Anémie patří mezi nejčastější onemocnění vůbec. Celosvětově postihuje až 25 % populace. Anémie z nedostatku železa (Fe) pak patří mezi vedoucí. Není také překvapením, že sideropenická anémie postihuje především ženy. Anémie patří obecně mezi významné problémy na každém interním oddělení. Tento článek si nedělá ambici na podání komplexního přehledu o významu, diagnostice a léčbě anémie. Jeho cílem je upozornit na jedné straně na zdánlivě běžná (ale právě proto někdy opomíjená) fakta z každodenní praxe na interním oddělení, a na druhé straně poukázat na novinky v léčbě především parenterálním Fe. Význam anémie na interním oddělení. Už v úvodu je zmíněno, že anémie je na interně častá. Je to především anémie z nedostatku Fe a anémie chronických onemocnění. Anémií trpí především starší, polymorbidní nemocní (často s jedním dominujícím, někdy kryptogenním onemocněním). Jako určitý problém vidím fakt, že anémie je často vnímána především jako signál dalšího onemocnění sama o sobě, a pak tedy není cílem ani diagnostického ani léčebného snažení. Diagnostika a léčba. Nemocného má v péči obvykle kmenový lékař interního oddělení, pokud je ale anémie závažnější, vstupuje do procesu diagnostiky a léčby hematolog jako konziliář. Základní vyšetření představuje vyšetření Fe, ferritinu, transferinu, cirkulujících transferinových receptorů, případně dalších parametrů. Samozřejmě, že zejména u sideropenické anémie se soustředí pozornost na pátrání po příčině možných ztrát nebo v případě anémie chronických onemocnění po prvotní chorobě. Poznámky k léčbě Fe. Pokud je u nemocného prokázán deficit železa, je často (po zjištění příčiny) indikována terapie Fe. Železo se podává ve většině případů perorálně. Existuje však několik situací, v nichž je parenterální podání Fe nejen výhodnější, ale dokonce představuje jedinou možnost léčby. V současné době je nejvíce důkazů pro pozitivní účinky parenterálně podávaného Fe pro železitou karboxymaltózu, přípravek Ferinject. Parenterální podávání Fe v gastroenterologii. Gastroenterologie představuje v zásadě typickou oblast pro využití podávání Fe. Je to proto, že ztráty do GIT jsou časté, ale některá onemocnění GIT jsou navíc i příčinou poruchy vstřebávání Fe. Typickým příkladem mohou být nespecifické střevní záněty, zejména potom Crohnova choroba. Nejen kazuistická sdělení prokazují pozitivní působení železité karboxymaltózy u Crohnovy choroby. Parenterální podávání Fe v kardiologii. Nedávno publikovaná práce (kontrolovaná, dvojitě slepá, multicentrická, dostatečně statisticky silná) CONFIRM HF prokázala, že podání železité karboxymaltózy u nemocných se srdečním selháním zlepšila jejich funkční zdatnost, změnila k lepšímu klasifikaci podle NYHA a ne na poledním míst zlepšila kvalitu života. Nebyla to přitom studie této indikaci první. Pilotní studie u srdečního selhání dopadla rovněž pozitivně CONFIRM HF jen potvrdila její výsledky. Kvalita života – opomíjený parametr. Již při popisu výsledků studie CONFIRM HF je uvedena, jako jeden z výsledků, zlepšená kvalita života. Je bohužel pravdou, že je to parametr (jistě subjektivní a obtížně objektivizovatelný), kterému není dáván zdaleka takový význam jako „tvrdým endpointům“, cílovým ukazatelům intervenčních studií. Přitom je prokázáno, že léčba anémie má významný vliv na kvalitu života u chronické únavy (především u žen). Zlepšení příznaků vyčerpanosti bylo prokázáno po podání 1 000 mg železité karboxymaltózy. Kvalitu života zlepšuje terapie Fe také anemickým nemocným s nádorovým onemocněním., Anaemia is one of the most common diseases. Worldwide affects up to 25% of the population. Anaemia with iron deficiency (Fe) is the leading one. It is not surprising that iron deficiency mainly affects women. Generally, anaemia is one of the major problems in every department of internal medicine. There is no ambition to provide a comprehensive review of the diagnosis and treatment of anaemia. The aim is to point out the common (but sometimes neglected) facts from daily practice in internal department and on the other hand, to highlight the news in the treatment focusing on parenteral Fe. The importance of anaemia at the department of internal medicine. Mentioned above, anaemia is very frequent in internal medicine. Especially, it is anaemia of Fe deficiency and anaemia of chronic disease. Mostly elderly and polymorbid patients (often with one dominant, sometimes cryptogenic disease) suffer from anaemia. I am concern about the fact that anaemia is often seen only as a sign of other disease and usually is not the target of diagnostic and therapeutic efforts. Diagnosis and treatment. The internal department physician is responsible for patient care, but cooperates with haematologist in case of severe anaemia in diagnostic and therapeutic process. Basic examination contains analysis of Fe, ferritin, transferrin, circulating