Cíl. S nárůstem sportovních aktivit a vyšším počtem přístrojů magnetické rezonance se jeví potřeba vymezit úlohu magnetické rezonance ve vyšetřování sportovních úrazů. Autoři se pokusili na základě dlouholetých zkušeností určit přínos MR k objasňování sportovních traumat a ukázat na některá specifika vyšetření. Metoda. Byly sledovány sportovní úrazy jak rekreačních, tak závodních sportovců ve spádové oblasti v časovém období od roku 2004 do června 2012 tak, jak je přinášel sportovní život. Validita nálezů byla ověřována zpětnou vazbou v úzké spolupráci s ošetřujícími sportovními klubovými lékaři. Výsledky. MR vyšetření potvrdilo nebo vyloučilo akutní trauma ve všech případech již v časném období po úrazu. U chronických potíží dokázalo určit bližší specifikaci onemocnění. Závěr. MR je spolehlivou a nepřínosnější metodou ve vyšetřování sportovních poranění svalově-šlachové jednotky a kloubů., Aim. The increased spectrum of the sport activities and also the increased number of the installed MR suites need to define the role of MRI more precise. The authors presented their own experience with the imaging of the sports injuries using MRI. Methods. The period 2004-2012 was assessed according the occurrence of the sports injuries as the life brought them. Validity of the imaging results was compared with the preoperative or clinical data. Results. MRI confirmed or excluded with high accuracy almost all injuries caused by the sports trauma. Conclusion. Here we report the case of a 31-year-old man admitted to our hospital with echocardiografic and Cardiac Magnetic Resonance signs of myocarditis complicated by ventricular tachycardia, initially resolved with direct current shock. After the recurrence of ventricular tachycardia the patient was submitted to electrophysiological study revealing a re-entrant circuit at the level of the medium segment of interventricular septum, successfully treated with transcatheter ablation. This case highlights how the presence of recurrent ventricular arrhythmias at the onset of acute myocarditis, suspected or proven, could be associated with a pre-existing arrhythmogenic substrate, therefore these patients should be submitted to electrophysiological study in order to rule out the presence of arrhythmogenic focuses that can be treated with transcatheter ablation., Emanuele Cecchi, Serena Fatucchi, Elena Crudeli, Cristina Giglioli, and Literatura 15
Postavení plátce ve zdravotnictví je zajímavou a často diskutovanou problematikou v mnoha zdravotních systémech. Česká republika v tomto směru není výjimkou. Již od roku 1993 se zde, méně či více bouřlivě, vedou debaty o tom, zda je, po vzoru národních služeb, lépe mít zdravotní systém s jedním plátcem či zda dát prostor vícero plátcům. Záměrem autorky je přispět do této polemiky a na základě poznatků literatury se zamyslet nad tím, jakou roli by vlastně plátci ve zdravotnictví měli plnit. Následně k tomu vymezit a diskutovat, jaké podmínky musí být splněny, aby tyto role plátci plnit skutečně mohli. Jinými slovy, cílem článku je specifikovat role plátců ve zdravotních systémech a vymezit a diskutovat podmínky vedoucí k tomu, aby tyto role plnit mohli., The role of payers in the healthcare systems is an interesting and often discussed issue in many countries. The Czech Republic is not an exception. Since 1993, there has been a debate, more or less heated, of whether it is better to have a single payer system (like NHS) or whether the pluralistic system should take place. The purpose of the author is to contribute to this polemics and based on the literature review, to think about what is/should be the role of the payer in the healthcare systems and, in accordance to this, to specify and to discuss the conditions that must be fulfilled to accomplish those roles. Cíle studie: Cílem této studie je sledování vzájemného vztahu plasmatických hladin imatinibu s jeho denní dávkou a časovým intervalem od poslední užité dávky u pacientů s chronickou myeloidní leukemií na terapii imatinibem. Chronická myeloidní leukemie (CML) je myeloproliferativní onemocnění charakteristické