INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease. Úvod: Fyloidní tumor je velmi zřídka se vyskytující nádor prsu. Vyskytuje se pouze v 1 % případů všech nádorů prsu. Diagnostika fyloidního tumoru může být obtížná vzhledem k malému počtu případů. Léčba fyloidního tumoru je vždy chirurgická. Metody: V letech 2004−2013 jsme na Chirurgické klinice FN Plzeň operovali 12 pacientek s fyloidním nádorem prsu z celkového počtu 1564 operací pro nádory prsu (0,8 %). Hodnotili jsme věk, biologické chování nádoru v závislosti na velikosti nádoru a délce trvání, vzdálené metastázy, terapii a přežití. Výsledky: Průměrný věk v době operace byl 50 let (26−84), délka trvání nemoci do operačního řešení byla 1 měsíc až 10 let. Velikost tumoru se pohybovala v rozmezí 2−29 cm, tumory menší než 5 cm byly vždy benigní. 7x pro benigní fyloidní nádor byla provedena excize tumoru. 5x byl diagnostikován maligní fyloidní tumor a byla vždy provedena mastektomie, ve 3 případech s exenterací axily, kdy uzliny byly vždy benigní. V jednom případě po mastektomii následovala radioterapie pro dosahování nádoru do okraje resekátu, ostatní pacientky byly pouze dispenzarizovány. 2 pacientky měly generalizaci do plic diagnostikovanou 5 a 10 měsíců po operaci prsu. 1 pacientka s generalizací zemřela, ostatní žijí bez lokální recidivy onemocnění. Medián přežití je 52 měsíců, interval bez nemoci je 50 měsíců. Závěr: Z našeho souboru vyplývá, že pokud je fyloidní nádor diagnostikován včas, jedná se téměř výhradně o benigní variantu, při déle trvající anamnéze a s rostoucí velikostí tumoru nádory malignizují. Chirurgická léčba je dostačující i v případě maligních forem. Výkon na prsu není třeba doplňovat o exenteraci axily., Introduction: Phyllodes tumour is a breast tumour occurring very rarely. It accounts for only in 1% of all cases of breast tumour. The diagnosis of phyllodes tumours can be difficult in consideration of the small number of cases. Treatment of phyllodes tumours is always surgical. Methods: In 2004–2013, we operated on twelve female patients with phyllodes tumours out of the total number of 1564 surgeries for breast tumours (0.8%) at the Department of Surgery at Teaching Hospital in Pilsen. We evaluated the age, the biological behaviour of the tumour depending on the tumour size and duration, the distant metastases, therapy and survival. Results: The average age at the time of surgery was fifty years (26−84), the duration of disease to the surgical solution ranged from one month to ten years. Tumour size was in the range of two to twenty-nine centimetres, tumours measuring less than five centimetres were always benign. Tumour excision for benign phyllodes tumour was performed seven times. Malignant phyllodes tumour was diagnosed five times with mastectomy performed in each case, and the axilla was exenterated in three cases where nodes were benign in each of them. In one case, mastectomy was followed by radiotherapy because the tumour reached the edge of the resected part; the other patients were only monitored. In two patients, tumour spreading into the lungs was diagnosed at five to ten months after breast surgery. One patient with generalized disease died, the other ones live with no local recurrence of this disease. Median survival is fifty-two months; the disease-free interval is fifty months. Conclusion: The results show that if phyllodes tumour is diagnosed in time, it is almost exclusively benign. If the case history is longer and the tumour is growing, the likelihood of malignancy increases. Surgical treatment is also sufficient in the case of malignant forms. Poruchy spánku patria k najčastejším non‑motorickým prejavom u pacientov s Parkinsonovou chorobou (PCh), v populačných štúdiách je uvádzaný ich výskyt až u 60 % pacientov. Štúdie tiež uvádzajú rozdielnosť prejavov PCh medzi mužmi a ženami. Cieľ: Zistiť, či sú u pacientov s PCh na Slovensku prítomné medzi pohlaviami rozdiely v klinických prejavoch PCh a výskyte porúch spánku. Materiál a metódy: Zisťovanie bolo robené dotazníkovou