Farmakogenetika je vedný odbor, ktorý skúma efekt jednotlivých liekov v závislosti od genotypu. V súčasnosti sú liečebné odporúčania pre liečbu niektorých monogenénových diabetov založené na genetickej diagnostike. Aj v oblasti farmakogenetiky perorálnych antidiabetík boli už publikované prvé štúdie, ktoré zistili asociácie jednotlivých génových variantov s liečebnou odozvou. Odozva na deriváty sulfonylurey bola signifikantne asociovaná s variantmi KCNJ11/ABCC8, TCF7L2 a CYP2C9. Odozva na liečbu metformínom bola asociovaná s variantmi génov ATM a SLC47A1. Odozva na liečbu glitazónmi bola asociovaná s variantom génu PPARG. Terapeutická odozva na liečbu gliptínmi bola asociovaná s variantmi génov TCF7L2 a CTRB1/2. Je možné očakávať, že v blízkej budúcnosti budú farmakogenetické poznatky využívané aj pri personalizácii liečby diabetu 2. typu., Pharmacogenetics is the study of how genes (individual genotypes) affect a person‘s response to drugs. At present, recommendations made about the treatment of some monogenic forms of diabetes are based on genetic diagnostics. The first studies in the field of pharmacogenetics of oral antidiabetics have now been published which have identified associations of individual genetic variants with response to treatment. The response to sulfonylurea derivatives was significantly associated with the variants KCNJ11/ABCC8, TCF7L2 and CYP2C9. The response to metformin treatment was associated with the genetic variants ATM and SLC47A1. The response to treatment with glitazones was associated with the genetic variant PPARG. The therapeutic response to the treatment with gliptins was associated with the genetic variants TCF7L2 and CTRB1/2. Fekálna mikrobiálna terapia (FMT) je liečebná metóda, pri ktorej sa prenesie fekálna mikroflóra chorému jedincovi od zdravého darcu, a tým sa obnoví normálne mikrobiálne zloženie čreva. V súčasnosti vzrastá záujem o využitie FMT pri liečbe rôznych chorôb, avšak neexistujú štandardné terapeutické protokoly. Postupy pri realizácii FMT sa líšia vo viacerých aspektoch, ako je výber darcu, príprava fekálnych materiálov, príprava príjemcu a spôsob aplikácie. FMT sa zdá byť najúspešnejšia v liečbe rekurentnej infekcie Clostridium difficile, randomizované kontrolované štúdie dokázali úspešnosť približne 90 %. Existujú aj obmedzené výsledky pre liečbu ulceróznej kolitídy, publikované boli malé série prípadov, ktorých výsledky však nie sú jednotné. Využitie FMT sa skúma aj v liečbe iných chorôb, pri ktorých je narušená črevná mikroflóra, ako sú napr. kardiovaskulárne, autoimunitné a metabolické ochorenia. Nateraz je pri FMT veľa nezodpovedaných otázok, a preto je nevyhnutný ďalší výskum v tejto oblasti., Fecal microbiota transplantation (FMT) is a therapeutic method, in which the fecal microflora from healthy donors is transmitted to the patient to restore the healthy microbial composition of the gut. In the recent years, there is a growing interest in the therapeutic potential of FMT in various diseases. The standard FMT protocols do not exist. Procedures of FMT vary in several aspects such as donor selection, preparation of fecal material, preparation of the recipient and administration way. FMT appears to be the most successful in the treatment of recurrent Clostridium difficile infection (CDI), randomized controlled studies reported 90 % success rate. There is a limited evidence for FMT as a treatment of ulcerative colitis. FMT has been also studied as treatment of diseases with impaired gut microbiota, such as cardiovascular, autoimmune and metabolic diseases. Incidence karcinomu endometria ve věkové skupině žen mladších 40 let narůstá, a proto roste i zájem o fertilitu šetřící léčbu. Nejčastěji používaná konzervativní léčba progestiny může být zvážena u pečlivě selektovaných pacientek s grade 1, stage IA karcinomem endometria bez myometriální invaze, které si přejí zachovat reprodukční funkce, a jsou ochotny podstoupit intenzivní léčbu a sledování a také určité riziko chirurgicky nestážovaného onemocnění. Před léčbou je potřeba co nejpřesněji zhodnotit biologický charakter a rozsah onemocnění. Hormonální terapie je aplikována obvykle 6 měsíců. Response rate se pohybuje přibližně v hodnotách 60–80 %, recurrence rate 25–40 %. Live birth rate je uváděn okolo 30 %. Je potřeba informovat pacientky o reálné možnosti recidivy, obvykle dobře léčitelné, i o výhodě použít metody asistované reprodukce, o potřebě intenzivního follow-up a doporučení definitivní léčby po dokončení reprodukčních