Úvod: Cílem tohoto článku je prezentovat naše zkušenosti s transanální endoskopickou mikrochirurgií (TEM) jako jedním ze základních operačních přístupů k selektovaným pacientům s nádory konečníku. Metody: Retrospektivně jsme vyhodnocovali soubor 80 pacientů, u kterých byla pro nádor konečníku provedena operační léčba pomocí TEM. U těchto pacientů jsme sledovali a statisticky vyhodnocovali konkrétní předoperační a pooperační ukazatele, které bychom chtěli prezentovat: předoperační bioptické vyšetření a pooperační výsledek histologického vyšetření, velikost nádoru a jeho uložení v konečníku, hloubku resekce a hodnocení základních demografických ukazatelů. Výsledky: Z našich zkušeností u zmíněných pacientů vyplývá, že není významný rozdíl mezi věkem operovaných mužů a žen ani při rozlišení na benigní a maligní nádory. Nejčastěji operovanými pacienty byli muži, průměrně ve věku 64 let, kteří byli primárně vyšetřeni pro krvácení z konečníku. Nejčastějším nálezem byl low-grade adenom, který byl potvrzen i v definitivním resekátu. Celkem u 17 pacientů byl nalezen v definitivním histologickém vyšetření maligní tumor, který nebyl zachycen předoperačně. Závěr: V rukách zkušeného chirurga představuje TEM reálnou možnost provedení radikální resekce i maligního, lokálně nepokročilého nádoru konečníku, pokud je v dosahu operačního rektoskopu. TEM je metodou bezpečnou, bez negativního vlivu na funkce análních svěračů. Vzhledem k možnosti falešně negativního předoperačního histologického vyšetření preferujeme radikální odstranění nádoru s celou šířkou stěny konečníku. I když optimální lokalizace pro použití operačního rektoskopu je na zadní straně konečníku, téměř u poloviny našich pacientů se tyto nádory vyskytovaly na stěně přední. Vzhledem k technické proveditelnosti sutury stěny rekta je vhodné, aby báze tumoru nepřesahovala polovinu obvodu konečníku. Technika TEM je vhodná zejména pro nízce uložené, nestenotizující, lokálně nepokročilé nádory konečníku. Naše dobré výsledky jsou dány především přísnou selekcí pacientů, technickým zvládnutím operačního postupu a letitými zkušenostmi., Introduction: The purpose of this paper is to present our experience with transanal endoscopic microsurgery (TEM) as an operative approach in a selected group of patients with rectal tumours. Methods: We present a retrospective study of 80 patients operated for rectal tumour using TEM. Specific preoperative and postoperative indicators were gathered and statistically evaluated. We assessed histological examination before the surgery and definitive histology of the resected tumour, resection depth and basic demographic data of our patients. Results: Our experience suggests that there is no significant difference between the age of patients according to sex, even if malignant and benign tumours are assessed separately. The majority of our operated patients were male patients, mean age of 64 years who were primarily referred to our department because of bleeding low-grade adenomas of the rectum. In 17 patients a malignant tumour was diagnosed based on final histopathologic examination. None of them had signs of malignancy before the surgery. Conclusion: In the hands of an experienced surgeon, TEM is a feasible option of radical removal of rectal, locally not advanced malignancies if within reach of TEM rectoscope. TEM is a safe procedure that does not result in any alteration of rectal sphincter functions. Possible false negative preoperative benign diagnosis leads us to approach radical removal of the tumour with full wall thickness resection. Although the posterior aspect of the rectum is an optimal location for the rectoscope, anterior tumours were found in almost one half of our patients. Considering technical feasibility of the rectal wall suture, the base of the tumour should not extend beyond one half of the rectal circumference. Low non-stenotising noninvasive tumours of the rectum are suitable for TEM. Úvod: Tato práce referuje první zkušenosti s laparoskopicky asistovanou transanální totální excizí mezorekta u pacientů s karcinomem středního a dolního rekta. Metody: V období od října 2013 do května 2014 bylo na dvou pracovištích (CHK FN Brno, CHO KOC Nový Jičín) operováno 17 pacientů technikou transanální totální mezorektální