Cíl: Cílem šetření bylo zjistit, jak studenti ošetřovatelství hodnotí svoji klinickou praxi, spolupráci s mentorem, jaké aspekty ovlivňují jejich spokojenost s praxí a jak se hodnocení praxe liší na jednotlivých odděleních. Metodika: Výzkumný soubor zahrnoval 59 studentů druhého a třetího ročníku oboru všeobecná sestra. Dotazníkové šetření bylo realizováno standardizovaným nástrojem pro hodnocení klinické výukové prostředí a supervize CLES (Saarikoski, 2002) po absolvované klinické praxi studentů (2008). Výsledky: Byly potvrzeny rozdíly v hodnocení supervize a klinického výukového prostředí vzhledem k typu oddělení, kdy interní oddělení byla studenty hodnocena kladněji. Studenti hodnotili nejvyššími známkami dimenzi ošetřovatelská péče na oddělení, nejhůře pak vztah mezi mentorem a studentem. Současně byly potvrzeny pozitivní korelace mezi celkovou spokojeností studentů a jejich hodnocením. Statisticky významné vztahy byly identifikovány mezi zátěží sester na oddělení a hodnocením studentů – s vyšší fyzickou zátěží sester byla supervize a klinické výukové pracoviště hodnoceno kladněji. Závěr: Výsledky šetření ukazují na význam supervize a kvalitního klinického výukového prostředí pro ošetřovatelské praxe. Praktická výuka tvoří významnou část vzdělávacího programu sester, a proto je nezbytné sledovat zkušenosti a reflexe studentů., Objective: The aim of this study was to find out how nursing students evaluate their clinical practice, co-operation with the mentor, what aspects affects their satisfaction with the practice and how the evaluation of practice varies from wards. Methods: The sample comprised of 59 nursing students of the second and third year of study. The survey was realized by standardized Clinical Learning Environment and Supervision instrument CLES (Saarikoski, 2002) after the completion of students clinical practice (2008). Results: Differences were confirmed in the evaluation of the supervision and clinical learning environment depending on the type of wards, when the internal wards were evaluated by students more favourably. Students rated the dimension of nursing care at the word the highest marks, the lowest was rated the relationship between mentors and students. At the same time were confirmed positive correlations between students’ overall satisfaction and their evaluation. Statistically significant relationship were identified between the nurses´ workload in the wards and students´ assessment – with the higher workload of nurses was the supervision and clinical learning environment evaluated more favourably. Conclusion: The results of survey show the importance of clinical supervision and quality of clinical learning environment for nursing practice. Cíl: Cílem výzkumu bylo zjistit kvalitu života seniorů s demencí pohledem rodinných příslušníků. Dále pak porovnat vnímání kvality života seniorů s demencí rodinnými příslušníky v závislosti na prostředí, ve kterém senior žije. Metodika: Byl proveden kvalitativní výzkum, metodou sběru dat byl nestandardizovaný rozhovor. Výzkumu se zúčastnilo 9 rodinných příslušníků pacientů s demencí. Na základě analýzy dat byly stanoveny 4 kategorie: 1. změny v psychické oblasti, 2. změny kognitivních funkcí, 3. změny v oblasti aktivit denního života a instrumentálních všedních činností a 4. celkové vnímání kvality života. Výsledky: Z pohledu rodinných příslušníků nejvíce zhoršuje kvalitu života seniorů s demencí ztráta paměti, která je vyřazuje z běžného života, dále ztráta schopnosti orientace v prostoru a čase a ztráta schopnosti vykonávat činnosti běžného denního života. Kvalitu života snižují také smyslové poruchy, které pacienty uvádějí do ještě větší sociální izolace a mohou v nich podporovat nárůst paranoidity. Rodinní příslušníci hodnotili kvalitu života svých blízkých v domácím prostředí výrazně lépe v porovnání s kvalitou života pacientů s demencí umístěných v domově pro seniory. V domácím prostředí měli senioři více podnětů, byli více aktivizováni a vtahováni do rodinného a společenského života. Po nástupu do institucionální péče došlo k zhoršení příznaků onemocnění a postupnému vyřazení nemocných z aktivizačních programů. Závěr: Důležitou součástí zvýšení kvality života pacientů s demencí