Cíl: Posoudit efekt samoléčby a autodiagnózy dysmenorey u studentek univerzity v Maiduguri. Metody: 289 studentek trpících dysmenorhoeou z celkového počtu 25 000 studentek univerzity v Maiduguri ze státu Borno, Nigerie, bylo randomizováno k léčbě dysmenorey. Studentky byly vybrány na základě dotazníku, který vyplnily v lednu 2011. Výsledky: Střední věk ± směrodatná odchylka (SD) účastnic byl 22.50 ± 3.12 roku. 112 (38.8 %) užívalo analgetika a nefarmakologické prostředky k tišení bolesti, buď současně, nebo následně (p < 0.001). Nejčastějšími nemedikamentózními prostředky byly horká lázeň (15.2 %) a ohřívací dečka umístěná na podbřišek (5.5 %). Nejčastějšími léky byly nesteroidní antirevmatika (NSAID) užívané při nástupu menstruace. V 72 % byla léčba účinná, ve 4.2 % nebyla. Většina účastnic se medikovala sama a nezabývala se možnými vedlejšími účinky (p < 0.001). Většina účastnic (63 %) utratila 300 Nigerijských Naira a méně (1.9 US Dolarů a méně) za léčbu dysmenorey. Závěry: Výsledky potvrdily, že NSAID byly nejčastěji užívané medikamenty k léčbě dysmenorey, ale většinou bez znalosti potencionálních vedlejších účinků. Proto doporučujeme zapojení profesionálních zdravotníků do tohoto procesu., Objective: This study was performed to evaluate self-medication for the treatment of self-diagnosed dysmenorrhoea among a group of University of Maiduguri female students. Methods: The population of this randomized study was 289 female students suffering from dysmenorrhoea out of over 25 000 students attending a college of medicine and eight non-medical faculties of university of Maiduguri in Borno state, Nigeria. The students were interviewed using a self-administered pre-tested structured questionnaire in January, 2011. Results: The Mean ± Standard Deviation (SD) of age of the participants was 22.50 ± 3.12 years. Significant number 112 (38.8%) of participants use drugs to remedy menstrual pains followed by non-drug therapy while few combine both the drug and non-drug therapy for the pain management (p < 0.001). The most common non drug remedies employed by the participants were taking a hot bath (15.2%) and placing a heating pad on abdomen (5.5%). However, the most common drugs used by the participants were Non-Steroidal Anti-Inflammatory Drugs (NSAIDs) which were usually taken orally at the onset of menstruation. Among the participants that used drug, 72% found it effective while 4.2% reported ineffectiveness. Significant majority of the participants usually self-prescribed their drugs, and were ignorant of the common side effects of these drugs (p < 0.001). Most participants (63%) spent 300 Nigerian Naira and below (1.9048 US Dollar and below) on drugs for dysmenorrhoea every month. Conclusion: The results confirmed that NSAIDs were the most common drugs used for dysmenorrhoea treatment, but frequently used without knowledge of potential side effects due to self-medication. Úvod: Léčba intratékálním baklofenem (ITB) kontinuálně pomocí pumpových systémů je důležitou léčebnou modalitou u nemocných s těžkou spasticitou, která již nezabírá na perorální léky nebo když se objeví výrazné nežádoucí účinky. Tato metoda je relativně velmi bezpečná, přesto se ale mohou vyskytnout komplikace spojené s touto léčbou. Nejčastěji se objevují komplikace spojené s implantací systému. Jde zejména o problémy s katétrem. Rotace pumpy je extrémně vzácnou komplikací a její výskyt je méně než 0,3 %. Kazuistika: U 74leté ženy s těžkou spasticitou po chronickém míšním poranění ve výši Th9 byla úspěšné provedena implantace baklofenové pumpy (Medtronic, Synchromed II). Pacienka měla bezprostřední úlevu se snížením spasticity již na dávce baklofenu 100 ?g/den. Na první ambulantní kontrole se opakovaně nezdařilo doplnit rezervoár pumpy. Bylo vysloveno podezření na její rotaci. Pacientka byla okamžitě odeslána na rentgenologické vyšetření, kde bylo potvrzeno přetočení pumpy o 180 stupňů. Tentýž den byla pacienta v lokální anestezii operována, pumpa byla otočena a důkladně zafixována stehy. Vzhledem k rychlé revizi se u pacientky nerozvinul syndrom z náhlého odnětí baklofenu při prázdném rezervoáru pumpy. Závěr: Rotace pumpy je vzácná komplikace léčby ITB. Je nutné okamžité vyřešení tohoto problému, neboť hrozí