Cíle studie: 1. Zhodnocení buněčností likvoru (CSF, z angl. cerebrospinal fluid = mozkomíšní mok), koncentrací glukózy v CSF, hodnot glukózového kvocientu (Q glu. ), koncentrací laktátu v CSF a hodnot koeficientu energetické bilance (KEB) coby ukazatelů intenzity zánětlivého procesu v centrálním nervovém systému (CNS) ve skupinách pacientů bez postižení CNS, pacientů s mírnými serózními záněty CNS neinfekční etiologie, pacientů se serózními záněty CNS infekční etiologie a pa- cientů s purulentními záněty CNS s extracelulárními bakteriemi v patogenezi. 2. Porovnání informačního potenciálu parametrů energetického metabolismu glukózy v likvorovém kompartmentu v soubo - ru vyšetřených pacientů, tzn. koncentrací glukózy v CSF, hodnot Q glu. , koncentrací laktátu v CSF a hodnot KEB. Typ studie: Retrospektivní studie. Materiál a metody: Vyšetřili jsme 133 vzorků CSF pacientů bez postižení CNS, 227 vzorků CSF pacientů s mírnými serózními záněty CNS s intrathekální syntézou imunoglobulinů neinfekční etiologie, 208 vzorků CSF pacientů se serózními záněty CNS infekční příčiny a 140 vzorků CSF pacientů s purulentními záněty CNS s extracelulárními bakteriemi v patogenezi. Oblastí našeho zájmu byly buněčnost CSF, koncentrace glukózy v CSF, hodnoty Q glu. , koncentrace laktátu v CSF a hodnoty KEB. Pro statistické hodnocení vybraných parametrů jsme použili D’Agostinův omnibus test, Kruskalův-Wallisův test s následnou post hoc analýzou Dunnovou metodou, Spearmanův korelační koeficient a multinomiální logistickou regresní analýzu. Výsledky: V likvorovém kompartmentu pacientů bez postižení CNS jsme nezjistili žádné změny na úrovni buněčností CSF ani změny energetické. V likvorovém kompartmentu pacientů s mírnými serózními záněty CNS neinfekční etiologie byly pouze v některých případech patrny zvýšené buněčnosti CSF a jisté odchylky hodnot Q glu. , koncentrací laktátu v CSF a hod - not KEB od normálu. V likvorovém kompartmentu pacientů se serózními záněty CNS infekční etiologie byly pozorovány projevy nápadně zvýšené intenzity zánětlivého procesu v likvorovém kompartmentu především na úrovni buněčností CSF, koncentrací laktátu v CSF a hodnot KEB. Vysoká intenzita purulentního zánětlivého procesu bakteriální etiologie v CNS se výrazně projevila na úrovni všech sledovaných parametrů. Nejtěsnější vztah jsme v našem souboru pacientů zjistili mezi buněčnostmi CSF a hodnotami KEB ( ρ = -0,770), následoval vztah mezi buněčnostmi CSF a koncentracemi laktátu v CSF ( ρ = 0,734), hodnotami Q glu. ( ρ = -0,676) a koncentracemi glukózy v CSF ( ρ = -0,544). Využitelnost uvedených parametrů k predikci intenzity zánětlivého procesu v CNS jsme ověřili pomocí multinomiální logistické regresní analýzy. Nejvyšší pre - dikční potenciál měly buněčnost CSF se 71,9 % a KEB se 71,6 % správně zařazených pacientů. Následovaly koncentrace laktátu v CSF s 64,7 %, Q glu. s 58,8 % a koncentrace glukózy v CSF s 54,7 % správně klasifikovaných pacientů. Závěr: Získané výsledky podpořily vhodnost využití úrovní buněčností CSF, koncentrací glukózy v CSF, hodnot Q glu. , kon - centrací laktátu v CSF a hodnot KEB k diagnostice postižení a monitorování intenzity zánětů v CNS. Navíc umožnily vytvořit pořadí energetických parametrů v likvorovém kompartmentu pacientů našeho souboru podle úrovně jejich informačního potenciálu. Nejpříznivějších výsledků pro hodnocení intenzity zánětlivého procesu v CNS tak bylo dosaženo v případě hod - not KEB, následovaly je koncentrace laktátu v CSF a hodnoty Q glu. a nejméně příznivých výsledků jsme dosáhli v případě koncentrací glukózy v CSF., Objective: 1. Design: Retrospective study. Material and Methods: We examined 133 CSF specimens in patients without CNS impairment, 227 CSF specimens in pa - tients with slight serous inflammation with intrathecal synthesis of immunoglobulins