Imunostimulace polybakteriálním lyzátem (Urivac®) v prevenci recidivujících infekcí dolních močových cest. Infekce močových cest (IMC) zaujímají v evropských zemích i u nás dlouhodobě druhé místo ve výskytu infekčních onemocnění v populaci a představují tak významný socioekonomický problém. Spolu s respiračními infekty se rozhodující měrou podílejí na preskripci antimikrobiálních přípravků. Extenzivní antimikrobiální léčba je jednou z nejvýznamnějších příčin nárůstu bakteriální rezistence. Určitou možnost prevence recidiv infekcí dolních močových cest a snížení spotřeby antimikrobiálních preparátů představuje imunostimulace. Prospektivní multicentrická studie hodnotila klinický účinek, snášenlivost a přínos k léčbě recidivujících zánětů při podávání imunostimulačního přípravku Urivac®. Počet atak infekcí dolních močových cest se po imunostimulaci významně snížil jak v jejím průběhu, tak po jejím ukončení (p = 2,2x10-16). V průběhu imunostimulace bylo bez příznaků 85 % nemocných a při dalším sledování 70,8 %. Snížil se také počet aplikovaných antimikrobiálních přípravků. Subjektivní zlepšení přetrvávalo ve více než 57 % i po ukončení léčby. Mikrobiologická odpověď dosáhla 77 %, v odstupu od ukončení léčby se snižuje. U exacerbací byla uropatogenní flóra detekována v 61 %. Nežádoucí účinky u dvou nemocných vedly k předčasnému ukončení terapeutického režimu., In European countries, including the Czech Republic, urinary tract infections (UTIs) have long occupied the second place in the incidence of infectious diseases in the population, thus representing a major socioeconomic problem. Along with respiratory infections, they are the major contributors to the prescription of antimicrobial agents. Extensive antimicrobial therapy is one of the most important causes of increased bacterial resistance. Immunostimulation is a possible way of preventing recurrent lower urinary tract infections and reducing the consumption of antimicrobial agents. A prospective multicentre study evaluated the clinical effi cacy, tolerability, and benefi t to treatment of recurrent infections with the immunostimulatory drug Urivac®. The number of attacks of lower urinary tract infections following immunostimulation reduced signifi cantly both during its course and after its cessation (p = 2.2 x 10–16). During immunostimulation, 85 % of patients were symptom-free, and during subsequent follow-up it was 70.8 %. There was associated with a reduction in the number of the antimicrobial agents administered. Subjective improvement persisted in more than 57 % even after treatment cessation. Microbiological response reached 77 %; it decreases with increased time from treatment cessation. In the case of exacerbations, uropathogenic fl ora was detected in 61 %. This research paper presents the results of ovarian reserve estimation for 125 women with the Polycystic Ovary Syndrome (PCOS) who have undergone various methods of surgical treatment resection of the ovaries, thermokauterisation and drilling by laser (HoYag). Ovarian reserve was estimated according to the amount of antral follicles, level of follitropin and Müllerian inhibiting substance (MIS), also named antiMüllerian hormone (AMH). Blood flow in ovarian tissue was also examined after various methods of surgical treatment. Cíl: Sémontův repoziční manévr se v praxi užívá k léčbě pacientů s benigním paroxyzmálním polohovým vertigem zadního polokruhového kanálku. Cílem studie bylo zhodnotit interindividuální a intraindividuální variabilitu při provádění tohoto manévru a definovat parametry, které jeho provedení nejvíce ovlivňují. Metody: Tři zkušení terapeuti provedli manévr oboustranně u 10 zdravých jedinců. Inerciální měřicí jednotka složená z akcelerometru a gyroskopu získávala informace o trajektorii pohybu hlavy v prostoru a její rychlosti. Úhlové změny byly zaznamenány v souřadnicovém referenčním systému. Z odchylek od uvažované roviny polokruhového kanálku byla vyhodnocena data o přesnosti provedení manévru. Výsledky: Parametry ovlivňujícími přesnost manévru jsou výška probanda (p = 0,0252), fáze pohybu (p < 0,0001) a dále terapeut