Takotsubo kardiomyopatie je náhle vzniklé onemocnění charakterizované reverzibilní poruchou kinetiky levé srdeční komory. V typickém případě se rozvíjí následkem silného stresového podnětu a prezentuje se symptomy, EKG a laboratorním nálezem, které připomínají akutní koronární syndrom. Od toho se liší nepřítomností onemocnění koronárních tepen a ve většině případů nekomplikovaným průběhem s následnou velmi dobrou dlouhodobou prognózou. V kazuistice popisujeme případ 63leté ženy, u které se počátek onemocnění projevil fibrilací komor. Díky okamžité kardiopulmonální resuscitaci a následné odborné péči ve specializovaném kardiocentru, která mimo jiné zahrnovala implantaci implantabilního kardioverteru-defibrilátoru v rámci sekundární prevence náhlé srdeční smrti, se pacientka zotavila bez jakýchkoliv následků. Takotsubo kardiomyopatie je důležitou jednotkou v diferenciální diagnóze akutního koronárního syndromu. I přes dobrou prognózu, kterou se vyznačuje ve většině případů, ji nelze podcenit a zaslouží si náležitou pozornost a léčbu, která umožní předcházet případným komplikacím., Takotsubo cardiomyopathy (TTC) is an acute syndrome characterized by rapid onset of transient systolic dysfunction of the left ventricle. Symptoms, ECG and laboratory findings resemble acute coronary syndrome, from which TTC differs by the absence of coronary artery disease. In typical cases, TTC is triggered by exposure to unexpected stress and the clinical course and prognosis are very good. We present a case of a 63-year-old woman, in whom the onset of the disease was complicated by ventricular fibrillation. The patient recovered without sequelae thanks to immediate cardiopulmonary resuscitation and following care in a specialized cardiocenter, which involved also implantation of implantable cardioverter defibrillator to prevent sudden cardiac death. Takotsubo cardiomyopathy is an important entity in the differential diagnosis of acute coronary syndrome. Hereditární hemoragická teleangiektázie neboli morbus Rendu‑Osler‑Weber je fibrovaskulární dysplazie s autozomálně dominantní dědičností charakteristická přítomností teleangiektázií a arteriovenózních malformací kůže, sliznic a vnitřních orgánů. Plicní arteriovenózní malformace jako zdroj septických embolů mohou zapříčinit vznik mozkového abscesu, který tak bývá prvním a často jediným projevem heriditární hemoragické teleangiektázie. V práci jsou prezentovány tři kazuistiky pacientů trpících touto chorobou, kde prvním a závažným příznakem byl vzniklý mozkový absces. Protože se jedná o relativně vzácné onemocnění, nebývá vždy zvažováno v rámci diferenciální diagnostiky a tím pádem ani diagnostikováno. U každého pacienta s mozkovým abscesem je nezbytné na toto onemocnění pomýšlet a v případě podezření je nutné jej řádně vyšetřit. Při verifikované plicní arteriovenózní malformaci je třeba ji ošetřit a zabránit tak vzniku recidivujících mozkových abscesů. Péče o tyto pacienty je komplexní a multioborová., Hereditary hemorrhagic telangiectasia or Rendu‑Osler‑Weber disease is an autosomal‑dominant disorder that results in fibrovascular dysplasia and is characterized by telangiectases and arteriovenous malformation of the skin, mucosa and viscera. Pulmonary arteriovenous malformations as a source of septic emboli may give rise to brain abscess that so often is the first and only clinical manifestation of hereditary hemorrhagic telangiectasia. Three cases of patients suffering from this disease with brain abscess as the first and serious symptom are presented. Since this is a relatively rare disease, it is not always considered in the differential diagnosis and thus not diagnosed. This disease has to be considered in patients with brain abscess and, in case of a suspicion, it must be properly examined. When pulmonary arteriovenous malformation is verified, it has to be treated in order to prevent recurrent brain abscesses. Care of these patients should be comprehensive and multidisciplinary. Karcinom prsu v diseminovaném stadiu je nevyléčitelné onemocnění s definovanou dobou přežití, která v případě viscerálních metastáz dosahuje jen několika měsíců. U pacientek s viscerálním postižením, symptomatických a hormonálně nedependentních je základem léčby chemoterapie. U pacientek HER2 negativních (resp. triple negativních) má své místo antiangiogenní léčba bevacizumabem, která je schválena v I. linii léčby metastatického HER2 negativního karcinomu prsu v kombinaci s paclitaxelem, a to na základě významného prodloužení PFS ve studii III. fáze E2100 (1). Kazuistika: 53letá pacientka onemocněla triple negativním karcinomem prsu v roce 2008. Absolvovala neoadjuvantní chemoterapii, radikální mastektomii s exenterací axilly s pooperačním stagingem ypT1a ypN1. Následovala adjuvantní chemoterapie, ozáření. V květnu 2012 prokázány metastázy v plicích a v mediastinálních uzlinách dle CT. Provedena bronchoskopie a histologicky verifikovány metastázy duktálního karcinomu, triple negativního, dle FISH nebyla nalezena amplifikace genu HER2. Indikována ke kombinované léčbě paclitaxel + bevacizumab. Došlo ke kompletní remisi onemocnění. Léčba samotným