Úvod: Synchronní nádory pankreatu a jiné primární nádory nejsou časté. V autoptických materiálech se tato koincidence uvádí kolem 5,6 %. Současné resekce pankreatu a jiného orgánu pro synchronní malignity jsou publikovány pouze ve zcela ojedinělých souborech případů či jednotlivých kazuistikách. Autoři předkládají tři případy řešené chirurgicky v jedné době. Kazuistiky: V sestavě 400 resekcí pankreatu (2006−2014) byli tři nemocní s karcinomem pankreatu a jinou synchronní maligní duplicitou. Dva nemocní se prezentovali symptomatologií nádoru pankreatu – bolesti břicha, úbytek na váze, obstrukce žlučových cest. Druhými nádory byly asymptomatický tumor antra žaludku a tumor ledviny. Pacienti podstoupili pravostrannou pankreatoduodenektomii (PDE) a subtotální gastrektomii s lymfadenektomií, resp. levostrannou nefrektomii. Histopatologické nálezy byly vývodový adenokarcinom hlavy pankreatu pT2N1M0, G3 a adenokarcinom žaludku pT1N1M0, G2, resp. vývodový adenokarcinom hlavy pankreatu pT3N1M0, G2 a renální papilární karcinom pT1bN0M0, G3. Pooperačně u obou následovala chemoterapie gemcitabinem. První nemocný zemřel 12 měsíců od operace s generalizací do jater. Druhý nemocný zemřel 19 měsíců od operace, s nově zjištěným karcinomem prostaty. Třetí nemocný byl vyšetřován pro bolesti epigastria a při gastroskopii byl nalezen tumor antra žaludku, při CT pak asymptomatický malý tumor hlavy pankreatu. Podstoupil pravostrannou PDE a subtotální gastrektomii s lymfadenektomií. Histopatologické nálezy byly: vývodový karcinom hlavy pankreatu pT2N0M0, G1 a adenokarcinom žaludku pT2bN2M0, G3. Následovaly radioterapie a chemoterapie 5-fluorouracilem a leukovorinem. Žije téměř 5 let od operace, bez recidivy. Závěr: Přežití nemocných operovaných pro duplicitní synchronní malignitu pankreatu a jiného primárního nádoru v naší sestavě významně ovlivnily vlastnosti pankreatického tumoru. Pokud se duplicita projevila symptomy pankreatického tumoru, bylo přežití významně kratší nežli u incidentálně zjištěného tumoru pankreatu při manifestaci jiné malignity., Introduction: The occurence of synchronous pancreatic cancer and other primary cancer is not frequent and reaches about 5.6% as reported in autoptic studies. Double resections of the pancreas with another organ due to synchronous malignancies have been published only in quite sporadic sets of cases or individual case reports. The authors present three cases of synchronous pancreatic malignancies and stomach or renal cancers treated with surgery, including results and survival. Cases: Three patients with synchronous double cancer were identified in a series of 400 pancreatic resections (2006−2014). Two patients presented with symptoms of pancreatic periampullary tumors (bile duct obstruction , weight loss and abdominal pain). The second malignancies were identified as incidental during diagnostic work-up (asymptomatic cancer of the stomach, kidney). Pancreatoduodenectomies (PDE) with lymphadenectomies were performed due to ductal adenocarcinomas (pT2N1M0 G3 and pT3N1M0 G2). The second procedures included subtotal gastrectomy with lymphadenectomy (gastric adenocarcinoma pT1N1M0, G2) and nephrectomy (renal papillary carcinoma pT1bN0M0, G3). Postoperative adjuvant chemotherapy with gemcitabine was given in both patients. Survival rates were 12 and 19 months, respectively. The third patient suffered from abdominal pain and weight loss. Diagnostic work-up revealed stomach carcinoma and early pancreatic adenocarcinoma. Double resection – subtotal gastrectomy with lymphadenectomy and pancreatoduodenectomy with lymphadenectomy – was performed. Gastric adenocarcinoma pT2N2M0, G3 and pancreatic ductal papillary-mucinous adenocarcinoma pT2N0M0, G1 were found in the specimens. Adjuvant radiochemotherapy with 5-fluorouracil and leukovorine was given postoperatively. This patient is still alive nearly 5 years after the surgery, without any reccurence. Conclusion: The survival of patients with double synchronous pancreatic malignancies and other primary tumors in our set seems to be influenced by the stage and biology of pancreatic cancer. The survival was worse when the duplicity was presented with symptoms of pancreatic cancer. Synoviální sarkom je maligní mezenchymální vřetenobuněčný nádor s variabilní epiteliální diferenciací. Prezentujeme