There are various complications of prosthetic valvular surgeries. Among them, leaflet perforation should be emphasized and brought to mind when there have been eccentric valvular regurgitation. The presence of a broad spectrum of autoantibodies in Sjögren's syndrome (SjS) patients is the result of abnormal B-cell regulation that can be at least partially explained by abnormal BAFF/BAFFR regulation. The objective of this study was to determine both membrane and intracellular expression of BAFF/BAFFR in monocytes and B-cells in peripheral blood of 19 primary Sjögren's syndrome patients and 20 healthy controls using flow cytometry. We also measured sBAFF in serum. Compared to healthy controls, both surface and intracellular expression of BAFF was significantly increased in monocytes and B-cells of SjS patients. Also serum sBAFF level was elevated. Expression of BAFFR on B-cells of SjS patients was surprisingly decreased, but there was no clear increase or decrease within monocytes. Our results indicate that activated monocytes communicate with B-cells via BAFF and BAFFR, so that B-cells are stimulated, but BAFF is also produced to stimulate cells in autocrine way. The decrease of BAFFR expression in SjS patients suggests that there is the mechanism that attempts to take over in order to balance the high level of BAFF. and J. Krejsek, M. Koláčková, I. Cíl: Zjistit průměrnou incidenci infekční endokarditidy (IE) v okrese Tábor za sledované období a srovnat získané údaje s dříve publikovanou celorepublikovou studií. Metodika: Retrospektivní monocentrická observační studie sledující výskyt a charakteristiky infekční endokarditidy v definované spádové oblasti za roky 2009–2013. Zařazeni byli pouze pacienti s prokázanou a léčenou infekční endokarditidou a pacienti, u nichž byla diagnóza stanovena až pitvou. Výsledky: Ve sledovaném období bylo zachyceno celkem 39 případů infekční endokarditidy, což odpovídá průměrné incidenci 7,8 případů na 100 000 obyvatel ročně. Medián věku pacientů v době diagnózy byl 67 let. V souboru nemocných výrazně převažovali muži, kteří byli postiženi 3krát častěji než ženy. Přibližně stejně četně byla postižena mitrální (36 %) a aortální (33 %) chlopeň a elektroda kardiostimulačního systému (31 %). Ve srovnání s referenční prací bylo zachyceno více infekcí u pacientů s kardiostimulátory. Dvě třetiny infekcí byly způsobeny stafylokoky, což je proti pilotní studii výrazný nárůst. Naopak mortalita se podstatně neměnila. Závěr: Charakteristiky infekční endokarditidy se vyvíjejí, hospitalizační mortalita pacientů s IE je vysoká, kolem 30 %. Samo onemocnění IE je z hlediska roční úmrtnosti špatným prognostickým faktorem. Klíčová slova: incidence – infekční endokarditida, Aim: To find out an average incidence of infective endocarditis (IE) in Tabor district in a monitored period of time and compare the obtained data with previously published national study. Methods: Retrospective monocentric observational study monitoring occurence and characteristics of infective endocarditis in a defined region in years 2009–2013. Only patients with proved and treated infective endocarditis and patients whose diagnosis was revealed by autopsy were involved. Results: In the monitored time 39 cases of infective endocarditis occured, which means that the average incidence was 7.8 cases on 100 000 people per year. Median age in the time of diagnosis was 67. The group of patients was strongly dominated by men, who were affected three times more often than women. Almost the same rates of infective endocarditis were localized on mitral (36 %) and aortic (33 %) valve and electrode of a pacemaker (31 %). In comparison with the reference study there were more pacemaker-related infections detected. Two thirds of infections were caused by staphylococci. That means a prominent growth when compared to the pilot study. On the contrary, there was no significant change in mortality. Conclusion: Characteristic features of infective endocarditis are evolving, patients’ in-hospital mortality rate is high, about 30 %. Cílem práce je vyhodnocení výsledků MESA/TESE (MESA – microsurgical sperm aspiration, TESE – testicular sperm extraction) u pacientů s azoospermií v průběhu devíti let spolupráce Urologické kliniky a Centra asistované reprodukce FN Brno a dopad na gravidity, kterých jsme dosáhli. Celkem bylo provedeno 70 operačních výkonů. Výsledkem bylo dosaženo 17 gravidit vlastními spermiemi, z toho byly dva aborty, dále osm pacientů bez embryotransferu., The objective of this study is to evaluate the results of microsurgical sperm aspiration and testicular sperm extraction in patients with azoospermia during nine years of cooperation of Urological Department and Center for Assisted Reproduction University Hospital Brno and its impact on the obtained gravidity. In total 70 surgical interventions were performed, which resulted in 17 cases of gravidity using own sperm, including two abortions, and eight patients without embryo transfer., and Sanalla T., Lousová E.