serum transferrin receptors or other parameters. Of course, the focus in iron deficiency anaemia is on its possible loss or in case of chronic disease anaemia on primary disease. Notes to Fe treatment. If the patient has iron deficiency the Fe treatment is often indicated (after finding the cause). Iron is administered orally in most cases. There are several situations when parenteral Fe is not only preferable, but also represents the only therapeutic option. Currently, the best evidence for the positive effects is observed in parenterally administered Fe ferric carboxymaltose, Ferinject. Parenteral administration of Fe in gastroenterology. Gastorenterology is a common field for the use of Fe administration. The losses in the GIT are frequent as well as malabsorption of Fe in several gastrointestinal diseases. A typical example may be inflammatory bowel disease, especially Crohn´s disease. Not only case reports demonstrate the positive effect of ferric carboxymaltose in Crohn´s disease. Parenteral administration of Fe in cardiology. Recently published work (multicentric randomized controlled double-blinde trial) CONFIRM HF have proved that administration of ferric carboxymaltose in heart failure patients have improved their functional ability, NYHA stratification and quality of life. The pilot studies performed before resulted positively and CONFIRM HF only confirmed their conclusions. Quality of life – neglected parameter. Conclusions of CONFIRM HF introduce the improved quality of life as one of the main result. Unfortunately, it is a parameter (subjective, difficult to objectify) which doesn´t have such importance as endpoint – indicator of intervention trials. It is shown that treatment of anaemia is important in chronic fatigue (especially women) and affects quality of life. Improvement in symptoms of fatigue has been demonstrated after administration of 1000 mg of ferric carboxymaltose. Antifosfolipidový syndrom (APS) je autoimunitně podmíněné onemocnění s celou řadou potenciálně život ohrožujících manifestací. Vyskytuje se samostatně v primární podobě nebo jako sekundární doprovázející řadu chorobných stavů (autoimunitní, nádorová onemocnění atd.). Diagnostická kritéria zahrnují jak klinickou, tak laboratorní složku. Mezi klinické manifestace patří výskyt trombóz arteriálních či žilních a dále komplikace v těhotenství. Laboratorně musí být opakovaně prokázána přítomnost antifosfolipidových protilátek v minimálním odstupu 12 týdnů. Klinická manifestace APS může být velmi pestrá a často vyžaduje multidisciplinární přístup. Obávanou, ale vzácnou komplikací je tzv. katastrofický APS spojený s vysokou morbiditou a mortalitou. Včasná diagnóza spolu s terapií výrazně ovlivňuje prognózu pacientů s APS. APS occurs in a primary form with no associated conditions, or in a secondary form associated with various other illnesses (autoimmune, neoplastic and others). APS is diagnosed according to clinical and laboratory criteria. It is characterized by recurrent arterial or venous thrombosis and/or pregnancy‑related complications and laboratory findings of antiphospholipid antibodies in a minimum time interval of 12 weeks between individual laboratory tests. Clinical manifestations of APS can vary and a multidisciplinary approach is needed. Catastrophic APS is a very serious and life‑threatening condition associated with high mortality and morbidity, although it is rare. Sekundární prevence aterotrombotických příhod je doménou protidestičkové léčby, podle rizika jedním lékem či kombinací acetylsalicylové kyseliny s blokátory ADP receptorů. Význam kombinace duální protidestičkové léčby v kombinaci s xabany či s dabigatranem prověřovalo 6 klinických studií. Pouze jedna z nich (ATLAS ACS 2-TIMI 51) ukázala, že léčba malými dávkami rivaroxabanu (2krát 2,5 mg) může být přidána ke kombinaci kyseliny acetylsalicylové s klopidogrelem. Pro několikanásobně zvýšené riziko velkých krvácivých příhod se však čistý klinický přínos pohybuje jen kolem 0,5 % ročně. Duální léčba s kombinací kyseliny acetylsalicylové s prasugrelem či s tikagrelorem je přínosnější. V druhé části přehledu je rozebírán vyšší výskyt infarktu myokardu v kontrolovaných studiích při léčbě dabigatranem v porovnání s warfarinem. Vztah není dořešen, nicméně u nemocných s vyšším rizikem koronárních příhod je při indikaci antikoagulační léčby přímými antikoagulancii vhodnější volit ze skupiny xabanů (apixaban či rivaroxaban)., Secondary prevention of atherothrombotic events is the domain of antiplatelet therapy and according to present risk is used one drug strategy or combination of acetylsalicylic acid with ADP receptor blockers. The importance of the combination of dual antiplatelet therapy together with xabans or dabigatran was investigated in 6 clinical trials. Only one of them (ATLAS ACS 2-TIMI 51) indicated that treatment