INTRODUCTION: Cellulitis remains a very serious disease even today. Mortality, which varied between 10-40%, has been reduced owing to the standard securing of airway patency and use of an appropriate surgical treatment approach. MATERIALS AND METHODS: A total of 195 patients were hospitalised for cellulitis at the University Hospital in Hradec Králové during 2007-2011. The following parameters were evaluated: age, gender, dependence of incidence of the disease on the season of the year, frequency of attacks of the particular areas and their clinical characteristics, aetiology of the inflammation, types of patient complaints, prevalence of current systemic diseases, results of microbiological and selected laboratory analyses, socio-economic status of the patients, and duration of patient stay at the hospital. Statistical analysis was performed by using Pearson's correlation coefficient, the statistical significance level was p < 0.05. RESULTS: The mean age of the patients was 39.8 years. The group of 195 patients included 108 (55%) males and 87 (45%) females. The mean time between the first symptoms of the disease and admission to the Department was 5 days. From among the 195 patients, 116 (59.5%) were working persons, 79 (40.5%) were non-working (children, students, unemployed persons, women on maternity leave, retired people). The odontogenic origin of the disease was verified in 173 (88.7%) patients. In total, 65 (33.3%) patients had no coinciding complicating systemic disease, 22 (11.3%) patients had diabetes mellitus. The most frequent symptom of cellulitis was painful swelling, found in 194 (99.5%) patients, followed by jaw contracture, found in 153 (78.5%) patients. CONCLUSION: The results are largely very similar to those of previous studies performed in other countries, except that we found no correlation between the prevalence of cellulitis and the socio-economic status, nor have we confirmed Klebsiella pneumoniae sp. as the cause of cellulitis in patients with diabetes mellitus. and H. Úvod: Biliární ileus představuje 1−4 % mechanických obstrukcí gastrointestinálního traktu, ale u pacientů nad 65 let již 25 % nestrangulačních mechanických ileů tenkého střeva. Objemný kámen žlučníku obvykle vytvoří tlakovou nekrózu, protlačí se vzniklou biliodigestivní píštělí a obturuje gastrointestinální trakt. Kazuistiky: Mezi daty 1. 1. 2000 a 31. 1. 2015 bylo na Chirurgické klinice FN v Plzni operováno 35 pacientů pro poruchy pasáže s nálezem biliárního ileu. Průměrný věk pacientů v době operace byl 79 let (medián 78 let, min. 58 let, max. 92 let). Aerobilie na rentgenovém nativním snímku břicha byla přítomna z 27 pořízených snímků v 7 případech (26 %). Předoperačně byla provedena dvakrát kolonoskopie a devětkrát ezofagogastroduodenoskopie. Z operačních výkonů byla 21x provedena extrakce kamene z enterotomie, třikrát extrakce z gastrotomie, sedmkrát resekce tenkých kliček, jedenkrát Hartmannova resekce, jedenkrát resekce ileoascendentoanastomózy a jedenkrát extrakce konkrementu s následnou duodenojejunální anastomózou a s cholecystektomií v jedné době. Jedenkrát byla provedena ileotransverzoanastomóza. Pooperační 30denní mortalita byla 14,3 %. Pooperační morbidita dle klasifikace Clavien-Dindo činila u I. stupně 22,8 %, u II. stupně 5,7 %, u III. stupně 11,5 % a u IV. stupně 11,5 %. K recidivě biliárního ileu došlo u 8,8 % pacientů. Primární ošetření píštěle a cholecystektomie byla provedena v jednom případě. Závěr: Rozhodnutí o ošetření cholecystoenterální píštěle a cholecystektomii v první době bude vždy záležet na celkovém stavu pacienta a místním nálezu v oblasti žlučníku. Při ponechání píštěle je nutné počítat s rizikem recidivy ileu. Při operaci je nutné též zrevidovat celé střevo k vyloučení přítomnosti dalších konkrementů a snížit tak riziko časné recidivy. However, the rate of non-strangulated small bowel