metódou s účasťou pacientov a neurológov z jednotlivých regiónov Slovenskej republiky. Dotazníky boli vyplňované počas rutinných kontrolných návštev. Boli zisťované socioekonomické premenné ako pohlavie, vek, vek pri začiatku ochorenia, stupeň poškodenia hodnotený škálou Hoehna a Yahrovej, dominujúci fenotyp ochorenia a používaná medikácia. Pri hodnotení porúch spánku bola použitá slovenská verzia dotazníka PDSS. Výsledky: Získali sme dáta od 1 067 pacientov. Pri porovnávacích analýzach neboli zistené podstatné rozdiely vo väčšine hodnotených parametrov. U mužov bol o niečo vyšší podiel pacientov s hodnotou škály Hoehna a Yahrovej 4 a boli zistené signifikantne vyššie dávky levodopy v monoterapii (p < 0,01). Významné rozdiely boli zistené v skóre škály PDSS pre hodnotenie rušivých halucinácií počas noci – skóre u mužov bolo 8,22, u žien 8,48 (p < 0,05). Podobný výsledok sme zaznamenali v skóre hodnotiacom nutnosť močenia počas noci – u mužov bolo skóre 5,90, u žien 6,53 (p < 0,01). Differences in symptoms between men and women have been reported. Aim: In the present study, we assessed whether there are gender differences in clinical presentation of PD and prevalence of sleep disturbances in individuals diagnosed in the Slovak Republic. Material and method: Questionnaires were distributed to participating neurologists and patients in outpatient practices across the Slovak Republic. Sociodemographic variables – gender, age, age at onset, disease severity according to Hoehn and Yahr stage, phenotype of the dominating symptom of Parkinson’s disease and type of medication – were collected. The Slovak language version of the PDSS was used in a questionnaire to test sleep disturbances. Results: Data from 1,067 outpatients with PD were collected. Comparative analyses showed males and females not to be significantly different on the majority of the demographic and medical characteristics collected. Males had a slightly higher proportion of individuals with Hoehn and Yahr score 4 and, among those taking levodopa medication as monotherapy, males took significantly higher levodopa dose than females (p < 0.01). A significant difference in the distribution of PDSS subscores between males and females was observed on item 7 (distressing hallucinations at night) – score for males 8.22, for females 8.48, p < 0.05. Gliomy limbického a paralimbického systému představují anatomicky a tím pádem i operačními přístupy unikátní skupinu mozkových nádorů. Mikrochirurgie v kombinaci s moderními perioperačními postupy zvyšuje šance na radikální odstranění těchto gliomů. Z hlediska lokalizace je dělíme do tří základních skupin: 1. gliomy inzuly, 2. amygdalohippokampálního komplexu a 3. cingula. V minimonografii se věnujeme anatomii i funkci limbického a paralimbického systému a v návaznosti na ni představujeme ucelený pohled na jednotlivé neurochirurgické přístupy k těmto oblastem. Maximální resekce za zachování funkční integrity mozku je předpokladem následné smysluplné onkologické léčby. Taktéž prezentujeme výsledky operací našeho pracoviště ve smyslu morbidity a radikality za posledních sedm let., Limbic and paralimbic gliomas represent a unique group of brain tumors both from an anatomic and surgical point of view. Microsurgery, together with modern perioperative procedures, enables a wider extent of resection of these gliomas. Based on localization, we divide gliomas into three basic groups: 1. insular, 2. amygdalohippocampal complex and 3. cingulum gliomas. In our review, we describe the anatomy and function of the limbic and paralimbic system and in relation to that, we present a description of the specific neurosurgical approaches to these regions. Maximal resection with brain function preservation represents the basis for subsequent oncologic therapy. We also present the surgical results of our department in the last seven years with regard to morbidity and radicality. Konečné produkty pokročilé glykace (AGEs) hrají významnou roli v patogenezi diabetu i diabetických komplikací. Jejich hromadění v oční čočce odráží celkový stav glykace v organizmu. Nedávno byl na trh uveden nový konfokální biomikroskop ClearPath DS-120, který umožňuje rychlé a neinvazivní stanovení AGEs v oční čočce na základě autofluorescence (LAF) některých AGEs. V naší pilotní studii jsme vyšetřili LAF u 69 diabetiků a 49 zdravých kontrol a srovnali se současně měřenou kožní autofluorescencí (SAF) na předloktí pomocí AGE-Readeru. Bylo pozorováno významné zvýšení LAF (DM1T: 0,27 ? 