with small dose of rivaroxaban (2 × 2.5 mg) may be added to dual strategy of acetylsalicylic acid and clopidogrel. The risk of major bleeding event is increased and net clinical benefit is only about 0.5 % per year. Dual therapy with aspirin and prasugrel or tikagrelor is beneficial. In the second part of the review is discussed higher incidence of myocardial infarction in controlled group in the trial comparing treatment of dabigatran with warfarin. Literature data support that green tea and its major component epigallocatechin gallate (EGCG) have powerful antioxidant effects. Contrary, hepatotoxicity can be induced by high-dose EGCG. The timing of exposure to green tea in relation to administration of hepatotoxic agent plays an import role too. The aim of our work was a verification of antioxidative effect of EGCG on D-galactosamine-induced injury in primary culture of rat hepatocytes. Hepatocytes were incubated with EGCG at concentrations of 1.25-10 μM and toxic D-galactosamine (GalN) for 24 hrs. Alternatively, hepatocytes were pretreated with EGCG for 24 hrs, and then incubated with EGCG and GalN for further 24 hrs. Cytotoxicity was analysed by lactate dehydrogenase activity, functional capacity by albumin production. Oxidative stress was evaluated from a production of malondialdehyde and glutathione content in the cells. EGCG protected hepatocytes against GalN-induced cytotoxicity but preventive treatment of intact hepatocytes with EGCG was required to diminish the development of hepatocyte injury. Oxidative stress induced in our study seems to overcome the ability of hepatocytes to improve GSH depletion and albumin production. Invazivní metody léčby chronické formy ischemické choroby srdeční byly po dlouhou dobu považovány za kauzální terapii, která bezpochyby snižuje morbiditu a mortalitu nemocných. Na druhé straně životní styl a farmakologická terapie byla nahlížena jako spíše doplňující, ale nikoliv rozhodující léčba. V posledních letech je možno vidět změnu v pohledu na ischemickou chorobu srdeční. Je to mnohem difuznější proces v koronráním řečišti a není to tedy jen otázka kritických stenóz, ale stejně nemocného ohrožují i další úseky věnčitých tepen, kde se hovoří o vulnerabilních placích. Překvapením pro experty byly závěry řady studií potvrzujících neschopnost invazivch metod prodloužit život nemocným s chronickou formou ICHS. Na druhé straně přibývají důkazy o významném vlivu úpravy životního stylu a rozrůstajících se možnostech farmakolgoické terapie s pozitivním vlivem na morbiditu a mortalitu. By contrast, lifestyle and pharmacological therapy were seen as valuable but not lifesaving. In the last few years a change can be seen in the view on coronary artery disease. It is a much more diffusive and complicated process in the coronary tree. It is not merely a question of the critical stenotic areas; the new idea of vulnerable plaque, which presents the same danger of complication as critically stenotic areas, is also important. A great surprise for experts was the result of various studies confirming the inability of invasive procedures to prolong life in patients with chronic coronary disease. On the other hand, there has been more evidence that lifestyle changes and expanding pharmacological possibilities can have a significant positive effect on morbidity and mortality. Vzniku trombózy v arteriálním nebo venózním cévním systému předchází složitá souhra faktorů vnějšího prostředí a faktorů genetických. Rozsáhlé genomové asociační studie se ukazují jako velice přínosné v identifikaci lokusů spojených s kardiovaskulárními onemocněními a souvisejícími rizikovými faktory. Ještě zbývá mnoho práce v identifikaci určujících genů a jejich kauzálních variant, stejně jako v definici mechanizmů, které ovlivňují vývoj a progresi onemocnění. Z výsledků studií lokusů spojených s dříve identifikovanými onemocněními se očekává zlepšení našich znalostí o patofyziologii kardiovaskulárních onemocnění a možnost identifikovat nové cíle pro léčbu. V oblasti aterotrombózy a trombofilie jsou významné výsledky z asociačních studií zaměřených na oblast ischemické choroby srdeční, ischemické cévní mozkové příhody a žilního tromboembolizmu. INTRODUCTION: The issue of resistance to antiplatelet therapy has raised many questions in the area of neurovascular diseases. The first objective of this work was to determine the prevalence of aspirin resistance in neurovascular patients with clinical non-responsiveness to aspirin treatment and a high-risk of atherothrombotic complications using two interpretable and independent methods (aggregation and PFA 100). The second objective was to find the correlation between both assays and to evaluate the results in groups at risk for various cerebrovascular diseases. MATERIAL AND METHODS: Laboratory tests of aspirin resistance were performed in 