Úvod: Kombinace monoklonální antiCD20 protilátky rituximabu s chemoterapií je vysoce účinná v léčbě chronické lymfocytární leukemie (CLL). Hlavním nežádoucím účinkem rituximabu jsou infuzní reakce, např. syndrom uvolnění cytokinů (cytokine release syndrome – CRS), který zpravidla doprovází první podání rituximabu. Cílem naší retrospektivní analýzy bylo zhodnotit četnost výskytu infuzních reakcí v rutinní klinické praxi, jejich vztah k parametrům nádorové nálože a zjistit možnou spojitost infuzních reakcí s účinností léčby. Dále byla posouzena bezpečnost rychlé infuze s rituximabem u nemocných s CLL. Charakteristika souboru: Hodnoceno bylo 108 nemocných s CLL léčených chemoimunoterapií s rituximabem od května roku 2005 do března roku 2011 v našem centru. V rámci 1. linie (n = 66, 71 % mužů, věkový medián 63 let) byly použity tyto léčebné režimy: fludarabin, cyklofosfamid, rituximab – FCR (n = 43) a nízkodávkovaný FCR (n = 13); 10 nemocných bylo léčeno jinými protokoly. 42 nemocných (76 % mužů, věkový medián 65 let) podstoupilo 2. linii: rituximab-dexametazon (n = 18), FCR/nízkodávkované FCR (n = 14) a 10 nemocných jiný režim. Součástí premedikace před infuzí rituximabu byl metylprednisolon 80 mg i.v., paracetamol 1 000 mg p. o. a bisulepin 1 mg i. v., všichni nemocní měli nitrožilní hydrataci minimálně 2 000 ml/den a alopurinol 300–600 mg/den. V 1. cyklu byl rituximab podáván frakcionovaně v celkové dávce 375 mg/m2 a od 2. cyklu formou rychlé infuze v dávce 500 mg/m2. Výsledky: K rozvoji infuzní reakce po rituximabu došlo u 32 % nemocných v 1. linii a 19 % v 2. linii. Jednalo se převážně o mírné reakce a k závažné toxicitě (stupeň III/IV dle NCI CTCAE – National Cancer Institute – The Common Terminology Criteria for Adverse Events) došlo výjimečně (3 % nemocných v 1. linii a 2 % v 2. linii). Reakce se nejčastěji projevila třesavkou, zimnicí, dušností, horečkou či hypotenzí. U všech nemocných bylo možné dokončit infuzi rituximabu v plánované dávce ve stejný den. Hodnocení léčebné odezvy (overall response rate – ORR) neprokázalo statisticky významné rozdíly mezi nemocnými s reakcí či bez infuzní reakce. Nemocní s infuzní toxicitou měli vyšší absolutní počet lymfocytů (1. linie, 87 vs 56 × 109/l, p = 0,21; 2. linie, 101 vs 14 × 109/l, p = 0,043). Při mediánu sledování 36 měsíců nebyl prokázán statisticky významný rozdíl v PFS (doba přežití bez progrese – progression-free survival) a OS (celkové přežití – overall survival) mezi oběma skupinami. Závěry: Výskyt infuzních reakcí u nemocných s CLL léčených rituximabem je poměrně běžný (32 %), avšak při pečlivé premedikaci jsou reakce velmi dobře zvladatelné a těžká forma je vzácná. Potvrdili jsme velmi dobrou snášenlivost rychlé infuze rituximabu. Naše data neprokázala statisticky významnou souvislost mezi výskytem infuzních reakcí a efektem léčby., Background and Aims: Rituximab in combination with chemotherapy is an effective treatment of patients (pts) with chronic lymphocytic leukemia (CLL). The most frequent adverse event of rituximab is infusion-related toxicity, e.g. cytokine-release syndrome that occurs usually during the first infusion. However, there is scarce data on feasibility and tolerability of rituximab infusions in CLL outside clinical trials. Therefore, we performed a single-center retrospective analysis of the frequency of rituximab infusion-related adverse events during the first- and the second line CLL treatment administered in the routine practice. We also analyzed its relation to parameters of tumor load and possible association with treatment efficacy. The safety of rapid infusion of rituximab in CLL pts was also evaluated. Patients and Methods: We analyzed 108 pts with CLL treated with rituximab-containing regimens between March 2005 and May 2011 at our institution. The most common first-line regimens (n = 66, 47 males, median age 63 years) were FCR (43 