obstructions reaches as much as 25% in patients over 65 years of age. Usually, a pressure necrosis is created by a large gallstone that passes through the developed biliodigestive fistula, subsequently obturating the gastrointestinal tract. Case reports: 35 patients underwent a surgical procedure at the Department of Surgery, Teaching Hospital Pilsen for biliary ileus from January 1, 2000 to January 31, 2015. Mean age was 79 years (median 78 years; min. 58 years; max. 92 years). Aerobilia was visible in abdominal X-ray scans in 7 cases (26%) of 27 acquired images. Preoperatively, colonoscopy was done 2 times and esophagogastroduodenoscopy 9 times. Regarding surgical procedures, enterolithotomy was done in 21 cases, extraction of the gallstone from gastrotomy was done 3 times, small bowel resection 7 times, Hartmann resection 1 time, resection of ileoascendentoanastomosis 1 time, and extraction of the gallstone from duodenotomy with duodenojejunoanastomosis and cholecystectomy was done in 1 case. Ileotransversoanastomosis was performed in 1 case. Postoperative 30-day mortality was 14.3%. Postoperative morbidity according Clavien-Dindo was 22.8% for grade 1, 5.7% for grade 2, 11.5% for grade 3, and 11.5% for grade 4. A recurrence was observed in 8.8%. Primary treatment of the fistula and cholecystectomy were done in 1 case. Conclusion: The indication of primary biliodigestive fistula treatment always depends on the general condition of the patient and on the local finding in the gallbladder area. Residual fistula increases the risk of reccurence, but primary treatment is associated with higher mortality. Cíl: Na základě existujících cizojazyčných testů sestavit a validovat vlastní, v českém jazyce dosud chybějící test funkcionální komunikace u pacientů s afázií. Metodika: Byl vytvořen Dotazník funkcionální komunikace (DFK), který hodnotí stav komunikace v reálných situacích pomocí 20 položek v oblastech: I. bazální komunikace, II. sociální komunikace, III. čtení a psaní a IV. čísla a orientace; každá položka je hodnocena šestistupňovou škálou (0–5 bodů). Maximální možná hodnota Indexu funkcionální komunikace (Index FK) dosahuje 100 bodů. DFK byl validován na souborech zdravých dobrovolníků (n = 110, medián věku 63 let), pacientů s afázií (n = 38, medián věku 62 let) a s Alzheimerovou demencí (n = 8, medián věku 81,5 let). Výsledky: Hodnoty Indexu FK korelovaly významně s věkem (Spearman r = –0,354; p = 0,000148), proto byly stanoveny normativní hodnoty Indexu FK pro věk: 50–59 let: > 90; 60–69 let: > 85; 70 a více let: > 76. Pomocí ROC analýzy byla potvrzena vysoká diagnostická validita DFK v diskriminaci mezi normálními jedinci a pacienty s afázií (senzitivita při cut‑off hodnotě Indexu FK 86,5 je 89,5 % a specificita je 93,6 %; AUC = 0,974; p < 0,001). Hodnoty Indexu FK u pacientů s afázií významně korelovaly se stupněm jazykového deficitu vyjádřeného testem MASTcz a nezjistili jsme rozdíl oproti hodnotám Indexu FK u pacientů s poruchami komunikace u Alzheimerovy demence. Potvrdili jsme test‑retest reliabilitu opakovaným vyšetřením 10 pacientů s afázií. Byl vytvořen terapeutický materiál, který navazuje na položky DFK. Závěr: DFK je nový validovaný test v českém jazyce, který doplňuje paletu diagnostických nástrojů pro pacienty s afázií. Výzkum naznačil jeho výborné psychometrické vlastnosti, jež je třeba ověřit na větších souborech., Aim: To develop and validate a new, in the Czech language still missing test of functional communication for patients with aphasia. Methods: Functional Communication Questionnaire (FCQ) comprises 20 items that evaluate communication in four areas of real situations: I. Basal communication, II. Social communication, III. Reading and writing, and IV. Calculation and orientation. Every item is evaluated on 6–degree scale (0–5 points), and the sum (Functional Communication Index – FCI) of 100 