je podpora rodinných pečovatelů, aby nemocní mohli zůstat co nejdéle ve svém přirozeném prostředí., Objective: The aim of the research was to determine the quality of life of seniors with dementia from the perspective of family members. Furthermore, to compare the perceptions of quality of life of seniors with dementia by family members depending on the environment in which the senior lives. Methodology: A qualitative research was performed, for the method of data collection was used non-standardized interview. Research was attended by nine family members of patients with dementia. Based on analysis of data there were determined four categories: 1. changes in the psychological field, 2. changes in cognitive function, 3. changes in the area of activities of daily living and instrumental activities of everyday life and 4. overall perception of quality of life. Results: From the perspective of family members most deteriorating quality of life of seniors with dementia is the memory loss, which excludes them from normal life, as well as loss of ability to orientation in space and time, and loss of ability to perform normal activities of daily life. The sensory disturbance also reduces the quality of life, which leads patients into a greater social isolation and this may promote the growth of the paranoia. Family members rated the quality of life of their loved ones at home as significantly better in comparison with the quality of life of patients with dementia placed in the home for the elderly. In a domestic environment have seniors more stimulations, are more activated, and are more drawn into the family and social life. After coming into institutional care occurs worsening of symptoms and the gradual removal of patients from activation programs. Cieľ: Cieľom výskumu bolo u detí s bronchiálnou astmou zistiť vplyv ochorenia a stupňa kontroly astmy na kvalitu života dieťaťa a kvalitu života jeho rodičov. Metodika: Na zber empirických údajov sme použili štandardizované dotazníky na kvalitu života – Paediatric Asthma Quality of Life Questionnaire (PAQLQ) a Paediatric Asthma Caregiver´s Quality of Life Questionnaire (PACQLQ) a na hodnotenie kontroly astmy – Asthma Control Questionnaire (ACQ). Výsledky boli analyzované pomocou metód deskriptívnej a induktívnej štatistiky. Výsledky: Vo vzorke detí (n = 72) sme zistili, že 27 (37,5 %) detí malo astmu pod kontrolou a 28 detí (38,9 %) bolo v pásme nekontrolovanej astmy. Celkové skóre ACQ (1,24 ±1,07) potvrdilo stredný stupeň kontroly astmy. Celkové skóre dotazníka PAQLQ u detí bolo 5,34 ±1,16, čo indikuje, že deti s astmou vyjadrujú dobrú kvalitu života. U detí ochorenie viac vplývalo na aktivity ako na emocionálne prežívanie a výskyt symptómov. Celkové skóre dotazníka PACQLQ u rodičov bolo 4,53 ±1,07, u rodičov nebol zaznamenaný rozdiel v doménach emocionálne prežívanie a aktivity. Výskum potvrdil, že stupeň kontroly astmy štatisticky významne ovplyvňuje kvalitu života dieťaťa aj jeho rodičov, pričom čím je vyššia kvalita života dieťaťa, tým je vyššia kvalita života rodičov. Záver: Cieľom komplexného manažmentu liečby je dosiahnuť čo najoptimálnejší stupeň kontroly astmy, a tým vytvoriť predpoklady k vyššej kvalite života dieťaťa a jeho rodičov. Využívaním posudzovacích nástrojov na hodnotenie kvality života je možné identifikovať závažný dopad choroby na každodenný život detí aj rodičov., Aim: The aim of the research was to find out the impact of the disease and the degree of asthma control on quality of life in children with bronchial asthma and quality of life in their parents. Methods: To collect empirical data we have used standardized questionnaires concerning quality of life – Paediatric Asthma Quality of Life Questionnaire (PAQLQ) and Paediatric Asthma Caregiver’s Quality of Life Questionnaire (PACQLQ) and for the assessment of asthma control we have used questionnaire Asthma Control Questionnaire (ACQ). The results were analysed using the methods of descriptive and inductive statistics. Results: In the sample of children studied (n = 72) we have found that 27 children (37.5%) have their disease under control and 28 children (38.9%) were in the range of uncontrolled asthma. Total ACQ score (1.24±1.07) confirmed