Předmětem této práce je objektivně posoudit dostupná výzkumná data týkající se bezpečnosti biologické terapie v těhotenství a kojení pacientek s idiopatickými střevními záněty. Biologická léčba se zdá být v graviditě bezpečná bez zvýšeného rizika vrozených malformací. Dostupné klinické údaje naznačují, že účinnost infliximabu a adalimumabu v dosažení klinické odpovědi a udržení remise těhotných pacientek převažuje teoretické riziko ohrožení plodu těmito léky. Terapie anti-TNFα preparáty by měla být, pokud je to možné, ukončena na konci 2. trimestru pro transplacentární přestup a z toho vyplývající riziko pro plod. Užití infliximabu a adalimumabu je pravděpodobně slučitelné s kojením. Biological therapies appear to be safe in pregnancy, as no increased risk of malformations has been demonstrated. Available clinical results suggest that the efficacy of infliximab and adalimumab in achieving clinical response and maintaining remission in pregnant patients might outweigh the theoretical risks of drug exposure to the fetus. If possible, anti-TNF therapy should be stopped by the end of the second trimester due to transplacental transfer and potential risk for the fetus. The use of infliximab and adalimumab is probably compatible with breastfeeding. Denosumab is a human monoclonal antibody representing a novel therapy of osteoporosis. Contrary to always other antiosteoporotic drugs, it is not contraindicated in advanced chronic kidney disease, as its pharmacokinetic does not differ from patients with normal kidney function. However, published case reports in chronic kidney disease (CKD) patients stopped the therapy after single dose because of hypocalcemia. Eribulin je netaxanový inhibitor dynamiky mikrotubulů. Je syntetickým analogem halichondrinu B. Působením na mikrotubuly eribulin účinkuje antimitoticky v G2/M fázi buněčného cyklu, ničí mitotické vřeténko a způsobuje apoptózu. Specificky inhibuje růstovou fázi mikrotubulů a sekvestruje tubulin v neproduktivní agregáty. V poslední době byly publikovány výsledky preklinického výzkumu „nemitotického“ mechanizmu účinku eribulinu (zejm. vliv na vaskularizaci nádoru, epiteliálně mezenchymální přechod, invazivitu a schopnost migrace)., Eribulin is a non-taxane microtubule dynamics inhibitor. It is a synthetic analogue of halichondrin B. By acting on microtubules, eribulin has an antimitotic effect in the G2/M phase of cell cycle, disrupting the mitotic spindle and inducing apoptosis. Ovarian cancer is the fifth most common malignancy in the world's female population and with the highest lethality index among gynecological tumors. The prognosis of metastatic disease is usually poor, especially in platinum-resistant cases. There are several options for the treatment of metastatic disease resistant to platinum derivates (e.g. paclitaxel, topotecan and pegylated liposomal doxorubicin), all of which are considered equipotent. Pegylated liposomal doxorubicin (PLD) is a liposomal form of the anthracycline antibiotic doxorubicin. It is characterized by more convenient pharmacokinetics and a different toxicity profile. Cardiotoxicity, the major adverse effect of conventional doxorubicin, is reduced in PLD as well as hematotoxicity, alopecia, nausea and vomiting. Skin toxicity and mucositis, however, emerge as serious issues since they represent dose and schedule-limiting toxicities. The pharmacokinetics of PLD (prolonged biological half-life and preferential distribution into tumor tissue) provide new possibilities to address these toxicity issues. The extracorporeal elimination of circulating liposomes after PLD saturation in the tumor tissue represents a novel and potent strategy to diminish drug toxicity. V současné době je věnována familiární hypercholesterolemii (FH) po letech opět zvýšená pozornost. Je tomu tak především proto, že se toto geneticky podmíněné onemocnění stává cílem velmi účinných nových hypolipidemik, PCSK-9 inhibitorů, lomitapidu a mipomersenu. Předložené sdělení se zabývá popisem 2 souborů nemocných (vždy ve své době z největších na světě), před 50 lety a nyní. I když přímé statistické srovnání není možné, lze přeci jen najít v klinickém obrazu FH jisté vývojové změny. Faktem však je, že základní charakteristika FH se zásadně nezměnila. V biochemickém obrazu dominuje těžká izolovaná hypercholesterolemie s hodnotami celkového cholesterolu 9–10 mmol/l, s hodnotami LDL-cholesterolu 7–8 mmol/l a s normálními hodnotami triglyceridů. Je zajímavé, že právě triglyceridy se při srovnání s dobou před 50 lety významně zvyšují a dosahují hraničních hodnot. Hladiny HDL-cholesterolu jsou normální. Manifestace ICHS ve věku od 40 let u mužů a od 50 let u žen není výjimkou (ojediněle se vyskytují i