Dyskinetické extrapyramidové syndromy mohou vzácně vést k akutnímu zhoršení do život ohrožujícího stavu, který si vyžádá hospitalizaci na jednotce intenzivní péče, hlubokou analgosedaci a umělou plicní ventilaci. V případě neúspěšnosti symptomatické farmakologické léčby lze uvažovat o stereotaktickém neurochirurgickém výkonu (pallidotomii nebo hluboké mozkové stimulaci). V příspěvku uvádíme naše dosavadní zkušenosti se třemi pacienty, kteří v akutní fázi dyskinetického syndromu takový výkon podstoupili. U dvou dětských pacientů (12 a 6 let) pro status dystonicus s geneticky verifikovanou hereditární generalizovanou dystonií (DYT‑6 a DYT‑1) byla provedena hluboká mozková stimulace vnitřního pallida a u jedné pacientky (21 let) s Huntingtonovou nemocí byla provedena pallidotomie pro akutní zhoršení generalizované chorey s rozvojem balizmu. Přes počáteční kritický stav došlo u všech tří pacientů v odstupu 1–6 týdnů od výkonu k významnému potlačení mimovolných pohybů. Stereotaktická neurochirurgická léčba by proto měla být zvažována jako metoda volby v terapii akutních, život ohrožujících hyperkinetických stavů, kde farmakologická terapie selhala., Dyskinetic movement disorders may rarely progress to an acute life-threatening condition requiring admission to an intensive care unit, deep sedation and mechanical ventilation. Stereotactic neurosurgery (pallidotomy or deep brain stimulation) should be considered in all patients who did not respond to pharmacological treatment. We report three patients who underwent stereotactic neurosurgery due to acute dyskinetic syndrome. Two boys (12 and 6 years) with generalized hereditary dystonia (DYT-6 and DYT-1) developed status dystonicus and were treated with deep brain stimulation of the globus pallidus interna. The third patient was a 21-years-old woman with Huntington’s disease who underwent a pallidotomy due to acute worsening of generalized chorea with ballistic movements. Despite the initial critical state of all three patients, their abnormal movements significantly subsided within six weeks after the neurosurgery. The stereotactic neurosurgical procedure should be considered as the method of choice in patients with acute life-threatening hyperkinetic movement disorders that failed to improve with pharmacological treatment. Brown tumors are bony lesions caused by rapid osteoclastic activity, which rare involved jaws. Renal osteodystrophy (ROD) is associated with different pathogenetic mechanisms--disorder of calcium-phosphate metabolism, impaired metabolism of vitamin D, increased parathyroid activity that lead to extreme concentrations of parathormone. The authors report two cases of jaw enlargement in patients received haemodialysis with excessive increase values of alkaline phosphatase and parathormone in serum. The patients were treated surgically with corrective procedures in maxillo-facial area. ROD of the jaws could be severe complication in dialysis patients with end stage of CKD if no appropriate care aimed at correction or prevention of parathyroid hyperfunction was applied to them. and PF. The authors present the case of a young woman with newly diagnosed Takayasu’s arteritis. This woman, with arterial hypertension, was investigated for the unspecific symptoms at the beginning. Afterwards, the transthoracic echocardiography showed dysfunction of the left ventricle and the abdominal sonography showed a stenosis of the right renal artery. PET/CT scan showed chronic modification after inflammatory processes on the wall of the thoracic and abdominal aorta. The present article describes two unrelated cases of progressive myoclonic epilepsy (PME) of the Lafora's disease and Unverricht-Lundborg types who were treated with topiramate (TPM) as add-on therapy for their myoclonus. After the initiation of topiramate therapy both cases responded with marked decrease in myoclonic seizure frequency and improvement of quality of life. Užívání hormonální antikoncepce je považováno za rizikový faktor vzniku hluboké žilní trombózy a plicní embolie. Od zavedení hormonální antikoncepce v 60. letech 20. století byl u jejích uživatelek pozorován častější výskyt těchto komplikací. Nejnovější studie pak ukázaly dvakrát vyšší riziko vzniku žilní trombózy při užívání kombinovaných preparátů obsahujících progestiny třetí generace (desogestrel, gestoden, norgestimate) ve srovnávání s progestiny druhé generace (levonorgestrel a norgestrel), ačkoli progestiny třetí generace neprokázaly žádný klinicky významný benefit. Přičemž riziko vzniku hluboké žilní trombózy je ještě vyšší u neperorálních forem hormonální antikoncepce (vaginální kroužek, transdermální náplast)., Since the early 1960s, it has been well documented that combined hormonal contraceptives increase the risk of venous thrombosis. Second and third generation oral contraceptives (OCs) differ in their progestin component. Third generation OCs contain desogestrel, gestodene or norgestimate while