Cílená léčba představuje vedoucí odvětví onkologického výzkumu. Poznávání molekulárních jevů, které stojí za nádorovými procesy, umožňuje používat léčiva, proti těmto procesům cíleně zamířená. Jde buď o monoklonální protilátky, tj. látky proteinové struktury, nebo o small drugs, menší chemické molekuly. V obou případech je jejich cílem protein exprimovaný nádorovou buňkou. I klasická cytostatika si zachovávají své místo v terapii onkologických onemocnění. Jednak proto, že s jejich podáváním je velké množství zkušeností, jednak proto, že u některých diagnóz nejsou zatím jejich nástupci z nových skupin k dispozici. Vlastnosti obou skupin se pak spojují v konjugovaných molekulách, kdy se toxicky působící konvenční molekula díky vazbě na monoklonální protilátku nebo small drug dopraví v těle přímo do nádorové buňky., Targeted therapy represents leading branch of oncological research. Understanding of molecular phenomena that stand behind cancer processes enables us to use drugs that are targeted against these processes. Either monoclonal antibodies, e.g. protein molecules, or small drugs, i.e. smaller chemical entities are involved. In both cases, the target is protein molecule exprimed by tumour cell. Conventional cytostatics play still major role in the therapy of oncological diseases. This is based on much experience with their use as well as the fact that for some diagnoses there have not been any targeted drugs available yet. Úkolem diagnostické terminologie v jakékoli morfologické metodě je poskytnutí jednoznačně interpretovatelné informace klinickému specialistovi tak, aby mohl zvolit adekvátní terapeutický postup. Cytologická klasifikace dysplastických změn na děložním hrdle se od původní Papanicolauovy pětistupňové postupně zjednodušila na dvoustupňovou. V histopatologické rovině hodnocení se rychle prosadila fúze kategorií CIN III a CIS, další vývoj je však v porovnání s cytopatologickým hodnocením pomalejší. Navrhovaná dvoustupňová klasifikace histopatologických lézí děložního hrdla nese v sobě potenciál zpřesněné komunikace klinika a patologa ve vyhodnocování cervikálních lézí a jejich následném ošetřování. V porovnání k předchozím je plně převoditelná. Zachovává si ovšem i předchozí omezení ve vyhodnocení základního histopatologického nálezu, zejména závislost na primárně reprezentativním vzorku. Hlubší porozumění procesům karcinogeneze děložního hrdla však rozšiřuje spektrum zpřesňujících prognostických markerů a možnosti individualizovaných léčebných postupů., The function of diagnostic terminology in any morphology method is to provide information to the clinical specialist that is interpreted unequivocally and which enables him to choose the appropriate treatment. The former Papanicolaou five-tiered cytological classification of dysplastic changes on the uterine cervix has been simplified to a two-tiered approach. In the histopathological evaluation, fusion of the categories CIN III and CIS was rather smooth. Further progress toward simplification of the histopathology terminology is proceeding more slowly. The proposed two-tiered classification of the histopathological lesions of the cervix has the potential to create a more precise communication between the clinician and the pathologist in the evaluation and subsequently in the treatment of the cervical lesions. In comparison to the previous classifications, it is fully convertible. It preserves the previous limits in the evaluation of the histopathological finding, mainly the dependence on the primary representativeness of the specimen. Deeper understanding of the cervical cancerogenesis will be achieved with the increasing spectrum of prognostication markers. Biologické cirkadiánní hodiny mají všichni obratlovci, včetně člověka. Jejich receptory jsou na oční sítnici, informace o střídání dne a noci jsou vedeny do centra v hypothalamu a odtud do epifýzy. Ta produkuje hormon melatonin (N-acetyl-5-metoxytryptamin), téměř výhradně v období tmavé části dne. Hormon je rychle vyplavován do krve a mozkomíšního moku a je bezprostředním nosičem informace o fázi cirkadiánního cyklu buňkám celého těla, které podřizují fyziologické funkce orgánů a organismu střídání dne a noci. Podle dosavadních výzkumů se melatonin účastní