zde raritní případ mladé pacientky s primárním synoviálním sarkomem jater, jehož diagnóza byla ověřena molekulárně patologickou detekcí fuzního transkriptu SYT/SSX. Multidisciplinarita, ale i vysoké požadavky na odbornost předurčují pacienty se sarkomem k tomu, aby byli léčeni v centrech, která jim poskytnou potřebnou komplexní péči., Synovial sarcoma is malignant mesenchymal fuso-cellular tumor with variable epithelial differentiation. Kardiomyopatie představuje heterogenní skupinu onemocnění srdečního svalu, provázenou poruchou srdeční funkce. Tachykardií indukovaná kardiomyopatie (TIC) je způsobena déle trvající tachykardií vedoucí k dilataci a systolické dysfunkci s klinickými projevy srdečního selhání, které jsou reverzibilní po normalizaci tepové frekvence. Diagnóza je obvykle stanovena retrospektivně po normalizaci tepové frekvence a následné úpravě funkce levé komory (LKS). Více než 100 let po prvním dokumentovaném případu (roku 1913 popsána u mladého nemocného s fibrilací síní a projevy srdečního selhání [25]) jsou nadále znalosti patofyziologických mechanizmů omezené. Mezi nejčastější arytmie odpovědné za TIC patří fibrilace síní [1,2], flutter síní [3], incesantní supraventrikulární tachykardie [4], komorová tachykardie (KT) [5] a frekventní komorové extrasystoly (KES) [6]. Vzhledem k potenciální reverzibilitě je proto rozpoznání TIC a včasný terapeutický zásah stěžejní. Mezi stávající možnosti léčby patří farmakoterapie a chirurgická či katetrová ablace., Cardiomyopathy is a heterogeneous group of diseases of heart muscle accompanied with impaired cardiac function. Tachycardia-induced cardiomyopathy (TIC) is caused by prolonged tachycardia leading to dilatation and systolic dysfunction with clinical manifestation of heart failure. This state is reversible after normalization of heart rate. The diagnosis is usually made retrospectively after normalization of heart rate and recovery of left ventricular function (LVF). More than 100 years after the first documented case (described in 1913 in a young patient with atrial fibrillation and symptoms of heart failure [25]) is still limited knowledge of pathophysiological mechanisms. The most common arrhythmias responsible for the TIC include atrial fibrillation [1,2], atrial flutter [3], incessant supraventricular tachycardia [4], ventricular tachycardia (VT) [5] and frequent ventricular extrasystoles (VES) [6]. Těhotenství žen bez domova představuje specifický sociálně-zdravotní problém, který má řadu aspektů. Prvním z nich je minimální prevence otěhotnění plynoucí jednak z menší dostupnosti antikoncepčních prostředků z ekonomických důvodů, jednak z nízkého intelektu, případně z naděje, že těhotenství je cesta k vyřešení tíživé sociální situace za pomoci státu nebo dobročinných organizací. Vážným problémem je také značné zdravotní riziko pro těhotnou i pro plod plynoucí z nedostatečné intimní hygieny, z absence pravidelné gynekologické péče, z rizika nákazy sexuálně přenosnými chorobami, ze závislosti na návykových látkách i z malnutrice. Podobně je sociálně-zdravotně problematická i perinatální péče. Tato situace je demonstrována ilustrativní kazuistikou. The first one is minimum prevention of pregnancy as a result of a more difficult access to contraceptives for economic reasons, because of low intelligence or maybe a hope that pregnancy offers a way to solve a precarious social situation with the support of the state or charitable organizations. A serious problem is also a considerable health risk for the pregnant woman and fetus, resulting from inadequate intimate hygiene, the absence of regular gynecological care, risk of sexually transmitted diseases, addictions and malnutrition. Similarly problematic are the social and health aspects of perinatal care. The described state is illustrated by case studies. Autoři referují o vzácném případu traumatické totální avulze očního bulbu s optickým nervem v úrovni chiazmatu. Kazuistické sdělení retrospektivně popisuje průběh ročního sledování mladé ženy, která byla zraněna při hasičských závodech. Avulze bulbu a optiku, kombinovaná s poraněním chiazmatu, rezultovala v hemianopický defekt druhostranného zorného pole. Při traumatu vzniklo subarachnoidální krvácení do oblasti cisteren chiazmatu, jež spontánně ustoupilo. Nedošlo k rozvoji