OBJECTIVES: The main aim of the study was to investigate the physiological and social needs of patients hospitalized with schizophrenia to uncover potential issues in these areas. METHODS: The relevant self-evaluating CANSAS questionnaire for physiological and social needs was used by nurses in a cohort of hospitalized schizophrenic patients undergoing rehabilitation before discharge from the mental hospital. RESULTS: Two hundred and forty-four patients (women N = 115) aged 18-58 years were involved in the study. Intimate relations, financial matters, treatment of psychotic symptoms, and sexual life were among the most pressing physiological and social needs in our study subjects. CONCLUSION: The results of our study should stimulate psychiatric nurses in their effort not only to detect but also address the problems of schizophrenic patients concerning unfulfilled needs. and J. Úvod: Prevalence rizikových faktorů aterosklerózy se posouvá do mladších věkových kategorií. U vybrané již preselektované skupiny dětí s obezitou, dyslipidemií a metabolickým syndromem jsme sledovali antropometrické a laboratorní ukazatele během měsíčního redukčního pobytu dětí v Léčebně dr. Filipa, s.r.o., v Poděbradech. Současně byly sledovány genetické parametry spolupodílející se na vzniku obezity. Soubor: V rámci projektu byly statisticky zpracovány výsledky u 350 obézních dětských pacientů ve věku 8–16 let, průměrného věku 13,7 ? 2,1 roku, průměrné výšky 163 ? 10,6 cm. Metody: Při vstupu a při ukončení hospitalizace byly změřeny antropometrické parametry (výška, váha, tloušťka kožních řas, obvody tělesných částí), změřen krevní tlak a pulz. Byl stanoven lipidogram a další metabolické ukazatele, izolována DNA ke stanovení polymorfizmů genů pro FTO a MC4R. Výsledky: Během pobytu dětí došlo ke statisticky významnému poklesu metabolických ukazatelů a redukci antropometrických hodnot. Současně jsme prokázali skutečnost, že u nositelů rizikových polymorfizmů asociovaných s obezitou byla lepší odezva na režimová a dietní opatření. Závěr: Identifikovat jedince s horší odezvou na režimová opatření bude velice důležité, jelikož u těchto jedinců bude nutné časněji zahájit farmakologickou léčbu s cílem zabránit rozvoji komplikací plynoucích z přítomnosti těchto rizikových faktorů., Objectives: The study was aimed to determine risk factors of atherosclerosis after one month lifestyle intervention in overweight/obese children and also FTO and MC4R gene variants associated with obesity. Design and Methods: 350 non-diabetic Czech children (age 13.7 ? 2.1 years, 163 ? 10.6 cm hight) was examined. Before and after 4 weeks of lifestyle intervention (comprising a reduction of energy intake), biochemical and anthropometrical measurements were performed. Results: The mean weight loss achieved was 6.2 ? 2.1 kg (P < 0.001). Significant associations between BMI decrease and FTO and MC4R variants were found. Carriers of the FTO GG genotype and/or MC4R CC genotype lost significantly more body weight in comparison to the non-carriers (P < 0.0009 for BMI and P < 0.002 for body weight). The differences remain significant after adjustment for sex age and baseline values (P = 0.004 for BMI and P = 0.01 for body weight). Conclusions: It is necessary to look for the risk individuals with wrong response to the regime intervention. Nová doporučení v oblasti srdeční resynchronizační léčby uvádějí klinické a EKG rysy pro největší přínos resynchronizační léčby, klíčové důkazy pro indikaci k resynchronizační léčbě, indikace resynchronizační léčby u pacientů se srdečním selháním a nízkou ejekční frakcí levé komory při sinusovém rytmu i při fibrilaci síní. Cíl: Cílem sledování bylo zhodnotit efektivitu zavedených nefarmakologických aktivit na podporu a zachování kognitivních funkcí na ženském gerontopsychiatrickém oddělení psychiatrické léčebny v Kroměříži a zhodnotit spokojenost hospitalizovaných klientek s těmito aktivitami. Metodika: Na začátku a ke konci hospitalizace bylo použito škály Mini-Mental State Examination (MMSE) a testu kreslení hodin (CDT) k orientačnímu zhodnocení kognitivních funkcí a na závěr hospitalizace bylo použito řízeného rozhovoru. Výsledky: Ke konci hospitalizace nebylo prokázáno statisticky signifikantního zlepšení ve stavu kognitivních funkcí (krátké období pozorování je nejpravděpodobnějším důvodem). Klientky však hodnotí psychoterapeutické aktivity k ovlivnění kognitivních funkcí kladně. Závěr: Nefarmakologické postupy mají významné místo v léčbě demence a doplňují farmakologické i podpůrné léčebné postupy., Aim: Aim of the study was to evaluate the effectiveness of non-pharmacological approaches influencing cognitive functions that are used in routine practice in inpatient women psycho-geriatric ward in Mental hospital in Kromeriz. Furthermore the study tried to evaluate satisfaction with these activities in hospitalised clients. Methods: At the beginning and at the end of hospitalisation was used range of Mini-Mental State Examination (MMSE) and Clock Drawing Test (CDT) to make an indicative assessment of cognitive functions, and at the end of hospitalization was used a controlled interview. Results: At the end of hospitalisation there was no significant evidence of improving cognitive functions (short period of research is the most probable reason). Nevertheless, psychotherapeutic activities were evaluated by clients positively. The class of commutative dually residuated lattice ordered monoids ($DR\ell $-monoids) contains among others Abelian lattice ordered groups, algebras of Hájek’s Basic fuzzy logic and Brouwerian algebras. In the paper, a unary operation of negation in bounded $DR\ell $-monoids is introduced, its properties are studied and the sets of regular and dense elements of $DR\ell $-monoids are described.
The inotropic effects of insulin in the rat heart are still incompletely understood. In this study, the effects of insulin on cardiac contraction were studied in right ventricular papillary muscles from both control rats and rats with chronic diabetes (lasting 16 weeks). Diabetes was induced by the application of streptozotocin (STZ) and the development of diabetes was documented by increased levels of blood glucose, by reduction in body weight and by decreased plasma concentrations of insulin. The contraction was significantly smaller in diabetic rats. Insulin (80 IU/l) reduced the contraction force in both control and diabetic groups. The post-rest potentiation of contraction was not influenced by insulin in control rats, but insulin increased it in diabetic rats. The negative inotropic effect of insulin was preserved in the presence of cyclopiazonic acid (3 μmol/l), a blocker of sarcoplasmic reticulum (SR) Ca2+ pump, in both control and diabetic groups. In contrast, the negative inotropic effect of insulin was completely prevented in the presence of nifedipine (3 μmol/l), a blocker of L-type Ca2+ current. We conclude that insulin exerts a significant negative inotropic effect in rat myocardium, both control and diabetic. This effect is probably related to processes of SR Ca2+ release triggering, whereas SR Ca2+ loading is not involved.
