Akutní myeloidní leukemie (AML) je onemocnění především staršího věku s mediánem při stanovení diagnózy 69 let. Bohužel výsledky intenzivní léčby u nemocných starších 60 let s AML, tedy pacientů s nejvyššími riziky rozvoje AML, jsou neuspokojivé. Důvodem těchto neuspokojivých výsledků jsou zvýšená rizika mortality při samotné intenzivní chemoterapii u starších nemocných a dále samotná biologická podstata AML ve vyšším věku, která je spojena se zvýšenou rezistencí k intenzivní léčbě a s vyšším rizikem relapsu. Možnost provedení alogenní transplantace krvetvorných buněk po redukované přípravě v rámci konzolidační léčby za současně se zlepšující podpůrné péče v potransplantačním období může dle publikovaných výsledků zlepšit nepříznivou prognózu AML u nemocných starších 60 let. Limitací uvedeného postupu je fakt, že alogenní transplantaci v současné době nemůže podstoupit většina starších pacientů kurativně léčených pro AML z důvodu nedosažení kontroly základního onemocnění nebo z důvodu neuspokojivého celkového stavu neumožňujícího provedení transplantace. Zavedení nových léčebných postupů do léčby AML a další zlepšení samotných transplantačních postupů může v budoucnu potenciálně zvýšit možnost zařazení transplantace do léčby pacientů starších 60 let s AML., Acute myeloid leukemia (AML) is disease of older age with the median of age at diagnosis 69 years. Unfortunately results of intensive treatment of patients older than 60 years with AML, thus patients with high-risk of AML development, are unsatisfactory. The reasons for this unsatisfactory results are higher risks of treatment-related mortality among older patients treated with intensive chemotherapy and biological characteristics of AML in older age with higher risk of AML resistance to intensive treatment and higher risk of AML relapse. According to published data reduced-intensity allogeneic transplantation as part of consolidation treatment and advancement in supportive care after transplantation can improve outcome of patients older than 60 years with AML. The limitation of this strategy is a fact that majority of older patients cannot undergo allogeneic transplantation due to unsatisfactory control of AML or due to diminished performance status which does not alow transplant procedure. INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease. Tento článek se zabývá karcinomem prostaty v jeho nejčastější podobě, přináší stručný přehled jeho patofyziologie a diagnostiky. Uvádí také jednotlivé strategie hormonoterapie, možnosti chemoterapie, poukazuje na významnou roli androgenních receptorů při léčbě hormonálně dependentního i rezistentního onemocnění, v neposlední řadě zohledňující kvalitu života. Pro přehlednost jsou zde uvedené i nové nadějné preparáty, jejichž využití je zatím omezené ve fázi klinického zkoumání., This article focuses on prostate cancer in its most common form and presents a brief overview of its pathophysiology and diagnostics. The individual strategies of hormonal therapy and chemotherapy are outlined. It highlights the important role of androgen receptors in hormone dependent and resistant cancer treatment, taking into account the quality of life of the patient. Během posledních 30 let se transplantace pankreatu stala rutinním postupem, který je zvláště vhodný pro pacienty s diabetem 1. typu, kteří současně podstupují transplantaci ledviny. Zlepšuje kvalitu života a v porovnání s transplantací samotné ledviny zlepšuje dlouhodobé přežívání. Poskytuje nezávislost na inzulinu s téměř normální glukózovou tolerancí, uvolnění diety, klid od hypoglykemií a možnost zastavení postupu nebo dokonce regresi mikroangiopatických komplikací. Samostatně se transplantace pankreatu provádí zejména u vybraných pacientů s rekurentními epizodami těžkých hypoglykemií při syndromu porušeného vnímání hypoglykemie nebo jako následný výkon u příjemců s funkčním štěpem ledviny od zemřelého či žijícího dárce. 