63letý pacient, hypertonik, celoživotní kuřák, byl na kliniku TRN FN Plzeň přijat pro tumor pravé plíce zjištěný pro bolesti v pravém hemitoraxu. Na základě stážování byla stanovena diagnóza adenokarcinomu pravé plíce s generalizací do obou plic a levé nadledviny, st. IV. Byla zahájena chemoterapie Paclitaxel (200 mg/m2) – Carboplatina (AUC) – Avastin (15 mg/kg) v rámci studie SAIL firmy Roche s velmi dobrou tolerancí. Po 3. cyklu chemoterapie byla potvrzena parciální remise dle CT hrudníku, dále pokračováno ve stejném schématu chemoterapie s Avastinem do 6. cyklu. Kontrolní CT hrudníku prokázalo další regresi tumoru. Vzhledem k velmi dobrému efektu podávané léčby bylo dále pokračováno v monoterapii Avastinem v třítýdenních intervalech zatím do 22 cyklů. Po 22 měsících léčby s Avastinem bylo dosaženo trvající parciální remise nálezu dle PET/CT trupu, pacient je klinicky zcela bez potíží, neomezen v běžném životě, tedy s velmi dobrou kvalitou života (KS 90 bodů), bez vedlejších nežádoucích účinků Avastinu. Chemoterapie již nemá potenciál pro další prodlužování přežití, je tedy potřebné hledání nových léčebných strategií. Avastin jako inhibitor angiogeneze vede poprvé k prodloužení celkového přežití nad 12 měsíců. Významné prodloužení přežití bez progrese onemocnění (PFS) umožňuje delší přežití bez zhoršení symptomů., A 63-year-old hypertensive patient, a life-long smoker, presented to the Department of Tuberculosis and Respiratory Disease of the Plzeň Teaching Hospital with a right-lung tumour detected due to pain in the right hemithorax. Based on staging, a diagnosis of stage IV right lung adenocarcinoma with generalization into both lungs and the left adrenal gland was made. Chemotherapy with Paclitaxel (200 mg/m2) – Carboplatin (AUC) – Avastin (15 mg/kg) was initiated within the SAIL study of the Roche company with very good tolerance. Following the third chemotherapy cycle, partial remission was confirmed based on CT of the chest; the same chemotherapy regimen with Avastin was continued for six cycles. Follow-up CT of the chest revealed further tumour regression. Given the very good effect of the treatment administered, monotherapy with Avastin was continued in three-week intervals for 22 cycles so far. Following 22 months of Avastin treatment, sustained partial remission was achieved according to PET/CT of the trunk; the patient is clinically free of complaints, without limitations in everyday life, thus with a very good quality of life (KS of 90) and free of adverse effects of Avastin. Chemotherapy has no more potential for further extension of survival; therefore, new therapeutic strategies have to be looked for. As an angiogenesis inhibitor, Avastin leads to an extension of overall survival beyond 12 months. Autoři prezentují sestavu 76 nemocných radikálně operovaných pro karcinom hlavy pankreatu a analyzují pooperační komplikace a přežívání. Materiál a metodika: V období 1/2006–9/2010 podstoupilo 76 nemocných potenciálně kurativní hemipankreatoduodenektomii pro adenokarcinom hlavy pankreatu. Pacienti jsou prospektivně sledováni, diskutovány jsou komplikace a přežití. Výsledky: 30denní pooperační mortalita je 2,6 %. Na základě TNM klasifikace 11 % mělo stadium I, 78 % stadium II a 3 % stadium III. Nejzávažnější komplikací byla insuficience pankreatojejunální anastomózy (6,5 %) a opožděná evakuace žaludku (11,8 %). Ve stadiu I přežívá 1., 2., 3. a 4. rok 82 %, 52 %, 35 % a 35 % operovaných. Závěr: Resekce hlavy pankreatu je bezpečná operace s nízkou mortalitou, ale neuspokojivým dlouhodobým přežíváním. Toto je zřejmě způsobeno agresivní biologickou aktivitou nádoru a pozdní diagnostikou., The authors refer seventy six patients radically operated for pancreatic head carcinoma and analyzed postoperative complications and survival. Material and Methods: During 1/2006–9/2010 76 patients