vaskulárních spazmů mozku. Celková kortikoterapie a neuroprotekce neměla vliv na průběh onemocnění. Během sledovací doby objektivně došlo k descendentní atrofii korespondujících axonů a retinálních gangliových buněk. V současné době neexistuje efektivní terapie poranění v oblasti chiazmatu., The authors report a rare case of complete traumatic avulsion of the eyeball with the optic nerve at the level of chiasm. The case report retrospectively describes a year-long monitoring of a young woman who was injured in fire-fighter races. Avulsion of the globe and optic nerve combined with injured chiasm resulted in contralateral hemianopic visual field defect. The trauma led to subarachnoid hemorrhage in cisterns of chiasm, which spontaneously resolved. There was no development of vascular spasms in the brain. Systemic corticosteroid therapy and neuroprotection did not affect the course of the disease. During the observation period, descending atrophy of the corresponding axons and retinal ganglion cells developed. There is currently no effective therapy of chiasm injuries., and L. Hejsek, V. BACKGROUND: Intussusception of the large bowel in adults is a very rare pathological condition. However, it has its clinical importance because intussusception is very often associated with an intraluminal lesion. CASE REPORT: We report two cases of the large bowel intussusception, ileocolic and colorectal. Both intussusceptions were associated with a malignant tumor. However, the clinical presentation was different. One of the intussusceptions was of non-transient character, while the second one resolved spontaneously before operation. Both patients underwent surgery and malignant tumors were found and removed. RESULTS: The purpose of the article is to draw attention to intussusception and emphasize that intussusception, either transient or non-transient, should be further examined. Cíl práce: V kazuistice prezentujeme raritní nález primárního plazmablastického lymfomu penisu, který klinicky imitoval karcinom glans penis. Jedná se o druhý publikovaný případ tohoto typu lymfomu penisu ve světě. Kazuistika: Pacient, narozen 1929, byl odeslán na naše pracoviště ošetřujícím urologem pro nově zjištěný exofytický tumor glans penis, indikovali jsme parciální amputaci penisu. Histologické vyšetření amputátu prokázalo primární plazmablastický lymfom. Následná restagingová vyšetření neprokázala systémové onemocnění, pacient je nyní 22 měsíců po výkonu bez známek onemocnění a zůstává v hematologické dispenzární péči. Závěr: Primární plazmablastický lymfom penisu představuje extrémně raritní diagnózu, ve světové literatuře je popsán pouze jediný případ. Jedná se o agresivní typ lymfomu, který obvykle vede v krátké době ke smrti pacienta. Ačkoli léčba těchto nádorů je převážně založena na chemoterapii, u našeho pacienta vedl radikální operační výkon k úplnému vyléčení., We report a case of an elderly man who has presented with tumorous affection involving the uncircumcised penile glans and prepuce. Primary clinical examination gave strong suspicion for primary carcinoma of penis, therefore a suprapubic cystostomy and partial amputation of penis were performed. Histopatological examination revealed uncommon diagnosis of plasmablastic lymphoma involving glans penis and prepuce. The vast majority of penile tumours are squamous cell carcinomas, primary malignant lymphoma of penis is extremely rare. As staging results were negative, no adjuvant systemic chemotherapy was neccesary. Úraz podtlakem v bazénu je skupina specifických raritních poranĕní s velkým rizikem fatálních důsledků. Častĕjší obĕtí s častĕji vážnějšími důsledky jsou zejména malé dĕti. Při tomto druhu poranĕní dochází ve vĕtší či menší míře k tonutí postiženého. Případ 19-letého pacienta zachyceného v oblasti hýždí nezajištěným otvorem sací drenáže zajímavĕ ilustruje nĕkteré zvláštnosti této problematiky. Preventivní opatření jsou v současnosti již známá, problém bývá vĕtšinou s jejich důslednou aplikací. K fatálním příčinám patří kromĕ utonutí, hypovolemický šok z náhlé redistribuce tekutin třetího prostoru a eviscerace s devastací vnitřních orgánů. Lokalizované často bizardní a velké otoky a sufúze lze v naprosté vĕtšinĕ případů řešit konzervativnĕ. Pro prevenci je klíčové rozšíření informace o existenci těchto poranĕní