rozvoj abstinenčního syndromu z náhlého odnětí baklofenu. Léčbu ITB je třeba provádět v centrech se specializovanou péčí o tyto pacienty. Despite generally favorable outcomes and relative safety of this method, complications associated with ITB can be observed. The most common problems include surgery‑related complications resulting from system implantation, including catheter failure. Pump rotation is a rare complication with incidence less than 0.3%. Case report: Seventy‑four‑years‑old female suffered from spasticity after chronic spinal cord injury at T9 level. e underwent successful implantation of ITB pump (Medtronic, Synchromed II) with prompt spasticity relief on baclofen dose of 100 ?g/day. During the first outpatient appointment, it was difficult to puncture and fill the reservoir with baclofen. Plain x‑ray revealed rotation of the pump with reservoir in downright position. On the same day, the patient underwent emergency surgical revision with correction of the pump position and refill of the reservoir. No baclofen withdrawal syndrome or other complications occurred and the patient recovered well. Conclusion: Pump rotation is a very unusual and rare complication of ITB treatment. Na nedávném kongresu Evropské urologické asociace (EAU) ve švédském Stockholmu byla prezentována data, která jednoznačně ukazují na to, že se lékaři při léčbě příznaků syndromu dolních cest močových u mužů až příliš soustřeďují na evakuační (obstrukční) symptomy a opomíjejí příznaky jímací (iritační), které zřejmě ještě negativněji ovlivňují kvalitu života jejich pacientů. In order to study cardiac pathology in different subtypes of ischemic stroke we examined 165 stroke patients. The 1st group included 90 (54.5%) patients with hemodynamic stroke, the 2nd one - 75 (45.5%) patients with cardioembolic stroke. Control group consisted of 45 individuals without cerebrovascular diseases. Cardiac pathologies with the prevalence of various types of ischemic heart disease were observed in all groups. The parameters of left ventricular stroke volume and ejection fraction were lower in patients with hemodynamic stroke than in other groups, but were within normal limits. Potential sources of cardiogenic embolism were found in all groups: in the 1st group at 74.4%, in the 2nd one at 100%. The disabling nature and lifelong impact of multiple sclerosis (MS) imposes considerable socioeconomic burden despite its relative low prevalence compared to other chronic diseases. This is the first study of MS in the Slovak Republic that provides information about healthcare and social expenditures including costs of productivity loss caused by reduced work capacity of patients with MS. Retrospectively direct and indirect costs of MS were investigated by prevalence based "bottomup" approach. The societal and health insurance perspective was used to perform the overall economic burden caused by MS in Slovakia. The human capital method was used for the calculation of indirect costs, as this is the most common method applied in published studies. As not all detailed data on expenditures were available, the missing data were collected in the retrospective patient research. Total annual costs in 2010 for 6.100 dispensed patients with MS in Slovakia were 54.723.592. Indirect costs (31.728.757) prevailed over direct costs (22.994.834). The highest part of both costs were 25.207.512 and 12.641.052 for loss productivity due to patients sickness and invalidity pensions and diseasemodifying drugs respectively. The average cost per patient independently of disease severity was 8.971. MS causes a high economic burden, with a strong predominance of indirect costs. Cíl: Správný pooperační režim je důležitou součástí rekonvalescence po dekompresi karpálního tunelu. Cílem této prospektivní randomizované studie je zjistit, zda pooperační dočasné dlahování má klinicky významný efekt, nebo zda není naopak zbytečné. Výsledky dosud publikovaných prací nedávají jasné, prokazatelné a jednoznačné doporučení. Soubor a metodika: Do souboru bylo zařazeno celkem 72 pacientů s klinicky a elektromyograficky prokázaným syndromem karpálního tunelu, kteří byli randomizováni do dvou skupin. V první skupině byla pacientům po operaci na dva týdny přiložena