points represents maximum possible value. FCQ was validated in groups of healthy volunteers (n = 110, median age 63 years), patients with aphasia (n = 38, median age 62 years), and patients with Alzheimer dementia (AD) (n = 8, median age 81.5 years). Results: Values of FCI correlated significantly with age (Spearman r = –0.354, p = 0.000148); and different normal limits of FCI were established for decades: 50–59 years: > 90; 60–69 years: > 85; 70–79 years: > 76. Using ROC analysis we confirmed high diagnostic validity of FCQ in discrimination between healthy controls and patients with aphasia (sensitivity of 89.5% and specificity of 93.6% for cut‑off value 86.5; AUC = 0.974; p < 0.001). Degree of functional communication impairment quantified with FCI in patients with aphasia correlated with the degree of language deficit quantified with MASTcz, while there was no significant difference in FCI values between patients with aphasia and AD. Repeated evaluation in 10 patients with aphasia proved very high test‑retest reliability of FCQ. New therapeutic material based on FCQ was finally introduced into clinical practice. Conclusion: FCQ extends the repertoire of diagnostic tests for patients with aphasia available in the Czech language. Our results showed very good psychometric characteristics of FCQ that should be confirmed by further research. Úvod: Je uváděno, že prevalence neurodegenerativních onemocnění v malých evropských komunitách izolovaných od okolí je vyšší ve srovnání s běžnou populací. Tentýž jev jsme pozorovali v malém, relativně izolovaném regionu jihovýchodní Moravy. Cíl: Zjištění prevalence neurodegenerativního parkinsonizmu v izolovaném regionu jihovýchodní Moravy. Metodika: Byla použita metoda třífázového vyšetření. V první fázi byly do ordinací praktických lékařů distribuovány dotazníky, které byly vyplněny všemi pacienty, kteří se z jakéhokoliv důvodu dostavili v průběhu tří měsíců do ordinace praktického lékaře. Ve druhé fázi byly u pozitivních respondentů příznaky parkinsonizmu objektivizovány vyšetřením edukovaným ambulantním neurologem. Ve třetím stupni byli respondenti, u kterých byly při skríningovém vyšetření příznaky parkinsonizmu objektivizovány, přijati k detailnímu vyšetření za hospitalizace v terciárním centru pro diagnostiku a léčbu neurodegenerativních onemocnění. Výsledky: Celková prevalence parkinsonizmu v populaci starší 50 let byla 2,8 % (95% CI: 2,2-3,4); prevalence v populaci 50-64 let byla 1,9 % (95% CI: 1,2-2,5); a v populaci nad 65 let byla 4,06 % (95% CI: 2,9-5,1). Byly získány tři velké rodokmeny s patrnou autozomálně dominantní dědičností parkinsonizmu. Závěr: Zjištěná prevalence byla překvapivě vysoká a podstatně odlišná od hodnot prevalence publikovaných v jiných evropských zemích. Vzhledem k charakteristice regionu je předpokládán vliv genetických faktorů. Současný výzkum je zaměřen na genetické pozadí a DNA analýzu probandů z rodokmenů, ve kterých byla identifikována autozomálně dominantní dědičnost parkinsonizmu. We recently observed this phenomenon in a small specific region of south-eastern Moravia. Objective: To assess the prevalence of neurodegenerative parkinsonism in an isolated region with a rural population in south-eastern Moravia. Methods: A three-stage method of data collection was used. In the first phase, originally designed questionnaires were distributed to general practitioners and completed by all patients who visited them for any reason during a three-month period. In the second phase, positive responders were examined by trained primary care neurologists. Finally, the diagnosis was confirmed or excluded by a movement disorders specialist. Results: The overall prevalence in the population older than 50 years of age was 2.8% (95% CI: 2.2-3.4); the prevalence in the population from 50 to 64 years was 1.9% (95% CI: 1.2-2.5), and it was 4.06% (95% CI: 2.9-5.1) in