- Cíl: Průzkumné šetření zjišťovalo schopnosti spolupráce nemocných při léčbě Warfarinem a dodrţování léčebného reţimu v průběhu dlouhodobé antikoagulační terapie Warfarinem. Konkrétně byla mapována a vyhodnocena úroveň edukace sestrou, informovanost a adherence k léčbě u pacientů léčených Warfarinem. Metodika: Průzkumný soubor tvořilo 108 pacientů evidovaných v ambulantním léčení interní a kardiologické ambulance při interním oddělení Nemocnice ve Frýdku-Místku. Průzkumné šetření probíhalo v prosinci roku 2010 a lednu roku 2011. Pro sběr dat byla vyuţita metoda dotazníkového šetření. Výsledky: Výsledky průzkumného šetření ukazují, ţe naprostá většina pacientů léčených Warfarinem se aktivně podílí na léčebném procesu. Nemocným se od zdravotníků dostává poučení o léčbě, sestry se na edukaci podílejí v 75 %. Většina dotázaných dochází na pravidelné kontroly a zná zpaměti uţívanou dávku léku. Čtvrtina nemocných však není schopna uvést krvácení jako hlavní riziko léčby. Převáţná většina respondentů si je vědoma skutečnosti, ţe účinek léčby můţe být ovlivněn sloţením přijímané potravy, méně neţ polovina respondentů zná interakci Warfarinu s jinými léky či doplňky stravy. Nemocní mají na zřeteli, ţe je nutno informovat kaţdého lékaře o této léčbě, jen polovina respondentů však informuje i lékárníka. Závěr: Adherence k léčbě ze strany pacienta je základem úspěšné a bezpečné terapie Warfarinem. Cílem všech zdravotníků by měla být snaha o kvalitní a efektivní komunikaci s nemocnými, snaha o dosaţení co nejvyšší úrovně spolupráce, ochoty a schopnosti pacientů řídit se pokyny zdravotníků. Tím se výrazně zvýší úspěšnost léčby a eliminuje se riziko moţných komplikací léčby., Aim: Exploratory investigation ascertained capabilities of cooperation of patients when treated with warfarin and adherence to treatment regimen during long term anticoagulant therapy by warfarin. Specifically was mapped and assessed the level of education by nurse, foreknowledge and adherence to treatment in patients treated with warfarin. Methods: The exploratory set consisted of 108 patients, who were registered in ambulant treatment of internal and cardiac ambulance at internal department of Hospital in Frydek-Mistek. Exploratory investigation took place in December 2010 and January 2011. For data collection was used method of questionnaire survey. Results: The results of questionnaire survey show, that absolute majority of patients treated with warfarin is actively participating in the treatment process. The patients receive from the health care providers education about the treatment, nurses are participating in education in 75%. Majority of respondents have regular check ups and knows by heart the dose of the drug. A quarter of patients is not able to state bleeding as the main risk of treatment. The vast majority of respondents are aware of the fact, that the treatment effect may be influenced by the composition of the diet, less than half of the respondents know the warfarin interactions with other medication or supplements. Patients have in mind that every doctor should be informed about this treatment, only half of the respondents, however, informs as well the pharmacist. Conclusion: Adherence to treatment by the patient is the foundation of a successful and safe warfarin therapy. The goal of all health professionals should be to aim for quality and effective communication with patients, effort to achieve the highest level of cooperation, willingness and ability of patients to follow medical instructions. Cílem tohoto článku je vytvořit představu o podílu nákladů na léčbu karcinomu prsu, který lze přiřadit konzumaci alkoholu v ČR v roce 2007. - V těhotenství je anafylaktická reakce naštěstí sice relativně vzácná, její důsledky ale mohou vést až k závažnému postižení matky i plodu. Ve většině publikovaných případů těžké anafylaxe v těhotenství bylo těžké postižení dokonce spíše na straně plodu než matky. Vznik anafylaxe se většinou nedá předvídat, první symptomy jsou někdy netypické, pouze lokální, ale rychle mohou přecházet v generalizované postižení celého organismu. Ať už jsou zprostředkovány protilátkami typu IgE (pravá anafylaxe), nebo vznikají na jiném podkladě (anafylaktoidní reakce), klinický obraz bývá obdobný. Je třeba myslet na to, že i klinicky nenápadné počáteční symptomy mohou mít průběh závažný a život ohrožující. Každé opomenutí a prodleva v diagnostice a léčbě anafylaxe může vést k fatálnímu konci. O úspěšném výsledku léčby rozhoduje velkou měrou medicínská zkušenost a včasnost zásahu. Protože je toto téma v současné literatuře zmiňováno spíše jen výjimečně, je cílem tohoto článku shrnout problematiku anafylaxe v těhotenství z hlediska patofyziologie, klinického průběhu, diagnostiky i léčby., Anaphylaxis in pregnancy is a relatively rare reaction but it can lead to serious maternal and fetal impairment. In published cases of severe anaphylaxis in pregnancy there were even more severe disabilities in the fetus than the mother. Development of anaphylaxis is usually unpredictable, the first symptoms are sometimes atypical and only local, but can move