excize. V souboru bylo 11 mužů a 6 žen ve věku 49–81 let. Vzdálenost dolního okraje tumoru od análního okraje byla 30–80 mm, medián 60 mm. U všech pacientů byla konstruována ručně šitá kolo-anální anastomóza. Výsledek: U všech operovaných byl dosažen negativní distální resekční okraj. Cirkumferentní resekční okraj byl pozitivní ve dvou případech. Kvalita totální mezorektální excize byla hodnocena v 8 případech jako kompletní a v 9 případech jako téměř kompletní. Dehiscence kolo-anální anastomózy byla zaznamenána ve dvou případech (12 %). Operační čas se pohyboval v rozsahu 212–375 minut (medián 280 minut). Medián pooperační hospitalizace byl 9 dnů v rozsahu 6 až 30 dnů, v případě řešení anastomotické a rané komplikace. Závěr: Na základě prvních získaných zkušeností hodnotíme výkon jako realizovatelný a bezpečný v podmínkách našich chirurgických pracovišť s uspokojivými patologickými výsledky. Přesné indikace zatím nelze jednoznačně doporučit. Předpoklad je, že k transanální totální mezorektální excizi by měli být indikováni pacienti s nízce uloženým karcinomem rekta, event. s úzkou pánví, objemným mezorektem a objemnou prostatou., Introduction: First experiences with laparoscopic low anterior resection with transanal mesorectal excision in patients with low and mid rectal cancer are referred. Methods: During a 9 month period (between October 2013 and May 2014), 17 laparoscopically assisted rectal resections with transanal total mesorectal excision, hand-sewn colo-anal anastomosis and protective ileostomy were performed at two institutions (CHK FN Brno, CHO KOC Nový Jičín). There were 11 males and 6 females, with a mean age 68 years (range 49–81 years) in our cohort. The tumor was located 30–80mm from the anal verge (median 60 mm). Results: A negative distal resection margin was achieved in all patients. The circumferential resection margin was positive in two cases. The TME quality was described as complete in 8 cases and nearly complete in 9 cases. Mean operative time was 280 min (range 212–375 min) with no intra-operative complications. Anastomotic leakage was observed in two patients (12%). Median length of postoperative hospital stay was 9 days with a range of 6–30 days, in case of anastomotic and wound complications. Conclusion: According to our first experience we evaluated this method as feasible and safe with satisfactory pathological outcomes. We cannot recommend exact indications yet. BACKGROUND: Intussusception of the large bowel in adults is a very rare pathological condition. However, it has its clinical importance because intussusception is very often associated with an intraluminal lesion. CASE REPORT: We report two cases of the large bowel intussusception, ileocolic and colorectal. Both intussusceptions were associated with a malignant tumor. However, the clinical presentation was different. One of the intussusceptions was of non-transient character, while the second one resolved spontaneously before operation. Both patients underwent surgery and malignant tumors were found and removed. RESULTS: The purpose of the article is to draw attention to intussusception and emphasize that intussusception, either transient or non-transient, should be further examined. Cil: Klasická neuralgie trigeminu (CTN) je charakterizována epizodickou ostrou neuropatickou obličejovou bolestí a s výjimkou přítomnosti neurovaskulárního konfliktu (NVC) nebyla v našem souboru podmíněna jinou známou abnormitou či patologií. V poslední době je diskutovaná atrofie trigeminu na straně bolesti. Našim cílem bylo ověřit tyto nálezy a vztáhnout je k dalším klinickým datům. <p align="">Soubor a metodika: Pomocí magnetické rezonance (MR) jsme změřili objemy trojklaného nervu na bolestí postižené a nepostižené straně a vyhodnotili výskyt a typ NVC u 45 nemocných s CTN. Následně jsme provedli korelace mezi objemy postiženého nervu, NVC, klinickými a demografickými daty (trvání nemoci, věk nemocného). <p align="">Výsledky: NVC jsme zjistili u 69 % postižených nervů, 51 % nepostižených nervů a u 42 % nemocných oboustranně. Signifikantně snížený objem trojklaného nervu byl zjištěn na bolestí postižené straně (p < 0,001). Žádné korelace jsme nezaznamenali mezi objemem postiženého nervu, přítomností a typem NVC ani mezi klinickými