0,09; DM2T: 0,22 ? 0,06; kontroly: 0,17 ? 0,04 AU; p < 0,0001) i SAF (DM1T: 2,0 ? 0,4; DM2T: 2,3 ? 0,6; kontroly: 1,8 ? 0,3 AU; p < 0,0001) u pacientů s diabetem oproti zdravým kontrolám. Ve všech skupinách byl popsán významný vztah mezi LAF a SAF (DM1T: r = 0,53; p < 0,005; DM2T: r = 0,37; p < 0,05; kontroly: r = 0,30; p < 0,05). Naopak závislost LAF ani SAF na HbA1c nebyla přesvědčivá, což asi souvisí s tím, že hodnota glykovaného hemoglobinu dostatečně neodráží mnohaletý stav glykace v organizmu. Stanovení oční autofluorescence možná přináší robustní marker dlouhodobé kompenzace diabetu predikující riziko budoucích komplikací, avšak definitivní potvrzení takové hypotézy si vyžádá četnější a delší klinické studie., Advanced glycation end-products (AGEs) play a crucial role in the pathogenesis of diabetes and its complications. Their accumulation in the lens reflects total glycation rate in the human body. Recently, a new confocal biomicroscope ClearPath DS-120 quickly measuring lens autofluorescence (LAF) has been developed. Our pilot study included 69 patients with diabetes and 49 healthy controls, in all subject LAF was measured and compared with skin autofluorescence (SAF) assessed by AGE-Reader. Both LAF (T1DM: 0,27 ? 0,09; T2DM: 0,22 ? 0,06; controls: 0,17 ? 0,04 AU; p < 0,0001) and SAF (T1DM: 2,0 ? 0,4; T2DM: 2,3 ? 0,6; controls: 1,8 ? 0,3 AU; p < 0,0001) was significantly higher in patients with diabetes. In all groups significant relationship between LAF and SAF was observed (T1DM: r = 0,53, p < 0,005, T2DM: r = 0,37, p < 0.05; controls: r = 0,30, p < 0,05). On the contrary, LAF and SAF relationship with glycated hemoglobin (HbA1c) was rather poor, since HbA1c cannot wholly reflect long-term glycation process. Cíl studie: Metabolismus železa musí být velmi pečlivě regulován. Klíčovým systémovým regulátorem je peptidový hormon hepcidin, který způsobuje “uzamčení” železa uvnitř buňky; to pak nemůže být utilizováno. Dialyzovaní pacienti často trpí anémií, která je způsobena mnoha faktory, mj. stavem mikrozánětu a také zřejmě zvýšenou retencí hepcidinu v důsledku snížené glomerulární filtrace. Naším cílem bylo popsat vztahy mezi hepcidinem a dalšími parametry metabolismu železa, zánětu a erytropoezy u těchto pacientů. Typ studie: Observační Název a sídlo pracoviště: Ústav klinické biochemie a hematologie LF UK a FN v Plzni, Alej Svobody 80, 304 60 Plzeň Materiál a metody: Do studie bylo zařazeno 164 chronicky dialyzovaných pacientů (věk 66 ± 13, 25 - 92 let; 101 mužů a 63 žen) a 37 zdravých kontrol (věk 55 ± 20, 21 - 92 let; 16 mužů a 21 žen), u kterých byl stanoven jejich kompletní krevní obraz, parametry metabolismu železa, zánětu a výživy. Výsledky: Koncentrace železa, transferrinu a hemoglobinu byla významně nižší u dialyzovaných pacientů (p<0,0001), naopak ferritin (p<0,0001), solubilní transferrinové receptory(p<0,05), hepcidin (p=0,0003), CRP a IL-6 (p<0,0001) byly u těchto pacientů významně vyšší než u zdravých kontrol. Hepcidin slabě koreloval pozitivně s CRP a ferritinem a negativně s transferrinem u dialyzovaných pacientů, u zdravých kontrol nebyla nalezena žádná korelace hepcidinu s ostatními parametry. Závěr: Zdá se, že vliv zánětu na hladiny hepcidinu není u hemodialyzovaných pacientů zcela zásadní a na jeho koncentraci se podílí i jiné faktory, např. jeho retence., Objective: Peptide hormone hepcidin is a key systemic regulator of the iron metabolism. Hepcidin binds to the iron cell exporter ferroportin so iron is kept in the cells unavailable for erythropoiesis. The synthesis of hepcidin is up-regulated