a strana pohybu, přičemž tyto faktory vykazují vliv především ve své interakci. Byl hodnocen vztah fáze pohybu k výšce probandů (p = 0,0130), terapeuta k fázi pohybu (p = 0,0001), terapeuta k výšce probandů (p < 0,0252). Největší zaznamenaný rozsah odchylky od senzorické roviny byl v rozmezí –37,17° až 31° se standardní odchylkou 16,6°. Závěry: Získaná data poukazují na překvapivě vysokou variabilitu při provádění Sémontova manévru ať v porovnání mezi jednotlivými měřeními u jednoho terapeuta nebo v porovnání mezi terapeuty. Analýza vlivu těchto odchylek na úspěšnost terapie benigního polohového paroxyzmálního vertiga a bližší analýza ovlivňujících parametrů by měly být předmětem dalšího výzkumu., Aim: Sémont liberatory manoeuvre is used in the treatment of posterior semicircular canal in patients with benign paroxysmal positional vertigo. The aim of the study was to determine the inter-individual and intra-individual variability in the processing of the manoeuvre and to define the parameters that affect it the most. Methods: Three experienced therapists applied the manoeuvre bilaterally in 10 healthy probands. Inertial measurement unit consisting of the accelerometer and gyroscope recorded the trajectory and speed. Angle changes were placed in the coordinate reference system. Accuracy of the manoeuvre was evaluated based on the deviations from the projected sensory plane. Results: Parameters affecting the accuracy of the manoeuvre included height of a proband (p = 0.0252), phase of the movement (p < 0.0001), therapist and the side of the movement. The effect of these factors is the most pronounced when combined. We assessed interactions of the phase of the movement and the height of the probands (p = 0.0130), the therapist and the phase of movement (p = 0.0001), the therapist and the height of the probands (p < 0.0252). The largest magnitude of deviation from the sensory plane was in the range of –37.17° to 31° with a standard deviation of 16.6°. Conclusions: The data highlight high variability in the implementation of the Sémont liberatory manoeuvre, whether measurements by a single therapist or inter-individually between the therapists are compared. Analysis of the impact of these deviations on therapeutic efficacy in patients with benign paroxysmal positional vertigo and detailed analysis of influencing parameters should be the subject of further research. We assessed the intensive care unit (ICU) patients for Invasive aspergillosis (IA) with culture and non-culture based diagnostic methods from Iran. Thirty-six ICU patients with underlying predisposing conditions for IA were enrolled in the study. Sixty eight Bronchoalveolar lavage (BAL) samples were collected by bronchoscope twice a weekly. BAL samples were analyzed by microscopic examination, fungal culture and galactomannan (GM) detection. The Platelia Aspergillus GM EIA was used to quantify GM indices. Samples with a BAL GM index > or = 1 were considered as positive for GM. Patients were classified as having probable or possible IA. Out of 36 suspected patients to IA, 36.1% of cases showed IA which were categorized as: 4 cases of possible IA and 9 of probable IA. 76.2% of BAL samples were positive for GM. From 13 patients with IA, 11 (84.6%) had at least one positive BAL GM index. Of these patients, 9 (81.8%) showed probable IA. The main underlying predisposing conditions were neutropenia (53.8%) and COPD (30.8%). Our study has indicated that COPD must be considered as one of the main predisposing condition for occurrence of aspergillosis in ICU patients. Poúrazový hydrocefalus způsobený poruchami cirkulace mozkomíšního moku často komplikuje klinický průběh a proces léčby po kraniocerebrálním poranění. Hydrocefalus po poranění míchy je popisován pouze výjimečně. Sdělení popisuje dvě kazuistiky nemocných s kompletní lézí krční míchy, u kterých v poúrazovém období došlo k rozvoji hydrocefalu. Rozbor obou případů a přehled literárních dat potvrzují rozhodující úlohu extraspinálních faktorů pro rozvoj hydrocefalu po míšním poranění. I když k rozvoji hydrocefalu po míšním poranění dochází výjimečně, tato