of non-infectious aetiology in the CNS, 208 CSF specimens in patients with serous inflammation of infectious aetiology in the CNS and 140 CSF specimens in patients with purulent inflammation in the CNS with extracellular bacteria in pathogenesis. The objects of our interest were numbers of cells in the CSF, concentrations of glucose in the CSF, values of the Q glu. , concentrations of lactate in the CSF and values of the KEB. The D’Agostino Omnibus test, the Kruskal-Wallis test with follow-up post hoc analysis using the Dunn’s method, the Spearman correlation coefficient and the multinomial logistic regression analysis were used for statis - tical analysis of the examined parameters. Results: We did not find any changes in the numbers of cells in the CSF and in energy ratios in the CSF compartment of patients without CNS impairment. We found raised numbers of cells in the CSF and slight alterations of the glucose quo - tients, lactate concentrations in the CSF and the values of the KEB only in some patients with slight serous inflammations of non-infectious aetiology in the CNS. We observed manifestations of conspicuously increased intensity of inflammation in the numbers of cells in the CSF, lactate concentrations in the CSF and the values of the KEB in patients with serous inflam - mations of infectious aetiology in the CNS. Very high intensity of purulent inflammation in the CNS of bacterial aetiology was well apparent in all the evaluated parameters. Concerning the relationship, either direct or indirect, between the number of cells in the CSF and the other parameters, we found the highest correlation between the number of cells in the CSF and the values of the KEB ( ρ = -0.770), followed by the lactate concentrations in the CSF ( ρ = 0.734), the Q glu. ( ρ = -0.676) and the glucose concentrations in the CSF ( ρ = -0.544). We verified the applicability of the parameters mentioned above for prediction of the intensity of inflammation in the CNS via multinomial logistic regression analysis. The number of cells and the KEB, with 71.9 % and 71.6 % respectively, has the highest prediction potential of the correctly classified patients. They were followed by the lactate concentration in the CSF with 64.7 %, the Q glu. with 58.8 % and the glucose concentration with 54.7 % of the correctly classified patients. Conclusion: Our study supports the applicability of the numbers of cells in the CSF, the glucose concentrations in the CSF, the values of the Q glu. , the lactate concentrations in the CSF and the values of the KEB for diagnosing CSF impairment and for monitoring the intensity of inflammation in the CNS. Further, the results enabled determination of the information potential of the energy parameters. The values of the KEB were most suitable for evaluation of the intensity of inflammation in the CNS. Less suitable results were achieved in case of the lactate concentrations in the CSF. INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease. Primární nádory mozku jsou heterogenním onemocněním. Nejzávažnější diagnózu představuje glioblastoma multiforme, který tvoří 75 % z high grade gliomů. Jedná se o agresivní nádory s omezenými terapeutickými možnostmi a špatnou prognózou. Medián přežití od stanovení diagnózy je bez léčby 4,6 měsíce, s léčbou kolem 12 měsíců. Zlatým standardem terapie je v dnešní době radikální operace, chemoradioterapie a následně adjuvantní chemoterapie temozolomidem. V naší práci jsme se zaměřili na přínos přidání temozolomidu ke standardní radioterapii v porovnání s jeho toxicitou., Primary brain tumors are a heterogeneous disease. The most important is glioblastoma multiforme, which constitutes 75 % of the high grade gliomas. These are aggressive tumors with limited therapeutic options and poor prognosis. Median survival from