underwent potentially curative hemipancreato-duodenectomy for adenocarcinoma of the head of pancreas. The patients are prospectively followed, complications and survival are discussed. Results: The 30-days postoperative mortality was 2,6 %. According to TNM classification 11 % of patients had stage I, 78 % stage II and 3 % stage III. Most severe complications were leak from pancreato-jejunal anastomosis (6,5 %) and delayed gastric emptying (11.8 %). The survival analyses showed 1,2,3 and 4 years survival as follows: 82 %, 52 %, 35 %, 35 % respectively. Conclusion: Resection of the pancreatic head is a safe procedure with low mortality but disappointing long-term survival. The target of research was the implementation of the comparative analysis of conservative therapy of preparation of basic therapy and also new methods of treatment (anticytokine therapy). Appointment of salofalk in twos tablets (0, 5) 34 times a day per os, and also 2,0 gr in the form of a rectal enema unitary for a period of seven days, then had transferd the patients to reception 2,0 gr salofalk per os, allowed to achieve substantial improvement patients cliniclaboratory and endoscopic symptoms with a heavy intensification of ulcer colitis variation, providing full cliniclabaratory remissions achievements of disease at 8 (34 %) patients. In the Minister of the Uzbek Public Health of the Republican Science Coloproctological Center at 10 patients with the IDI have spent the therapy remicade, 7 of them had NUC and 3 patients had the Crohn's disease. Therefore, all patients no longer than on the first week after introduction of remicade had improvement of a course disease which was expressed in defering stool, extinction or reduction of pathological impurity in excrement and subsidence bellyaches. During colonoscopical inspection, in 1218 weeks after the first infusion at 80% of the patients had observed disappearance of ulcers and erosion. In terms of 6 months of supervision of relapse of disease was not noted. All new directions in treatment of the IDI, apparently, are perspective, but while are proved only theoretically and had no practical development. Colonic polyps and polyposis are studied in 183 patients. It has been found, that sensitivity of rectoscopy is 66.7%, irrigoscopy 61.1%, virtual colonoscopy computeraided tomography of colon (VCCATC) 79.1%. Endoscopic polypectomy was the main treatment method used in patents with colonic polyps and polyposis (18.6%). Jako light chain escape fenomén (LCE) je označována změna biologického chování nádoru u pacientů s mnohočetným myelomem (MM). Při diagnóze jsou v séru přítomny kompletní monoklonální imunoglobulin (mIg) a monoklonální volné lehké řetězce (mFLC), při relapsu nebo progresi onemocnění dochází k prudkému vzestupu koncentrace pouze mFLC, obvykle při stabilní nebo klesající koncentraci mIg, společně s těžkým orgánovým postižením. Pro časnou diagnostiku LCE u nemocných s MM je doporučováno sériové vyšetřování FLC. LCE byl popsán v literatuře zatím pouze u pacientů po hematologické terapii. V tomto sdělení prezentujeme kazuistiku pacienta vstupně hodnoceného hematologem jako doutnající myelom, bez nutnosti hematologické terapie. Domníváme se, že vývoj nálezu odpovídal kritériím LCE., The light chain escape phenomenon is characterized as a change in tumor biological movement in patient with multiple myeloma. Complete monoclonal immunoglobulin molecules (mIg) together with monoclonal free light chains (mFLC) are present in diagnosis, only mFLC great increase is observed in relapse or disease progression together with heavy organ impairment whether mIg concentration is usually stable or decreasing. Serum FLC monitoring is recommended for early diagnostics LCE phenomenon in patients with MM. To this time LCE phenomenon was described only in patients after hematological