možnost by měla být zvážena při pozdním zhoršení klinického stavu nemocného po poranění krční míchy, zvláště při přítomnosti anatomických anomálií mokového prostoru a poúrazovém subarachnoidálním krvácení., Posttraumatic hydrocephalus caused by cerebrospinal fluid circulation disturbances frequently complicates the clinical course and treatment after craniocerebral injury. Hydrocephalus complicating spinal cord injury is only exceptionally reported. The paper presents two cases of complete cervical spinal cord injury with subsequent development of hydrocephalus. The analysis of both cases and literature data confirmed the dominant role of non-spinal factors in the development of hydrocephalus after spinal cord injury. Despite the exceptional occurrence of hydrocephalus after spinal cord injury, this diagnosis should be considered in cases of delayed deterioration of a patient with cervical spinal cord injury, particularly if cerebrospinal fluid space abnormalities and posttraumatic subarachnoid haemorrhage are present., and J. Úvod: Karcinom prsu je nejrozšířenějším zhoubným nádorem u žen. Nejčastěji se toto onemocnění vyskytuje u žen mezi 60. a 70. rokem života, nicméně v posledních letech dochází k nárůstu počtu pacientek i v mladších věkových skupinách. Incidence karcinomu prsu má celosvětově vzestupný charakter, zejména ve Spojených státech a západní Evropě. Jeho mortalita má však zejména díky screeningovému programu a pokročilým léčebným metodám spíše mírně sestupnou tendenci. Metody: Do souboru byly zařazeny pacientky s karcinomem prsu, které podstoupily operaci pro tuto diagnózu na I. chirurgické klinice FN Olomouc v letech 2008– 2012. U každé pacientky byl vypočítán věk v době stanovení diagnózy, respektive v době operace. Pacientky byly v každém roce rozděleny do věkových skupin po 10 letech. Současně byla v každém roce vytvořena podskupina pacientek s karcinomem prsu mladších 45 let. Byl porovnán počet pacientek v odpovídajících skupinách, resp. podskupinách, v jednotlivých letech. V každém roce byl rovněž stanoven průměrný věk a medián pacientek. Ke zhodnocení přínosu mamárního screeningu bylo porovnáno zastoupení stadií operovaných tumorů v jednotlivých sledovaných letech. Výsledky byly statisticky zpracovány a vyhodnoceny. Výsledky: V letech 2008–2012 bylo operováno na I. chirurgické klinice FN Olomouc celkem 980 pacientek ve věku od 20 do 88 let, z toho 101 pacientek mladších 45 let. Průměrný věk pacientek byl 59 let a medián 60 let. V jednotlivých letech se celkový počet pacientek zvýšil ze 153 v roce 2008 až na 240 v roce 2012. V podskupině pacientek do 45 let nedošlo ve sledovaných letech k signifikantnímu nárůstu počtu nemocných. V roce 2012 bylo diagnostikováno více pacientek ve stadiu II a méně pacientek ve stadiu III v porovnání s rokem 2010. Závěr: Analýzou našeho souboru byla potvrzena vzestupná tendence ve výskytu karcinomu prsu. Nebyl prokázán statisticky významný nárůst počtu pacientek v tzv. předscreeningovém věku, tedy mladších 45 let. Zároveň byl zjištěn častější záchyt časnějších stadií onemocnění, zejména v posledních třech sledovaných letech., Introduction: Breast cancer is the most common malignancy in women. It affects mostly women between the ages of 60 and 70; however, in the past years, the number of younger female patients has been increasing. The incidence of breast cancer has been rising worldwide, especially in the United States and Western Europe. Breast carcinoma mortality, on the other hand, has shown a slight decrease due to early screening programmes and advanced treatment methods. Methods: We included patients who had undergone surgery for breast carcinoma in the 1st Department of Surgery at Teaching Hospital in Olomouc between 1 January 2008 and 31 December 2012. In each patient, her age at the time of diagnosis/surgery was calculated. All patients were divided into 10-year age groups. At the same time, a sub-group of breast cancer patients younger than 45 years was created. We compared the numbers of patients in the respective groups and sub-groups in every year. The mean age and the median of age were also calculated. To evaluate the benefit of mammary screening, we compared the staging of operated tumours in the different years studied. The results were statistically processed and evaluated. Results: The total number of 980 patients underwent surgery for breast carcinoma at our department between 2008 and 2012 with age ranging from 20 to 88 years. 