pts) and low-dose FCR (13 pts); 10 pts were treated by other protocols. Forty-two pts (32 males, median age, 65 years) underwent second line treatment: 18 pts rituximab-dexamethasone, 7 pts FCR, 7 pts low-dose FCR and 10 pts other regimens. Intravenous hydration (2 000 ml daily on days 0 and 1 of cycle), allopurinol 300–600 mg p. o. daily and premedication with methylprednisolone 80 mg i. v., acetaminophen 1 000 mg p. o. and bisulepine 1 mg i. v. were administered before rituximab infusion. Rituximab was given by fractionated infusion (100 mg for 2 hours, then if tolerated well, the rest of the dose with infusion rate escalation from 100 mg/hour up to 400 mg/hour) at the dose of 375 mg/m2 in the first cycle. Subsequent doses (500 mg/m2) were administered by rapid-infusion protocol. Results: Rituximab infusion-related toxicity occurred in 32 % pts (n = 21) during the first line treatment and 19% pts (n = 8) during the second line treatment. Adverse events were predominantly mild and NCI CTCAE grade III/IV occurred rarely (3% in the first line, 2% in the second line). Infusion toxicity manifested predominantly as rigors, chills, fever and hypotension. All patients with adverse events could finish rituximab infusion as initially planned on the same day. Treatment response analysis did not demonstrate statistically significant differences between patients with and without rituximab infusion toxicity. Patients who developed rituximab infusion toxicity had higher absolute lymphocyte count (first line, 87 vs 56 × 109/l, p = 0.21; second line, 101 vs 14 × 109/l, p = 0.043). At the median follow-up of 36 months, there were no statistically significant differences in PFS or OS in both cohorts. Conclusions: Rituximab infusion-related toxicity in pts with CLL is relatively frequent (32%). However, occurrence of infusion-related symptoms can be reduced by proper premedication and severe adverse events are uncommon. In our experience, all patients were able to receive the planned dose of rituximab. Subsequent doses of rituximab could be safely administered by rapid-infusion protocol. We assessed the intensive care unit (ICU) patients for Invasive aspergillosis (IA) with culture and non-culture based diagnostic methods from Iran. Thirty-six ICU patients with underlying predisposing conditions for IA were enrolled in the study. Sixty eight Bronchoalveolar lavage (BAL) samples were collected by bronchoscope twice a weekly. BAL samples were analyzed by microscopic examination, fungal culture and galactomannan (GM) detection. The Platelia Aspergillus GM EIA was used to quantify GM indices. Samples with a BAL GM index > or = 1 were considered as positive for GM. Patients were classified as having probable or possible IA. Out of 36 suspected patients to IA, 36.1% of cases showed IA which were categorized as: 4 cases of possible IA and 9 of probable IA. 76.2% of BAL samples were positive for GM. From 13 patients with IA, 11 (84.6%) had at least one positive BAL GM index. Of these patients, 9 (81.8%) showed probable IA. The main underlying predisposing conditions were neutropenia (53.8%) and COPD (30.8%). Our study has indicated that COPD must be considered as one of the main predisposing condition for occurrence of aspergillosis in ICU patients. Cíl: Ukázat na možnosti a bezpečnost IRE v léčbě nádorů. Metodika: Uvádíme soubor 23 nemocných s adenokarcinomem pankreatu, dvou nemocných se solitární jaterní metastázou kolorektálního karcinomu a jednoho adenokarcinomu solitární ledviny. Všichni byli léčeni pomocí IRE perkutánně pod CT kontrolou nebo při laparotomii s US asistencí. Výsledky: IRE je bezpečná metoda, 30denní mortalita nulová. Umožňuje destrukci nádorů v lokalitách, kde ostatní ablační metody selhávají. U neresekabilních nádorů pankreatu však v naší sestavě nepřinesla významné prodloužení života. Průměrná doba přežití ve skupině nádorů pankreatu je 10 měsíců. Závěr: IRE je bezpečná a užitečná ablační metoda zvláště pak nádorů umístěných u kritických struktur., Aim: To show the safety and clinical usefulness of IRE. Methods: A total of 23 pancreatic adeno-carcinomas, two liver mets of colorectal carcinoma and 1 renal adeno carcinoma have undergone IRE percutaneously under CT control or during laparotomy with US guidance. Results: IRE is safe method, 30 days mortality 0. There is no significant prolongation of survival in the group of unresectable pancreatic cancer. Survival in the group of