diagnosis is 4,6 months without treatment in the management of around 12 months. The gold standard therapy today is radical surgery, chemoradiotherapy followed by adjuvant chemotherapy with temozolomide. Cíl. Záměrem práce bylo zhodnocení radiační zátěže pracovníků na Klinice nukleární medicíny a endokrinologie 2. LF UK a FN Motol (KNME) v letech 2006-2011 spolu s posouzením radiační ochrany pracoviště. Stanovení radiační zátěže na tomto pracovišti bylo vyhodnoceno na základě retrospektivní studie opírající se o příslušné údaje a výsledky monitorování. Metoda. Výsledky monitorování pracovníků KNME se vztahují k období 2006-2011. Na základě osobních a prstových dozimetrů byla stanovena průměrná kolektivní efektivní dávka a průměrná kolektivní ekvivalentní dávka na ruce od prstů až po předloktí personálu. Vyhodnocení bylo prováděno do března 2009 firmou Celostátní služba osobní dozimetrie Praha, od dubna 2009 monitoring realizovala firma VF a.s., a to v obou případech po měsíčních intervalech. Průměrná roční zpracovaná aktivita radiofarmak (radionuklidy 99mTc, 111In, 153Sm, 123I, 131I) se v tomto časovém období pohybovala v rozmezí 2,79-5900 GBq. Výsledky. Průměrná kolektivní efektivní dávka činila v jednotlivých letech 2006, 2007, 2008, 2009, 2010 a 2011 pro radiologické asistenty a sestry s atestací v oboru nukleární medicína na ambulanci hodnotu 1,14 mSv, 1,39 mSv, 1, 29 mSv, 0,99 mSv, 1,17 mSv a 1,11 mSv. Pro pracovníky radiofarmaceutické laboratoře na ambulanci byla průměrná kolektivní efektivní dávka za shodné časové období 0,29 mSv, 0,82 mSv, 0,52 mSv, 0,17 mSv, 0,36 mSv a 0, 50 mSv. Sestry na 2. lůžkové stanici obdržely průměrnou kolektivní efektivní dávku v daném časovém období, 0,73 mSv, 0,35 mSv, 0,22 mSv, 0,21 mSv a 0,39 mSv. V radiojódové laboratoři získali pracovníci průměrnou kolektivní efektivní dávku 3,00 mSv, 3,04 mSv, 3,33 mSv, 3,85 mSv, 2,04 mSv a 2,84 mSv. Průměrná kolektivní ekvivalentní dávka na ruce od prstů až po předloktí pracovníků (stanovována v témže časovém rozmezí 2006-2011) ve farmaceutické laboratoři dosahovala 23,00 mSv, 23,99 mSv, 23,19 mSv, 55,75 mSv, 167,81 mSv a 136,00 mSv. Pro radiologické asistenty na ambulanci a sestry s atestací nukleární medicína na ambulanci činila průměrná kolektivní ekvivalentní dávka na končetiny 4,35 mSv, 2,86 mSv, 0,88 mSv, 1,99 mSv, 3,48 mSv a 6,24 mSv. V radiojódové laboratoři byla vyhodnocena průměrná kolektivní ekvivalentní dávka ruce od prstů až po předloktí na 20,70 mSv, 18,21 mSv, 22,79 mSv, 44,76 mSv, 44,98 mSv a 54,03 mSv. Závěr. Hodnoty z osobních i prstových dozimetrů jsou pod dávkovými limity stanovenými dozorným orgánem, i když se ozáření pracovníků v jednotlivých letech mírně zvyšuje, což zřejmě souvisí s vyšším počtem a rozmanitostí prováděných diagnostických i léčebných úkonů. Na základě prezentovaných výsledků lze konstatovat, že radiační ochrana je na KNME zajištěna v souladu s požadavky Státního úřadu pro jadernou bezpečnost., Aim. The intention of the paper was the assessment of radiation burden at the Clinic of Nuclear Medicine and Endocrinology 2nd LF UK and FN Motol (KNME) during the period of 2006-2011 together with the examination of the level of radiation protection. The determination of radiation burden at this workplace was based on the retrospective study relying on relevant monitoring data and results. Method. Monitoring results of the personnel of the KNME are covering the period of 2006-2011. Using personal and finger dosimeters the average collective effective dose and average collective equivalent dose related to fingers up to forearms have been assessed. Until March 2009 the monitoring was carried out by the National Personal Dosimetry Service in Prague while as of April this monitoring was performed by VF Ltd. In both cases the monitoring interval was one