do obecného povědomí. Pro jejich správný management je pak důležité prohlubování povědomí a sdílení zkušeností s řešením těchto raritních poranĕní v odborné veřejnosti., Swimming pool suction injuries are unique and rare with a substantive risk of fatal consequences. Little children under the age of 8 are the most frequent victims with serious injuries. Drownings of different seriousness are also a usual part of accidents. The case of a 19 year old man trapped in the gluteal area by a unsecured suction drainage hole illustrates the uniqueness of this problem in an interesting way. Prophylactic arrangements are well known but the problem is with their strict application. Fatal causes excluding drowning include hypovolemic shock from the sudden redistribution of intersticial fluid and blood and also the evisceration of the bowel and other abdominal organs. Localised often bizarre and large swellings and sufusions can be treated nonoperatively in the vast majority of cases. For the prevention of these injuries it is important to inform the public and increase their awareness to these injuries. „Upside-down stomach syndrome“ je sporadicky se vyskytující typ velké paraezofageální hiátové hernie vyžadující při známkách uskřinutí neodkladné chirurgické řešení. Autoři článku prezentují kazuistiku 53letého nemocného s volvulem žaludku při „upside-down stomach syndromu“. Chirurgický výkon byl komplikován poraněním abdominální části jícnu, které bylo úspěšně léčeno mimo jiné použitím samoexpandibilního metalického stentu. Přes závažný klinický pooperační průběh s nutností revize dutiny břišní, zavedení jícnového stentu, provedení torakotomie pro absces mediastina a sekundárního hojení laparotomie byl pacient po padesáti dvou dnech propuštěn do domácího léčení v uspokojivém stavu, plně realimentován a rehabilitován, se zhojenou perforací jícnu i laparotomií., Upside-down stomach syndrome is a rare type of a large paraoesophageal hiatal hernia, which requires an immediate surgical treatment in case of incarceration. The authors present a case report of a 53-year-old male patient with gastric volvulus related to the upside-down stomach syndrome. Surgical treatment was complicated by an injury to distal oesophagus, which was successfully treated using a self-expandable metallic stent among other methods. Despite the complicated postoperative course with a necessity of reoperation, insertion of an oesophageal stent, thoracotomy for a mediastinal abscess and secondary healing of the laparotomy, the patient was discharged in a good condition with healed oesophageal perforation and laparotomy after 52 days., and L. Epilepsia je chronické ochorenie, ktoré je často sprevádzané psychiatrickou komorbiditou. Najčastejšou z nich je depresia, ktorá je sprevádzaná subsyndromálnymi, atypickými a chronifikovanými príznakmi depresie. Incidencia depresie je u pacientov s epilepsiou vyššia ako u iných neurologických alebo chronických ochorení. Psychopatológia spojená s epilepsiou obťažuje pacienta častokrát viac ako samotné ochorenie. Cieľom práce je prezentácia a analýza kazuistiky 37-ročného muža s diagnózou epilepsie so sprievodnou psychiatrickou symptomatikou, ktorá znížila kvalitu jeho života oveľa viac ako samotné neurologické ochorenie. V kazuistike popisujeme klinické symptómy neuropsychiatrického ochorenia – epilepsie ako aj životné udalosti, ktoré sú u každého pacienta jedinečné a majú závažný vplyv na priebeh a liečbu epilepsie., Epilepsy is a chronic disease that is often accompanied by psychiatric comorbidity. The most frequent of them is depression accompanied by subsyndromic, chronic and atypical symptoms of depression. The incidence of depression in patients with epilepsy is higher than in other neurological or chronic diseases. Psychopathology associated with epilepsy often bothers the patient more than the disease itself. The aim of this work is to present and analyze a case report of a 37‑year‑old man diagnosed with epilepsy and associated psychiatric symptomatology that reduced his quality of life much more than the neurological disease itself. In this case report, we describe clinical symptoms of neuropsychiatric disease – epilepsy as well as life events that are unique to each patient and have a significant impact on the course and treatment of epilepsy.