101 of them were younger than 45 years. The mean age of the patients was 59 years, the median was 60 years. The total number of patients increased from 153 in 2008 to 240 in 2012. There was no significant increase in the number of patients younger than 45 years. There were more patients diagnosed with stage II carcinoma and fewer patients with stage III carcinoma in 2012 than in 2010. Conclusion: The analysis of our group of patients confirmed the increasing tendencies of breast cancer incidence in total. We did not prove a statistically significant increase in the number of patients in pre-screening age (i.e., younger than 45 years). Cíl: Cílem šetření bylo zjistit, jak studenti ošetřovatelství hodnotí svoji klinickou praxi, spolupráci s mentorem, jaké aspekty ovlivňují jejich spokojenost s praxí a jak se hodnocení praxe liší na jednotlivých odděleních. Metodika: Výzkumný soubor zahrnoval 59 studentů druhého a třetího ročníku oboru všeobecná sestra. Dotazníkové šetření bylo realizováno standardizovaným nástrojem pro hodnocení klinické výukové prostředí a supervize CLES (Saarikoski, 2002) po absolvované klinické praxi studentů (2008). Výsledky: Byly potvrzeny rozdíly v hodnocení supervize a klinického výukového prostředí vzhledem k typu oddělení, kdy interní oddělení byla studenty hodnocena kladněji. Studenti hodnotili nejvyššími známkami dimenzi ošetřovatelská péče na oddělení, nejhůře pak vztah mezi mentorem a studentem. Současně byly potvrzeny pozitivní korelace mezi celkovou spokojeností studentů a jejich hodnocením. Statisticky významné vztahy byly identifikovány mezi zátěží sester na oddělení a hodnocením studentů – s vyšší fyzickou zátěží sester byla supervize a klinické výukové pracoviště hodnoceno kladněji. Závěr: Výsledky šetření ukazují na význam supervize a kvalitního klinického výukového prostředí pro ošetřovatelské praxe. Praktická výuka tvoří významnou část vzdělávacího programu sester, a proto je nezbytné sledovat zkušenosti a reflexe studentů., Objective: The aim of this study was to find out how nursing students evaluate their clinical practice, co-operation with the mentor, what aspects affects their satisfaction with the practice and how the evaluation of practice varies from wards. Methods: The sample comprised of 59 nursing students of the second and third year of study. The survey was realized by standardized Clinical Learning Environment and Supervision instrument CLES (Saarikoski, 2002) after the completion of students clinical practice (2008). Results: Differences were confirmed in the evaluation of the supervision and clinical learning environment depending on the type of wards, when the internal wards were evaluated by students more favourably. Students rated the dimension of nursing care at the word the highest marks, the lowest was rated the relationship between mentors and students. At the same time were confirmed positive correlations between students’ overall satisfaction and their evaluation. Statistically significant relationship were identified between the nurses´ workload in the wards and students´ assessment – with the higher workload of nurses was the supervision and clinical learning environment evaluated more favourably. Conclusion: The results of survey show the importance of clinical supervision and quality of clinical learning environment for nursing practice. Kolorektální karcinom z prstenčitých buněk je vzácné agresivní nádorové onemocnění. Incidence u mladých pacientů je velmi nízká a zaujímá do 1 % všech chirurgicky léčených kolorektálních karcinomů. Raritní výskyt u mladých pacientů koresponduje s diagnostikováním tohoto onemocnění v pokročilých stadiích a s nepříznivou prognózou. Pro verifikaci karcinomu z prstenčitých buněk je zásadní bioptické vyšetření