month. The average activities of used radiopharmaceuticals (including 99mTc, 111In, 153Sm,123 I, 131I) was in the range of 2.79-5.900 GBq. Results. The average collective effective dose of radiological assistants and nurses certified in nuclear medicine in individual years, namely 2006, 2007, 2008, 2009, 2010 and 2012 was 1.14 mSv, 1.39 mSv, 1.29 mSv, 0.99 mSv, 1.17 mSv a 1.11 mSv, respectively. During the same period the personnel working in the radioiodine laboratory received average collective effective dose 0.29 mSv, 0.82 mSv, 0.52 mSv, 0.17 mSv, 0.36 mSv a 0.50 mSv. The average collective effective dose of nurses at the 2nd Ward was reported to be in those years 0.73 mSv, 0.35 mSv, 0.22 mSv, 0.21 mSv a 0.39 mSv. With respect to extremities, the average collective equivalent dose of the personnel in the radiopharmaceutical laboratory was thought out the above mentioned period was as follows 23.00 mSv, 23.99 mSv, 23.19 mSv, 55.75 mSv, 167.81 mSv a 136.00 mSv while the average collective equivalent dose received by radiological assistants on ambulance and certified on ambulance nurses amounted to 4.35 mSv, 2.86 mSv, 0.88 mSv, 1.99 mSv, 3.48 mSv a 6.24 mSv. The average collective equivalent dose to extremities of workers from the radioiodine laboratory during the period of 2006, 2007, 2008, 2009, 2010 a 2011 was 20.70 mSv, 18.21 mSv, 22.79 mSv, 44.76 mSv, 44.98 mSv a 54.03 mSv, respectively. Conclusion. The results of personal as well as finger dosimeters have been found to be below the relevant dose limits set by the Regulatory Authority although over some of the recent years the exposure of workers has slightly been increasing which apparently is related to the increased number and variety of diagnostic and therapeutic procedures. Periprosthetic fractures are the third most common reason for revision total hip arthroplasty. Surgical treatment of periprosthetic fractures belongs to the most difficult procedures due to the extensive surgery, elderly polymorbid patients and the high frequency of other complications. The aim of this study was to evaluate the results of operatively treated periprosthetic femoral fractures after total hip arthroplasty. We evaluated 47 periprosthetic fractures in 40 patients (18 men and 22 women) operated on between January 2004 and December 2010. The mean follow-up period was 27 months (within a range of 12-45 months). For the clinical evaluation, we used modified Merle d'Aubigné scoring system. In group of Vancouver A fractures, 3 patients were treated with a mean score of 15.7 points (good result). We recorded a mean score of 14.2 points (fair result) in 6 patients with Vancouver B1 fractures, 12.4 points (fair result) in 24 patients with Vancouver B2 fractures and 12.7 points (fair result) in 7 patients with Vancouver B3 fractures. In group of Vancouver C fractures, we found a mean score of 16.2 points (good result) in 7 patients. Therapeutic algorithm based on the Vancouver classification system is, in our opinion, satisfactory. Accurate differentiation of B1 and B2 type of fractures is essential. Preoperative radiographic images may not be reliable. If in doubt, checking the stability of the prosthesis fixation during surgery should be performed. and M. Korbel, P. Sponer, T. Kucera, E. Cíl: Stanovit, jak často je SPECT/CT potřebné ke specifikaci abnormálního nálezu na planárním WB kostním skenu, zda je užitečné porovnat scintigrafický nález s předcházejícím vyšetřením, v kolika případech SPECT/CT určí definitivní diagnózu a kolik ložisek abnormální aktivity nacházíme na kostním skenu u benigních a maligních afekcí. Pacienti a metoda: Retrospektivně jsme prostudovali 471 WB skenů u 450 pacientů s karcinomem prsu, plic a prostaty, kteří v roce 2008 podstoupili kostní scintigrafii. SPECT/CT nebo CT jsme použili v 11,9 % vyšetření u 12,4 % pacientů. Opakovaná scintigrafie (319 pacientů po kostní scintigrafii a 36 pacientů po SPECT/CT) sloužila jako referenční vyšetření. Výsledky: Srovnání s předchozí scintigrafií bylo užitečné u pacientů s karcinomem prsu před indikací SPECT/CT. S použitím SPECT/CT jsme mohli specifikovat nejisté scintigrafické léze u 90,9 % vyšetření. Pro detekci kostních metastáz dosáhlo CT ve SPECT/CT vyšetření senzitivitu 72,8 %, specificitu 96 %, pozitivní a negativní prediktivní hodnotu 88,9 %. V pravděpodobnosti rozvoje kostních metastáz do 2 let od vyšetření mezi pacienty s jedním suspektním ložiskem ve skeletu a pacienty bez suspektního ložiska jsme nezaznamenali signifikantní rozdíl (p = 0,4272), ovšem riziko vzrůstalo, jestliže jsme nalezli suspektních ložisek více (p ≤ 0,0001). Závěr: SPECT/CT zvyšuje specificitu kostní scintigrafie. Po porovnání scintigrafického nálezu s předchozím vyšetřením se zdá rozumné uvažovat o použití SPECT/CT v 10-20 % vyšetření. Účel studie: Retrospektivně jsme zpracovali výsledky léčby mozkových nádorů stereotaktickou radioterapií a radiochirurgií. Použité metody: Pacienti podstoupili plánovací CT a MR vyšetření. Poté bylo provedeno plánování cílového objemu a ozařovací techniky v BrainLab systému a pacienti byli ozářeni na X-noži Varian s vícelamelovým kolimátorem BrainLab. Radiochirurgie byla aplikována jednorázově s fixací ve stereotaktickém rámu. Pacienti, kteří podstoupili frakcionovanou stereotaktickou radioterapii, byli fixováni pomocí speciální stereotaktické masky. Statistické zpracování bylo provedeno ve spolupráci s Ústavem matematiky a statistiky PřF MU Brno. Výsledky: Celkem bylo ozářeno 101 pacientů s primárními mozkovými tumory. Nejčastěji se jednalo o meningeomy a high-grade gliomy. Medián sledování byl 22,4 měsíců. Pacienti, kteří podstoupili radiochirurgii, měli medián dávky 18 Gy. Pacienti, ozařovaní frakcionovaně, měli medián dávky 25 Gy. Toxicita léčby byla nízká. Stabilizace onemocnění byla zaznamenána u 68 % pacientů, parciální remisi dosáhlo 10 % a jeden pacient měl kompletní remisi. Závěr: Výsledky naší léčby jsou srovnatelné s publikovanými studiemi. Stereotaktické ozařovací metody mají své důležité místo v léčbě primárních tumorů mozku., We analysed retrospectively our treatment of primary brain tumors using stereotactic radiotherapy and radiosurgery. Methods: Patients underwent CT and MR examinations. Target volume and technique were planned in BrainLab system. Patients were irradiated on X knife. Radiosurgery was apllied with fixation using stereotactic frame. Stereotactic mask was used in cases of stereotactic radiotherapy. Statistical processing was performed at Department of mathematics and statistics, Faculty of Science, Masaryk university Brno. Results: 101 paients were included in our study. In the most of cases there were diagnosis of meningeoma and high-grade glioma. Median follow up was 22,4 months. Median of dose of radiosurgery was 18 Gy. Patients, whose underwent stereotactic radiotherapy, were median of doses 25 Gy. Treatment toxicity was low. Stable disease was recorded in 68 % of cases, partial remission achieved in 10 % patients and one patient had the complete remission. Conclusion: Results of our study are comparable with results other publicated studies. Cíl: Cílem studie je stanovit prevalenci chronického selhání ledvin u 101 pacientů s monoklonální gamapatií neurčeného významu (MGUS). Typ studie: retrospektivní studie. Materiál a metody: Porovnali jsme glomerulární filtraci odhadovanou ze sérového kreatininu a cystatinu C a hodnotili jsme korelaci mezi koncentrací volných lehkých řetězců v séru a funkcí ledvin. U všech pacientů jsme provedli elektroforézu pro - teinů séra s denzitometrickou kvantifikací monoklonálního imunoglobulinu a změřili jsme koncentraci