5leté přežívání funkce štěpu pankreatu činí při kombinované transplantaci, transplantaci samotného pankreatu či transplantaci pankreatu následně po ledvině více než 75, 50 a 62 %. V rozporu se zlepšujícími se výsledky je určitá stagnace počtu výkonů v některých evropských zemích a v USA. Ačkoliv se na tom podílí také horší kvalita dárců, zavádění ostrůvkové transplantace jako méně invazivní procedury a naděje v technologická řešení, hlavní příčina spočívá v nedostatečném povědomí o současné úspěšnosti a menším počtu pacientů na čekací listině. ČR se v oblasti transplantační léčby diabetu tradičně řadí k nejaktivnějším zemím. Pro pacienty jsou s ohledem na klinickou potřebu k dispozici různé varianty včetně transplantace Langerhansových ostrůvků., During the past 30 years pancreas transplantation evolved into a routine procedure especially suitable for type 1 diabetic recipients undergoing simultaneously kidney transplantation significantly improving quality of life and life expectancy as compared with kidney only recipients. It provides insulin independence with near-normal glucose control without special dietary restriction, freedom from hypoglycemia and chance for halting or regression of microangiopathic diabetes complications. As a separate procedure, pancreas transplantation is carried out mainly in selected subjects suffering from severe hypoglycemic episodes and impaired hypoglycemia awareness or as a subsequent procedure in type 1 diabetic kidney recipients from both cadaveric or living donors. Five-year insulin independence rate following combined pancreas and kidney, pancreas only and pancreas after kidney procedures currently exceed 75, 50 and 62 %, respectively. Though the outcomes still continue to improve, the rate of pancreas transplants has reached a plateau in several European countries or even declines in the United States. Main reasons for that include fewer referrals from diabetes specialist, decreased donor quality, introduction of islet transplantation as a less invasive procedure but probably most of all probably insufficient information on the latest progress and trends achieved in this area. V uplynulých 25 letech se průměrná očekávaná délka života mužů prodloužila o čtyři roky, zatímco věk, kdy je u muže detekován karcinom prostaty, se snížil průměrně o deset let. Onemocnění je navíc stále častěji diagnostikováno v časném stadiu, kdy je stále možná kurativní léčba. Tyto změny ve věku a rozsahu maligního onemocnění v době diagnostikování poukázaly na limitace běžných terapeutických modalit pro léčbu karcinomu prostaty s kurativním záměrem, jako jsou např. významné riziko recidivy agresivního karcinomu nebo riziko dlouhodobé morbidity v močopohlavním systému a jejich významný dopad na kvalitu života pacienta (QoL). Větší povědomí o limitacích radikální prostatektomie, ozařování zevním paprskem i brachyterapie podnítilo vývoj alternativních terapií s kurativním záměrem, které by poskytovaly srovnatelnou kontrolu karcinomu při menší morbiditě, což by umožnilo lépe zachovat QoL. Cílený ultrazvuk o vysoké intenzitě (HIFU) disponuje vlastnostmi, které jej řadí mezi atraktivní kurativní terapeutické modality. HIFU je neinvazivní technika, při níž je pomocí přesně cílené ultrazvukové energie dosaženo nekrózy nádorových buněk bez nutnosti provádět ozařování nebo operační excizi. V současné době se v urologické onkologii HIFU využívá při léčbě karcinomu prostaty a v experimentálním stadiu probíhá testování tohoto přístupu u pacientů s karcinomy ledviny a prsu. Klinický výzkum v oblasti využití HIFU pro léčbu lokalizovaného karcinomu prostaty byl zahájen v 90. letech minulého století a v současné době již tuto léčbu podstoupilo přibližně 30 000 pacientů s karcinomem prostaty, ve většině případů pomocí zařízení Ablatherm (EDAP TMS, Lyon, Francie). Od roku 2000 se užívá kombinace transuretrální resekce prostaty a HIFU s cílem redukovat velikost prostaty, usnadnit destrukci tkáně a minimalizovat výskyt vedlejších účinků. Předpokládá se, že zdokonalení zobrazovacích technologií povede k dalšímu zlepšení vynikajících výsledků v oblasti účinnosti a bezpečnosti léčby. Probíhající výzkum testující využití HIFU v rámci fokální, záchranné i paliativní terapie pomůže rozšířit indikace HIFU jako univerzální neinvazivní modality pro léčbu karcinomu prostaty., Over