A hybrid approach to elephant trunk technique for treatment of thoracic aortic aneurysms combines a conventional surgical and endovascular therapy. Compared to surgery alone, there is a presumption that mortality and morbidity is reduced. We present a case report of a 42-year-old man with a giant aneurysm of the entire thoracic aorta, significant aortic and tricuspid regurgitation and ventricular septum defect. Hereditární hemoragická teleangiektázie neboli morbus Rendu‑Osler‑Weber je fibrovaskulární dysplazie s autozomálně dominantní dědičností charakteristická přítomností teleangiektázií a arteriovenózních malformací kůže, sliznic a vnitřních orgánů. Plicní arteriovenózní malformace jako zdroj septických embolů mohou zapříčinit vznik mozkového abscesu, který tak bývá prvním a často jediným projevem heriditární hemoragické teleangiektázie. V práci jsou prezentovány tři kazuistiky pacientů trpících touto chorobou, kde prvním a závažným příznakem byl vzniklý mozkový absces. Protože se jedná o relativně vzácné onemocnění, nebývá vždy zvažováno v rámci diferenciální diagnostiky a tím pádem ani diagnostikováno. U každého pacienta s mozkovým abscesem je nezbytné na toto onemocnění pomýšlet a v případě podezření je nutné jej řádně vyšetřit. Při verifikované plicní arteriovenózní malformaci je třeba ji ošetřit a zabránit tak vzniku recidivujících mozkových abscesů. Péče o tyto pacienty je komplexní a multioborová., Hereditary hemorrhagic telangiectasia or Rendu‑Osler‑Weber disease is an autosomal‑dominant disorder that results in fibrovascular dysplasia and is characterized by telangiectases and arteriovenous malformation of the skin, mucosa and viscera. Pulmonary arteriovenous malformations as a source of septic emboli may give rise to brain abscess that so often is the first and only clinical manifestation of hereditary hemorrhagic telangiectasia. Three cases of patients suffering from this disease with brain abscess as the first and serious symptom are presented. Since this is a relatively rare disease, it is not always considered in the differential diagnosis and thus not diagnosed. This disease has to be considered in patients with brain abscess and, in case of a suspicion, it must be properly examined. When pulmonary arteriovenous malformation is verified, it has to be treated in order to prevent recurrent brain abscesses. Care of these patients should be comprehensive and multidisciplinary. Prezentujeme případ mladé ženy, jež prodělala v dospělém věku operaci vrozené cysty pravého hepatiku, kterou vzhledem k těsnému kontaktu s velkými cévami nebylo možné kompletně resekovat. Byla provedena parciální resekce a cysto-jejuno-anastomóza (CJA) kdrenáži žluči. Pacientka byla 4 roky bez obtíží. Posléze se dostavily bolesti břicha, subikterus a laboratorní známky cholestázy Kvůli těmto potížím a opakovaným cholangoitidám prodělala řadu vyšetření a intervencí, které měly zajistit ozřejmění příčiny obstrukce a obnovení odtoku žluči. Použité zobrazovací metody neprokázaly jednoznačnou příčinu obtíží, proto bylo přistoupeno k operační revizi, při níž byl peroperační ultrasonografií prokázán tumor v oblasti CJA. Nález byl vzhledem k lokalizaci inoperabilní. Pacientka měla následně zjištěnu diseminaci malignity v játrech i v uzlinách a zemřela 5 měsíců od zjištění maligní diagnózy na hepatorenální selhání. Tento poměrně raritní případ dokládá zvýšené riziko maligního zvratu vrozených malformací žlučových cest především v případě jejich diagnózy a léčby v dospělém věku. V dostupné literatuře existuje jen málo dokumentovaných případů metachronního karcinomu po resekci žlučových cest., We present a case of a young woman, who underwent surgery for congenital bile duct cyst in adulthood. Due to location of the cysts it was impossible to completely remove it. The cyst was partially resected and it was performed cyst-jejunal anastomosis (CJA) to drain of bile. After operation the patient was 4 years without any problems. After this interval appeared pain in the right upper quadrant and subicterus with laboratory evidence of cholestasis. The patient suffered from recurrent cholangitis due to which a number of diagnostic tests and interventions were performed. Used imaging metods did not demonstrate an unequivocal cause of problems, therefore subsequently surgical revision was performed and