second generation OCs contain norgestrel or levonorgestrel. In spite of the fact that the third generation OCs have not shown any clinically significant benefit over the second generation OC, multiple studies and two meta‑analyses show that the third generation OC containing desogestrel are associated with approximately doubled risk of venous thrombosis. The newer generation of oral formulations, as well as non‑oral contraceptives (transdermal and vaginal), were recently studied for thrombotic risk. Poúrazový hydrocefalus způsobený poruchami cirkulace mozkomíšního moku často komplikuje klinický průběh a proces léčby po kraniocerebrálním poranění. Hydrocefalus po poranění míchy je popisován pouze výjimečně. Sdělení popisuje dvě kazuistiky nemocných s kompletní lézí krční míchy, u kterých v poúrazovém období došlo k rozvoji hydrocefalu. Rozbor obou případů a přehled literárních dat potvrzují rozhodující úlohu extraspinálních faktorů pro rozvoj hydrocefalu po míšním poranění. I když k rozvoji hydrocefalu po míšním poranění dochází výjimečně, tato možnost by měla být zvážena při pozdním zhoršení klinického stavu nemocného po poranění krční míchy, zvláště při přítomnosti anatomických anomálií mokového prostoru a poúrazovém subarachnoidálním krvácení., Posttraumatic hydrocephalus caused by cerebrospinal fluid circulation disturbances frequently complicates the clinical course and treatment after craniocerebral injury. Hydrocephalus complicating spinal cord injury is only exceptionally reported. The paper presents two cases of complete cervical spinal cord injury with subsequent development of hydrocephalus. The analysis of both cases and literature data confirmed the dominant role of non-spinal factors in the development of hydrocephalus after spinal cord injury. Despite the exceptional occurrence of hydrocephalus after spinal cord injury, this diagnosis should be considered in cases of delayed deterioration of a patient with cervical spinal cord injury, particularly if cerebrospinal fluid space abnormalities and posttraumatic subarachnoid haemorrhage are present., and J. Kolorektální karcinom z prstenčitých buněk je vzácné agresivní nádorové onemocnění. Incidence u mladých pacientů je velmi nízká a zaujímá do 1 % všech chirurgicky léčených kolorektálních karcinomů. Raritní výskyt u mladých pacientů koresponduje s diagnostikováním tohoto onemocnění v pokročilých stadiích a s nepříznivou prognózou. Pro verifikaci karcinomu z prstenčitých buněk je zásadní bioptické vyšetření a léčba je obdobná jako u ostatních kolorektálních karcinomů. Autoři popisují případ mladé ženy s diagnostikovaným kolorektálním karcinomem z prstenčitých buněk. Cílem sdělení je upozornit na raritní výskyt vzácného kolorektálního karcinomu u mladé nemocné., Signet ring cell colorectal carcinoma is a rare aggressive tumor. The incidence of this desease is very low in young patients and accounts up to 1% of all surgical patients with colorectal cancer. The infrequency of the disease among young patients makes the diagnosis more difficult and the prognosis less favourable. Biopsy results are crucial for verification of signet ring cell carcinoma. Treatment is similar as in other types of colorectal cancer. We report a case of signet ring cell colon cancer in a 22 years old female patient. Condylomata acuminata představují významný epidemiologický problém, a to zejména v teenagerovské populaci. Podstatou onemocnění je infekce tzv. lidským papilomavirem – HPV. Lidský papilomavirus neumíme léčit, proto terapie kondylomat většinou probíhá pouze formou lokální destrukce bradavic a nijak neřeší kauzální podstatu onemocnění. I samotné odstranění kondylomat, zejména při větším rozsahu onemocnění, může být obtížné a zdlouhavé, stejně tak jako i následné hojení pacienta, nezřídka komplikované časnými recidivami. Jakýkoliv nový preparát, který rozšíří možnosti terapie tohoto nepříjemného onemocnění, je velkým přínosem. Celkově působící imunomodulační preparát Isoprinosine je z tohoto pohledu velká naděje pro pacienty, a to zejména pro jeho systémové působení s efektem na všechny kožní a slizniční povrchy, pro jeho šetrnost vůči pacientovi, ale především pro jeho předpokládanou schopnost snížit procento recidiv. Na ambulanci onkogynekologické prevence při porodnicko-gynekologickém oddělení Městské nemocnice Ostrava a jejím přidruženém laserovém pracovišti jsme sledovali efekt léčby Isoprinosinem v klinické praxi a výsledky jsou velmi nadějné. Na základě našich pozorování se rovněž snažíme dát návod, jak lék používat, v jaké dávce a jak dlouho. Pro zajímavost uvádíme 1 kazuistiku i s fotodokumentací, v níž jsme léčbu Isoprinosinem využili s velmi dobrým efektem., Condylomata acuminata present a considerable epidemiological problem, particularly in a teenage population. The illness develops through the infection by a human papillomavirus- HPV. We are unable to treat the human papillomavirus, therefore the treatment of anogenital