by high iron stores and inflammation. Dialyzed patients have very often impaired iron management – they suffer from anemia, which is caused by many factors including the state of micro inflammation and hepcidin retention due to decreased glomerular filtration rate. Our aim was to describe the relationship of hepcidin and other parameters of iron metabolism, erythropoiesis and inflammation. Design: Observation Settings: Institute of Clinical Biochemistry and Hematology, Charles University Hospital in Pilsen, Alej Svobody 80, 304 60 Plzeň Material and Methods: Complete blood cell count, iron, ferritin, transferrin, CRP, albumin, creatinine, hepcidin, soluble transferrin receptors (sTfR) and IL-6 were measured in samples from 164 patients included in chronic hemodialysis program (age 66 ± 13, 25 - 92 years), 63 women and 101 men and 37 control healthy volunteers (age 55 ± 20, 21 - 92 years), 21 women and 16 men. Results: Iron, transferrin and hemoglobin were significantly lower in the patients group (p<0.0001) while ferritin (p<0.0001), sTfR (p<0.05), hepcidin (p=0.0003), CRP and IL-6 (p<0.0001) were significantly higher in the patients group. Hepcidin weakly positively correlated with CRP and ferritin and weakly negatively correlated with transferrin in hemodialyzed patients. No correlation of hepcidin with other biochemical parameters in controls was found. Conclusion: It seems that the influence of inflammation on hepcidin levels in hemodialyzed patients is not crucial and other factors including its retention in end-stage renal disease participate., T. BACKGROUND: In the cholesteatoma surgery ossicles can be replaced to reconstruct middle ear function. It is important that these ossicles are free of squamous epithelium, to prevent residual disease. This study focuses on the histological findings of the malleus and incus harvested during cholesteatoma surgery. MATERIALS AND METHODS: Eighty middle ears ossicles were examined in vivo and histologically to consider the relationship of cholesteatoma to ossicles, grade of bone destruction and invasion of cholesteatoma to deeper layers of bone. RESULTS: Serious ossicular destruction was observed more frequently in incus compared to malleus (p=0.0065). Difference of ossicles destruction between children and adults was not significant (p=0.3032). Deep invasion of cholesteatoma into the vascular spaces or inner core of the bone was not observed. CONCLUSIONS: Autograft ossicles from cholesteatomatous ears should not necessarily be rejected for reconstruction of the ossicular chain. Cíl:Cílem naší studie bylo kvantitativně vyhodnotit poruchy písma u pacientů s Parkinsonovou nemoci (PN) ve srovnání s věkově a pohlavím vázanými zdravými kontrolami (ZK) pomocí digitalizačního tabletu. Soubor a metoda: Prospektivně jsme zařadili 40 pacientů s PN (průměrný věk 68,6 ? 11,36 let, délka trvání nemoci 8,02 ? 4,79 let) a 40 věkem a pohlavím vázaných ZK (průměrný věk 62,55 ? 11,22 let). Všichni jedinci byli praváci bez přítomnosti deprese či demence. Každý subjekt podstoupil sedm cvičení pro vyšetření písma a kresbu Archimédovy spirály a elips s pomocí digitalizačního tabletu. Byly hodnoceny rychlostní parametry mikrografie a kresby při pohybu pera po tabletu i nad tabletem. Pro statistickou analýzu dat jsme použili Mann‑Whitneyho U test a Spearmanovy korelace s korekcí na opakovaná měření (Benjamini‑Hochbergova metoda). Výsledky: U PN ve srovnání se ZK jsme při psaní na tabletu zjistili statisticky významné snížení v parametrech: okamžitá rychlost, okamžité zrychlení, okamžitá změna zrychlení v čase. Změny se zvýrazňovaly s délkou psaného segmentu. Ještě významnější byly rozdíly mezi oběma skupinami při hodnocení pohybu pera nad tabletem, tj. před vlastním zahájením psaní, při přípravě na pohyb. Zaznamenali jsme pokles sledovaných hodnot až o 20 % ve srovnání se ZK. Závěr: U pacientů s PN jsme prokázali specifické změny nejen při vlastním psaní, ale i ve fázi přípravy na psaní, které lze kvantifikovat pomocí digitalizačního tabletu. Výsledky studie mohou mít přímý klinický dopad: umožní nám studovat mikrografii jako možný časný klinický marker rozvoje PN., Aim: The aim of this study was to assess micrographia in patients with Parkinson’s disease (PD) as compared to healthy controls (HC) using a digitizing tablet. Methods: We included 40 PD (mean 68.6 ? 