the in-traoperative ultrasonography demonstrated tumor in anastomosis. The finding was inoperable due to inappropriate location. In the following period liver and lymphnode metastases appeared and patient died five months after diagnosis of malignancy. This relatively rare case illustrates an increased risk of malignant transformation of congenital malformations of the biliary tract. Denosumab is a human monoclonal antibody representing a novel therapy of osteoporosis. Contrary to always other antiosteoporotic drugs, it is not contraindicated in advanced chronic kidney disease, as its pharmacokinetic does not differ from patients with normal kidney function. However, published case reports in chronic kidney disease (CKD) patients stopped the therapy after single dose because of hypocalcemia. Cílem práce je ukázat v průběhu 18letého sledování kognitivní, emoční a behaviorální změny u pacientky s roztroušenou sklerózou mozkomíšní (RS). Předkládaná kazuistika dokumentuje maladaptaci na diagnózu RS a dlouhodobé maladaptivní zvládání, prolínání a vliv afektivních složek na inhibici kognitivních schopností a psychického stavu, vliv stresu na vznik „pseudoatak“ RS, psychosociální problematiku a kvalitu života. Diagnóza RS měla negativní vliv na pacientku a rodinu. Nepředvídatelný průběh ovlivňoval potíže v adaptaci a v plánování podpory. Vliv stresu souvisejícího s onemocněním a zároveň vedlejší negativní efekt léčby vedli k mnoha psychosociálním potížím, které snížily kvalitu života pacientky a měly negativní vliv na její spolupráci při léčbě a rehabilitaci. Součástí komprehensivní diagnostiky i léčby RS byla opakovaná neuropsychologická vyšetření a v závěru sledování, ve fázi chronicko‑progresivní RS, i kognitivně‑behaviorální terapie cílená na snížení zátěže způsobené chronicitou nemoci a zlepšení kvality života., The aim of the paper is to show cognitive, emotional and behavioural changes in a multiple sclerosis (MS) patient over eighteen years of observation. Case study documents maladaptation to MS diagnosis and prolonged maladaptive coping, diffusion and influence of affective components on inhibition of cognitive abilities and mental health status, the contribution of stress to development of „pseudo-relapses“ of MS, psychosocial problems and quality of life. The diagnosis of MS was devastating for the patient herself and her family. Its unpredictable course complicated adjustment and created difficulties in planning for support. Effects of disease-related stress as well as a negative side-effect of treatment led to many psychosocial problems that impaired the patient’s quality of life and affected compliance with treatment and rehabilitation. Repeated neuropsychological assessment was a part of a comprehensive diagnostic process and treatment of MS and, at the end of observation, in chronic-progressive phase of MS, was complemented with cognitive-behavioural therapy aimed at reducing burden of disease and improving the quality of life., and A. Cílem našeho sdělení je poukázat na možné komplikace po lokální aplikaci cizích látek do podkoží penisu. Prezentujeme případy tří pacientů (průměrný věk 31 let) s lokálními komplikacemi po injekční aplikaci borové masti (acidum boricum) do podkoží penisu. Dle našich dosavadních zkušeností se tato problematika týká užší skupiny sociálně slabších pacientů, kteří se pro tyto amatérské zákroky rozhodli ve snaze zvětšit si průměr svého penisu. V místě aplikace došlo k těžkým zánětlivým změnám, induraci a s latencí několika měsíců (let) až k rozpadu ložiska v nehojící se ulcerózní lézi. Vždy je indikováno chirurgické řešení, a to excize ložiska, případně krytí defektu dermoepidermálním štěpem., The aim of this communication is to describe severe complications caused by the local application of inorganic material into the subcutaneous penile tissue. Three patients (mean age 31 years) with local complications after the injection of boric ointment (acidum boricum) into the subcutaneous tissue of the penis are described. According to our experience, this condition is limited to a group of socially weak patients who underwent this intervention in order to increase the volume of their penis. The injection of this inorganic material leads to severe infl ammatory changes at the site of application. After several months (years) these deposits