a demografickými daty. <p align="">Závěr: Naše výsledky naznačují, že NVC může být sice spoušťovým mechanizmem CTN u vnímavých subjektů, ale není spolehlivým ukazatelem nemoci. Snížený objem trojklaného nervu se objevuje nezávisle na NVC a odráží spíše predisponující asymetrii trigeminu než vzniklou atrofii., Aim: Classic trigeminal neuralgia (CTN) is characterized by episodic neuropathic facial pain that, with exception of neurovascular conflict (NVC), was not attributed to any underlying disorder in our sample. Using magnetic resonance imaging, we evaluated volumes of affected and non‑affected nerves and presence and type of NVC in a large group of CTN subjects prior to gamma knife treatment. <p align="">Design: Correlation between affected nerve volume and NVC, clinical data and demographic characteristics (duration of pain, age) were explored in 45 subjects diagnosed with CTN. <p align="">Results: NVC was detected in 69% of affected nerves, 51% of non‑affected nerves, and in 42% of subjects bilaterally. Lower trigeminal nerve volume was detected on the affected side (p < 0.001), however, no correlation between affected nerve volume and the presence and type NVC or demographic data was detected. <p align="">Conclusion: Our results suggest that NVC may trigger CTN in susceptible subjects but is not a reliable disease marker. Cíl: Cílem práce je poukázal na možnosLi diagnosLiky sakroileitidy jako jedné z prvotních známek chronického zánětlivého onemocnění skeletu, typu ankylozující spondylitidy (AS) či jiného onemocnění z kategorie séronegativních spondylartritid (SpA). Nezpochybnitelnou roli v diagnostice časných stadií hraje magnetická rezonance (MR), která je schopna časně zobrazit přítomnost revmatoidního zánětu v oblasti sakroiliakálních kloubů (SI), a to v tzv. preradiologickém období, kdy ostatní diagnostické metody (CT a RTG) zatím žádné změny nedetekují. Předpokládáme, že v určitých stadiích onemocnění a také v závislosti na věku vyšetřovaných pacientů ztrácí MR svou výsadní roli a může být zdrojem falešně negativních nálezů, a proto se snažíme navrhnout optimálnější protokol vyšetření v závislostí na věku. Metodika: Zkoumali jsme 82 MR vyšetření SI kloubů, zaměřili jsme se na přítomnost edému kostní dřeně v subchondrální kosti jako známky aktivního zánětu a zároveň na přítomnost kostních změn. V případě pozitivního nálezu edému kostní dřeně jsme dále hodnotili rozsah edému, rozdělili jsme do čtyř kategorií: O - žádný edém, 1 - edém postihující méně než 1/3 rozsahu subchondrální kosti, 2 - edém postihující 1/3-2/3 rozsahu subchondrální kostí, 3 - rozsáhlý edém postihující více než 2/3 rozsahu subchondrální kosti. Hodnotili jsme oba SI klouby, každý samostatně a započítávali jsme vždy vyšší hodnotu. Výsledky: Naše zkušenosti ukazují, že MR je suverénní diagnostickou metodou potvrzující přítomnost revmatoidního zánětu v oblasti SI kloubů v časných stadiích zánětlivého postižení a především v nižších věkových kategoriích. Potvrdili jsme náš předpoklad, že MR ve věkové kategorii nad 35 let (35+) již ztrácí svou suverénní roli v diagnostice SpA a může být zdrojem falešně negativního nálezu, protože přítomnost kostních změn při absenci edému kostní dřeně může vést hodnotícího radiologa falešně k diagnóze artrózy. Prokázali jsme, že ve věkové kategorii 35+ je edém subchondrální kosti málo častým nálezem, a pokud se vyskytuje, je spíše menšího rozsahu. Naopak pravidelně se objevují kostní změny a navíc se v této věkové kategorii již běžně objevují artrotické změny na kloubech, které působí diferenciálně diagnostické rozpaky. Závěr: Na základě našich zkušeností navrhujeme u pacientů s podezřením na sakroileitidu ve věkové kategorii 35+ používat jako metodu první volby nativní CT SI kloubů., Aim: To refer about the posibility of diagnosis of sacroiliitis, which is one of the first signs of chronical inflammatory disease of the skeleton, the type ankylosing spondylitis (AS) or other diseases from the group of seronegative spondyloarthritis (SpA). An important role in the diagnosis plays magnetics resonance (MR), which can assess early the presence of rheumatoid inflammations in the sacroiliac joints (SI), primarily in the so called preradiological period, even when other metods (CT, RTG) do not