therapy. Výsledky na bázi dat Národního onkologického registru ČR z období 1976–2005 uvádí: a) Z 125 262 primárních novotvarů (52,1 % mužů, 47,9 % žen) bylo evidováno v časných klinických stadiích 37,1 % nádorů u mužů a 46,6 % u žen, v pokročilých stadiích 9,3 % u mužů a 9,5 % u žen. Neznámá stadia převažovala nad ostatními stadii až do roku 1994. Za celé období převažovali muži ve stadiu IV nad ženami. Nejvíce časných stadií se vyskytlo v Praze, pokročilých stadií v kraji Plzeňském. Po vyloučení neznámých stadií bylo z 54 187 duplicit u mužů 63,8 % v časných stadiích, u žen 71,3 %, v pokročilých stadiích u mužů 18,4 % a 15,8 % u žen. Z ostatních 10 292 multiplicit bylo 16,2 % časných stadií u mužů a 11,7 % u žen, v pokročilých stadiích 1,6 % u mužů a 1,2 % u žen. b) Z 165 050 následných novotvarů (54,4 % mužů, 45,6 % žen) – u případů po primárních stadiích I a II. bylo evidováno 21,7 % časných klinických stadií u mužů a 27 % u žen, 5 % pokročilých stadií u mužů a 6 % u žen – u případů po primárních stadiích III a IV bylo evidováno 2,5 % časných stadií u mužů a 3,2 % u žen, 1,8 % pokročilých stadií u mužů a 2,2 % u žen. Pozoruhodných bylo u mužů 12 418 následných nádorů v pokročilých stadiích (13,8 % ze všech 89 796 následných), u žen 10 189 (13,5 % ze všech 75 254 následných) po primárních nádorech stadia III a IV. Mezi následnými pokročilými stadii převažovalo – u mužů 36,9 % nádorů trávicích a 33,1 % dýchacích cest, 10,3 % pohlavních orgánů, 8,1 % močových cest – u žen 37 % nádorů trávicích cest, 21 % prsů, 17,2 % rodidel, 12,8 % dýchacích a 4,5 % močových cest. Je varující, že z vícečetných onemocnění byl každý sedmý nádor gastrointestina u mužů a pátý u žen po primárním novotvaru zjištěn v některém pokročilém klinickém stadiu. c) Ze 41 161 přežívajících k 17. 10. 2007 bylo 54,7 % časných stadií u mužů a 58 % u žen, 5 % pokročilých stadiích u mužů a 5,2 % u žen, neznámých stadií 40,3 % u mužů a 36,9 % u žen. Z 84 101 zemřelých bylo 30 % časných stadií u mužů a 39,8 % u žen, 11 % pokročilých stadií u mužů a 12,1 % u žen, neznámých stadií 59 % u mužů a 48,1 % u žen. Z výsledků lze doporučit, aby přehledy vícečetných diagnóz a jejich stadia se staly součástí ročních statistických přehledů novotvarů u české populace., The results based in the Czech Cancer Registry in 1976–2005: a) Of the 125,262 primary neoplasms (52.1 % males, 47.9 % females) there were registered 37.1 % of early clinical stages in males and 46.6 % in females, 9.3 % of advanced stages in males and 9.5 % in females. Unknown stages prevailed over the other stages until 1994. The cancers of stage IV prevailed in males during the total period. The most of early stages were in Prague, of advanced stages in Pilsen region. After exclusion of unknown stages, there were of 54,187 duplicities 63.8 % cases of early stages in males and 71.3 % in females; 18.4 % of advanced stages in males and 15.8 % in women. Of other 10,292 multiplicities there were 16.2 % of early stages in males and 11.7 % in females, 1.6 % of advanced stages in males and 1.2 % in females. b) Of the 165,050 subsequent neoplasms (54.4 % males, 45.6 % females) – in cases following the primary stages I and II there were registered 21.7 % of early stages in males and 27 % in females, 5 % of advanced stages in males and 6 % in females, – in cases following the primary stages III and IV there were registered 2.5 % of early stages in males and 3.2 % in females, 1.8 % of the advanced stages in males and 2.2 % in females. There were uncommon the number of 12,418 subsequent tumors of advanced stages in men (13.8 % of total 89,796 subsequent ones) and 10,189 in women (13.5 % of total 75,254 subsequent ones), following the primary cancers of stages III, IV. Between these subsequent and advanced stages predominated – in males 36.9 % cancers of gastrointestinal and 33.1 % respiratory