BACKGROUND: Radio frequency (RF) and chemical peels have been used for nonablative skin rejuvenation. Both of these cause collagen remodeling in the dermis and neo-collagen formation resulting in facial rejuvenation. There is limited literature on the evaluation of collagen remodeling by objective methods. OBJECTIVE: To compare the benefits of monopolar radiofrequency and glycolic acid peels in facial rejuvenation with regards to histopathology and Ultrabiomicroscopic sonography (UBM). METHODOLOGY: In this study, forty patients with mild to moderate photoaging received four treatments with 3 weeks interval of monopolar radiofrequency on one side of face and glycolic acid peels in increasing concentrations (NeostrataR) on the other side. Pre and post treatment, 2 mm biopsies were taken from both preauricular areas and Ultrasonography using a 35 MHz probe was done from outer canthus of eye and nasolabial folds from both sides of face. A blinded assessment was done to measure the increase in the grenz zone and dermal thickness. RESULTS: In 35/40 patients there was a significant increase in the grenz zone on histopathology and decrease in subepidermal low-echogenic band (SLEB) on UBM of the nasolabial folds on both sides of the face (p < 0.05). Úvod: Alveolární echinokokóza je vzácné a závažné zoonotické parazitární onemocnění. Kazuistika: Autoři demonstrují případ mladého nemocného s postižením jater, bránice a plic touto formou infekce. Diagnóza echinokokové infekce byla stanovena na základě anamnézy, klinické symptomatologie v kombinaci s USG, CT, MRI a sérologickými metodami. Byla provedena radikální bloková resekce 7. jaterního segmentu, dolního plicního laloku vpravo a bránice. Definitivní stanovení diagnózy alveolární echinokokózy bylo provedeno histopatologicky a pomocí PCR metody ze vzorku resekované tkáně. Závěr: Nemocný je 8 měsíců po operaci bez potíž, trvale dispenzarizován s long-life terapií albendazolem., Introduction: Alveolar echinococcosis is a life-threatening zoonotic parasitic disease. Its incidence is rare. In some cases, the correct and timely diagnosis can be difficult. Case report: The authors present the case of a young patient with liver, diaphragm and lung involvement. The suspicion of echinococcus infection was made on the basis of medical history, clinical symptoms, and a combination of ultrasonography, computed tomography, magnetic resonance imaging tests and serological methods. The patient underwent multimodal treatment with albendazole and en-bloc resection of the liver, lung and diaphragm. The definitive diagnosis of alveolar echinococcosis was determined from samples of the resected tissues using histopathology and polymerase chain reaction methods. The patient has been followed regularly and is on life-long treatment with albendazole. Pseudocysta pankreatu patří mezi nejčastější cystické léze pankreatu. Pseudocystu lze definovat jako tekutinovou intrapankreatickou, peripankreatickou nebo extrapankreatickou kolekci s definovanou stěnou vznikající nejčastěji na pokladě chronického nebo akutního zánětu pankreatu. Jde o poměrně častou komplikaci akutní nebo chronické pankreatitidy. V diagnostice má zásadní význam UZ pro svou neinvazivitu a dostupnost. Mezi další vyšetření vykazující nejvyšší přesnost, specificitu a senzitivitu patří MDCT a ERCP. Na základě MDCT a ERCP vyšetření lze stanovit klasifikaci a na základě této klasifikace následně algoritmus léčby. MDCT vyšetření by optimálně mělo předcházet ERCP. K odlišení od maligních cystických lézí pak prokazuje největší význam EUS s FNAB nebo aspirací obsahu se stanovením onkomarkerů a AMS v aspirátu. Funkční klasifikace pseudocyst je důležitým předpokladem pro úspěšný algoritmus léčby. Klasifikace vycházejí zejména z anatomického vztahu pseudocysty k pankreatickému vývodu a morfologii parenchymu pankreatu. V léčbě se uplatňují zejména perkutánní drenáž, endoskopické a chirurgické metody., The pseudocyst of the pancreas is one of the most common cystic lesions of the pancreas. The pseudocysts can be defined as intrapancreatic, peripancreatic or extrapancreatic fluid collections with a defined wall formed mostly on the basis