bevacizumabem pokračuje dosud. Pacientka přežívá od zjištění generalizace dosud 43 měsíců! Kvalita života je výborná., Metastatic breast cancer is an incurable disease with a defined time of survival. This time is limited to only a few months when visceral metastases are present. Chemotherapy is the crucial treatment for patients with visceral involvement, symptomatic and hormone-independent disease. There is a role as well for an antiangiogenic therapy with bevacizumab in HER2-negative disease. Bevacizumab is approved for the treatment of metastatic HER2-negative breast cancer in combination with paclitaxel. This approval is based on significant improvement in PFS reported in phase III trial E2100. Case report: 53-years old patient, female, was diagnosed with triple-negative breast cancer in 2008. Chemotherapy was delivered in neoadjuvant setting, followed by radical mastectomy with dissection of axilla. Pathological staging was ypT1a ypN1. Adjuvant chemotherapy was applied as well as radiation therapy. In May 2012 the evidence of metastatic disease in lung and mediastinal lymph nodes was proved by CT scan. Triple negative breast cancer was verified from bronchology exam. FISH analysis of HER2 was also negative. Combination treatment with paclitaxel + bevacizumab was indicated. Brown tumors are bony lesions caused by rapid osteoclastic activity, which rare involved jaws. Renal osteodystrophy (ROD) is associated with different pathogenetic mechanisms--disorder of calcium-phosphate metabolism, impaired metabolism of vitamin D, increased parathyroid activity that lead to extreme concentrations of parathormone. The authors report two cases of jaw enlargement in patients received haemodialysis with excessive increase values of alkaline phosphatase and parathormone in serum. The patients were treated surgically with corrective procedures in maxillo-facial area. ROD of the jaws could be severe complication in dialysis patients with end stage of CKD if no appropriate care aimed at correction or prevention of parathyroid hyperfunction was applied to them. and PF. Nádory oblasti hlavy a krku představují v Evropě u mužů čtvrté nejčastější nádory. Nejvyšší výskyt je pozorován v zemích okolo Středozemního moře. Incidence nádorů orofaryngu v České republice je 0,91/100 000 obyvatel s mortalitou 0,65/100 000 obyvatel, nejvíce zastoupeným klinickým stadiem nádoru je klinické stadium IV (58,9 %). U nádorů patra je incidence 0,34/100 000 obyvatel s mortalitou 0,15/100 000 obyvatel. Nejvíce zastoupeným klinickým stadiem je také stadium IV (26,7 %). Postižení žen je výrazně nižší než u mužů, v ČR je poměr mužů k ženám přibližně 4:1. Z hlediska rizikových faktorů hraje rozhodující vliv kouření a konzumace alkoholu. Průměrný věk pacientů se pohybuje mezi 50 a 60 lety. V kazuistikách bude představen pacient s nádorem orofaryngu a pacient s nádorem měkkého a tvrdého patra a patrové tonzily. V obou případech šlo o pokročilé lokální nálezy bez vzdálených metastáz u „typických“ pacientů mužského pohlaví s dlouholetým abúzem alkoholu a tabakizmem., Head and neck tumours are the fourth most common cancer among men in Europe. The highest incidence is in the Mediterranean countries. The incidence rate of oropharyngeal cancer in the Czech Republic is 0.91/100 000 inhabitants with a mortality rate of 0.65/100 000 inhabitants; the most prevalent clinical stage of the tumour is clinical stage IV (58.9%). In palatine cancer, the incidence rate is 0.34/100 000 inhabitants with a mortality rate of 0.15/100 000 inhabitants. The most prevalent clinical stage is also clinical stage IV (26.7%). Autoři uvádějí kazuistiku pacienta s epiteloidním hemangioendoteliomem jater (HEHE), který byl léčen chemoembolizací (TACE). Při dlouhodobém sledování 9 let od stanovení diagnózy je klinicky bez obtíží a bez progrese onemocnění při vyšetření zobrazovacími metodami. Chylózní ascites představuje zřídka se vyskytující komplikaci radikálních onkogynekologických operačních výkonů v retroperitoneu, především jako následek systematické pánevní a zejména paraaortální lymfadenektomie. Terapeutický přístup je vysoce individuální, primárně konzervativní s důrazem na zavedení dietních omezení a vede k výsledkům za 4–8 týdnů. V případě jeho selhání, je indikovaná chirurgická terapie, která může být metodou první volby s vysokou úspěšností řešení problému v případě jasně detekované lokalizace úniku lymfy., Chylous ascites is a rare complication of oncogynaecological surgery in retroperitoneum as a result of systematic pelvic and paraaortal lymphadenectomy. Therapeutical approach is highly individual, primary conservative with accent to dietary restriction and leading to results in 4–8 weeks. Clear cell odontogenic carcinoma (CCOC) is a rare odontogenic tumor associated with aggressive clinical behavior, metastasis and low survival. We report a case of CCOC affecting the maxilla of 62 year-old woman. It was first described as a clinicopathological entity in 1985 and to date only 67 cases were described in the English literature. We are understanding of the behavior of this carcinomas was depend on limited case reports.