the past 25 years, the average life expectancy for men has increased by four years, and the age of prostate cancer detection has decreased an average of 10 years with diagnosis increasingly made at early‑stage disease where curative therapy is possible. These changing trends in the age and extent of malignancy at diagnosis have revealed limitations in conventional curative therapies for prostate cancer, including a significant risk of aggressive cancer recurrence, and the risk of long‑term genitourinary morbidity and its detrimental impact on patient quality of life (QoL). Greater awareness of the shortcomings in radical prostatectomy, external radiotherapy and brachytherapy have prompted the search for alternative curative therapies that offer comparable rates of cancer control and less treatment‑related morbidity to better preserve QoL. High‑intensity focused ultrasound (HIFU) possesses characteristics that make it an attractive curative therapy option. HIFU is a non‑invasive approach that uses precisely‑delivered ultrasound energy to achieve tumor cell necrosis without radiation or surgical excision. In current urological oncology, HIFU is used clinically in the treatment of prostate cancer, and is under experimental investigation for therapeutic use in renal and breast malignancies. Clinical research on HIFU therapy for localized prostate cancer began in the 1990s, and there have now been approximately 30,000 prostate cancer patients treated with HIFU, predominantly with the Ablatherm (EDAP TMS, Lyon, France) device. Transurethral resection of the prostate has been combined with HIFU since 2000 to reduce prostate size, facilitate tissue destruction, and to minimize side effects. Advances in imaging technologies are expected to further improve the already superior efficacy and morbidity outcomes, and ongoing investigation of HIFU as a focal therapy and in salvage and palliative indications are serving to expand the role of HIFU as a highly versatile non‑invasive therapy for prostate cancer., and Thüroff S., Chaussy C. Transplantaci jater jako kurativní léčebnou metodu je možno použít u vybraných primárních tumorů jater, především u hepatocelulárního karcinomu a vzácnějších semimaligních tumorů, jako je epiteloidní hemangioendoteliom, dále u infiltrace jater metastázami neuroendokrinních tumorů (za předpokladu, že metastázy jsou lokalizovány pouze v játrech a primární tumor byl odstraněn) a u benigních tumorů (hemangiomů a adenomů) s útlakovými příznaky a velikostní progresí. Cholangiokarcinom není v CKTCH Brno k transplantaci jater indikován. V posledních letech dochází k nárůstu transplantací jater pro hepatocelulární karcinom a hepatocelulární karcinom se také častěji nachází až ex post v explantovaných játrech. Transplantace jater je indikována u vybraných pacientů, u nichž je dobrá šance na dlouhodobé přežití po transplantaci jater (přijímanou hranicí je 50% 5leté přežití po transplantaci). V CKTCH Brno bylo k 20. 3. 2015 transplantováno 38 pacientů – u 25 z nich byl hepatocelulární karcinom diagnostikován před transplantací a u 13 byl zjištěn z explantátu jater. 5leté přežití po transplantaci je u této kohorty pacientů 53 %. Chronickou hepatitidou C trpělo 32 % pacientů. Nejdéle (32 let) přežívající pacient v CKTCH Brno byl transplantován pro velký fibrolamelární hepatocelulární karcinom, což ukazuje na prognostický význam histologie tumoru – kritérium zohledňované z praktických důvodů pouze v některých indikačních schématech. Benigní tumory jater (adenomatóza, cystadenom, hemangiom s útlakovými příznaky) jsou spíše raritní indikace a výsledky transplantace jsou příznivé. Pro infiltraci jater karcinoidem byli transplantováni 4 pacienti, u 1 došlo k rekurenci tumoru., Liver transplantation as a curative treatment method can be used for selected primary liver tumours, in particular for hepatocellular carcinoma and rather rare semi-malignant tumours such as epithelioid hemangioendothelioma, further for infiltration of liver by metastatic neuroendocrine tumours (provided that metastases are only located in the liver and the primary tumour was removed) and