a léčba je obdobná jako u ostatních kolorektálních karcinomů. Autoři popisují případ mladé ženy s diagnostikovaným kolorektálním karcinomem z prstenčitých buněk. Cílem sdělení je upozornit na raritní výskyt vzácného kolorektálního karcinomu u mladé nemocné., Signet ring cell colorectal carcinoma is a rare aggressive tumor. The incidence of this desease is very low in young patients and accounts up to 1% of all surgical patients with colorectal cancer. The infrequency of the disease among young patients makes the diagnosis more difficult and the prognosis less favourable. Biopsy results are crucial for verification of signet ring cell carcinoma. Treatment is similar as in other types of colorectal cancer. We report a case of signet ring cell colon cancer in a 22 years old female patient. Cíl: Od roku 2002 lze v ČR využít evokované potenciály (EP) k potvrzení smrti mozku (SM). V článku informujeme o užitečnosti kombinace BAEP (sluchových kmenových evokovaných potenciálů) a SEP (somatosenzorických evokovaných potenciálů) jakož i opakovaného vyšetření, jež zvyšují senzitivitu průkazu SM. Metodika: Na Neurochirurgickém oddělení FN Plzeň bylo v letech 2002–2012 u 41 pacientů provedeno potvrzení SM pomocí EP. Všichni pacienti splňovali klinická kritéria SM a podstoupili potvrzení SM pomocí BAEP a SEP n. medianus. Výsledky: Senzitivita prvního vyšetření k průkazu SM byla u BAEP 37 %. Opakováním vyšetření BAEP pak došlo k potvrzení SM se senzitivitou 100 %. Kombinace vyšetření BAEP a SEP zvýšila senzitivitu průkazu SM na 75 %. Senzitivita SEP pro průkaz SM byla při prvním vyšetření 70 %, senzitivita druhého vyšetření dosáhla 100 %. Závěr: Kombinace BAEP a SEP a opakování vyšetření zvyšuje senzitivitu průkazu SM u pacientů s klinicky prokázanou SM., Aim: Since 2002, evoked potentials (EP) can be used to confirm brain death (BD) in Czech Republic. The aim of this study was to show an improvement in reliability of the diagnosis of brain death (BD) using serial recordings and a combination of auditory brain stem potentials (BAEPs) and median nerve somatosensory evoked potentials (SEPs). Methods: From 2002 to 2012, 41 patients had BAEP and SEP performed to confirm BD at the Department of Neurosurgery, University Hospital Pilsen. Results: There was a sensitivity of 37% at the first BEAPs registration to confirm BD. Serial BAEP recordings increased BD confirmation to 100%. BAEPs and SEPs combination increased sensitivity of BD confirmation to 75%. Sensitivity was 70% for the first recording SEP and 100% for serial recordings to confirm BD. Conclusion: Combined use of BAEPs and SEPs and serial recordings can improve BD confirmation sensitivity in all clinically BD patients. Úvod: Pectus excavatum je najčastejšou vrodenou deformitou hrudnej steny. Postihuje približne jedného zo 400 až 1000 novorodencov. Operačná korekcia sa stala metódou voľby u pacientov, ktorí spĺňajú indikačné kritéria. Najčastejšie využívanými technikami sú modifikovaná Ravitchova korekcia (HMRR) a minimálne invazívna korekcia pectus excavatum (MIRPE), pričom druhá menovaná je považovaná za metódou voľby u detských pacientov. V dospelej populácii je otázka najvhodnejšej techniky predmetom diskusie v odborných kruhoch. Metódy: Autori práce retrospektívne vyhodnotili súbor 60 dospelých pacientov, ktorí absolvovali MIRPE v období od októbra 2010 do marca 2015. Preferovanou technikou na pracovisku autorov bola „short-bar“ technika s pravostrannou torakoskopickou asistenciou. Získané údaje vyhodnotili s pomocou štatistického softvéru SPSS? verzia 21.0.0 (IBM, USA). Platnosť hypotéz a potvrdenie štatisticky významných vzťahov overili štatistickými testami. Klinické a demografické premenné sme vyhodnotili Studentovým t-testom pre parametrické dáta a Mann- Whitneyovým U-testom pre dáta neparametrické. Závislosť kvalitatívnych znakov boli otestované po zadaní do kontingenčných tabuliek pomocou Pearsonovho χ2 testu dobrej zhody. Za hladinu štatistickej významnosti sme zvolili p<0,05. Výsledky: V sledovanom období podstúpilo operačnú korekciu 45 mužov a 15 žien. Medián veku v súbore bol 24 rokov, bez štatisticky