warts largely only consists in local destruction of warts and does not treat the cause of the illness. The removing of warts itself, in particular when the affected site is larger-sized, can be difficult and protracted, as well as subsequent healing of the patient, often complicated by recurrences. Any new preparation that broadens the possibilities of treatment of this obtrusive illness means a great benefit. In this regard the overall effect of the immunomodulation preparation Isoprinosine is a great hope for patients, mainly due to its systemic effect on all cutaneous and mucosal surfaces, its patient friendliness, but mainly due to its assumed ability to reduce the percent of recurrence. At the outpatients‘ department of oncogynecological prevention affiliated to the Department of Obstetrics and Gynecology of the Ostrava City Hospital and its Laser centre, we followed the effect of the treatment with Isoprinosine in clinical practice and the results are very encouraging. Based on our observations we also try to provide guidance on how to use the medicine, on its dosing and length of application. Cíl: Autoři prezentují výsledky operační léčby tumorů periferních nervů. Operační výkony byly provedeny na Neurochirurgické klinice LF MU a FN Brno v letech 2003–2013. Ve sdělení je prezentován současný přehled histologické klasifikace a její vliv na možnosti chirurgické léčby. Součástí práce je i kazuistika dokumentující operační postup u raritního objemného tumoru n. ischiadicus v hloubce hýžďové krajiny. Soubor a metodika: Operační revizi podstoupilo celkem 24 pacientů, z toho 13 mužů a 11 žen. Průměrný věk pacientů v souboru byl 46,5 roku (min. 22, max. 74 let). Výkony byly provedeny standardní mikrochirurgickou operační technikou, od roku 2007 s užitím peroperační elektrofyziologické monitorace. Retrospektivní analýzou je soubor definován dle postiženého nervu, rozsahu resekce a histologické klasifikace. Je zhodnocen výsledný pooperační neurologický stav. Výsledky: Nejčastěji postiženým periferním nervem byl n. tibialis. V souladu s literárními zdroji se histologicky nejčastěji jednalo o benigní schwannom, a to celkem ve 20 případech. Jednou šlo o neurofibrom, jednou o intraneurální ganglionovou cystu, jedenkrát byla histologie uzavřena jako raritní extraoseální Ewingův sarkom a u jednoho pacienta byl tumor způsoben chronickým zánětlivým procesem. Závěr: V našem souboru bylo dosaženo totální resekce u všech operovaných pacientů, kromě jednoho pacienta. U něj byla provedena pouze revize s otevřenou biopsií. U pacienta s maligním Ewingovým sarkomem byla nutná kompletní rozšířená resekce postižených nervů s příslušným totálním motorickým a senzitivním deficitem. Ve dvou dalších případech došlo po operaci ke zhoršení neurologického stavu, jednou se jednalo o lehkou parézu, jednou o trvalou hypestezii. V ostatních případech klinické obtíže prakticky vymizely., Aim: The authors present surgical treatment results of peripheral nerve tumors. All the surgical procedures were performed between 2003 and 2013 at the Department of Neurosurgery, The University Hospital Brno. The paper provides an overview of current histological classification and its impact on surgical treatment. The paper also includes a case report describing a surgical procedure used for a rare, bulky tumor of the sciatic nerve in the depth of the gluteal region. Methods: In total, 24 patients, 13 men and 11 women, underwent surgical revision. The mean age of the group was 46.5 years (min. 22, max. 74 years). All the surgical procedures were completed using standard microsurgical tools. Perioperative electrophysiological monitoring has been used since 2007. A retrospective analysis according to the localization of the affected nerve, extent of resection and histological classification of the tumor was performed and the final outcome in terms of neurological status was evaluated in the study group. Results: A tumor of the tibial nerve was the most frequent target of the surgery in our study group. Benign schwannoma was the most common tumor in our study group (20 cases) and this is in accordance with previously published data. Other tumors were classified as neurofibroma (one case) and intraneural ganglion cyst (one case). Histology confirmed the presence of a rare extraosseous Ewing’s sarcoma in one case. Finally, one patient suffered from chronic inflammation. Conclusion: In our study group, total resection was performed in all patients except one. Revision with open biopsy was performed in a patient with nerve inflammation. Complete extended resection of the affected nerves was necessary in a patient with malignant Ewing’s sarcoma. This resulted in total motor and sensitive deficit of the corresponding area. Neurological status has also worsened after the surgery in two other cases, one had light paresis and the other had permanent hypoesthesia. The clinical symptoms disappeared after surgery in all other cases.