detect any changes. We supposed, that in some stages of disease and according to the age of examined people MR loses its sovereign role in diagnosis and may cause false-negative findings and therefore we try to propose more optimal protocol testing depending on the age of the pacient. Methods: We evaluated 82 MR examinations of SI, focused on the presence of bone marrow edema in subchondral bone as evidence of active inflammation and also the presence of bone changes. In the case of a positive finding of bone marrow edema we evaluated its extent. For this purpose we devided findings into 4 categories : 0 - no edema, 1 - edema affecting less than 1/3 extent of subchondral bone, 2 - edema affecting 1/3 till 2/3 extent of subchondral bone, 3 - edema affecting more than 2/3 of subchondral bone. We evaluated both SI joints separately and we always counted the higher value. Results: Our experience shows that MR is a sovereign diagnosis method which confirms the presence of rheumatoid inflammation in the SI in early stage of inflammatory diseases and especially at lower ages. We confirmed our assumption that MR in the age categories over 35 years loses its sovereign role in diagnosis SpA and can be a source of false-negative results, because the presence of bone changes in the absence of bone marrow edema can falsely lead the radiologist to diagnose arthrosis. We have shown that in the age category over 35 years subchondral bone edema is a rather rare finding, and if present at all, it is of minor extent. On the contrary, bone changes appear regularly and moreover in this age group arthritic changes commonly occur at the joints, causing differential diagnostic embarrassment. Cíl. Korelace ultrazvukového vyšetření polohy disku temporomandibulárního kloubu s vyšetřením na magnetické rezonanci. Metoda. Celkem bylo vyšetřeno 95 pacientů, 112 čelistních kloubů, soubor zahrnoval 10 mužů, 85 žen, věkové rozhraní 14-74 let, průměrný věk 37 let. Vyšetření na ultrazvuku probíhalo na přístroji Philips iu22 lineární 17 MHz sondou ve dvou fázích - klidové a dynamické - při otevírání úst. Na magnetické rezonanci byli pacienti vyšetření na přístroji Philips Achieva 1,5 T s povrchovou cívkou a mikroskopickou cívkou, každé vyšetření zahrnovalo sekvenci s postupným otevíráním úst. Výsledky. Souhlas mezi ultrazvukovým vyšetřením a magnetickou rezonancí při ventrální poloze byl 96%, z toho byla diagnostikována repozice disku v 94%, u pacientů bez repozice byla senzitivita nižší - 65%., Aim. The aim of this study was the correlation of an ultrasound examination of the disk of the temporomandibular joint with the magnetic rezonance examination. Method. In total has been examined 95 patients, 112 mandibular joints, file include 10 men, 85 women, age interface 14-74 years old, the average age of 37 years. The patients were examined using Philips iu22 with linear 17 MHz probe with dynamic phase - with opening the mouth. On the MRI, the patients were examined using Philips Achieva 1.5 T with the surface coil and microscopic coil, each examination included a sequence with the gradual opening of the mouth. Results. Bio-degradable stents are be made of different synthetic polymers (like polylactide or polyglycolide) or their co-polymers (polydioxanone). They can be used for treating benign stenoses of the small and large intestine, particularly in Crohn’s disease. Endoscopic introduction of bio-degradable stents into small and large intestinal stenoses is feasible and relatively simple. Initial results are encouraging and the complication rate is low. However, there are still some difficulties that need to be overcome. The rate of early stent migration is still rather high (up to one third of patients). This might be solved by changes in the shape or rigidity of the stents as well as by further improvement in the design. The pilot study deals with the clinical results of the regenerative therapeutical method in intraosseous periodontal defects done by surgical augmentation procedure using a material of a new generation prepared on the basis of a bioactive glass. A group of 10 individuals were treated properly for chronic periodontitis. Some of treated infraalveolar periodontal lesions, both periodontal pockets and interradicular