Farmakogenetika je vedný odbor, ktorý skúma efekt jednotlivých liekov v závislosti od genotypu. V súčasnosti sú liečebné odporúčania pre liečbu niektorých monogenénových diabetov založené na genetickej diagnostike. Aj v oblasti farmakogenetiky perorálnych antidiabetík boli už publikované prvé štúdie, ktoré zistili asociácie jednotlivých génových variantov s liečebnou odozvou. Odozva na deriváty sulfonylurey bola signifikantne asociovaná s variantmi KCNJ11/ABCC8, TCF7L2 a CYP2C9. Odozva na liečbu metformínom bola asociovaná s variantmi génov ATM a SLC47A1. Odozva na liečbu glitazónmi bola asociovaná s variantom génu PPARG. Terapeutická odozva na liečbu gliptínmi bola asociovaná s variantmi génov TCF7L2 a CTRB1/2. Je možné očakávať, že v blízkej budúcnosti budú farmakogenetické poznatky využívané aj pri personalizácii liečby diabetu 2. typu., Pharmacogenetics is the study of how genes (individual genotypes) affect a person‘s response to drugs. At present, recommendations made about the treatment of some monogenic forms of diabetes are based on genetic diagnostics. The first studies in the field of pharmacogenetics of oral antidiabetics have now been published which have identified associations of individual genetic variants with response to treatment. The response to sulfonylurea derivatives was significantly associated with the variants KCNJ11/ABCC8, TCF7L2 and CYP2C9. The response to metformin treatment was associated with the genetic variants ATM and SLC47A1. The response to treatment with glitazones was associated with the genetic variant PPARG. The therapeutic response to the treatment with gliptins was associated with the genetic variants TCF7L2 and CTRB1/2. Fekálna mikrobiálna terapia (FMT) je liečebná metóda, pri ktorej sa prenesie fekálna mikroflóra chorému jedincovi od zdravého darcu, a tým sa obnoví normálne mikrobiálne zloženie čreva. V súčasnosti vzrastá záujem o využitie FMT pri liečbe rôznych chorôb, avšak neexistujú štandardné terapeutické protokoly. Postupy pri realizácii FMT sa líšia vo viacerých aspektoch, ako je výber darcu, príprava fekálnych materiálov, príprava príjemcu a spôsob aplikácie. FMT sa zdá byť najúspešnejšia v liečbe rekurentnej infekcie Clostridium difficile, randomizované kontrolované štúdie dokázali úspešnosť približne 90 %. Existujú aj obmedzené výsledky pre liečbu ulceróznej kolitídy, publikované boli malé série prípadov, ktorých výsledky však nie sú jednotné. Využitie FMT sa skúma aj v liečbe iných chorôb, pri ktorých je narušená črevná mikroflóra, ako sú napr. kardiovaskulárne, autoimunitné a metabolické ochorenia. Nateraz je pri FMT veľa nezodpovedaných otázok, a preto je nevyhnutný ďalší výskum v tejto oblasti., Fecal microbiota transplantation (FMT) is a therapeutic method, in which the fecal microflora from healthy donors is transmitted to the patient to restore the healthy microbial composition of the gut. In the recent years, there is a growing interest in the therapeutic potential of FMT in various diseases. The standard FMT protocols do not exist. Procedures of FMT vary in several aspects such as donor selection, preparation of fecal material, preparation of the recipient and administration way. FMT appears to be the most successful in the treatment of recurrent Clostridium difficile infection (CDI), randomized controlled studies reported 90 % success rate. There is a limited evidence for FMT as a treatment of ulcerative colitis. FMT has been also studied as treatment of diseases with impaired gut microbiota, such as cardiovascular, autoimmune and metabolic diseases. Narrow particle size distribution basalt pebbles of mean particle size 11.5 mm conveyed by water in the pipe sections of different inclination were investigated on an experimental pipe loop, consisting of smooth stainless steel pipes of inner diameter D = 100 mm. Mixture flow-behaviour and particles motion along the pipe invert were studied in a pipe viewing section, the concentration distribution in pipe cross-section was studied with the application of a gamma-ray densitometer. The study refers to the effect of mixture velocity, overall concentration, and angle of pipe inclination on chord-averaged concentration profiles and local concentration maps, and flow behaviour of the coarse particle-water mixtures. The study revealed that the coarse particle-water mixtures in the inclined pipe sections were significantly stratified, the solid particles moved principally close to the pipe invert, and for higher and moderate flow velocities particle saltation becomes the dominant mode of particle conveying.