medium degree of asthma control (partially controlled asthma). Total score of PAQLQ questionnaire in children (5.34±1.16) indicates that children with asthma represent good quality of life. Bronchial asthma in children had more significant influence on activities than on emotions and incidence of symptoms. Total score of PACQLQ questionnaire in parents was 4.53 ±1.07 and we have not registered differences between domains of emotions and activities. The research has proved that degree of asthma control has statistically significant influence on the quality of life of the child and his parents, whilst the higher the quality of life of the child, the higher the quality of life of parents. Conclusion: The aim of comprehensive management of asthma therapy is to achieve the most optimal level of asthma control and thus to establish conditions for higher quality of live of the child and his parents. The article presents results of a study of the relationship 'locus of control health and subjective wellbeing. In a heterogeneous sample (N = 239) in the stage of youth (1930 years) shall be established: locus of control determines 11.56% of the changes in the assessment of health, changes in health contribute to 12.82% of the variation in assessment of happiness and 6.10% in the measurement of life satisfaction. Young people with an internal locus of control are healthier, happier and more satisfied with their lives than those with an external locus of control. In a study of cause effect relationship is established that internal locus of control determines a healthier lifestyle. Inadequate health care, more severe risk factors (stress, unhealthy diet, inadequate sleep, smoking) among young people with an external locus of control, outline prospects for greater vulnerability of this group of the population of the so called "Diseases of modern society." Introduction and Objectives: One of the characteristic changes of tumor formation is accumulation of genetic disorders in mitochondrial and nuclear genome. Mitochondrial disorders, from its side, are responsible for failure of metabolism, apoptosis, cell growth, formation of reactive oxygen species, etc. Overprpoduction of reactive oxygen species (ROS) significantly impacts the respiration chain enzymes and entirely the antioxidant system of mitochondria. Finally this may become a favorable condition for normal cells transformation. The purpose of the presented work was to study the mitochondrial defects and to establish their role in prostate cancer development. Results: Experimental results demonstrate significant increase of the activity of mitochondrial succinate dehydrogenaze (complex II) of the malignant epithelial cells of prostate, and slight changes in cytochrome oxydase (complex IV) activity. Also significant activation of the antioxidant system (glutathione-dependant system) of mitochondria in prostate malignant epithelial cells was revealed. Cieľ: Cieľom práce bolo zistiť výskyt komplikácií v závislosti od veku a pohlavia pacientov, od dĺžky zavedenia stimulačnej elektródy, od zvolenej cesty prístupu zavedenia stimulácie u pacientov hospitalizovaných na Oddelení arytmií a koronárnej jednotky v Univerzitnej nemocnici Martin. Metodika: Pre zber potrebných údajov sme použili obsahovú analýzu zdravotnej dokumentácie, nemocničný informačný systém, pozorovanie na základe vypracovaného záznamového hárku. Údaje sme analyzovali pomocou deskriptívnej a induktívnej štatistiky. V sledovanom súbore bolo 38 pacientov od 58 rokov do 93 rokov. Pacienti boli akútne prijatí na Jednotku intenzívnej starostlivosti za účelom realizácie dočasnej kardiostimulácie, ktorá bola užívaná v priemere 5,28 ± 2,33 dní. Výsledky: Na základe spracovania údajov sme u 17 pacientov zistili výskyt komplikácií, pričom najčastejšou komplikáciou bola lokálna infekcia. Štatistickou analýzou sme zaznamenali významné vzťahy medzi dĺžkou zavedenia stimulačnej elektródy a výskytom lokálnej infekcie, kde nám vznikla silná signifikantná korelácia r(S) = 0,518 na hladine významnosti p = 0,001 a rovnako pri výskyte hematómu sme zaznamenali koreláciu r(S) = 0,329 na hladine významnosti p = 0,023. Záver: Analýzou získaných údajov sme identifikovali najčastejšie