adult male cadaver. This intercordal neural communication between lateral and medial cords was oriented obliquely and measured 2.2 cm in length. Furthermore, the posterior cord revealed a variant branching pattern. It branched into three upper subscapular nerves and a common trunk for the thoracodorsal and lower subscapular nerves. The lowest of the three upper subscapular nerves gave a communicating twig to the thoracodorsal nerve.