quickly in a generalized impairment of the whole organism. Whatever they are mediated by IgE antibodies (right anaphylaxis), or are caused on another basis (anaphylactoid reactions), the clinical picture is similar. It should be kept in mind that even clinically inconspicuous initial symptoms may progress into serious and life-threatening state. Any omission and delay in diagnosis and treatment of anaphylaxis can lead to fatal end. A successful outcome is largely dependent on experience and timeliness of medical intervention. CT angiografie provedená na multidetektorovém CT přístroji má vysokou úspěšnost v detekci odstupu bronchiálních tepen a slouží k navádění při jejich katetrizaci a embolizaci., The authors describe a case of a 78-year-old woman with massive hemoptysis, which was from to hypertrophic bronchial artery anomalously originating from the internal thoracic artery. CT angiography performed on multidetector CT shows anomalous origin of the right bronchial artery. The bronchial bleeding was treated with embolization. The patient had no recurrent bleeding during 24 month follow-up. Ectopic origin of bronchial arteries is not rare, therefore it is necessary to consider it especially in cases when no bronchial artery arises between the upper margin of the fifth thoracic vertebra and the lower margin of the sixth thoracic vertebra. - The aim of our study was to determine the effect of selected cytostatics on a human ovarian cancer cell line A2780 as a model system for ovarian cancer treatment. This cell line is considered cisplatin-sensitive. Panel of tested cytostatics included cisplatin, paclitaxel, carboplatin, gemcitabine, topotecan and etoposide. These cytostatics have a different mechanism of action. - Spánek se významně podílí na celkovém zdraví a kvalitě života člověka, je důležitý nejen pro duševní a fyzickou regeneraci, ale také pro normální funkci metabolických pochodů, endokrinního a imunitního systému. V moderní společnosti dochází k postupnému zkracování délky spánku v důsledku celé řady faktorů. Epidemiologické studie v posledním desetiletí potvrdily, že spánek kratší než 7 hodin představuje rizikový faktor pro rozvoj obezity, diabetu mellitu 2. typu, hypertenze, kardiovaskulárních onemocnění, depresí a zvyšuje riziko mortality. Cílem této práce bylo zhodnotit délku spánku a posoudit vztah mezi délkou spánku a BMI u dospělé populace (n=478, 18–65 let). Metoda sběru dat zahrnovala 7denní záznam o délce spánku za den a antropometrická měření. Z výsledků vyplynulo, že průměrná délka spánku v souboru činila 7,45 (±0,91) hodin za den. 28 % dospělých osob přesto spalo méně než doporučených 7 hodin denně. Statisticky nejméně spali jedinci ve věku 40–49 let (průměr 7,2±0,80 hodin). Riziko obezity definované jako BMI ≥30 kg/m2 v souboru stoupalo se zkracující se délkou spánku: pro kategorie spánku [<6,3 h], [≥6,3 až <8,3 h] a [≥8,3 h] bylo riziko obezity na základě relativních četností vyjádřeno poměrem 2 : 1,5 : 1,2. Jedinci spící průměrně <6,3 hodiny vykazovali 1,3krát vyšší riziko obezity než ostatní jedinci spící déle i při zohlednění faktoru věku, tento závěr však není statisticky významný (OR=1,256; 95% IS: 0,631; 2,422). Z hlediska prevence obezity se dostatečný a kvalitní spánek jeví jako významný ochranný faktor, který by zejména v současné době rychlého životního tempa a při stoupajícím počtu obézních jedinců neměl být, vedle zdravé stravy a dostatečné pohybové aktivity, opomíjen., Sleep plays a significant role in overall health and in the quality of human life. It is not only important for mental and physical regeneration, but also for the normal functioning of metabolic processes, for the endocrine and immune systems. In modern society there is a gradual shortening of the length of sleep due to a number of factors. In the last decade a number of epidemiological studies suggest that sleeping less than 7 hours a day increases the risk of developing obesity, type 2 diabetes mellitus, hypertension, cardiovascular diseases, and depression and is associated with premature mortality. The aim of this study was to evaluate sleep duration and to assess the relationship between sleep duration and BMI in the adult population (n=478, 18 to 65 years). The method of data collection consisted of a 7-day record of the length of the sleep per day and anthropometric measurements. The results showed that the average sleep duration was 7.45 (±0.91) hours per day. However, 28% of adults slept less than a recommended 7 hours a day. The individuals aged 40–49 years slept statistically the shortest time (mean 7.2±0.80 hours). The risk of obesity increased indirectly with duration of sleep in this population, for sleep categories [<6.3 h], [≥6.3 to <8.3 h] and [≥8.3 h] the risk of obesity defined as BMI ≥30 kg/m2 was 2 : 1.5 : 1.2 based on the relative frequencies of obese and non-obese participants. Individuals sleeping on average <6.3 hours were at 1.3 times higher risk of obesity than other individuals sleeping longer after adjusting for age, this conclusion was not statistically significant (OR=1.256, 95% CI: 0.631; 2.422).