Cieľ: Včasná diagnostika a liečba infekcie horných močových ciest je v detskom veku náročná a veľmi dôležitá vzhľadom na prognózu ochorenia. Cieľom našej práce bolo overiť možnosti využitia merania indexu rezistencie počas akútnej pyelonefritídy pri diagnostike ochorenia a pri určovaní rozsahu postihnutia obličiek. Metódy: Do súboru bolo zaradených 53 detí (43 dievčat, 10 chlapcov) vo veku 0-19 rokov s prvým atakom akútnej pyelonefritídy. Pacienti boli rozdelení do troch vekových skupín - deti do 4 rokov, od 4 do 7 rokov a deti nad 7 rokov a porovnávali sa so skupinou 29 detí bez príznakov akútnej infekcie a s normálnou funkciou obličiek. U 37 detí sme urobili analýzu rozsahu postihnutia zápalovým procesom. U všetkých pacientov bolo realizované ultrasonografické vyšetrenie uropoetického traktu do 72 hodín od začiatku hospitalizácie zamerané na morfológiu obličiek a meranie indexu rezistencie (RI) prostredníctvom dopplerovskej sonografie. Vo všetkých troch vekových kategóriách boli zistené signifikantne vyššie hodnoty RI pri zápalom postihnutých obličkách oproti obličkám kontrolnej skupiny. Výsledky: U 24 detí (64,8 %) išlo o unilaterálny zápalový proces a u 13 detí (35,2 %) išlo o proces bilaterálny. Záver: Na základe výsledkov možno skonštatovať, že meranie RI má výrazný prínos v diagnostike nejednoznačných prípadov akútnej pyelonefritídy, ako aj v možnosti určenia rozsahu postihnutia obličiek a možno ho odporučiť ako diagnostický postup doplňujúci štandardné diagnostické metodiky., Aim: Early diagnosis and treatment of infections of the upper urinary tract in children is difficult and very important with regard to the prognosis of the disease. The aim of our study was to verify the possibility of using measurements of resistivity index during acute pyelonephritis in the diagnosis of disease and to determine the extent of renal involvement. Methods: 53 children were eligible in the group (43 girls, 10 boys) at the age of 0-19 years at the first attack of acute pyelonephritis. The patients were divided into three age groups - children up to 4 years, 4 to 7 years and children older than 7 years and they were compared with a group of 29 children without symptoms of acute infection and normal kidney function. Ultrasonography examination of uropoietic tract oriented to kidney morphology was performed in all patients up to 72 hours from the beginning of hospitaliza-tion and RI was measured by beans of Doppler sonography In all the three age categories the authors detected significantly higher values of RI in kidneys affected by inflammation as compared with kidneys of the control group. Results: In 24 children (64.8%) was a unilateral inflammatory process and in 13 children (35.2%) was a bilateral process. Poruchy spánku patria k najčastejším non‑motorickým prejavom u pacientov s Parkinsonovou chorobou (PCh), v populačných štúdiách je uvádzaný ich výskyt až u 60 % pacientov. Štúdie tiež uvádzajú rozdielnosť prejavov PCh medzi mužmi a ženami. Cieľ: Zistiť, či sú u pacientov s PCh na Slovensku prítomné medzi pohlaviami rozdiely v klinických prejavoch PCh a výskyte porúch spánku. Materiál a metódy: Zisťovanie bolo robené dotazníkovou metódou s účasťou pacientov a neurológov z jednotlivých regiónov Slovenskej republiky. Dotazníky boli vyplňované počas rutinných kontrolných návštev. Boli zisťované socioekonomické premenné ako pohlavie, vek, vek pri začiatku ochorenia, stupeň poškodenia hodnotený škálou Hoehna a Yahrovej, dominujúci fenotyp ochorenia a používaná medikácia. Pri hodnotení porúch spánku bola použitá slovenská verzia dotazníka PDSS. Výsledky: Získali sme dáta od 1 067 pacientov. Pri porovnávacích analýzach neboli zistené podstatné rozdiely vo väčšine hodnotených parametrov. U mužov bol o niečo vyšší podiel pacientov s hodnotou škály Hoehna a Yahrovej 4 a boli zistené signifikantne vyššie dávky levodopy v monoterapii (p < 0,01). Významné rozdiely boli zistené v skóre škály PDSS pre hodnotenie rušivých halucinácií počas noci – skóre u mužov bolo 8,22, u žien 8,48 (p < 0,05). Podobný výsledok sme zaznamenali v skóre hodnotiacom nutnosť močenia počas noci – u mužov bolo skóre 5,90, u žien 6,53 (p < 0,01). Redakční rada potvrzuje, že rukopis práce splnil ICMJE kritéria pro publikace zasílané do biomedicínských časopisů., Sleep disturbances are one of the most common non-motor symptoms in patient with Parkinson’s disease (PD) with community‑based studies reporting prevalence data of 60%. Differences in symptoms between men and women have been reported. Aim: In the present study, we assessed whether there are gender differences in clinical presentation of PD and prevalence of sleep disturbances in individuals diagnosed in the Slovak Republic. Material and method: Questionnaires were distributed to participating neurologists and patients in outpatient practices across the Slovak Republic. Sociodemographic variables – gender, age, age at onset, disease severity according to Hoehn and Yahr stage, phenotype of the dominating symptom of Parkinson’s disease and type of medication – were collected. The Slovak language version of the PDSS was used in a questionnaire to test sleep disturbances. Results: Data from 1,067 outpatients with PD were collected. Comparative analyses showed males and females not to be significantly different on the majority of the demographic and medical characteristics collected. Males had a slightly higher proportion of individuals with Hoehn and Yahr score 4 and, among those taking levodopa medication as monotherapy, males took significantly higher levodopa dose than females (p < 0.01). A significant difference in the distribution of PDSS subscores between males and females was observed on item 7 (distressing hallucinations at night) – score for males 8.22, for females 8.48, p < 0.05. Od objavenia mimobunkovej fetálnej DNA v krvi matky uplynulo už 16 rokov. Avšak až v priebehu posledných 5 rokoch prebieha čoraz viac výskumov za účelom stanovenia jej presnej charakteristiky, koncentrácie a molekulárnej veľkosti, ktoré sa neskôr premietli do jednotlivých testov. Tie sú ale dostupné len v posledných 2 rokoch. Pri aktuálnom trende preferovania neinvazívnych metód prenatálnej diagnostiky a ich posúvania do obdobia prvého trimestra sa stanovenie mimobunkovej fetálnej DNA v krvi matky ukazuje ako veľmi perspektívnou metódou diagnostiky chromozómových aberácií u plodov. Jej širšie uplatnenie je limitované malým počtom štúdií na nerizikových skupinách pacientok a v nemalej miere aj ekonomickou náročnosťou vyšetrenia. Aj napriek tomu sa však táto metóda čoraz viac rozširuje zo Spojených Štátov Amerických do celého sveta, čo znižuje náklady na ňu, a stáva sa dostupnejšou pre čoraz väčší počet pacientok. Ukazuje sa, že táto metóda by v rámci podrobnejšieho vyšetrovania u rizikových pacientok mohla perspektívne minimalizovať riziko fetálnych strát, ktoré sú závažnou komplikáciou invazívnych metód prenatálnej diagnostiky., It has been 16 years since there has been a discovery of cell-free fetal DNA (cffDNA) in mother‘s blood. However in the last 5 years there has been more researching done in the purpose of its specific diagnosis, concentration and molecular size, which have later on reflected into particular tests. These have been available only in the past 2 years. Within the actual trend in preference of non-invasive methods of prenatal diagnosis and its shifting into first trimester the examination of mother’s blood has been showing as a very perspective method for early cell-free fetal DNA discovering of chromosome aberration diagnosis of fetus. Its wider enforcement is unfortunatelly limited by a small amount of studies within non-risk pregnancy groups of patients as well as its higher financial cost. Despite of this information this method is extending further more from United States of America into whole world which lowers its costs and its becoming more accesible for a wider amount of patients. Fyziologická gravidita je sprevádzaná zmenami v lokálnej a systémovej koagulácii a fibrinolýze. Hemostáza zasahuje do procesu implantácie blastocysty a placentácie. Udržiava optimálne prekrvenie placenty, zabezpečuje rýchle a adekvátne zrážanie počas pôrodu. Mnohé koagulačné proteíny sú exprimované na začiatku embryonálneho vývoja a hrajú úlohu mimo koaguláciu v proliferácii a diferenciácii buniek. Mechanizmus regulácie v placente nie je dostatočne chápaný. Poruchy hemostázy sa podieľajú na peripartálnom krvácaní, gestačnom tromboembolizme a považujú sa za rizikový faktor neúspešnej reprodukcie a tehotenských komplikácií., Physiological pregnancy is accompanied by changes in local and systemic coagulation and fibrinolysis. Haemostasis interferes with the implantation of the blastocyst and placentation. Maintain optimal blood flow placenta, ensures prompt and adequate clotting during the labour. Many coagulation proteins are expressed early in embryonic development and play roles outside of coagulation in cells proliferation and differentiation. Mechanism regulating haemostasis within the placenta remain poorly understood.