vyskytujúce sa komplikácie v spojitosti s dočasnou kardiostimuláciou. Najčastejšie sme zaznamenali výskyt lokálnej infekcie, ktorý súvisí s následnou ošetrovateľskou starostlivosťou, ale aj s významnými prediktormi, ktoré môžu do značnej miery ovplyvniť ich vývoj., Aim: The aim of our study was to determinate the incidence of complications depending on the age and gender of patients, duration of implantation of stimulation electrode, of selected access point of implementation stimulation in patients hospitalized at the Arrhythmia unit and Coronary care unit in University hospital in Martin. Methods: For the collection of required data we have used content analysis of medical records, hospital information system, surveillance based on own designed sheet. The data were analysed using descriptive and inductive statistic. The sample consisted of 38 patients in the range from 58 to 93 years of age. Patients were acutely admitted to ICU with the aim of realization of temporary cardiac pacing, which lasted 5.28 ± 2.33 days in average. Results: On the basis of processed data, we found incidence of complications in 17 patients, where the most frequent complication was local infection. Using statistical analysis we found a significant relationship between duration of implementation of stimulation electrode and incidence of local infection, which resulted in strong significant correlation (r(S) = 0.518, p = 0.001) and similarly, we noted correlation in the incidence of haematoma (r(S) = 0.329, p = 0.023). Conclusion: Using analysis of the obtained data we have identified the most frequently occurring complications in conjunction with the temporary cardiac pacing. Cíl: Cílem studie bylo zjistit potencionální riziko hypertenze u dětí ve věku 14 – 15 let dvojím standardním měřením krevního tlaku a zmapovat moţný vztah mezi hodnotami krevního tlaku dětí a jejich vybranými sociodemografickými faktory a tělesnými parametry. Metodika: Výzkumný soubor tvořilo 143 ţáků 8. a 9. tříd základních škol v Ostravě zapojených do projektu Zdravá škola. Data byla získána měřením fyziologických funkcí a antropometrických dat a doplňujícími otázkami na vybrané rizikové faktory kardiovaskulárních chorob. Výsledky: Měření krevního tlaku (TK) proběhla opakovaně, při druhém měření byla prokázána odlišnost naměřených hodnot TK mezi pohlavími, kdy u chlapců byly hodnoty TK vyšší neţ u děvčat. Bylo potvrzeno, ţe na hodnoty TK má vliv pohybová aktivita, a to zejména u dětí s vysokými hodnotami BMI. U normálních hodnot BMI korelace mezi hodnotou krevního tlaku a pohybovou aktivitou nebyla statisticky významná. Z rizikové skupiny 14 dětí s rodinnou predispozicí hypertenze bylo evidováno 29 % dětí s průměrnou hodnotou TK v pásmu pro 95. percentil (hypertenzní hodnoty) vzhledem k danému věku, pohlaví a výšce. Závěr: Výskyt vysokého TK byl četnější u dětí s nadváhou a obezitou. Pohybová aktivita ovlivňovala hodnotu krevního tlaku, zvláště pak právě u dětí s nadváhou a obezitou, kdy hodnoty TK u těchto dětí, které necvičily, byly vyšší neţ u dětí cvičících. Pravidelným měřením a správným zhodnocením TK můţe sestra včas zachytit děti a adolescenty s rizikovými parametry., Aim: The aim of the thesis is to examine potential risk of hypertension in children aged 14 - 15 years by two standard measurements of blood pressure and to find out possible relation between values of blood pressure in children and their selected socio-demographic factors and physical parameters. Methods: The research was carried out on 143 pupils from elementary schools in Ostrava, which participate in the project „Zdravá škola“(Healthy School). The data were obtained from measuring of physiological functions, survey of anthropometric data and further questions focused on particular risk factors for cardiovascular diseases. Results: The second measurement revealed a difference in values of blood pressure between sexes, when boys had higher blood pressure than girls. The research confirmed that blood pressure is influenced by physical activity, especially in children with high value of BMI. In children with lower values of BMI, the correlation between the value of blood pressure and physical activity was not statistically