the population over 65 years of age. Three large families with an autosomal-dominant inheritance patterns of parkinsonism were identified. Conclusions: The prevalence rates were surprisingly high; they substantially differed from the published prevalence rates in other European countries. Due to the characteristics of the region, we expected a particular impact of genetic factors, most probably the autosomal-dominant inheritance of parkinsonism. Poruchy spánku patria k najčastejším non‑motorickým prejavom u pacientov s Parkinsonovou chorobou (PCh), v populačných štúdiách je uvádzaný ich výskyt až u 60 % pacientov. Štúdie tiež uvádzajú rozdielnosť prejavov PCh medzi mužmi a ženami. Cieľ: Zistiť, či sú u pacientov s PCh na Slovensku prítomné medzi pohlaviami rozdiely v klinických prejavoch PCh a výskyte porúch spánku. Materiál a metódy: Zisťovanie bolo robené dotazníkovou metódou s účasťou pacientov a neurológov z jednotlivých regiónov Slovenskej republiky. Dotazníky boli vyplňované počas rutinných kontrolných návštev. Boli zisťované socioekonomické premenné ako pohlavie, vek, vek pri začiatku ochorenia, stupeň poškodenia hodnotený škálou Hoehna a Yahrovej, dominujúci fenotyp ochorenia a používaná medikácia. Pri hodnotení porúch spánku bola použitá slovenská verzia dotazníka PDSS. Výsledky: Získali sme dáta od 1 067 pacientov. Pri porovnávacích analýzach neboli zistené podstatné rozdiely vo väčšine hodnotených parametrov. U mužov bol o niečo vyšší podiel pacientov s hodnotou škály Hoehna a Yahrovej 4 a boli zistené signifikantne vyššie dávky levodopy v monoterapii (p < 0,01). Významné rozdiely boli zistené v skóre škály PDSS pre hodnotenie rušivých halucinácií počas noci – skóre u mužov bolo 8,22, u žien 8,48 (p < 0,05). Podobný výsledok sme zaznamenali v skóre hodnotiacom nutnosť močenia počas noci – u mužov bolo skóre 5,90, u žien 6,53 (p < 0,01). Differences in symptoms between men and women have been reported. Aim: In the present study, we assessed whether there are gender differences in clinical presentation of PD and prevalence of sleep disturbances in individuals diagnosed in the Slovak Republic. Material and method: Questionnaires were distributed to participating neurologists and patients in outpatient practices across the Slovak Republic. Sociodemographic variables – gender, age, age at onset, disease severity according to Hoehn and Yahr stage, phenotype of the dominating symptom of Parkinson’s disease and type of medication – were collected. The Slovak language version of the PDSS was used in a questionnaire to test sleep disturbances. Results: Data from 1,067 outpatients with PD were collected. Comparative analyses showed males and females not to be significantly different on the majority of the demographic and medical characteristics collected. Males had a slightly higher proportion of individuals with Hoehn and Yahr score 4 and, among those taking levodopa medication as monotherapy, males took significantly higher levodopa dose than females (p < 0.01). A significant difference in the distribution of PDSS subscores between males and females was observed on item 7 (distressing hallucinations at night) – score for males 8.22, for females 8.48, p < 0.05. Cíl studie: Metabolismus železa musí být velmi pečlivě regulován. Klíčovým systémovým regulátorem je peptidový hormon hepcidin, který způsobuje “uzamčení” železa uvnitř buňky; to pak nemůže být utilizováno. Dialyzovaní pacienti často trpí anémií, která je způsobena mnoha faktory, mj. stavem mikrozánětu a také zřejmě zvýšenou retencí hepcidinu v důsledku snížené glomerulární filtrace. Naším cílem bylo popsat vztahy mezi hepcidinem a dalšími parametry metabolismu železa, zánětu a erytropoezy u těchto pacientů. Typ studie: Observační Název a sídlo pracoviště: Ústav klinické biochemie a hematologie LF UK a FN v Plzni, Alej Svobody 80, 304 60 Plzeň Materiál a