Úvod: Léčba nekrotizující pankreatitidy se stále vyvíjí. Standardní léčebnou metodou infikované pankreatické nekrózy a následných septických komplikací je otevřená chirurgická drenáž. Rozvoj radiologických zobrazovacích metod a intervenční radiologie umožnily rozvoj miniinvazivních postupů, mezi něž patří cílené transkutánní punkce a drenáž pod CT/USG kontrolou (PCD), endoskopická transgastrická nekrektomie (ENE), laparoskopická transperitoneální nekrektomie (LNE) a retroperitoneální přístupy k pankreatické nekróze (RENE). Metody: Do studie byli zahrnuti nemocní léčení s akutní pankreatitidou (AP) v letech 2002–2013 (n=932). Ve skupině s nekrotizující formou AP byly porovnány výsledky chirurgické léčby nemocných, léčených klasickou laparotomickou cestou (skupina A) s výsledky nemocných léčených miniinvazivními metodami (skupina B). Statisticky byly výsledky hodnoceny chi-kvadrát testem. Výsledky: V uvedeném období bylo léčeno s lehkou formou AP 677 (72,6 %), s těžkou formou 255 (27,4 %) nemocných. Poměr mužů a žen byl 1,4:1. Ve skupině s těžkou formou nemoci bylo 171 léčeno konzervativně, letalita 16,4 % (28/171). Chirurgická léčba byla indikována u 84 nemocných, letalita 26,2 % (22/84). Laparotomickou cestou bylo operováno 52 nemocných (skupina A), mimiinvazivní výkony byly užity celkem u 32 nemocných (skupina B), u kterých bylo provedeno 37 výkonů. Úhrnná letalita ve skupině A byla 30,8 % (16/52) vs. 18,8 % (6/32) ve skupině B, p = 0,224. Průměrná doba hospitalizace byla delší u skupiny A (65,4 dne, median 52,4) oproti skupině B (49 dnů, median 36,5). Nejčastějším miniinvazivním výkonem byla PCD u 19 nemocných, 5 z nich zemřelo na pokračující sepsi a multiorgánové selhání a 2 museli být znovu operováni laparotomickou cestou. RENE byla provedena u 8 nemocných, z toho 5x z lumbotomie, ENE u 2 nemocných, 1 zemřel, a LNE byla provedena u jednoho nemocného. Fasciotomie v linea alba byla užita u 2 nemocných při intraabdominální hypertenzi. Závěr: Standardní léčebnou metodou infikované pankreatické nekrózy je otevřená chirurgická drenáž. Miniinvazivní metody jsou vhodnou alternativou léčby zvláště u kriticky nemocných, s nižší mortalitou a morbiditou. Tyto metody musí respektovat zejména lokalizaci léze a individuální stav pacienta. The standard therapeutic method for infected pancreatic necrosis and its subsequent septic complications is open surgical drainage. The advances in radiological imaging and interventional radiology have enabled the development of minimally invasive procedures, i.e. percutaneous drainage (PCD) under CT/USG control, endoscopic transgastric necrosectomy (ENE), laparoscopic transperitoneal necrosectomy (LNE) and retroperitoneal access to pancreatic necrosis (RENE). Methods: Patients with acute pancreatitis treated from 2002 to 2013 (n=932) were included in the study. In patients with a severe form of the disease, results obtained in two groups of patients were compared: the first group was treated by classic laparotomy (group A), the second one was treated by means of minimally invasive procedures (group B). Statistical analysis employed the chi-square test. Results: During the mentioned period, 677 (72.6%) patients with a mild form and 255 (27.4%) with a severe form of the disease were treated. The male/female ratio was 1.4:1. In the group of patients suffering from a severe form of acute pancreatitis, 171 patients were treated conservatively, mortality rate being at 16.4% (28/171). Surgery was indicated in a total of 84 patients, mortality rate reaching 26.2% (22/84). Fifty-two of the patients underwent laparotomy (group A), minimally invasive procedures were used in a total of 32 patients (group B). Overall mortality in group A was 30.8% (16/52) vs. 18.8% (6/32) in group B, p = 0.224. The average length of hospitalization was longer in group A (65.4 days; median 52.4 vs. 49 days; median 36.5 in group B). PCD was the most frequent procedure performed in 19 patients; 5 of them died due to ongoing sepsis and multiorgan failure and 2 of them underwent revisional laparotomy. RENE was performed in 8 