řady významných fyziologických funkcí: regulace spánku a cirkadiánních rytmů, nálady, imunitní, onkostatické a antioxidační aktivity. Syntéza melatoninu klesá s věkem. Umělé osvětlení večer nebo brzy ráno zkracuje tmavou část dne, také jen krátkodobé rozsvícení v noci redukuje množství melatoninu produkovaného během 24 hodin. Syntézu melatoninu významně narušuje letecký přesun přes několik časových zón. Zdravotní následky narušení biologických rytmů zahrnují poruchy spánku, zvýšený výskyt některých zhoubných nádorů, obezity, metabolického syndromu, diabetu, zhoršení projevů některých neurodegenerativních onemocnění. The hormone is quickly discharged into the bloodstream and cerebrospinal fluid into all cells. Such immediate information about the circadian phase of day/night changes allows to organs and organism to regulate their physiological functions. Current research suggests that melatonin is involved in several important physiological functions: sleep regulation and circadian rhythms, mood, immunity, oncostatic and antioxidant activities. Synthesis of melatonin reduces with age. Artificial light in the evenings and early mornings reduces the dark part of the day and even brief use of artificial light at night reduces the levels of melatonin produced over a 24-hour period. Air transport over a number of time-zones likewise significantly disrupts melatonin production. The health consequences of the disruption of biologcal rhythms include sleep disorders, higher frequency of some types of cancer, obesity, metabolic syndrome, diabetes and aggravation of some neurodegenerative symptoms. Malé regulační molekuly, tzv. mikroRNA, se podílejí na regulaci všech základních životních procesů v buňkách. Jsou regulátory exprese genů na posttranskripční úrovni. Ovlivňují tuto expresi vazbou na cílové mRNA, která vede k blokaci translace a štěpení mRNA. Nejnovější výzkumy ukázaly, že jejich výskyt není omezen jen na buňky, ale mikroRNA se vyskytují také v tělních tekutinách. Vzhledem k tomu, že diferenciální exprese určitých mikroRNA je často asociovaná s mnoha patologickými procesy, detekce deregulovaných mikroRNA v diagnosticky relevantních vzorcích (např. krvi, plazmě/séru, moči) se může stát základem nových diagnostických postupů. U gynekologických nádorových onemocnění je nalezení nových diagnostických biomarkerů důležitým cílem současných výzkumů. Assaultive acts committed by people with a mental illness is a major public health issue that affects patients with their families, law enforcement authorities, and the public at large. Failure to provide treatment is in fact a major predictor of assaultive acts in patients with schizophrenia living in the community. Considering that the indigenous ethnic groups of Central Asia have similar sociocultural characteristics, these factors may be reflected in individuals with schizophrenia who have committed serious assaultive acts in Uzbekistan. Objectives: The aim of the work was to identify the sociocultural and clinical characteristics of schizophrenic representatives of indigenous ethnic groups of Central Asia who have committed violent crimes in Uzbekistan and have been found insane in regard to their offence, and to compare these subjects to ones belonging to the other ethnic groups. Material and methods: The data were collected in 2010–2013 in the Tashkent High Security Psychiatric Hospital via face-to-face interviews and also from the patients’ charts and from forensic psychiatric examination statements. Results: The sample consisted of 201 individuals. The sample was 90.1 percent (n = 181) male, with a predominance of the paranoid schizophrenia subtype according to the ICD-10 criteria. Of the subjects, 174 ones (86.6%) were representatives of the indigenous ethnic groups of Central Asia, and 27 ones (13.4%) were representatives of other ethnic groups. The duration of illness among the subjects belonging to the indigenous ethnic groups of Central Asia was less than in the other group; the individuals were rarely referred to psychiatric care because of the popularity of alternative medicine and the stigma attached to mental illness. A positive correlation between violence and various psychotic symptoms, such as delusions, hallucinations, and