dorzální dlaha fixující zápěstí v neutrální poloze a v druhé skupině dlaha použita nebyla. Před operací i po operaci byla hodnocena distální motorická latence, síla stisku a míra subjektivních obtíží pomocí specializovaných dotazníků DASH (Disabilities of Arm, Shoulder and Hand Questionnaire) a Bostonského dotazníku (Boston Carpal Tunnel Questionnaire). Výsledky: Po operaci došlo ke zkrácení distální motorické latence a mírnému snížení síly stisku, ale mezi skupinami nebyly nalezeny statisticky významné rozdíly (p > 0,05). Po operaci došlo také k předpokládanému zmírnění subjektivních obtíží, objektivizovanému dotazníkovými koeficienty, ale mezi skupinami nebyl nalezen statisticky významný rozdíl (p > 0,05). Závěr: Podle našich zjištění dočasné dlahování po otevřené dekompresi karpálního tunelu ve srovnání s nedlahovanou skupinou nezlepšuje výsledky chirurgické léčby. Používání dlahy v této indikaci se jeví jako nadbytečné jak z lékařského, tak z ekonomického pohledu. Na druhou stranu dočasné dlahování po operaci výsledky operační léčby nezhoršuje., Aim: Correct post-operative care is an important part of recovery after carpal tunnel decompression. The aim of this prospective randomized study was to determine whether postoperative temporary splinting has a clinically significant effect, or whether it is unnecessary. The results of published data do not give clear, demonstrable and unambiguous recommendations. Material and methods: A total of 72 patients with clinically and electrodiagnostically established diagnosis of carpal tunnel syndrome were randomized into two groups. The first group included patients with a dorsal splint, fixing the wrist in a neutral position for two weeks after surgery, whereas the second group included patients, in whom the splint was not used after surgery. Distal motor latency, grip strength and degree of subjective symptoms were assessed before and after surgery using specific questionnaires, namely the DASH questionnaire (Disabilities of Arm, Shoulder and Hand) and the Boston questionnaire (Boston Carpal Tunnel Questionnaire). Results: After surgery, there was a reduction of distal motor latency and a moderate reduction of grip strength, but there were no statistically significant differences (p > 0.05) between the two groups. After surgery, the anticipated improvement of subjective symptoms was achieved, objectified by questionnaire coefficients. However, there was no statistically significant difference (p > 0.05) between the two groups either. Conclusion: According to our findings, temporary splinting after open carpal tunnel decompression does not improve the results of surgical treatment compared to the non-splinted group. Using the splint in this indication seems to be redundant from the medical and economic point of view. On the other hand, temporary splinting after surgery does not cause worsening of the results of surgical treatment. Práce se zabývá základními nástroji hodnocení efektivity ve zdravotnictví a jejich možnou aplikací do zdravotního systému. Zabývá se definicí efektivnosti a jejím terminologickým odlišením od účinnosti. Stanovuje základní druhy efektivnosti a typy nákladové analýzy, která se v oblasti zdravotnictví používá. Druhá část práce nabízí pohled na jednotlivé hráče zdravotního systému a jejich možnost zapojit analýzu efektivnosti a jejich výstupy do vlastní racionální činnosti. Ukazuje se, že analýza efektivnosti není jen prvkem, který by měl být zaveden do reformy zdravotnictví, ale i samotným nástrojem reformy, který usnadní racionální rozhodování., This paper treats basic tools of efficiency evaluation and their implementation into health-care system. Activated hepatic stellate cells (HSC) are a major source offibrous proteins in cirrhotic liver. Inducing or accelerating their apoptosis is a potential way of liver fibrosis treatment. Extracellular matrix (ECM) surrounding cells in tissue affects their differentiation, migration, proliferation and function. Type I collagen is the main ECM component in fibrotic liver. We have examined how this protein modifies apoptosis of normal rat HSC induced by gliotoxin, cycloheximide and cytochalasin D in vitro and spontaneous apoptosis of HSC isolated from CCl4-damaged liver.