plánů., The diagnosis of endometrial carcinoma in young women of childbearing age is rare, but the incidence in growing. Fertility-preserving therapy of endometrial carcinoma could be considered in patients with a histological diagnosis of grade 1 stage IA endometrial carcinoma without myometrial invasion who wish to preserve their fertility, are willing to accept intensive treatment, close follow-up and risk of unstaged disease. Conservative treatment is based on 6 months medical treatment with high-dose oral progestins. Response rate is referred between 60 and 80 %, recurrence rate 25–40 %; live birth rate around 30 %. An unusual combination of median nerve's variations has been encountered in a male cadaver during routine educational dissection. In particular, the median nerve was formed by five roots; three roots originated from the lateral cord of the brachial plexus joined individually the median nerve's medial root. The latter (fourth) root was united with the lateral (fifth) root of the median nerve forming the median nerve distally in the upper arm and not the axilla as usually. In addition, the median nerve was situated medial to the brachial artery. Léčba nádorového onemocnění prsu patří nejen do rukou chirurgů a onkologů, ale také do rukou rehabilitačních lékařů a fyzioterapeutů, kteří mají nezastupitelné místo v následné léčbě. Komplexním cílem léčby je zajistit co nejrychlejší uzdravení, a to včasným předcházením všem možným komplikacím, tzn. i těm, které vznikají v pohybovém aparátu. Rehabilitace po operaci prsu má hospitalizační a posthospitalizační fázi. Fáze hospitalizační rehabilitace je zaměřena především na prevenci pooperačních komplikací – dechových, tromboembolických. Posthospitalizační fáze má několik cílů: předcházet vzniku sekundárních reflexních změn v pohybovém aparátu, edukovat pacientky, jaká režimová opatření jsou nutná k prevenci vzniku a rozvoje sekundárního lymfedému, snažit se o včasný záchyt prvních známek lymfedému a časné zahájení léčby, pokud vznikl. Nejúčinnější léčbou lymfedému je komplexní dekongestivní terapie doplněná dlouhodobým podáváním léků pro systémovou enzymoterapii. Intervence v oblasti rehabilitační a fyzikální medicíny může být u pacientek po operaci prsu velmi prospěšná, protože umožní předejít příznakům, které snižují kvalitu života (pohybová omezení, ztráta soběstačnosti v aktivitách denního života apod), a následně vedou u onkologických pacientek ke vzniku deprese a s ní souvisejícímu poklesu imunity., The treatment of breast cancer is not only in the hands of surgeons and oncologists, but it also involves the irreplaceable follow-up care of rehabilitation doctors and physiotherapists. The comprehensive goal of the treatment is to achieve recovery in the shortest possible time, specifically through early prevention of all kinds of complications, including those developing in the locomotive organs. The rehabilitation following breast surgery consists of hospitalization and post-hospitalization stages. The stage of hospitalization rehabilitation is primarily focused on the prevention of postoperative complications – breathing, thromboembolic. The post-hospitalization stage is mainly aimed at the prevention of development of secondary reflexive changes in locomotive organs and the prevention of secondary lymphedema. The most efficient prevention to prevent lymphedema from developing is the complete decongestive therapy complemented with systemic enzyme therapy. Interventions in the area of rehabilitation and physical medicine may be very beneficial for patients after breast cancer, since they mediate prevention of symptoms that erode the quality of life (locomotor activity limitations, loss of self-reliance in daily activities and more), and subsequently lead to oncology patients developing depression and the associated decrease in immunity., and Pavla Formanová