metody: Do studie bylo zařazeno 164 chronicky dialyzovaných pacientů (věk 66 ± 13, 25 - 92 let; 101 mužů a 63 žen) a 37 zdravých kontrol (věk 55 ± 20, 21 - 92 let; 16 mužů a 21 žen), u kterých byl stanoven jejich kompletní krevní obraz, parametry metabolismu železa, zánětu a výživy. Výsledky: Koncentrace železa, transferrinu a hemoglobinu byla významně nižší u dialyzovaných pacientů (p<0,0001), naopak ferritin (p<0,0001), solubilní transferrinové receptory(p<0,05), hepcidin (p=0,0003), CRP a IL-6 (p<0,0001) byly u těchto pacientů významně vyšší než u zdravých kontrol. Hepcidin slabě koreloval pozitivně s CRP a ferritinem a negativně s transferrinem u dialyzovaných pacientů, u zdravých kontrol nebyla nalezena žádná korelace hepcidinu s ostatními parametry. Závěr: Zdá se, že vliv zánětu na hladiny hepcidinu není u hemodialyzovaných pacientů zcela zásadní a na jeho koncentraci se podílí i jiné faktory, např. jeho retence., Objective: Peptide hormone hepcidin is a key systemic regulator of the iron metabolism. Hepcidin binds to the iron cell exporter ferroportin so iron is kept in the cells unavailable for erythropoiesis. The synthesis of hepcidin is up-regulated by high iron stores and inflammation. Dialyzed patients have very often impaired iron management – they suffer from anemia, which is caused by many factors including the state of micro inflammation and hepcidin retention due to decreased glomerular filtration rate. Our aim was to describe the relationship of hepcidin and other parameters of iron metabolism, erythropoiesis and inflammation. Design: Observation Settings: Institute of Clinical Biochemistry and Hematology, Charles University Hospital in Pilsen, Alej Svobody 80, 304 60 Plzeň Material and Methods: Complete blood cell count, iron, ferritin, transferrin, CRP, albumin, creatinine, hepcidin, soluble transferrin receptors (sTfR) and IL-6 were measured in samples from 164 patients included in chronic hemodialysis program (age 66 ± 13, 25 - 92 years), 63 women and 101 men and 37 control healthy volunteers (age 55 ± 20, 21 - 92 years), 21 women and 16 men. Results: Iron, transferrin and hemoglobin were significantly lower in the patients group (p<0.0001) while ferritin (p<0.0001), sTfR (p<0.05), hepcidin (p=0.0003), CRP and IL-6 (p<0.0001) were significantly higher in the patients group. Hepcidin weakly positively correlated with CRP and ferritin and weakly negatively correlated with transferrin in hemodialyzed patients. No correlation of hepcidin with other biochemical parameters in controls was found. Conclusion: It seems that the influence of inflammation on hepcidin levels in hemodialyzed patients is not crucial and other factors including its retention in end-stage renal disease participate., T. Imunostimulace polybakteriálním lyzátem (Urivac®) v prevenci recidivujících infekcí dolních močových cest. Infekce močových cest (IMC) zaujímají v evropských zemích i u nás dlouhodobě druhé místo ve výskytu infekčních onemocnění v populaci a představují tak významný socioekonomický problém. Spolu s respiračními infekty se rozhodující měrou podílejí na preskripci antimikrobiálních přípravků. Extenzivní antimikrobiální léčba je jednou z nejvýznamnějších příčin nárůstu bakteriální rezistence. Určitou možnost prevence recidiv infekcí dolních močových cest a snížení spotřeby antimikrobiálních preparátů představuje imunostimulace. Prospektivní multicentrická studie hodnotila klinický účinek, snášenlivost a přínos k léčbě recidivujících zánětů při podávání imunostimulačního přípravku Urivac®. Počet atak infekcí dolních močových cest se po imunostimulaci významně snížil jak v jejím průběhu, tak po jejím ukončení (p = 2,2x10-16). V průběhu imunostimulace bylo bez příznaků 85 % nemocných a při dalším sledování 70,8 %. Snížil se také počet aplikovaných antimikrobiálních přípravků. Subjektivní zlepšení přetrvávalo ve více než 