kreatininu, cystatinu C, volných lehkých řetězců kappa a lambda, β 2 mikroglobulinu. Výsledky: Chronické selhání ledvin jsme nalezli u 38,6 % pacientů na základě odhadu ze sérového cystatinu C a u 34,6 % pacientů na základě odhadu ze sérového kreatininu. B2mikroglobulin má největší počet významných korelací; cystatin C (0,90), CysC GFR (-0,69), Kreatinin (0,70), MDRD GFR (-0,60), MIg koncentrace (0,24), κ FLC (0,22) λ FLC (0,34) a věk (0,40). Tento velký počet korelací lze vysvětlit tím, že β 2mikroglobulin odráží jak zátěž organismu maligními buňkami, tak pokles glomerulární filtrace. Závěr: Chronické selhání ledvin jsme nalezli u 38,6 % pacientů při odhadu glomerulární filtrace z cystatinu C a u 34,6 % při odhadu ze sérového kreatininu. Třída monoklonálního imunoglobulinu může mít vliv na výsledky odhadu glomerulární filtrace ze sérového cystatinu C, odhad glomerulární filtrace ze sérového kreatininu není ovlivněn třídou imunoglobulinu. Diabetici neměli statisticky signifikantně nižší glomerulární filtraci než pacienti bez diabetu mellitu., Background: Renal function is more frequently decreased in patients with monoclonal gammopathy. 10 % of patients with multiple myeloma present with acute renal failure and renal function is included in CRAB criteria for the diagnosis of multiple myeloma. Aim: The aim of this study is to determine the prevalence of chronic renal failure (CRF), defined as a glomerular filtration rate (GFR) below 1.0 ml/s/1.73m 2 , in 101 patients with monoclonal gammopathy of undetermined significance (MGUS). Design: retrospective study Material and methods: We compared GFR estimates from serum creatinine (MDRD GFR) and serum cystatin C (CysC GFR) and looked at the correlation between free light chains (FLC) concentration and renal markers. We performed serum protein electrophoresis (SPE) with quantification of monoclonal intact immunoglobulin (MIg) and measured serum creatinine, cysta - tin C, κ and λ FLC and β 2microglobulin in all patients. Results: We found CRF in 38.6% patients using estimation from serum cystatin C and 34.6 % using estimation from serum creatinine. B2microglobulin has the highest number of significant correlations; cystatin C (0.90), CysC GFR (-0.69), crea - tinine (0.70), MDRD GFR (-0.60), MIg concentration (0.24), κ FLC (0.22) λ FLC (0.34) and age (0.40). The best correlation results may be explained by the fact that β 2microglobulin reflects both malignant cells burden and renal function. Conclusions: We found CRF in 38.6 % MGUS patients using estimation from serum cystatin C and in 34.6 % using estima - tion from serum creatinine. The class of monoclonal immunoglobulin may influence results of glomerular filtration estimated from cystatin C but it does not influence GFR estimated from creatinine. The pilot study deals with the clinical results of the regenerative therapeutical method in intraosseous periodontal defects done by surgical augmentation procedure using a material of a new generation prepared on the basis of a bioactive glass. A group of 10 individuals were treated properly for chronic periodontitis. Some of treated infraalveolar periodontal lesions, both periodontal pockets and interradicular defects, persisted or recurred in posterior teeth. In order to eliminate them they were indicated for advanced periodontal surgery or regenerative surgical therapy stimulating healing processes of supportive tissues, mostly alveolar bone around treated teeth. Relevant clinical parameters, i. e. values of the pocket depth, gingival recession, and loss of attachment were evaluated before and after the periodontal surgery and compared with each other. The therapy led to significant improvement of clinical parameters in terms of the reduction of pocket depth and loss of attachment values. and R. Slezák, V. Paulusová