of acute or chronic pancreatitis. This is a relatively common complication of acute or chronic pancreatitis. Sonography is essential for the diagnosis given its noninvasivity and availability. Other assessments that show the highest accuracy, specificity and sensitivity include MDCT and ERCP. The MDCT and ERCP assessments can be used to classify the pseudocyst; the treatment algorithm is then determined based on this classification. MDCT should ideally be preceded by ERCP. EUS-FNAB or aspiration with the determination of oncomarkers and AMS in the aspirate have been shown to be most important to distinguish the pseudocyst from malignant cystic lesions. Functional classification of the pseudocyst is an important prerequisite for successful algorithm of the treatment. The classification is based mainly upon anatomical relations of the pseudocyst to the pancreatic outlet and morphology of the pancreatic parenchyma. Východisko a cíl: Prognóza inoperabilního karcinomu jícnu je velmi závažná, léčba těchto pacientů je paliativní. Cílem sdělení je retrospektivní hodnocení intraluminální brachyterapie vysokým dávkovým příkonem nádorové stenózy inoperabilního karcinomu jícnu. Pacienti a metody: Od února 1996 do června 2011 bylo intraluminální brachyterapií léčeno 41 pacientů s karcinomem jícnu, z toho v 19 případech se jednalo o dlaždicobuněčný karcinom a ve 22 případech o adenokarcinom. U všech pacientů byla před zahájením brachyterapie přítomna dysfagie. Výsledky: U většiny pacientů došlo po brachyterapii k úlevě polykacích obtíží. Medián doby přežití činil 396 dní (95 % CI: 270–492 dní). Nebyla pozorována mechanická komplikace zavedení brachyterapeutického aplikátoru. Závěr: Intraluminální brachyterapie je účinná a bezpečná paliativní metoda léčby dysfagie způsobené nádorovou stenózou jícnu. Úvod: Odběr ledvin z dárce s nezvratnou zástavou oběhu (DCD – donor after circulatory death) je důležitou součástí transplantačních programů v celosvětovém měřítku. V České republice byla první transplantace ledvin z DCD úspěšně provedena v roce 2002. Autoři diskutují vlastní a literární zkušenosti s tímto programem. Snahou je zvýšit potenciál odběru ledvin z DCD v České republice. Metoda: Od roku 2002 do roku 2015 bylo v plzeňském Transplantačním centru odebráno celkem 44 ledvin z DCD. Technikou byl „in situ“ odběr pomocí „double baloon triple lumen“ katetru při zachování 5–10 min. „no-touch“ intervalu. Metoda hypotermické pulzatilní perfuze byla použita k testování viability odebraných ledvin. Transplantováno bylo celkem 28 ledvin příjemcům o průměrném věku 51,1 roku (26–73 let). Dle maastrichtských kritérií bylo 16 (57,1 %) ledvin ve skupině II, 8 (28,6 %) ve skupině III a 4 (14,3 %) ve skupině IV. Výsledky: 30denní pooperační mortalita byla 0 a morbidita 10,7 tj. 14,3 % (N=4). Primární afunkce ledviny byla přítomna u 2 (7,1 %), opožděný nástup funkce pak u 5 (17,9 %) nemocných. Jeden rok, 5 a 10 let po transplantaci žije 100 %; resp. 86,4 % a 76,7% nemocných a ve stejných intervalech je funkčních 92,9; 69,6 a 61,9 % transplantovaných ledvin. Dlouhodobé výsledky jsou plně srovnatelné s transplantacemi ledvin od dárců se smrtí mozku. Závěr: DCD jsou významným zdrojem ledvin pro transplantace. Transplantace ledvin z DCD je metodou logisticky, ekonomicky a personálně náročnou s velmi dobrými dlouhodobými výsledky., Introduction: Kidney procurement from donors after circulatory death (DCD) is an important part of worldwide transplantation programmes. The first kidney transplantation from DCD was successfully performed in the Czech Republic in 2002. Method: Forty four kidneys from DCD were procured in the Transplant Centre of Pilsen between 2002 and 2015. We used the technique of “in situ“ procurement with the double balloon triple lumen catheter and 5−10 minutes of the no-touch interval. The method of pulsatile hypothermic perfusion was used to test the viability of the kidneys. Twenty eight recipients with mean age 51.1 (26−73) years were transplanted. Sixteen (57.1%) kidneys were from the 2nd, 8 (28.6%) from the 3rd and 4 (14.3%) from the 4th category according to the Maastricht criteria. Results: 30-day mortality and