79 patients with clinical non-responsiveness to aspirin treatment suffering from neurovascular diseases. Patients were divided into the two groups: expected low risk for aspirin resistance due to the first manifestation of a neurovascular disease (n = 34) and expected high risk due to the second clinical manifestation of a neurovascular disease (n = 45). RESULTS: The prevalence of aspirin resistance in both groups combined as determined by the PFA-100 and CPG techniques were 50.6% and 17.7%, respectively. No correlation was found between the two techniques. CONCLUSIONS: No significant prevalence of aspirin resistance was demonstrated by either method despite the heterogeneous pathophysiological mechanisms. Cíl: Převod Testu paměti pro záměry (Memory for Intentions (Screening) Test; MIST, 2010) do české verze zahrnoval standardizaci pomůcek, převod administrace a skórování, validační studii na české populaci, aby MIST bylo možné použít na české populaci. Úvod: Konstrukt prospektivní paměti (PP), tj. schopnosti si zapamatovat a realizovat záměry, je nezbytný pro uchování soběstačnosti, vykonávání aktivit denního života. V posledních letech hraje čím dál významnější roli pro diagnostiku poruch paměti. Standardizovaná a validovaná měřítka PP však v české diagnostice chybí. Metodika: Český převod MIST jsme administrovali 30 zdravým osobám. Výsledky: Celkový skór PP v testu MIST koreloval na střední úrovni s měřítky z neuropsychologické baterie zejména s retencí v paměti, mentální flexibilitou, odolností k interferenci a premorbidní inteligenční úrovní (všechna rho = 0,37–0,42, všechna p < 0,05). Reliabilita testu MIST ve smyslu vnitřní konzistence byla u základních osmi subškál nedostatečná (α = 0,50) i ve smyslu reliability obou polovin testu (split‑half reliabilita = 0,56), avšak reliabilita šesti subškál tříděných dle typu (oddálení, vodítka a modality odpovědi) byla vysoká (α = 0,88; split‑half = 0,95). Závěr: Výsledky validační studie naznačují, že český převod MIST je srovnatelný s verzí originální. Studie zpřístupňuje standardní vyšetření PP a umožňuje měření PP na klinických populacích. We included standardized testing material, translation of administration and scoring, and assessment of normative data for the MIST in the Czech population. Introduction: Prospective memory (PM), i.e., the ability to remember and implement intentions after a delay, is essential as a subsystem of episodic memory for the maintenance of independence and execution of activities of daily living. PM assessment thus plays an important role in the diagnosis of episodic memory disorders. However, there are currently no standardized and validated PM tools in Czech language. Methods: The Czech version of MIST was administered to 30 healthy persons. Results: The MIST Summary score correlated at a medium level with a range of neuropsychological measures including memory retention, mental flexibility, and resistance to interference (all rho = 0.37–0.42; all p < 0.05). The reliability of MIST in terms of internal consistency was insufficient when analyzing the eight individual MIST trials (α = 0.50), as was split‑half reliability (split‑half reliability = 0.56). In contrast, there was a high degree of reliability between six subscales classified by type (delay, cue and mode of response; α = 0.88, split‑half = 0.95). Conclusion: The reliability and validity of the Czech version of MIST is comparable to the original English version. Psoriasis patients are often susceptible to cardiovascular diseases (CVD), including atherosclerosis. Traditional markers (biochemical and inflammatory) and diagnostic tools could detect occlusive but not subclinical atherosclerosis. Carotid intima-media thickness (CIMT), has recently been recognised as a non invasive diagnostic tool for identification of premature atherosclerosis. Therefore we evaluated 80 psoriasis patients and 80 age sex matched healthy controls for serum leptin levels and apolipoprotein B/apolipoprotein A-I ratio (apoB/apoA-I ratio) in relation with CIMT of carotid artery. Carotid intima-media thickness and carotid plaques were simultaneously measured by carotid sonography. Serum concentration of leptin and apolipoprotein were measured using enzyme-linked immuno sorbent assay (ELISA) and nephelometry respectively. Raised CIMT correlated to age of onset of the disease, serum leptin and apoB/apoA-I ratio in psoriasis patients. Taking into account, values that were above the 75 percentile of the three markers (leptin, apoB/apoA-I ratio and CIMT) the odds ratio was 4.26 (2.06-8.80 CI). Leptin and apoB/apoA-I ratio showed significant cumulative association with CIMT. Results of predictive analysis supports measurement of CIMT along with estimation of serum leptin and apoB/apoA-I ratio for prediction of premature atherosclerosis in psoriasis patients. and K.