patients; lumbotomy was used in 5 of them. ENE was performed on 2 patients, 1 of them died, and LNE was used once. A less invasive procedure, the linea alba fasciotomy, was performed in 2 patients with intra-abdominal hypertension. Conclusion: Open surgical drainage represents the standard treatment for infected pancreatic necrosis. Minimally invasive procedures are suitable alternatives especially in critically ill patients providing lower morbidity and mortality rates. Úvod: V posledních letech prošla více než 20 let sloužící Atlantská klasifikace akutní pankreatitidy (AP) revizí. Přesto se prognostické ukazatele AP neustále hledají. Naše studie se pokouší zhodnotit odůvodnění 3 nově zavedených stupňů tíže revidované klasifikace akutní pankreatitidy a přínos 2 jednoduchých krevních laboratorních testů jako markerů rizika úmrtí a těžkého průběhu AP. Metody: Do studie bylo zahrnuto a sledováno celkem 159 pacientů s AP. Pacienti byli rozděleni do skupin podle Revidované Atlantské klasifikace a jednotlivé skupiny byly hodnoceny podle počtu úmrtí, délky hospitalizace a počtu stavem vyžadovaných intervencí včetně operací pankreatu. Počet leukocytů a distribuční šíře červených krvinek (RDW – red blood cell distribution width) v první den hospitalizace byly hodnoceny z hlediska prognózy AP. Výsledky: Podíl úmrtí, délka hospitalizace a počty operací pankreatu se statisticky významně lišily mezi všemi jednotlivými skupinami tíže podle Revidované Atlantské klasifikace. Počty bílých krvinek a RDW v 1. den hospitalizace byly identifikovány jako významné prediktory těžké AP a RDW také jako významný prediktor úmrtí. Souhrn: Nově zavedené kategorie lehké, středně těžké a těžké AP podle pravidel Revidované Atlantské klasifikace dobře vystihují prognózu, délku hospitalizace a riziko intervencí u nemocných. Počet bílých krvinek a RDW v první den hospitalizace se jeví jako možný adept mezi ukazatele k predikci úmrtí a rozvoje těžké formy AP., Introduction: In the last few years the Atlanta classification of acute pancreatitis (AP) have been revised. However prognostic markers of AP are still being searched for. The aim of this study is to validate the 3 severity cathegories proposed by the revised Atlanta classification. We also tried to reevaluate the association between two laboratory markers (leucocyte count and RDW – red cell distribution width) on admission and prognosis of the patients with AP. Methods: 159 patients were included into the study. The patients were classified according to revised Atlanta criteria and the subgroups evaluated according to mortality, lenght of hospital stay and need for interventions. Leucocyte count and RDW on admission was evaluated in the patients. Results: All the subgroups of patients were associated with significantly relevant differences in mortality, lenght of hospital stay and need for operations on pancreas. Leucocyte count and RDW were identified as significant predictors for severe AP and RDW was also identified as significant predictor for mortality in patients with AP. Conclusion: New categories of severity as defined by Revised Atlanta classification are describing well the mortality, lenght of hospital stay and need for interventions in the patients with AP. Akutní poškození ledvin je obecně definováno jako náhlé zhoršení funkce ledvin podmíněné poklesem glomerulární filtrace a je diagnostikováno hromaděním konečných produktů metabolizmu dusíku, sníženým výdejem moči nebo obojím. Akutní poškození ledvin je důsledkem řady onemocnění, které mohou akutně poškodit funkce ledvin. Jde o častou komplikaci hospitalizace a je velmi časté i u kriticky nemocných pacientů jako důsledek jiných závažných onemocnění. S včasnou diagnózou mohou pomoci nové diagnostické možnosti včetně renálních biomarkerů. Žádná specifická terapie akutního poškození, která by zmírnila tíži poškození anebo urychlila reparaci funkce ledvin, dosud neexistuje. Léčba nadále zůstává symptomatická. K náhradě funkce ledvin pomocí dialýzy přistupujeme v případě těžkého renálního poškození spojeného s nárůstem cirkulujícího volumu, biochemickými