pancreatic cancer is 10 months. Cíl: Cílem studie bylo zhodnotit přínos elektrofyziologického vyšetření v diferenciální diagnostice lumbální spinální stenózy (LS) a diabetické polyneuropatie (DPN). Soubor a metodika: Elektrofyziologické vyšetření bylo provedeno u 68 pacientů s klinicky symptomatickou LS, u 28 pacientů s DPN a 32 zdravých dobrovolníků. Výsledky: Vyšetřené elektrofyziologické parametry z horních končetin (latence F vlny n. ulnaris a amplituda senzitivního neurogramu n. radialis), všechny hodnocené latence z dolních končetin (F vlny n. tibialis, H reflexu m. soleus, spinální latence MEP) a amplituda senzitivního neurogramu n. suralis signifikantně odlišovaly pacienty s LS a DPN. ROC analýza nicméně prokázala pouze dva elektrofyziologické parametry jako efektivní nezávislé faktory pro odlišení pacientů s LS a DPN: latenci ulnarisové F vlny (optimální diskriminační hodnota je 24,2 ms, senzitivita 82,7 % a specificita 63,9 %) a amplitudu senzitivního neurogramu n. radialis (optimální diskriminační hodnota je 10,5 ?V, senzitivita 75,5 % a specificita 58,2 %). Vícerozměrná diskriminační analýza poskytla kanonické skóre s nejsilnější prediktivní hodnotou. Závěry: Elektrofyziologické vyšetření z horních končetin je užitečné v diferenciální diagnostice LS a DPN, nicméně nejlepší diskriminační efekt vykazuje kanonické skóre, které zahrnuje několik elektrofyziologických parametrů. Methods: Electrophysiological parameters were examined in 68 patients with clinically symptomatic LS, in 28 patients with DPN, and in a group of 32 healthy volunteers. Results: Electrophysiological parameters evaluated from the upper extremities (F-wave latency of the ulnar nerve and radial SNAP amplitude), all the latencies measured in the lower extremities (tibial F-wave, soleus H-reflex and spinal latency of MEP), and the sural SNAP amplitude contributed significantly to distinguishing the LS from DPN patients. ROC analysis, however, disclosed only two electrophysiological parameters as effective in independent discrimination between the LS and DPN patient groups: ulnar F-wave latency (cut-off point at 24.2 ms, sensitivity 82.7% and specificity 63.9%); and radial SNAP amplitude (cut-off point at 10.5 ?V, sensitivity 75.5%, specificity 58.2%). Multivariate discrimination provided a canonical score with the most powerful predictive value of all. Conclusions: Electrophysiological examination evaluated from the upper extremities proved very useful in the differential diagnostics of LS and DPN, but the most powerful predictive value was obtained by using a canonical score involving several electrophysiological parameters., and B. Víno pijeme nejméně 10 000 let nebo déle, v našem regionu již od doby keltské a germánské. Pokus používat víno k léčbě začíná v dávnověku, avšak teprve v současné době byl prokázán jeho příznivý účinek v prevenci infarktu myokardu, cévní mozkové příhody, diabetes mellitus a hypertenze. Pití velmi dobře působí také v pokročilém věku. Nejlepší účinky má každodenní pití při večeři, u mužů 20–40 g alkoholu, u žen polovina této dávky., We have been drinking wine at least for 10 000 years or longer, in our region already from Celtic and Germanic times. Favorable effects on prevention of myocardial infarction, stroke, diabetes and hypertension were demonstrated recently, following attempts to use alcohol for treatment in ancient times. Favorable effects of wine drinking could be seen also in aging population. The best results for men are reached by daily drinking of 20–40 g alcohol at dinner. Úvod: Z klinického hlediska je zjevné, že karcinom rekta a karcinom kolon jsou ve svém průběhu a léčbě odlišné nosologické jednotky. Cílem bylo analyzovat a objasnit rozdíly mezi chováním jaterních metastáz karcinomu kolon a karcinomu rekta. Studie těchto faktorů je důležitá pro stanovení přesné prognózy a indikace jejich co nejefektivnější terapie a léčby karcinomu kolon a karcinomu rekta jako systémového onemocnění. Metoda: Do našeho souboru bylo zařazeno 223 pacientů s metastatickým postižením jater kolorektálním karcinomem, kteří byli operováni na Chirurgické klinice LF UK a FN Plzeň od 1. 1. 2006 do 31. 1. 