tract, 10.3 % genital organs, 8.1 % urinary tract, – in females 37 % cancers of gastrointestinum, 21 % breast, 17.2 % genital organs, 12.8 % respiratory and 4.5 % urinary tract. It is worrying that of total multiple cases every the seventh gastrointestinal cancer in males and the fifth in females were detected at any advanced clinical stages after primary cancer. c) Of 41,161 survived up to October 2007 there were 54.7 % of early stages in males and 58 % in females, 5 % of advanced stages in males and 5.2 % in females, unknown stages 40.3 % in males and 36.9 % in females. Of 84,101 deaths there were 30 % of early stages in males and 39.8 % in females, 11 % of advanced stages in males and 12.1 % in females, unknown stages 59 % in males and 48.1 % in females. Objective: To prevent the possible functional complications after gastrectomy, to improve the quality of life and the process of early rehabilitation. Material and Methods: The study included 165 patients with gastric cancer of stage III (T3N12M0), which, depending on the method of recovery divided into 2 groups: Group 1 n = 80 patients who, after an extensive gastrectomy had reservoir formed, II group n = 85 patients who had gastrectomy performed without forming a reservoir. Results: There were no incidence of reflux esophagitis and dumping syndrome observed in the main group, while in the control group, these complications were respectively 61.2% and 25.8%. Weight gain in the main group was observed in 83.7% of patients, in the control group in 4.7%. Assessment of physical activity on the Karnovsky scale in the study group was 85.0%, while 62.0% in the control group. Within 3 months, physical rehabilitation was observed in 90.0% of patients of the main group, while in the control group it was 25.7%. Threeyear survival rate in the main group was 71.4+0.4%, while in the control group, 57.2+0.3% (P> 0.05). Conclusions: Creation of a stomach replacing reservoir after gastrectomy is adequate way in ridding patients from possible complications. In the department of coloproctology of NORC MH RUz 17 patients with disseminated forms of colorectal cancer was made the study of oncogenes and complex treatment by 2 protocols using FOLFOX4 regime and FOLFIRI regime. In second protocol there used 2 sessions of endolymphatical polychemotherapy FOLFOX4 regime against EHFhyperthermia. All patients were performed additional investigations directed to study the presence of multiple drug resistance in them where definition of р53, bcl2 oncogene expression. In our observations we followed resistance to FOLFOX4 scheme in 4 patients, and to FOLFIRI scheme in 2 cases. Cíl studie: Perzistující foramen ovale (PFO) je velmi často doprovázeno neurologickými komplikacemi (migrénou), ale patofyziologie tohoto vztahu není dosud zcela vyjasněna. Jedna z hypotéz popisuje významnou roli serotoninu v patofyziologii neurologických komplikací. Serotonin může pravolevým zkratem přes PFO pronikat do arteriální krve a vyvolat migrenózní ataku. Cílem studie bylo potvrdit hypotézu, že 24 hodin po perkutánním uzávěru PFO klesají hladiny serotoninu v arteriální krvi. Materiál a metody: U 8 pacientů s PFO byly odebrány vzorky venózní a arteriální krve na stanovení serotoninu před operací, během operace a 24 hodin po operaci. Hladiny serotoninu u pacientů i v kontrolní skupině byly stanoveny pomocí enzymové imunoanalýzy (EIA, Immunotech). Výsledky: Byly stanoveny následující koncentrace serotoninu ve venózní a arteriální krvi (mean ± SD): kontrolní skupina 906 ± 229,4 nmol/l, venózní krev před operací: 126 ± 69,6 nmol/l, venózní krev odebraná z centrálního žilního katétru: 182 ± 59 nmol/l, venózní krev po operaci: 140 ± 40 nmol/l, arteriální krev před operací: 165± 25 nmol/l, arteriální krev během operace: 182 ± 44 nmol/l, arteriální krev po operaci: 130 ± 36 nmol/l. Závěr: Přímé