morbidity rates were 0 and 10.7% i. e.14.3% respectively (N=4). Primary non-function was presented in 2 (7.1%), and delayed graft function in 5 (17.9%) cases. One, five and ten years of recipient and graft survival rates were 100%, 86.4% and 76.7%; and 92.9%, 69.6% and 61.9%, respectively. The long-term results are fully comparable with kidneys transplanted from donors after brain death. Conclusion: DCD are an important source for kidney transplantation. Still's disease is an inflammatory disorder of unknown etiology. First-line therapy is based on corticosteroids and non-steroidal anti-inflammatory drugs (NSAIDs) but the frequency of relapses and corticosteroid-induced adverse events are limiting factors. The efficacy of intravenous tocilizumab (TCZ) has been shown at a dose of 8 mg/kg but the corticosteroid-sparing effect of intravenous low-dose TCZ followed by subcutaneous (SC) injection in the course of the disease has been poorly investigated. We report the case of a 28-year old Caucasian woman presenting a relapse of Still's disease eleven months after diagnosis under treatment with 6 mg of methylprednisolone. TCZ at a dose of 4 mg/kg every 2 weeks was combined with 32 mg of methylprednisolone, followed by 162 mg SC every 3 weeks. Evolution was rapidly favourable with a decrease in corticosteroid doses. Je popsána resekce tumoru s částí stěny duté žíly i resekce metastázy v oblasti pánve. Dále autoři diskutují možnosti diagnostiky rozsevu tohoto typu nádoru a doporučenou adjuvantní onkologickou terapii., The authors present the case of a 57-year-old woman with a very rare extragenital malignant retroperitoneal Müllerian carcinosarcoma invading the inferior vena cava. Gliomy limbického a paralimbického systému představují anatomicky a tím pádem i operačními přístupy unikátní skupinu mozkových nádorů. Mikrochirurgie v kombinaci s moderními perioperačními postupy zvyšuje šance na radikální odstranění těchto gliomů. Z hlediska lokalizace je dělíme do tří základních skupin: 1. gliomy inzuly, 2. amygdalohippokampálního komplexu a 3. cingula. V minimonografii se věnujeme anatomii i funkci limbického a paralimbického systému a v návaznosti na ni představujeme ucelený pohled na jednotlivé neurochirurgické přístupy k těmto oblastem. Maximální resekce za zachování funkční integrity mozku je předpokladem následné smysluplné onkologické léčby. Taktéž prezentujeme výsledky operací našeho pracoviště ve smyslu morbidity a radikality za posledních sedm let., Limbic and paralimbic gliomas represent a unique group of brain tumors both from an anatomic and surgical point of view. Microsurgery, together with modern perioperative procedures, enables a wider extent of resection of these gliomas. Based on localization, we divide gliomas into three basic groups: 1. insular, 2. amygdalohippocampal complex and 3. cingulum gliomas. In our review, we describe the anatomy and function of the limbic and paralimbic system and in relation to that, we present a description of the specific neurosurgical approaches to these regions. Maximal resection with brain function preservation represents the basis for subsequent oncologic therapy. We also present the surgical results of our department in the last seven years with regard to morbidity and radicality. OBJECTIVE: The aim of the present study was to investigate the effect of allergic rhinitis on the success of the operation in chronic otitis surgery by using score for allergic rhinitis (SFAR). MATERIALS AND METHODS: In the present study; 121 patients, who underwent type 1 tympanoplasty were examined retrospectively. SFAR of all patients were recorded. The graft success rates of 26 patients with allergic rhinitis (AR) and 95 patients with no allergic rhinitis group (NAR) were compared. RESULTS: While the graft success rate in NAR group was 89.5%, this rate was 80.8% in the AR group. However, the difference between groups was not statistically significant (p = 0.311). CONCLUSION: These findings suggest that allergic rhinitis decreases the graft success rate of the pathologies occurring in eustachian tube, middle ear and mastoid although statistically significant difference wasn't found.