2012. Z celkového počtu 223 jedinců bylo 145 (65 %) mužů a 78 (35 %) žen. Jednalo se celkem o 275 výkonů. Resekční výkon byl proveden u 177 pacientů a ošetření pomocí radiofrekvenční ablace (RFA) v celkem 98 případech. Soubor byl rozdělen do kategorií podle lokalizace primárního tumoru na C (kolon) čítající 58 pacientů, S (c. sigmoideum) čítající 61 pacientů a R (rektum) čítající celkem 101 pacientů. Analýza významnosti jednotlivých studovaných parametrů (věk, pohlaví, TNM klasifikace, grading, typ operačního výkonu) byla provedena pomocí ANOVA testu. Celkové přežití (OS), bezpříznakové přežití (DFI) nebo období bez známek onemocnění (NED) bylo hodnoceno za pomoci Kaplan-Meierových křivek, které byly porovnávány pomocí Log-Rank a Wilcoxon testů. Výsledky: Z hlediska porovnávání primárního origa jaterních metastáz kolorektálního karcinomu bez ohledu na jejich ošetření (resekce i RFA) z naší studie vyplývá, že metastázy karcinomu rekta mají statisticky významně časnější recidivu (kratší NED/DFI). V případě ostatních faktorů byl pro prognózu časné recidivy statisticky významný lokálně pokročilý nález u primárního nádoru kolon a sigmoidea, dále podstoupená R2 resekce jaterních metastáz a pozitivita uzlinových metastáz v případě primárního nádoru kolon a sigmoidea. Dále jsme prokázali, že u nemocných s primárním postižením rekta nemá na DFI po resekci metastáz jater vliv pozitivita uzlinových metastáz ani lokální pokročilost primárního nádoru. Další studované faktory (časový odstup diagnózy orgánových metastáz od primární operace, grading, pohlaví či věk) nebyly pro prognózu OS i DFI (souhrnně u kolorektálního karcinomu) prokázány jako statisticky významné. Závěr: Z naší studie je zřejmé, že u karcinomu rekta a karcinomu kolon lze ve vztahu k jejich jaterním metastázám předpokládat různé chování se specifickými prognostickými faktory. Tyto rozdíly nejsou dosud plně objasněny a vyžadují další zkoumání a rozdělení, a to nejen na základě histopatologických, imunohistochemických a klinických faktorů, ale i molekulárně biologických parametrů. The aim of this study was to analyse and clarify the differences between the behaviour of liver metastases from colon and rectal cancer. The study of these factors is important for determining an accurate prognosis and indication of the most effective surgical therapy and oncologic treatment of colon and rectal cancer as a systemic disease. Method: 223 patients with metastatic disease of colorectal carcinoma operated at the Department of Surgery, University Hospital in Pilsen between January 1, 2006 and January 31, 2012 were included in our study. The group of patients comprised 145 men (65%) and 117 women (35%). 275 operations were performed. Resection was done in 177 patients and radiofrequency ablation (RFA) in the total of 98 cases. Our sample was divided into 3 categories according to the location of the primary tumor to C (colon), comprising 58 patients, S (c. sigmoideum) in 61 patients, and R (rectum), comprising 101 patients. Significance analysis of the studied factors (age, gender, staging [TNM classification], grading, presence of mucinous carcinoma, type of operation) was performed using ANOVA test. Overall survival (OS), disease-free interval (DFI) or no evidence of disease (NED) were estimated using Kaplan-Meier curves, which were compared with the log-rank and Wilcoxon tests. Results: As regards the comparison of primary origin of colorectal metastases in liver regardless of their treatment (resection and RFA), our study indicated that rectal liver metastases showed a significantly earlier recurrence than colon liver metastases (shorter NED/DFI). Among other factors, a locally advanced finding, further R2 resection of liver metastases and positivity of lymph node metastases were statistically significant for the prognosis of an early recurrence of the primary colon and sigmoid tumor. Furthermore, we proved that in patients with primary rectal carcinoma, DFI (after the resection of liver metastases) was not influenced by the positivity of lymph node metastases of primary tumor or locally advanced primary tumor. The other factors studied (time from diagnosis of organ metastases to primary operation, grading, sex or age) were not shown to be statistically significant for the prognosis of OS and DFI (colorectal cancer in total). Conclusion: As proven by our study, rectal cancer and colon cancer are two different nosological units with specific prognostic factors with respect to their liver metastases. These differences have not been fully understood yet and require further exploration and classification based not only on histopathological, immunohistochemical and clinical factors, but also on molecular biological parameters.