disintegrated into ulcerative lesions, which would not heal. Low-grade myofibroblastic sarcoma (LGMS) is a very rare, atypical myofibroblastic tumor with fibromatosis-like features with predilection mostly in head and neck region. LGMS occurs primarily in adult patients with a slight male predominance. Only few cases of LGMS affecting the larynx have been reported in literature to this date. We describe a case of low-grade myofibroblastic sarcoma of the larynx in a 40-year-old male patient. Cíl: Autoři prezentují výsledky operační léčby tumorů periferních nervů. Operační výkony byly provedeny na Neurochirurgické klinice LF MU a FN Brno v letech 2003–2013. Ve sdělení je prezentován současný přehled histologické klasifikace a její vliv na možnosti chirurgické léčby. Součástí práce je i kazuistika dokumentující operační postup u raritního objemného tumoru n. ischiadicus v hloubce hýžďové krajiny. Soubor a metodika: Operační revizi podstoupilo celkem 24 pacientů, z toho 13 mužů a 11 žen. Průměrný věk pacientů v souboru byl 46,5 roku (min. 22, max. 74 let). Výkony byly provedeny standardní mikrochirurgickou operační technikou, od roku 2007 s užitím peroperační elektrofyziologické monitorace. Retrospektivní analýzou je soubor definován dle postiženého nervu, rozsahu resekce a histologické klasifikace. Je zhodnocen výsledný pooperační neurologický stav. Výsledky: Nejčastěji postiženým periferním nervem byl n. tibialis. V souladu s literárními zdroji se histologicky nejčastěji jednalo o benigní schwannom, a to celkem ve 20 případech. Jednou šlo o neurofibrom, jednou o intraneurální ganglionovou cystu, jedenkrát byla histologie uzavřena jako raritní extraoseální Ewingův sarkom a u jednoho pacienta byl tumor způsoben chronickým zánětlivým procesem. Závěr: V našem souboru bylo dosaženo totální resekce u všech operovaných pacientů, kromě jednoho pacienta. U něj byla provedena pouze revize s otevřenou biopsií. U pacienta s maligním Ewingovým sarkomem byla nutná kompletní rozšířená resekce postižených nervů s příslušným totálním motorickým a senzitivním deficitem. Ve dvou dalších případech došlo po operaci ke zhoršení neurologického stavu, jednou se jednalo o lehkou parézu, jednou o trvalou hypestezii. V ostatních případech klinické obtíže prakticky vymizely., Aim: The authors present surgical treatment results of peripheral nerve tumors. All the surgical procedures were performed between 2003 and 2013 at the Department of Neurosurgery, The University Hospital Brno. The paper provides an overview of current histological classification and its impact on surgical treatment. The paper also includes a case report describing a surgical procedure used for a rare, bulky tumor of the sciatic nerve in the depth of the gluteal region. Methods: In total, 24 patients, 13 men and 11 women, underwent surgical revision. The mean age of the group was 46.5 years (min. 22, max. 74 years). All the surgical procedures were completed using standard microsurgical tools. Perioperative electrophysiological monitoring has been used since 2007. A retrospective analysis according to the localization of the affected nerve, extent of resection and histological classification of the tumor was performed and the final outcome in terms of neurological status was evaluated in the study group. Results: A tumor of the tibial nerve was the most frequent target of the surgery in our study group. Benign schwannoma was the most common tumor in our study group (20 cases) and this is in accordance with previously published data. Other tumors were classified as neurofibroma (one case) and intraneural ganglion cyst (one case). Histology confirmed the presence of a rare extraosseous Ewing’s sarcoma in one case. Finally, one patient suffered from chronic inflammation. Conclusion: In our study group, total resection was performed in all patients except one. Revision with open biopsy was performed in a patient with nerve inflammation. Complete extended resection of the affected nerves was necessary in a patient with malignant Ewing’s sarcoma. This resulted in total motor and sensitive deficit of the corresponding area. Neurological status has also worsened after the surgery in two other cases, one had light paresis and the other had permanent hypoesthesia. The clinical symptoms disappeared after surgery in all other cases.