přítomností chromozomu Philadelphia a leukemického fúzního genu BCR-ABL vedoucího k expresi konstitutivně aktivní tyrosinkinázy zodpovědné za maligní transformaci. Největší úspěšnost v léčbě byla zaznamenána prostřednictvím cílených inhibitorů vzniklého onkoproteinu, proto je dnes imatinib používán jako lék první linie. Terapeutické monitorování plasmatických hladin imatinibu u pacientů s CML je důležitým nástrojem k optimalizaci a individualizaci léčby. Typ studie: Klinická Materiál a metody: V naší studii jsme analyzovali soubor 1790 vzorků od 168 pacientů s CML na terapii imatinibem. Veškerá měření byla provedena metodou kapalinové chromatografie ve spojení s tandemovou hmotnostní spektrometrií v rámci rutinního vyšetřování plasmatických hladin tyrosinkinázových inhibitorů. Denní dávka imatinibu s pohybovala v rozmezí 100 až 800 mg; vzorky byly odebrány 1–120 hodin od poslední užité dávky. Výsledky a závěr: Analýza výsledků odhalila významnou korelaci plasmatické hladiny imatinibu s jeho denní dávkou (R2 = 0,075) a časovým intervalem od poslední užité dávky (R2 = 0,177). Z celého souboru bylo vybráno 616 vzorků odebraných 24±4 hodiny od poslední dávky od 111 pacientů s denní dávkou 400 mg imatinibu a byla stanovena inter- (48 %) a intraindividuální (36 %) variabilita., Objective: The aim of the study is monitoring of correlation of imatinib plasma levels with daily dose and sampling time in patients with chronic myeloid leukemia on imatinib therapy. Chronic myeloid leukemia (CML) is myeloproliferative disorder characterized by the presence of chromozome Philadelphia and leukemic fusion gene BCR-ABL leading expression of constitutively active tyrosin kinase responsible for malignant transformation. The most successful treatment was recorded via targeted inhibitors of generated oncoprotein and therefore imatinib is used as a first-line treatment, today. Therapeutic monitoring of imatinib plasma levels in patients with chronic myeloid leukemia is important tool for treatment individualization. Design: Clinical Materials and methods: In our study we have analyzed a group of 1790 samples from 168 patients with CML on imatinib therapy. All measurements were performed by the liquid chromatography coupled with tandem mass spectrometry method within routine monitoring of plasmatic levels of tyrosin kinase inhibitors. Imatinib daily dose vary from 100 to 800 mg and samples were obtained 1 - 120 hours after last drug administration. Results and conclusion: Analysis of the results revealed correlation between imatinib plasma level and daily dose (R2 = 0.075) or with sampling time (R2 = 0.177). Latitude gradients and secular trends in Europe and North America have been found in the male-female ratio at birth (M/F: male births divided by total births) which is expected to be 0.515. Annual national data for Czechoslovakia and the post-Czechoslovakian (Czech Republic and Slovakia) countries for male and female live births were obtained from the World Health Organisation and analysed with contingency tables. This study analysed 13,123,538 live births. An overall decreasing trend in M/F was found (p < 00001). No latitude gradient was noted. There was an overall deficit of 15,232 male births based on an M/F of 0.515. M/F is declining in this region, despite well developing economies that have resisted the worldwide slowdown. Aim: The principal aim was to establish reference paediatric data for the serum homocysteine levels in Czech children and adolescents. Methods and Results: 144 children either healthy or not sufferig from acute or chronic inflammation, autoimmune disorders including rheumatic diseases, inflammatory musculoskeletal disorders, inflammatory bowel disease, diabetes mellitus, hypercholesterolemia, epilepsy, chronic renal failure, aged 0–19.9 years (0–6.9 years, n = 40; 7–10.9 y, n = 28; 11–15.9 y, n = 45; 16–19.9 y, n = 31) had their blood samples collected and the serum homocysteine level (S-homocysteine) was evaluated by chemiluminescence. A significant age dependence of the S-homocysteine levels was observed (R = 0.35, p < 0.01); with highest values of upper reference range in the 11–15.9 and 16–19.9 years’ group, respectively. Článek představuje projekt UNICEF Škola přátelská dětem; seznamuje ve stručnosti s manuálem projektu. Projekt byl motivován skutečností, že denně navštěvuje základní a střední školy více než dvě miliardy dětí a školní docházka není pro všechny pozitivní zkušeností (např. pitná voda, školní nábytek, trestání, ponižování dětí atd.). Cílem projektu je vytvořit přátelské prostředí pro všechny děti. Klíčové principy projektu jsou: inkluzivní vzdělávání, demokratická participace dětí, podporující prostředí s cílem rozvoje osobnosti dětí a zajištění kvalitního vzdělávání. Jádrem projektu jsou práva dětí. Model Škola přátelská dětem zahrnuje následující oblasti: 1. umístění, exteriér a dispozice interiéru, 2. škola a komunita, 3. škola jako ochraňující prostředí a 4. vyučování. Záměrem projektu je, aby všechny školy mohly být identifikované jako školy přátelské dětem. To předpokládá, že si každá škola vypracuje svůj vlastní projekt a jeho realizaci ověří metodou dotazníkového šetření. Škola spolupracuje při realizaci projektu s orgány veřejného zdraví., The article presents the UNICEF project Child Friendly Schools; it acquaints us with the manual of projects in brief. The project was motivated by the reality that basic and secondary schools are visited by more then two billions children daily, and schooling is not for every child a positive experience (for example: drinking water, school furniture, punishment and humiliation and others). The aim of this project is the creation of a friendly school environment for all children. The key principles of the project are: inclusive education, democratic participation of children, a supporting environment with the aim of developing the child's personality and ensuzing an excellent education. The core of the project are the rights of children. The model of Child Friendly Schools consists of four areas: 1) location, design and construction, 2) the school and the community, 3) the school as a protective environment, and 4) learners, teachers and school management. The purpose of the project is for all schools to be identified us child friendly schools. It anticipates that each school elaborates its own project and its realization is verified by a questionnaire survey. Jedná se o prospektivní pilotní studii zaměřenou na vyšetřování vývoje tvaru a statiky páteře u dětí pomocí neinvazivní somatografické metody systémem DTP při vyšetřování kvality posturální funkce. Sledovaný soubor tvořilo 47 dětí v mladším školním věku a byly vyšetřeny dvakrát (v 1. a v 5. třídě ZŠ). Během sledování došlo ke statisticky významnému prohloubení krční lordózy v celém souboru, tj. u dívek i chlapců, k prohloubení bederní lordózy došlo pouze u chlapců. Celkově ke zlepšení držení těla došlo u 15 %, k celkovému zhoršení držení těla u 53 % a beze změny postury zůstala 4 % dětí vyšetřovaného souboru. Z našich šetření vyplývá 1) nestabilita posturální funkce v tomto sledovaném období, která naznačuje možnost efektivity preventivních opatření v podobě optimální zátěže; 2) možnost upřesnění stávajícího somatoskopického vyšetřování použitím neinvazivního přístroje., The aim of the prospective pilot study was the investigation of the development of postural function (spinal shape and statical properties of the spine) in children by the non-invasive DTP system. The sample consisted of 47 children of primary school age who were examined twice – in the 1st and 5th grade of elementary school. Exacerbation of cervical lordosis was statistically documented during follow-ups in both genders, while worsening of lumbar lordosis was proven only in boys. Overall, posture was improved in 15%, deteriorated in 53% and remained unchanged in 4% of examined children. Příspěvek pojednává o kontraktační povinnosti (smluvním přímusu) poskytovatelů zdravotních služeb na základě zákona o zdravotních službách. Autor poukazuje na to, že ačkoliv v některých situacích zde tento smluvní přímus skutečně existuje, jsou zákonem upraveny i výjimky.