57 % i po ukončení léčby. Mikrobiologická odpověď dosáhla 77 %, v odstupu od ukončení léčby se snižuje. U exacerbací byla uropatogenní flóra detekována v 61 %. Nežádoucí účinky u dvou nemocných vedly k předčasnému ukončení terapeutického režimu., In European countries, including the Czech Republic, urinary tract infections (UTIs) have long occupied the second place in the incidence of infectious diseases in the population, thus representing a major socioeconomic problem. Along with respiratory infections, they are the major contributors to the prescription of antimicrobial agents. Extensive antimicrobial therapy is one of the most important causes of increased bacterial resistance. Immunostimulation is a possible way of preventing recurrent lower urinary tract infections and reducing the consumption of antimicrobial agents. A prospective multicentre study evaluated the clinical effi cacy, tolerability, and benefi t to treatment of recurrent infections with the immunostimulatory drug Urivac®. The number of attacks of lower urinary tract infections following immunostimulation reduced signifi cantly both during its course and after its cessation (p = 2.2 x 10–16). During immunostimulation, 85 % of patients were symptom-free, and during subsequent follow-up it was 70.8 %. There was associated with a reduction in the number of the antimicrobial agents administered. Subjective improvement persisted in more than 57 % even after treatment cessation. Microbiological response reached 77 %; it decreases with increased time from treatment cessation. In the case of exacerbations, uropathogenic fl ora was detected in 61 %. Cíl: Zjistit průměrnou incidenci infekční endokarditidy (IE) v okrese Tábor za sledované období a srovnat získané údaje s dříve publikovanou celorepublikovou studií. Metodika: Retrospektivní monocentrická observační studie sledující výskyt a charakteristiky infekční endokarditidy v definované spádové oblasti za roky 2009–2013. Zařazeni byli pouze pacienti s prokázanou a léčenou infekční endokarditidou a pacienti, u nichž byla diagnóza stanovena až pitvou. Výsledky: Ve sledovaném období bylo zachyceno celkem 39 případů infekční endokarditidy, což odpovídá průměrné incidenci 7,8 případů na 100 000 obyvatel ročně. Medián věku pacientů v době diagnózy byl 67 let. V souboru nemocných výrazně převažovali muži, kteří byli postiženi 3krát častěji než ženy. Přibližně stejně četně byla postižena mitrální (36 %) a aortální (33 %) chlopeň a elektroda kardiostimulačního systému (31 %). Ve srovnání s referenční prací bylo zachyceno více infekcí u pacientů s kardiostimulátory. Dvě třetiny infekcí byly způsobeny stafylokoky, což je proti pilotní studii výrazný nárůst. Naopak mortalita se podstatně neměnila. Závěr: Charakteristiky infekční endokarditidy se vyvíjejí, hospitalizační mortalita pacientů s IE je vysoká, kolem 30 %. Samo onemocnění IE je z hlediska roční úmrtnosti špatným prognostickým faktorem. Klíčová slova: incidence – infekční endokarditida, Aim: To find out an average incidence of infective endocarditis (IE) in Tabor district in a monitored period of time and compare the obtained data with previously published national study. Methods: Retrospective monocentric observational study monitoring occurence and characteristics of infective endocarditis in a defined region in years 2009–2013. Only patients with proved and treated infective endocarditis and patients whose diagnosis was revealed by autopsy were involved. Results: In the monitored time 39 cases of infective endocarditis occured, which means that the average incidence was 7.8 cases on 100 000 people per year. Median age in the time of diagnosis was 67. The group of patients was strongly dominated by men, who were affected three times more often than women. Almost the same rates of infective endocarditis were localized on mitral (36 %) and aortic (33 %) valve