defects, persisted or recurred in posterior teeth. In order to eliminate them they were indicated for advanced periodontal surgery or regenerative surgical therapy stimulating healing processes of supportive tissues, mostly alveolar bone around treated teeth. Relevant clinical parameters, i. e. values of the pocket depth, gingival recession, and loss of attachment were evaluated before and after the periodontal surgery and compared with each other. Úvod: Důležitou součástí diagnostického algoritmu neuropatické bolesti jsou jednoduché dotazníkové nástroje založené na přítomnosti typických popisných charakteristik (tzv. deskriptorů). V českém jazykovém prostředí však není k dispozici validovaná verze žádného z mezinárodně akceptovaných dotazníků. Cíl: Cílem práce bylo vytvoření české jazykové verze jednoho z nejpoužívanějších dotazníků zaměřených na diagnostiku neuropatické bolesti a charakteristiku jednotlivých jejích subtypů, a to tzv. NPSI (Neuropathic Pain Symptom Inventory) a její validace. Soubory a metodika: Dotazník byl přeložen metodou „forward‑backward translation“. Pro potřeby validace byla touto českou jazykovou verzí následně vyšetřena skupina pacientů s neuropatickou bolestí periferního (n = 66) nebo centrálního (n = 30) původu (v rámci polyneuropatie či roztroušené sklerózy) a skupina nemocných s bolestí nociceptivní v důsledku pokročilé artrózy kyčelního nebo kolenního kloubu (n = 70). Za účelem hodnocení opakovatelnosti testu byl u části pacientů (n = 19) dotazník administrován dvakrát s odstupem přibližně tří hod. Výsledky: Srovnání jednotlivých hodnocených položek, dílčích škál i celkového NPSI skóre pomocí ROC analýzy prokázalo vysokou schopnost diskriminace mezi neuropatickou a nociceptivní bolestí (p < 0,001), ale nízkou schopnost odlišení neuropatické bolesti centrálního a periferního původu (p > 0,05). Opakovaná administrace testu potvrdila vysokou opakovatelnost testu (ICC, Intraclass Correlation Coefficient = 0,86 pro sumární skóre). Pomocí clusterové analýzy bylo identifikováno šest odlišných symptomových profilů. Závěr: Vytvořená česká verze dotazníku NPSI je schopna spolehlivě diskriminovat neuropatickou a nociceptivní bolest a identifikovat odlišné symptomové profily bolesti při akceptovatelné opakovatelnosti testu. Dotazník lze tedy doporučit jako vhodný a snadno použitelný nástroj v diagnostickém algoritmu neuropatické bolesti, využitelný i pro longitudinální sledování., Introduction: Simple questionnaires based on so called “neuropathic pain descriptors” are the most frequently used screening tools in the diagnosis of neuropathic pain, especially by non‑specialists. However, no validated Czech version of any of the internationally accepted nueropathic pain questionnaires is available. Aim: The aim of this study was to develop and validate a Czech version of the Neuropathic Pain Symptom Inventory (NPSI), a questionnaire that allows the user to characterize subtypes of neuropathic pain, is sensitive to change, and has discriminatory power. Patients and methods: The NPSI was translated into the NPSIcz using the forward‑backward translation method and validated in a group of patients suffering from neuropathic pain of peripheral (n = 66) or central origin (n = 30) due to polyneuropathy or multiple sclerosis, and in a group of individuals suffering from nociceptive pain (due to severe osteoarthritis of the knee or hip joint; n = 70). The NPSIcz was repeated within three hours in 19 individuals to determine test‑retest reliability. Results: The NPSIcz showed good diagnostic validity in discriminating between patients with neuropathic and nociceptive pain (p < 0.001) but low ability to discriminate between patients with central and peripheral neuropathic pain (p > 0.05). Repeated administration confirmed acceptable test‑retest reliability (intra‑class correlation coefficient of 0.86 for the NPSIcz sum score). Cluster analysis identified six different pain profiles. Conclusions: The NPSIcz is able to discriminate between neuropathic and nociceptive pain, to identify different pain profiles, and has acceptable test‑retest reliability. It can, therefore, be recommended as an easy‑to‑administer questionnaire suitable for the diagnosis of neuropathic pain, including long‑term follow up.