signifikantného rozdielu medzi mužmi (25 rokov) a ženami (23 rokov, p=0,368). Medián Hallerovho indexu v súbore bol 3,93, pričom mierne vyšší bol u ženských pacientov (p= 0,285). Asymetrická deformita bola prítomná u 20 pacientov (33 %), bez signifikantného rozdielu medzi pohlaviami (p=0,206). Medián dĺžky operačného výkonu v celom súbore pacientov bol 73 minút, pričom trvanie korekcie u mužov bolo mierne dlhšie (80 vs. 70 min, p=0,312). Dve dlahy sme implantovali 27 pacientom. Nezaznamenali sme pritom štatisticky signifikantný rozdiel medzi pohlaviami (p=0,454). Komplikácie boli prítomné u 15 pacientov (25 %), pričom sme nepotvrdili štatisticky signifikantný rozdiel podľa typu pohlavia (p=0,863), počtu implantovaných dláh (p=0,178) a tvaru deformity (p= 0,527). Najčastejšou komplikáciou boli recidivujúce ranové serómy u 6 pacientov (10 %). Medián pooperačnej hospitalizácie bol 5 dní, pričom sme nezaznamenali rozdiel medzi pohlaviami (p=0,653) a pacientami s rôznym počtom implantovaných dláh (p=0,600). Rehospitalizácia s potrebou ďaľšej chirurgickej intervencie bola indikovaná u 5 pacientov (8,3 %). Príčinou bola alergia na chróm (1×), ranová infekcia (1x) a recidivujúci seróm (3x) s potrebou V.A.C.? terapie (KCI, USA). Záver: MIRPE je možné ponúknuť ako bezpečnú a efektívnu techniku pri korekcii pectus excavatum v dospelej populácii aj na pracovisku bez predchádzajúcich skúseností s týmto výkonom u detských pacientov., Introduction: Pectus excavatum is the most common congenital chest wall deformity. Aproximatelly 1 out of 400 to 1000 newborns are affected by this diagnosis. Surgical correction is indicated in patients that fulfil the indication criteria. The highly modified Ravitch correction (HMRR) and minimally invasive pectus excavatum repair (MIRPE) are by far the most popular methods of correction. MIRPE has been established as the method of choice amongst children. Feasibility of the minimally invasive approach in the funnel chest correction in adult population still remains controversial. Methods: The authors present a retrospective analysis of adult patients treated by MIRPE in their institution. Sixty adult patients with pectus excavatum underwent MIRPE from October 2010 to March 2015. Various parameters were evaluated retrospectively. The effectiveness and safety of both techniques were evaluated with the use of statistical software SPSS? version 21.0 (IBM, USA). Data were evaluated using Student´s t-test or Mann-Whitney U-test for normally and non-normally distributed data, respectively. The comparison of categorical variables between the groups was evaluated by Pearson´s chi-square test. A p value of less than 0.05 was considered statistically significant. Results: 45 men and 15 women underwent MIRPE during the period. Median age in the group was 23 years without any statistically significant difference between males and females (p=0.386). Median Haller index was 3.93 and was slightly higher in women (p= 0.285). Asymmetric deformities were present in 20 patients (33.3%) without any statistically significant difference between the sexes (p=0.206). Median duration of the surgical correction was 73 minutes and was longer in males (p=0.312). Two bars were implanted in 27 patients with no significant difference between the sexes (p=0.454). Complication rate was 25% (15/60) without any significant difference based on the number of implanted bars (p=0.178), sex (p=0.863) or the presence of an asymmetric deformity (p=0.527). Wound seromas with 10% (6/60) were the most common complication. Median postoperative length of stay was 5 days without any significant difference between males and females (p=0.653) and patients with different number of bars implanted (p=0.600). The need for rehospitalisation and reoperation was indicated in 5 patients (8.3%). Allergy to chromium led to bar extraction in 1 patient and 4 patients were treated by V.A.C.? therapy (KCI, USA) because of wound abscess (1) and seroma (3). Conclusion: Minimally invasive pectus excavatum repair is an effective surgical technique for the treatment of pectus excavatum in adult patients.