relevant. From a high-risk group of 14 children with genetic predisposition, 29% of children had average value of blood pressure in range of 95. percentile (hypertension) given age, sex and height. Conclusion: The occurrence of high blood pressure is common in children who suffer from overweight and obesity. The value of blood pressure is influenced by physical activity, especially in overweight and obese children, when value of blood pressure in the children who did not practice, were higher in children than exercisers. V řadě případů se nejedná o problém ženy samotné, proto je velmi žádoucí, aby byl partner pacientky součástí diagnosticko-terapeutického procesu již od počátku. Současný stav: Výzkumné studie prokázaly, že nejenom rozdíly v pohlaví mají vliv na zdravotní stav, ale též etnická příslušnost a společenská třída. Imigranti jsou zranitelnou skupinou, která je ve vyšším riziku řady onemocnění ve srovnání s majoritou. Sociální determinanty zdraví jsou významné při hledání příčin sociálních nerovností ve zdraví (Bates et al., 2009; Lindström et al., 2001). Cíl: Hlavním záměrem výzkumu bylo zmapování využití zdravotní péče imigranty. Metodika: Kvantitativní metoda byla realizována pomocí dotazování. Výzkumné šetření probíhalo v období leden až červenec 2012. Bylo kontaktováno 236 imigrantů ve dvou regionech (Jihočeský kraj a hlavní město Praha). Byli osloveni legálně usazení imigranti, ve věkové kategorii 18-65 let, vietnamské, mongolské a ukrajinské národnosti. Výsledky: Čerpání zdravotní péče imigranty bylo ovlivněno typem zdravotního pojištění, délkou pobytu, pohlavím a znalostí českého jazyka. Imigranti, kteří mohli vstoupit do systému veřejného zdravotního pojištění, čerpali zdravotní péči častěji než imigranti s komerčním zdravotním pojištěním. Závěry: Mezi opatření, vedoucí k odstranění sociálních nerovností a zlepšení situace imigrantů v ČR patří zvyšování jazykových znalostí imigrantů a umožnění všem legálně usazeným imigrantům přístup k veřejnému zdravotnímu pojištění., Background: Research shows that health state is influenced not only by gender but also by class and ethnic membership. Immigrants form a vulnerable group in comparison to the majority, more susceptible to a wide range of diseases. Social determinants of health play an important role in search for reasons for social inequality in health (Bates et al., 2009; Limdström et al., 2001). Goal: The main aim of the research was to map use of healthcare by immigrants. Methodology: The quantitative method used were interviews. The survey was carried out in the period from January to July 2012. 246 immigrants were interviewed from two regions (South Bohemian Region and the capital Prague). They were legally settled immigrants in the age group 18-65, of Vietnamese, Mongolian and Ukrainian nationality. Results: The factors influencing use of healthcare by immigrants are the type of health insurance, length of residence, sex and knowledge of Czech language. Objective: To prevent the possible functional complications after gastrectomy, to improve the quality of life and the process of early rehabilitation. Material and Methods: The study included 165 patients with gastric cancer of stage III (T3N12M0), which, depending on the method of recovery divided into 2 groups: Group 1 n = 80 patients who, after an extensive gastrectomy had reservoir formed, II group n = 85 patients who had gastrectomy performed without forming a reservoir. Results: There were no incidence of reflux esophagitis and dumping syndrome observed in the main group, while in the control group, these complications were respectively 61.2% and 25.8%. Weight gain in the main group was observed in 83.7% of patients, in the control group in 4.7%. Assessment of physical activity on the Karnovsky scale in the study group was 85.0%, while 62.0% in the control group. Within 3 months, physical rehabilitation was observed in 90.0% of patients of the main group, while in the control group it was 25.7%. Threeyear survival rate in the main group was 71.4+0.4%, while in the control group, 57.2+0.3% (P> 0.05). Conclusions: Creation of a stomach replacing reservoir after gastrectomy is adequate way in ridding patients from possible complications.