thought disorder, has also been demonstrated in this group. Antrochoanal polyps are benign lesions originating from the mucosa of the maxillary sinus. Nasal obstruction and rhinorrhea are their main symptoms. Their endoscopical and radiological appearance makes them relatively easy to diagnose. These polyps are usually presented unilaterally, although bilateral presentation is also possible. We described two cases of atypically giant antrochoanal polyps: in a 15-year-old child and in a 38-year-old man. In both cases, the diagnosis was done by nasal endoscopy and computed tomography (CT) of the paranasal sinuses and supported by histopathological analysis. In the first patient, the excised polyp had the histological characteristic of an angiomatous antrochoanal polyp. Because of their unusual dimension, the combined transoral and endonasal endoscopic approach was performed for complete polyp excision. We discussed the clinical, histopathological and immunohistochemical characteristics of choanal polyps in comparison to inflammatory nasal polyps, and the applicable surgical techniques for treatment of these polyps. and C. The aim of our study was to determine the significance of lymphocyte-platelet adhesion (LPA), interleukins, transforming included 139 pregnant women aged between 17 and 27 years (21.3±4.22 years). GH was diagnosed in 119 women after 20 weeks of pregnancy. 20 patients (control group) were with physiological course of pregnancy. The distribution of patients by groups was carried out according to the level of blood pressure (BP) in accordance with ICD-10 (Geneva, WHO, 2002). The survey was conducted at the moment of detection pregnancy from 7 to 10 weeks and in dynamics of I, II and III trimesters of gestation. In the dynamics of gestation, were studied the number of desquamated endothelial cells circulating in the systemic circulation (CECs), nitrates levels, the adhesion of platelets by estimation their ability to form co-aggregates with lymphocytes by determining the percentage of lymphocytes aggregates with thrombocytes (lymphocyte-platelet plugs), at risk of hypertensive disorders, especially after 20-22 weeks of gestation and later, lymphocytes ability to platelets adhesion is rose, the concentrations of pro-inflammatory cytokines and NO level are increased. Srdeční selhání se zachovalou ejekční frakcí levé komory (heart failure with preserved ejection fraction – HFPEF, heart failure with normal ejection fraction – HFNEF) je epidemiologicky závažným onemocněním, jehož výskyt v populaci postupně narůstá. Etiologie jeho vzniku není ještě zcela objasněna. Nově bylo zjištěno, že prozánětlivý stav spojený s řadou komorbidit, jako např. obezita či diabetes, potencuje rozvoj HFPEF. Také sérové koncentrace některých působků, jako např. adiponektin, mají souvislost s tímto onemocněním. Hlavním znakem HFPEF je diastolická dysfunkce, která je ale doprovázena řadou dalších kardiálních i nekardiálních poruch organizmu. Prognóza se i přes dlouhodobě užívané, ale i nově zavedené terapeutické postupy nemění. Zlatým standardem v diagnostice HFPEF zůstává katetrizace srdce. K ozřejmění základní choroby vedoucí k srdečnímu selhání jsou využívána další vyšetření, jako elektrokardiografie (EKG), RTG srdce a plic, laboratorní odběry včetně markerů srdečního selhání a hlavně echokardiografie včetně pulzního dopplerovského měření., Heart failure with preserved ejection fraction of left ventricle (heart failure with normal ejection fraction, HFPEF, HFNEF) is frequent disease with serious consequences. Incidence of HFPEF in population is still growing. The exact pathophysiological mechanism of HFPEF remain unclear .Recent evidence suggests a relationship between inflammation associated with obesity or Diabetes mellitus and progression of HFPEF. Consistently, it has been reported that serum concentration of some pro-inflammatory markers such as adiponectin is positively related to HFPEF. By HFPEF is attended diastolic dysfunction. Diastolic dysfunction is linked to many other cardiac and non-cardiac diseases. Despite the great effort and new therapeutic approaches the prognosis of HFPEF does not improve. The gold standard in HFPEF diagnosis remains heart catheterization.