změnami a uremickými komplikacemi. Obvykle dojde k reparaci renálních funkcí a pacient není závislý na dialyzační léčbě. Pacienti po prodělaném akutním poškození ledvin mají zvýšené riziko chronického onemocnění ledvin. Obecně je akutní poškození ledvin spojeno se závažnými a dalekosáhlými důsledky nejen pro nemocného (nutnost dialýzy, zvýšení morbidity a mortality), ale i s významnými ekonomickými dopady pro zdravotní systém., Acute kidney injury is generally defined as a condition characterised by the rapid loss of the kidney's excretory function with a decline of the glomerular filtration rate and is typically diagnosed by the accumulation of the end products of nitrogen metabolism or decreased urine output, or both. It is the clinical manifestation of a number of disorders that can affect the kidney acutely. Acute kidney injury is common in hospital patients and very common in critically ill patients, and is often secondary to extrarenal causes. New diagnostic techniques including renal biomarkers may help with early diagnosis. No specific therapies have emerged that can attenuate acute kidney injury or expedite recovery; thus, treatment is supportive. Patients are given renal replacement therapy if acute kidney injury is severe and biochemical or volume‑related, or if uraemic‑toxaemia‑related complications are of concern. Patients typically recover to dialysis independence. However, evidence suggests that patients who have had acute kidney injury are at increased risk of subsequent chronic kidney disease. Mnohočetný myelom (MM) a solitární plazmocytom (SP) jsou nejčastější primární nádory páteře. Léčba MM a SP páteře je komplexní proces, který zahrnuje léčbu kauzální s cílem potlačit nádorový klon a dále léčbu podpůrnou, jejíž součástí je i chirurgická terapie. Operace přispívají k lepší kvalitě života a nabývají na významu a smysluplnosti zejm. díky zlepšené prognóze onemocnění. Navrhli jsme algoritmus operační léčby MM a SP a ten jsme dále prospektivně ověřovali a jeho vyhodnocení bylo cílem této studie. Pacienti byli operováni v průběhu 10 let, minimální doba sledování byl jeden rok. Soubor tvořilo 32 pacientů (20 mužů a 12 žen) s průměrným věkem 61 let. Operováno bylo celkem 61 obratlů. Důvodem k operaci byly projevy útlaku míchy a progredující neurologický deficit (hodnoceno Frankelovou škálou) nebo na konzervativní léčbu nereagující axiální bolest (hodnoceno VAS; Visual Analogue Scale) páteře. Provedena byla některá z těchto operací: vertebroplastika (VP), laminektomie, transpedikulární fixace, somatektomie a jejich kombinace, nebo okcipito‑cervikální fixace. Pooperační VAS a Frankelova škála byly vyhodnoceny za jeden rok, skiagramy a MR jsme provedli a zhodnotili každý jeden rok. U žádného pacienta nenastala lokální recidiva tumoru ani selhání stabilizace na operovaném úseku v celém sledovaném období. Průměrný předoperační VAS byl 6,8 a jeden rok po operaci se průměrný VAS snížil na 1,1. Všechny operační výkony vedly nejen ke kontrole bolesti, ale také k prevenci nebo zlepšení neurologického postižení po celou dobu sledování. Hodnotili jsme náš postup – algoritmus operací, míru radikality a jejich načasování jako optimální pro MM i SP., Multiple myeloma (MM) and solitary plasmacytoma (SP) are the most frequent primary tumors of the spine. Management of the spinal MM and SP is a complex process involving causal treatment to suppress the tumor clone, as well as supportive therapy, including surgery and radiotherapy. Surgery should be considered because of its favourable effect on disease prognosis. We proposed a surgical treatment algorithm in patients with spinal MM and SP and the aim of this article is to present prospective evaluation of this algorithm. Patients undergoing surgical treatment during the past 10 years were included in this study, with the minimum follow-up of one year. A total of 32 patients (20 males and 12 females) with the mean age of 61 years were included. Surgery of 61 vertebrae was performed. The procedures were indicated for progressing neurological deficit (Frankel score) and for axial