and electrode of a pacemaker (31 %). In comparison with the reference study there were more pacemaker-related infections detected. Two thirds of infections were caused by staphylococci. That means a prominent growth when compared to the pilot study. On the contrary, there was no significant change in mortality. Conclusion: Characteristic features of infective endocarditis are evolving, patients’ in-hospital mortality rate is high, about 30 %. The disease by itself is a poor prognostic factor in a one-year mortality rate. Key words: incidence infective – infective endocarditis, and Rudolf Koubek
Úvod: Kombinace monoklonální antiCD20 protilátky rituximabu s chemoterapií je vysoce účinná v léčbě chronické lymfocytární leukemie (CLL). Hlavním nežádoucím účinkem rituximabu jsou infuzní reakce, např. syndrom uvolnění cytokinů (cytokine release syndrome – CRS), který zpravidla doprovází první podání rituximabu. Cílem naší retrospektivní analýzy bylo zhodnotit četnost výskytu infuzních reakcí v rutinní klinické praxi, jejich vztah k parametrům nádorové nálože a zjistit možnou spojitost infuzních reakcí s účinností léčby. Dále byla posouzena bezpečnost rychlé infuze s rituximabem u nemocných s CLL. Charakteristika souboru: Hodnoceno bylo 108 nemocných s CLL léčených chemoimunoterapií s rituximabem od května roku 2005 do března roku 2011 v našem centru. V rámci 1. linie (n = 66, 71 % mužů, věkový medián 63 let) byly použity tyto léčebné režimy: fludarabin, cyklofosfamid, rituximab – FCR (n = 43) a nízkodávkovaný FCR (n = 13); 10 nemocných bylo léčeno jinými protokoly. 42 nemocných (76 % mužů, věkový medián 65 let) podstoupilo 2. linii: rituximab-dexametazon (n = 18), FCR/nízkodávkované FCR (n = 14) a 10 nemocných jiný režim. Součástí premedikace před infuzí rituximabu byl metylprednisolon 80 mg i.v., paracetamol 1 000 mg p. o. a bisulepin 1 mg i. v., všichni nemocní měli nitrožilní hydrataci minimálně 2 000 ml/den a alopurinol 300–600 mg/den. V 1. cyklu byl rituximab podáván frakcionovaně v celkové dávce 375 mg/m2 a od 2. cyklu formou rychlé infuze v dávce 500 mg/m2. Výsledky: K rozvoji infuzní reakce po rituximabu došlo u 32 % nemocných v 1. linii a 19 % v 2. linii. Jednalo se převážně o mírné reakce a k závažné toxicitě (stupeň III/IV dle NCI CTCAE – National Cancer Institute – The Common Terminology Criteria for Adverse Events) došlo výjimečně (3 % nemocných v 1. linii a 2 % v 2. linii). Reakce se nejčastěji projevila třesavkou, zimnicí, dušností, horečkou či hypotenzí. U všech nemocných bylo možné dokončit infuzi rituximabu v plánované dávce ve stejný den. Hodnocení léčebné odezvy (overall response rate – ORR) neprokázalo statisticky významné rozdíly mezi nemocnými s reakcí či bez infuzní reakce. Nemocní s infuzní toxicitou měli vyšší absolutní počet lymfocytů (1. linie, 87 vs 56 × 109/l, p = 0,21; 2. linie, 101 vs 14 × 109/l, p = 0,043). Při mediánu sledování 36 měsíců nebyl prokázán statisticky významný rozdíl v PFS (doba přežití bez progrese – progression-free survival) a OS (celkové přežití – overall survival) mezi oběma skupinami. Závěry: Výskyt infuzních reakcí u nemocných s CLL léčených rituximabem je poměrně běžný (32 %), avšak při pečlivé premedikaci jsou reakce velmi dobře zvladatelné a těžká forma je vzácná. Potvrdili jsme velmi dobrou snášenlivost rychlé infuze rituximabu. Naše data neprokázala statisticky významnou souvislost mezi výskytem infuzních reakcí a efektem léčby., Background and Aims: Rituximab in combination with chemotherapy is an effective treatment of patients (pts) with chronic lymphocytic leukemia (CLL). The most frequent adverse event of rituximab is infusion-related toxicity, e.g. cytokine-release syndrome that occurs usually during the first infusion. However, there is scarce data on feasibility and tolerability of rituximab infusions in CLL outside clinical trials. Therefore, we performed a