případy infarktu myokardu již ve 3. dekádě života). Klasickou klinickou manifestací FH je xantomatóza. Pravděpodobně v důsledku časné detekce nemocných s FH a účinné agresivní léčby se se šlachovými xantomy, s xanthelasma palpebrarum a arcus lipoides setkáváme méně často než před desítkami let. Obezita, diabetes mellitus (DM) ani hypertenze nepatří mezi typické klinické příznaky FH., Currently, the familial hypercholesterolemia (FH) rises the interest. The reason is that this genetic disorder is targeted by newly emerged and highly effective hypolipidemic agents, PCSK-9 inhibitors, lomitapid and mipomersen. Present paper discusses 2 patient study groups, before 50 years and nowadays. Although direct statistical analysis is impossible some changes in clinical features of FH might be found over the course of the time. In fact, the basic FH characteristic has not changed dramatically. Severe isolated hypercholesterolemia with total cholesterol 9–10 mmol/l, LDL-cholesterol 7–8 mmol/l and normal values of triglycerides dominates in laboratory analysis. Interestingly, the values of triglycerides increase and almost reach the pathological range in comparison to the values from the period 50 years ago. The values of HDL-cholesterol are normal. Manifestation of CHD in male patients over 40 years of age and in female patients over 50 years of age is not exceptional (rarely occur cases of myocardial infarction in third decade of age). Typical clinical manifestation of FH is xanthomatosis. The early detection and aggressive treatment in FH patients cause that xanthoma tendinosum, xanthelesma and arcus lipoides are less frequent as decades ago. Funkční hypotalamická amenorea (FHA) je vedle gravidity a syndromu polycystických ovarií nejčastější formou sekundární amenorey. FHA je důsledkem poruch v pulzní sekreci gonadoliberinu (GnRH), které pak vedou k poruše sekrece gonadotropinů FSH a LH. FHA je obranou organizmu v situacích ohrožujících životní funkce, které jsou chráněny pak přednostně před funkcí rozmnožovací. FHA je reverzibilní, po ukončení extrémní zátěže se zvolna sama může upravit. Uvádějí se 3 hlavní typy funkční amenorey: FHA se vztahem k úbytku hmotnosti, FHA v důsledku stresu a se vztahem k nadměrnému fyzickému výkonu. Konečným výsledkem jsou komplexní hormonální změny charakterizované hlubokou hypoestrogenemií. Vedle nízkých estrogenů tyto pacientky mají mírnou hyperkortizolemii, nízké hladiny inzulinu v séru, nízký IGF1 a nízký celkový trijodtyronin. Zdraví těchto žen je ohroženo ve více směrech, včetně stavu kostí, kardiovaskulárního systému a stavu mentálních funkcí. Pacientky mají sníženou kostní denzitu a jsou pak ohroženy zvýšeným rizikem fraktur. Osteopenie a osteoporóza jsou hlavní dlouhodobou komplikací FHA. Kardiovaskulární komplikace spočívají v dysfunkci endotelu a změnách v lipidogramu. Pacientky s funkční hypotalamickou amenoreou trpí častěji depresemi a úzkostmi a také problémy v sexuální oblasti než zdravé ženy., Functional hypothalamic amenorrhea (FHA) besides pregnancy and syndrome of polycystic ovary is one of the most common causes of secondary amenorrhea. FHA results from the aberrations in pulsatile gonadotropin-releasing hormone (GnRH) secretion, which in turn causes impairment of the gonadotropins (follicle-stimulating hormone and luteinizing hormone). FHA is a form of the defence of organism in situations where life functions are more important than reproductive function. FHA is reversible; it can be normalized after ceasing the stress situation. There are three types of FHA: weight loss related, stress-related, and exercise-related amenorrhea. The final consequences are complex hormonal changes manifested by profound hypoestrogenism. Additionally, these patients present mild hypercortisolemia, low serum insulin levels, low insulin-like growth factor 1 (IGF-1) and low total triiodothyronine. Women’s health in this disorder is disturbed in several aspects including the skeletal system, cardiovascular system, and mental problems. Patients manifest a decrease in bone mass density, which is related to an increase in fracture risk. Therefore, osteopenia and osteoporosis are the main long-term complications of FHA. Cardiovascular complications include endothelial dysfunction and abnormal changes in the lipid profile. Juxtapapilární divertikl duodena je literárně uváděn jako možná příčina řady pankreatobiliárních onemocnění. Etiologická souvislost však často není považována za jednoznačnou a v reálné klinické praxi bývá možná role divertiklu podceňována, až popírána. Tento článek na základě kazuistiky a přehledu literatury na tuto problematiku poukazuje. Za klinicky významnou známku možného podílu divertiklu na potížích