for benign tumours (hemangiomas and adenomas) with oppression symptoms and size progression. Cholangiocarcinoma is not indicated for liver transplantation at the CKTCH Brno. In recent years liver transplants for hepatocellular carcinoma have increased and hepatocellular carcinoma has also been more frequently found ex post, in the explanted livers. Liver transplantation is indicated in selected patients with a good chance of long-term survival after liver transplantation (a generally accepted limit is 5year survival of 50 % after transplantation). By 20 March 2015 there were liver transplants carried out on 38 patients – in 25 of them was hepatocellular carcinoma diagnosed before transplantation and in 13 it was found in the liver explants. 5year survival following transplantation is reached by 53 % of this cohort. 32 % patients suffered from chronic hepatitis C. The longest surviving (32 years) patient at CKTCH Brno had liver transplanted for a big fibrolamellar hepatocellular carcinoma, which points to the prognostic significance of tumour histology: the criterion only considered in some indication schemes for practical reasons. Bortezomib (Velcade) je lék ze skupiny inhibitorů proteasomu. Představuje zcela nový a jedinečný léčebný postup v léčbě mnohočetného myelomu (MM). Tato práce přináší retrospektivní analýzu léčebné účinnosti a nežádoucích účinků u nemocných s MM léčených v referenčních centrech CMG v České republice. Celkem bylo hodnoceno 424 nemocných MM léčených bortezomibem od června 2004 do června 2010. Medián délky sledování byl 12,4 měsíců. Průměrný věk nemocných byl 66 let (34,1–86,5). Jednalo se především o nemocné s relabujícím MM. Celkem 48,3 % (205/424) nemocných mělo bortezomib v rámci druhé léčebné linie. Léčebnou odpověď bylo možné hodnotit u 83,3 % (353/424) nemocných. Léčebné odpovědi (? 50% pokles MIg) bylo dosaženo u 52,7 % (186/353), z toho bylo u 14,2 % (50/353) nemocných dosaženo kompletní remise. Nejzávažnější toxicitou byla periferní neuropatie stupně 3 a vyšší u 14 % (49/358) nemocných. Trombocytopenie stupně 3 a vyšší byla u 28,8 % (103/357) nemocných. Infekční komplikace stupně 2 a vyšší byly u 33,6 % (121/359) nemocných. V průběhu léčby bortezomibem nedošlo v našem souboru nemocných ke zvýšení frekvence výskytu trombo embolických komplikací. Mimořádný přínos bortezomibu v klinické praxi je dán rychlým nástupem léčebné odpovědi a vysokým počtem dosažených kompletních remisí. Většina jeho nežádoucích účinků se zpravidla daří zvládat pomocí vhodných profylaktických opatření, případně krátkodobým přerušením a následným snížením dávky podle platných doporučení., Bortezomib (Velcade) is a representative of class of medicines called proteasome inhibitors. This drug represents quite new and unique medical treatment of the patients with multiple myeloma (MM). This article presents a retrospective analysis of effectiveness and adverse effects in patients with multiple myeloma treated at reference centers of CMG in the Czech Republic. A total 424 patients with multiple myeloma treated with bortezomib from June 2004 to June 2010 were evaluated. The median follow-up was 12,4 months. The mean age of patients was 66 year (34,1–86,5) and majority of them were patients with relapsed multiple myeloma. Of the total 424 patients, 205 patients had bortezomib as second line therapy (48,3 %). Assessment of terapeutic response was possible in 83,3 % of treated patients (353/424). Therapeutic response (defined as a ? 50 % decrease in the serum MIG) was achieved in 52,7 % (186/353). Among mentioned responses, complete response was obtained in 14,2 % (50/353). The most severe toxicity observed was peripheral neuropathy, grade 3 or higher was developed in 14 % (49/358). There was recorded a thrombocytopenia grade 3 or higher in 28,8 % (103/357), further occurance of infections grade 2 or higher in 33,6 % (121/359) and eventually we could observe increased number of thromboembolic disease as well. Rapid onset of therapeutic responses and high percentage of obtained complete responses are the main reasons of extraordinary clinical benefit of bortezomib.