stanovení hladin serotoninu u pacientů před perkutánním uzávěrem PFO a po něm je po eliminaci možných preanalytických chyb vhodným analytickým nástrojem pro studium změn serotoninu po uzávěru PFO. Výsledky ukazují pokles hladin arteriálního serotoninu během 24 hodin po operaci., Objective: Percutaneous closure of patent foramen ovale (PFO) has been associated with improvement of migraine headache. Tissues-borne serotonin bypassing the metabolic active pulmonary circulation through the PFO is the proposed trigger of migraine attacks in susceptible patients. Therefore a hypothetical decrease of arterial serotonin levels after PFO closure might explain the migraine improvement. Aim of the study: Aim of the study was the investigation of suitability of direct serotonin assay in venous and arterial blood in patients undergoing percutaneous PFO closure for further studying the mechanism of migraine improvement after PFO closure. Materials and methods: Arterial and venous serum serotonin levels were investigated in eight patients undergoing correctional surgery operation. The samples were collected as follows: Venous and arterial blood before operation, arterial blood during operation, venous sample from right atrial central catheter and then peripheral venous and arterial sample 24 hours after the operation. Serotonin levels were investigated in control group of 6 healthy patients. Serotonin was measured by commercially available enzyme immunoassay (EIA, Immunotech). Results: The results (mean ± SD) were as follows: Control group: 906.0 ± 229.4 nmol/l, Venous blood before the operation: 126.0 ± 69.6 nmol/l, venous blood after the operation: 140 ± 40 nmol/l, arterial blood before the operation: 165± 25 nmol/l, arterial blood during operation: 182 ± 44 nmol/l, arterial blood after the operation: 130 ± 36 nmol/l. Conclusion: Direct assay of serotonin with commercially available EIA is suitable for studying changes of serotonin levels after PFO closure. The Purpose of the study: define the indications and contraindications to sphincterpreserving operation in lowerampullar cancer of rectum. The Materials and methods: In coloproctological branch of National Oncological Scientific centre (NOSC) in 20052009 years 142 patients were performed radical operations, 64 (45,1%) patients were performed sphincterpreserving operations abdominoanal resection of rectum (AARR), medium age of patients 59,8 years, beside 78 (54,9%) patients were performed abdomeno perineal extirpation of rectum (APER), medium age 54,2 years. The Results: Studied results of the morphological research of removed preparation by histological structure of tumors and by form of the growing, depending on locations of the tumors on toothed line. The remote postoperative results studied beside 51 (79,7%) patient with AARR and beside 63 (80,1%) patients with APER. The Findings: sphincterpreserving operations (SPO) of the rectum can be the operation of the choice in treatment of high differentiated adenocarcinoma (HDA) and medium differentiated adenocarcinoma (MDA) in stages T23 in localization of the lower pole of tumors, not below 1 centimeter of toothed line (TL) and carries combined character, in location of the lower edge of tumors on toothed line in patients with HDA MDA in 4% and 2,2% cases are shown that completing intrasphincternal resections of rectum, in connection with particular aggressiveness LDA question about choice of the method of the treatment under their localizations directly on TL must be solved in favour of complete APER, in exophytic tumor located onto and above TL is indication for sphincterpreserving operation in type of AARR, indices recurrencefree and not metastatic period, oneyear deathrate, threeyear survivability after SPO at inferior ampular cancer of rectum has not an essential distinction from results after APER., A.