BACKGROUND: Occurrence of gastric metastasis as the first symptom of breast carcinoma with a long period of latency before presentation of the primary breast carcinoma is rare. CASE REPORT: A patient with gastric metastasis as the first symptom of lobular breast carcinoma, treated by neoadjuvant preoperative chemoradiotherapy and total gastrectomy, with complete local control. Fourteen months after presentation of the gastric metastasis a primary lobular breast carcinoma was discovered, treated by radiotherapy, chemotherapy and hormonal treatment with complete local response. Twenty-three months after diagnosis of breast cancer multiple colorectal metastases from the breast cancer occurred, which were treated by chemotherapy and hormonal treatment. Eighty-six months after diagnosis of gastric metastasis the patient died due to progression of cancer. CONCLUSIONS: Metastases to gastrointestinal or gynaecological tracts are more likely in invasive lobular carcinoma than invasive ductal cancer. The pathologist should determine whether or not they check estrogen and progesterone receptor status not simply by signet ring cell morphology but also by consideration of clinic-pathological correlation of the patient, such as the presence of a past history of breast cancer, or the colorectal localization of poorly differentiated carcinoma, which may occur less frequently than in the stomach. and D. Waldenströmova makroglobulinemie (WM) je vzácné lymfoproliferativní onemocnění ze skupiny monoklonálních gamapatií, s incidencí 3 případy na 1 milion obyvatel. Toto onemocnění je charakterizováno infiltrací kostní dřeně klonálními B-lymfocyty a přítomností monoklonálního imunoglobulinu třídy IgM v séru. Nejčastěji se jedná o indolentní onemocnění s mediánem přežití 6 let. Molekulární podstata není zcela objasněná, ale delece, trizomie chromozomů 4 a 8 a delece 13q se zdají být typické pro WM. Mutace MYD88L265P a CXCR4WHIM se u WM vyskytují velice často a mají vliv na růst a přežití nádorových buněk. Tato práce se zaměřuje na současné poznatky o chromozomových aberacích a genových mutacích spojených s patofyziologií WM. Klíčová slova: chromozomové aberace – somatické mutace – Waldenströmova makroglobulinemie, Waldenstöm macroglobulinemia (WM) is a rare lymphoproliferative disorder, currently classified as a monoclonal gammopathy, with incidence rate of 3 per million. The disease is characterized by presence of clonal B lymphocytes in the bone marrow and by presence of monoclonal immunoglobulin IgM in serum. It is mostly an indolent disorder, with median overall survival 6 years. Molecular pathogenesis of WM remains unclear, but deletion of 6q and 13q, trisomy of chromosomes 4 and 8 seem to be typical. Mutations of MYD88L265P and CXCR4WHIM are very common for WM and affect growth and survival of malignant cells. This work is aimed at the current knowledge of chromosomal aberrations and gene mutations connected to the pathophysiology of WM. Aneuryzma břišní aorty (AAA) je závažné onemocnění s vyšší prevalencí v pokročilém věku. Výdutě šíře nad 5,5 cm jsou indikovány k operačnímu nebo endovaskulárnímu řešení. Zřídka se dnes pro velké riziko jejich ruptury setkáváme s rozsáhlými výdutěmi. V kazuistice prezentujeme případ objemného AAA a jeho operačního řešení., Abdominal aortic aneurysm (AAA) is related with higher age and poses a serious threat to the patient´s life. Aneurysms larger than 5.5 cm in diameter are indicated for open or endovascular repair. Giant aneurysms are rarely encountered due to a high risk of rupture. Gliomy limbického a paralimbického systému představují anatomicky a tím pádem i operačními přístupy unikátní skupinu mozkových nádorů. Mikrochirurgie v kombinaci s moderními perioperačními postupy zvyšuje šance na radikální odstranění těchto gliomů. Z hlediska lokalizace je dělíme do tří základních skupin: 1. gliomy inzuly, 2. amygdalohippokampálního komplexu a 3. cingula. V minimonografii se věnujeme anatomii i funkci limbického a paralimbického systému a v návaznosti na ni představujeme ucelený pohled na jednotlivé neurochirurgické přístupy k těmto oblastem. Maximální resekce za zachování funkční integrity mozku je předpokladem následné smysluplné onkologické léčby. Taktéž prezentujeme výsledky operací našeho pracoviště ve smyslu morbidity a radikality za posledních sedm let., Limbic and paralimbic gliomas represent a unique group of brain tumors both from an anatomic and surgical point of view. Microsurgery, together with modern perioperative procedures, enables a wider extent of resection of these gliomas. Based on localization, we divide gliomas into three basic groups: 1. insular, 2. amygdalohippocampal complex and 3. cingulum gliomas. In our review, we describe the anatomy and function of the limbic and paralimbic system and in relation to that, we present a description of the specific neurosurgical approaches to these regions. Maximal resection with brain function preservation represents the basis for subsequent oncologic therapy. We also present the surgical results of our department in the last seven years with regard to morbidity and radicality.