spinal pain (VAS classification). We performed the following procedures: vertebroplasty, laminectomy, transpedicular fixation, somatectomy, and their combination, or occipito-cervical fixation. Frankel score and VAS was assessed one year after the surgery and X-ray and spinal MRI was performed every year. No local relapses of the tumor or stabilization failure were detected. The mean preoperative VAS was 6.8 and improved to 1.1 one year after the surgery. During the follow up period, we observed positive effect of surgery on pain control and on prevention or improvement of neurological dysfunction. The authors concluded that all surgical procedures, the extent of resection and timing were adequate in all subjects. Karcinom prsu je nejčastější nádorové onemocnění u žen. Každoročně postihuje více než 1 000 000 žen na celém světě. Z toho zhruba 11 % případů rakoviny prsu lze připisovat konzumaci alkoholu u žen. Cílem tohoto článku je vytvořit představu o podílu nákladů na léčbu karcinomu prsu, který lze přiřadit konzumaci alkoholu v ČR v roce 2007. Zjištěná výsledná částka, kterou je třeba vynaložit na léčbu pacientek pravidelně konzumujících alkohol, je 39 mil. Kč., Breast cancer is the most common cancer in women. Annually, it affects more than 1 million women worldwide. Thereof, some 11% of the breast cancer incidence can be attributed to alcohol consumption in women. The aim of this article is to create a notion of the proportion of the costs spent to treat breast cancer that can be ascribed to alcohol consumption in the Czech Republic in 2007. More than 60 neuropeptides that inhibit juvenile hormone synthesis by the corpora allata have been isolated from the brains of various insect species. Most of them are characterized by a common C-terminal pentapeptide sequence Y/FXFGL/I/V (allatostatin A family, allatostatin superfamily). Besides the allatostatin A family, allatostatic neuropeptides belonging to other two peptide families (W2W9)-allatostatins or allatostatin B family; lepidopteran allatostatin) were reported. Four peptides with allatostatic activity were isolated from brains of the Mediterranean field cricket, Gryllus bimaculatus. Three of them (Grb-AST A3: AGMYSFGL-NH2; Grb-AST A4: SRPFGFGL-NH2; Grb-AST A5: GPDHRFAFGL-NH2) belong to the wide-spread family of Y/FXFGL/I-amide peptides, the fourth (Grb-AST B5: AWDQLRPGW-NH2) is a member of the W2W9 - amide family of neuropeptides. All of these peptides are potent inhibitors of juvenile hormone (JH) biosynthesis by cricket corpora allata in vitro, causing 50% inhibition of JH biosynthesis at 0.4-3 × 10-8 M. The two peptides Grb-AST A5 and Grb-AST B5 have virtually the same potency and efficacy in inhibiting JH biosynthesis in vitro. No synergistic effect of the two peptide families with respect to the inhibition of JH biosynthesis could be observed. Peptides of both families decrease the accumulation of methylfarnesoate, the direct precursor of JH, within CA that have been incubated in farnesol-rich medium. Allozyme polymorphism was investigated in adult males of the stenotopic peat bog (tyrphobiontic) noctuid moth, Coenophila subrosea Stephens, from three isolated peat bog localities in Austria (Styria) and Czech Republic (South and North Bohemia). Of the eighteen enzyme loci examined, twelve were polymorphic and six monomorphic. Significant deviations of genotype frequencies from Hardy-Weinberg equilibrium were observed at about one third of polymorphic loci within the populations. The average heterozygosities for the populations from three geographically distinct localities ranged from 0.192 to 0.245, and 61% of the loci were polymorphic. The FST mean value of 0.0675 was higher than that found in most other Lepidoptera. The genetic distances based on allozyme heterozygosity ranged from 0.019 to 0.051, with the population from South Bohemia being the most distant. The genetic distances and FST values do not reflect the geographic distances between the populations. Morphometric analysis revealed a difference between the Austrian Purgschachen Moor and Bohemian populations.