lze považovat vyplnění lumen divertiklu bezoárem. Tento nález by, po vyloučení jiných příčin, měl vést ke zvažování etiologické souvislosti divertiklu s pankreatobiliárními potížemi. Řešení představují endoskopická papilosfinkterotomie jako metoda první volby a chirurgický výkon (divertikulektomie, biliodigestivní spojka či kombinace) v případě recidiv potíží a selhání méně invazivního postupu., Juxtapapillary duodenal diverticula are reported as a potential cause of many pancreatobiliary diseases. Pro klasifikaci prekanceróz vulvy se používá termín vulvární intraepiteliální neoplázie (VIN). Histologicky lze rozlišit dva základní typy VIN: VIN obvyklého typu (u-VIN) a VIN diferencovaného typu (d-VIN). Rozsah a intenzita dysplastických změn se v případě u-VIN vyjadřuje třístupňovým gradingem (u-VIN I, II a III). Prekancerózní potenciál d-VIN odpovídá vždy karcinomu in situ, proto se u této diagnostické jednotky grading neprovádí. Etiologicky je u-VIN spojena s infekcí vysoce rizikovými typy lidského papillomaviru (HPV), zatímco d-VIN je HPV negativní a rozvíjí se v terénu chronických vulvárních dermatóz typu lichen sclerosus a lichen simplex chronicus. u-VIN má tendenci k multifokalitě a k dlouhodobému recidivujícímu průběhu před transformací v invazivní dlaždicobuněčný karcinom. d-VIN se naopak vyskytuje spíše solitárně a má sklon k rychlé stromální invazi. u-VIN typicky postihuje ženy premenopauzálního věku s vyšší incidencí ostatních prekancerózních lézí dolního ženského pohlavního systému včetně perinea a anu. Pacientky s d-VIN bývají postmenopauzální ženy a není u nich pozorována asociace s výše zmíněnými dyspláziemi ostatních anatomických lokalizací. Nízká diagnostická reproducibilita kategorie u-VIN I, pochyby o prekancerózním potenciálu u-VIN I a problematické rozlišení u-VIN II a u-VIN III vedly k revizi současného diagnostického schématu. Podstatou změn je zrušení gradingu u-VIN se sloučením kategorií u-VIN II a u-VIN III a dále zrušení u-VIN I jako diagnostické jednotky. V revidované klasifikaci prekanceróz vulvy proto termín u-VIN reprezentuje high grade dysplastické léze asociované s HPV (dříve u-VIN II a u-VIN III) a d-VIN i nadále zůstává označením pro HPV negativní high grade prekancerózu., Classification of vulvar precancerous lesions is based on the concept of vulvar intraepithelial neoplasia (VIN). Two basic types of VIN are recognized histologically: VIN of the usual type (u-VIN) and VIN of the differentiated type (d-VIN). The three grade evaluation system is used to assess the intesity of dysplastic changes in u-VIN (u-VIN I, II and III). The precancerous potential of d-VIN corresponds to carcinoma in situ and therefore grading is not applied. u-VIN is etiologically linked to HPV infection, whereas d-VIN is HPV negative and emerges in the environment of chronic vulvar dermatoses (lichen sclerosus and lichen simplex chronicus). u-VIN has a tendency to multifocality and prolonged recurrent progression to the invasive squamous cell carcinoma. On the other hand, d-VIN represents rather a solitary lesion with a propensity to the rapid stromal invasion. u-VIN usually affects premenopausal women with a higher incidence of other precancerous lesions of the lower female genital tract including the perineum and anal area. Patients diagnosed with d-VIN are of postmenopausal age without any association with aforementioned dysplastic lesions of other anatomic locations. The low diagnostic reproducibility of the u-VIN I category, doubts about the precancerous potential of u-VIN I and the problematic distinction between u-VIN II and u-VIN III resulted in the modification of the current terminology. The grading of u-VIN was abandoned, u-VIN II and u-VIN III categories were merged and u-VIN I was removed from the classification scheme. The revised u-VIN category therefore represents high grade dysplastic lesions associated with HPV infection (former u-VIN II and u-VIN III categories) and the term d-VIN is still reserved for the HPV negative high grade vulvar precancerosis., and P. LDL-aferéza je extrakorporální eliminační metodika, která usiluje o pokud možno specifické odstranění LDL-cholesterolu z cirkulující krve. V současnosti je používáno 6 účinných metod k selektivnímu odstranění LDL-cholesterolu. Mezi hlavní indikace k léčbě LDL-aferézou patří diagnózy homozygotní familiární hypercholesterolemie, heterozygotní familiární hypercholesterolemie refrakterní nebo při intoleranci k standardní léčbě, a dále pacienti se zvýšením lipoproteinu(a) refrakterní nebo intolerantní k farmakoterapii. Dosud však není jednoty o tom, jaká hladina LDL-cholesterolu je rozhodující pro zahájení LDL-aferézy, názory se liší i v jednotlivých státech.