single-center retrospective analysis of the frequency of rituximab infusion-related adverse events during the first- and the second line CLL treatment administered in the routine practice. We also analyzed its relation to parameters of tumor load and possible association with treatment efficacy. The safety of rapid infusion of rituximab in CLL pts was also evaluated. Patients and Methods: We analyzed 108 pts with CLL treated with rituximab-containing regimens between March 2005 and May 2011 at our institution. The most common first-line regimens (n = 66, 47 males, median age 63 years) were FCR (43 pts) and low-dose FCR (13 pts); 10 pts were treated by other protocols. Forty-two pts (32 males, median age, 65 years) underwent second line treatment: 18 pts rituximab-dexamethasone, 7 pts FCR, 7 pts low-dose FCR and 10 pts other regimens. Intravenous hydration (2 000 ml daily on days 0 and 1 of cycle), allopurinol 300–600 mg p. o. daily and premedication with methylprednisolone 80 mg i. v., acetaminophen 1 000 mg p. o. and bisulepine 1 mg i. v. were administered before rituximab infusion. Rituximab was given by fractionated infusion (100 mg for 2 hours, then if tolerated well, the rest of the dose with infusion rate escalation from 100 mg/hour up to 400 mg/hour) at the dose of 375 mg/m2 in the first cycle. Subsequent doses (500 mg/m2) were administered by rapid-infusion protocol. Results: Rituximab infusion-related toxicity occurred in 32 % pts (n = 21) during the first line treatment and 19% pts (n = 8) during the second line treatment. Adverse events were predominantly mild and NCI CTCAE grade III/IV occurred rarely (3% in the first line, 2% in the second line). Infusion toxicity manifested predominantly as rigors, chills, fever and hypotension. All patients with adverse events could finish rituximab infusion as initially planned on the same day. Treatment response analysis did not demonstrate statistically significant differences between patients with and without rituximab infusion toxicity. Patients who developed rituximab infusion toxicity had higher absolute lymphocyte count (first line, 87 vs 56 × 109/l, p = 0.21; second line, 101 vs 14 × 109/l, p = 0.043). At the median follow-up of 36 months, there were no statistically significant differences in PFS or OS in both cohorts. Conclusions: Rituximab infusion-related toxicity in pts with CLL is relatively frequent (32%). However, occurrence of infusion-related symptoms can be reduced by proper premedication and severe adverse events are uncommon. In our experience, all patients were able to receive the planned dose of rituximab. Subsequent doses of rituximab could be safely administered by rapid-infusion protocol. We assessed the intensive care unit (ICU) patients for Invasive aspergillosis (IA) with culture and non-culture based diagnostic methods from Iran. Thirty-six ICU patients with underlying predisposing conditions for IA were enrolled in the study. Sixty eight Bronchoalveolar lavage (BAL) samples were collected by bronchoscope twice a weekly. BAL samples were analyzed by microscopic examination, fungal culture and galactomannan (GM) detection. The Platelia Aspergillus GM EIA was used to quantify GM indices. Samples with a BAL GM index > or = 1 were considered as positive for GM. Patients were classified as having probable or possible IA. Out of 36 suspected patients to IA, 36.1% of cases showed IA which were categorized as: 4 cases of possible IA and 9 of probable IA. 76.2% of BAL samples were positive for GM. From 13 patients with IA, 11 (84.6%) had at least one positive BAL GM index. Of these patients, 9 (81.8%) showed probable IA. The main underlying predisposing conditions were neutropenia (53.8%) and COPD (30.8%). Our study has indicated that COPD must be considered as one of the main predisposing condition for occurrence of aspergillosis in ICU patients.