11.36 years, duration of illness 8.02 ? 4.79 years) and 40 age- and sex-matched HC (mean 62.55 ? 11. 22 years). All subjects were right-handed, without the presence of depression or dementia. Each subject underwent seven exercises for writing and drawing of Archimedes spiral and ellipses using a digitizing tablet. The speed parameters of micrographia and drawing during the movement of a pen in the air and on the tablet were evaluated. The Mann-Whitney U test, Spearman correlation and Benjamini-Hochbergs method were used for statistical data analysis. Results: A statistically significant reduction in parameters of velocity, acceleration, and jerk was found when comparing both groups during writing. Changes were more pronounced with increased length of the written segment. The differences between the two groups were more pronounced when the in-air movements were assessed, i.e. during movement preparation. The values decreased up to 20% compared to HC. Conclusion: PD-specific changes assessed with a digitizing tablet were demonstrated not only during writing but also during preparation for writing. Cíl: Určení vztahu vybraných ukazatelů ke střednímu tlaku v plicnici (mPAP). Posouzení přítomnosti plicní hypertenze (PH) na základě RTG a HRCT obrazů. Metodika: 53 nemocných s chronickým plicním onemocněním podstoupilo pravostrannou srdeční katetrizaci, RTG hrudníku a HRCT plic. U 30 nemocných byla PH potvrzena. Skiagramy i HRCT byly hodnoceny třemi radiology, kteří se kromě jednotlivých nálezů vyjadřovali k přítomnosti PH. Byl vyhodnocen vztah relevantních parametrů k mPAP a shoda radiologů s objektivní přítomností PH. Výsledky: Nejsilnější vazba k mPAP byla na RTG hrudníku nalezena u poměru TI/ LHB (r = 0,256650, p = 0,02048) a šíře truncus intermedius (r = 0,188897, p = 0,023362). U HRCT vyšetření jsme nalezli významnou vazbu mezi hodnotou mPAP a průměrem kmene plicnice (r = 0,396894, p = 0,000001) a indexem PA/AA (r = 0, 382652, p = 0,000002). Hodnota rozhodovací meze pro PH u průměru kmene plicnice byla 31,2 mm se specificitou 80,769 % a senzitivitou 68,182 %. Shoda radiologů s mPAP při hodnocení RTG byla 58,49 %, 54,72 % a 64,15 %, při hodnocení HRCT byla 70,83 %, 60,42 % a 62,50 %. Závěr: Existuje vztah mezi šíří centrálních úseků plicních tepen a mPAP. Při pozitivitě několika specifických nálezů lze vyjádřit podezření na PH, při jejich absenci lze PH vyloučit s velkou pravděpodobností., Aim: To evaluate relationship between selected parameters and mean pulmonary artery pressure (mPAP). To assess prediction of pulmonary hypertension (PH) on chest radiographs (CXR) and High-resolution CT (HRCT) of lungs. Methods: 53 individuals with chronic lung disease underwent right heart cathetrisation, CXR and HRCT. 30 patients had PH confirmed. CXRs and HRCTs were reviewed by three radiologist who besides evaluation of findings stated whether PH is present. Relation between relevant parameters and mPAP and agreement between radiologists and real presence of PH were assessed. Results: The strongest correlation on CXR was found between mPAP and ratio of right descending pulmonary artery width and the left main bronchus (r = 0.256650, p = 0.02048) and right descending branch of pulmonary artery width (r = 0.188897, p = 0.023362). The strongest correlation on CXR was found between mPAP and pulmonary artery diameter (r = 0.396894, p = 0.000001) and PA/AA ratio (r = 0.382652, p = 0.000002. The cut-off value for PH for PA width was 31,2 mm with specificity 80.769% and sensitivity 68,182%. Agreement between radiologists and value of mPAP was in CXRS 58.49%, 54.72% and 64.15%, in HRCT of the lungs 70.83%, 60.42% and 62.50%. Conclusion: Width of pulmonary arteries and mPAP are related.
