- Pseudarhahdttsynachus yucatanensis sp. n. (Diplectanidae) is described from the gills of the red grouper, Epinephelus maria (Valenciennes, 1824) (Serranidae), from the coast of the Yucatan Peninsula, Mexico. This new species is characterized by having both the ventral and the dorsal squamodiscs composed of 10-12 rows of rods, with 0-1 innermost row forming a closed circle. Furthermore, the vagina of P. yucatanensis has a non-sclerotized ampulla with a fine, sclerotized duct leading to a sclerot-ized seminal receptacle. Prevalence (percentage of infected fish) and abundance (mean number of worms per examined fish) were estimated for P. yucatanensis from 8 localities along the coast of Yucatan. Prevalence varies from 38% to 100%, while abundance was between 1.2 ± 0.6 and 43.2 ± 17.8 worms per ftsh. The lack of linear relationship between the host length and the number of monogcneans per fish (regression F, = 0.56. p = 0.45) was probably due to the fact that the sample was restricted to juvenile hosts.

Metodika: Vzorku respondentov tvorilo 78 sestier pracujúcich v NsP v Levoči a Spišskej Novej Vsi. Na meranie psychickej záťaže bol použitý Meisterov dotazník neuropsychickej záťaže, na meranie výskytu depresie skríningový nástroj Patient Health Questionnaire 9 (PHQ 9) a na meranie subjektívnej pohody Personal Well-Being Index (PWI) pre zdravú dospelú populáciu. Výsledky: Bol potvrdený významný vzťah medzi psychickou záťažou, výskytom depresívnej symptomatológie a subjektívnou pohodou u sestier. Vyššia frekvencia výskytu depresívnej symptomatológie sa u nich spájala s vyššou psychickou záťažou a nižšou subjektívnou pohodou. Významným a zároveň alarmujúcim zistením bolo, že len 28 % sestier uvádzalo nevýznamnú depresívnu symptomatológiu. Až u 50 % sestier bola podľa PHQ 9 prítomná ľahká depresia. Štatisticky významné korelácie boli identifikované medzi všetkými tromi faktormi Meisterovho dotazníka, ako aj jeho hrubým skóre a PHQ 9. Najsilnejšie korelácie boli medzi Faktorom I (preťaženie) a PHQ 9. Záver: Výsledky poukazujú na skutočnosť, ţe práve aspekty pracovného preťaţenia (časová tieseň, problémy a konflikty, vysoká zodpovednosť) môžu u sestier viesť ku častejšiemu prežívaniu jednotlivých symptómov depresie (predovšetkým k únave a zníženému záujmu). V ďalšom výskume navrhujeme sa zamerať na sledovanie vzťahu troch premenných: depresia, burnout a psychická záťaž sestier., Aim: The study has a design of descriptive cross-sectional survey. The aim of this study was to investigate relationship between psychological load, depressive symptomatology and subjective well-being of nurses. Methods: Sample of respondents consisted of 78 nurses from two public hospitals (Levoča and Spišská Nová Ves). To measure the psychological load was used the Meister questionnaire of neuro-psychological load. To measure the incidence of depression was used the screening tool Patient Health Questionnaire 9 (PHQ 9). To measure subjective well-being was used Personal Well-Being Index (PWI) for a healthy adult population. Results: The results indicated that there is a significant correlation between psychological load, depressive symptomatology and subjective well-being of nurses. The higher frequency of depressive symptomatology was associated with higher level of psychological load and lower subjective well-being level. The depressive symptomatology has a significant relation with neuropsychological strain and subjective well-being. Important and at the same time alarming finding was that only 28% of nurses reported insignificant depressive symptomatology. According to PHQ 9 was present mild depression in up to 50% of nurses. Statistically significant correlations were identified between all three factors of Meister questionnaire, as well as his gross score and the PHQ 9. The strongest correlations were between Factor I (overload) a PHQ 9. Conclusion: The results show that mainly aspects of work overload (time constraints, problems and conflicts, high accountability) can lead in nurses to more frequent survival of individual symptoms of depression (especially the fatigue and reduced interests). - Výzkum se dnes široce zabývá tématem společného výskytu kardiovaskulárních onemocnění a deprese. Zvažuje různé aspekty, mezi nimiž je i pátrání po tom, zda existuje příčinná souvislost mezi depresí a kardiovaskulárními onemocněními nebo zda má deprese vliv na horší rekonvalescenci pacientů po infarktu myokardu a na vyšší výskyt komplikací. Deprese je spojována s výskytem komorbidit, jako je metabolický syndrom nebo diabetes mellitus. Určitou roli může hrát také horší funkce autonomního nervového systému nebo nezdravý životní styl. Deprese se vyskytuje u pacientů po infarktu myokardu ve větší míře než u normální populace a je považována za rizikový faktor, který zhoršuje prognózu pacienta. Proto je důležité se zaměřit na skríning deprese a případně i anxiety u těchto pacientů, tyto u nich včas odhalit a zajistit optimální léčbu., Researchers currently focus on a wide range of aspects associated with coincidence of cardiovascular diseases and depression. These include a search for a possible causal link between depression and coexisting cardiovascular disorder as well as the association between depression and poorer recovery after myocardial infarction or higher complication rate. Major depression is connected with comorbidities such as the metabolic syndrome and diabetes mellitus. Impaired autonomous nerve system function and unhealthy lifestyle can also play their role. Depression is more frequent in patients with a history of myocardial infarction and is considered a major risk factor of poorer prognosis. It is undoubtedly important to screen this patient group for both depression and anxiety so that appropriate treatment can be initiated if needed. - Bolest je subjektivní fenomén. Podněty k jejímu vzniku mohou být psychogenní nebo somatogenní. V její patogenezi se uplatňuji obdobně mechanizmy jako pn depresivní poruše. Akutní bolest má signální význam, což zpravidla vede lékaře k nalezeni její příčiny a ke kauzální léčbě. Naproti tomu chronická bolest takový význam nemá a léčíme ji často vedle působení na její patogenezu (např. u revmatických chorob) symptomaticky. Psychofarmaka se uplatňují bud jako adjuvantní léčba (antipsychotika, hypnotika, anxiolytika) nebo jako léky s vlastním analgetickým účinkem. Takovými léky jsou antidepresiva, která posílením noradrenergní a serotoninergní neurotransmise v descendentních míšních drahách tlumí bolest už na úrovni míchy. Z antidepresiv mají spolehlivý účinek především antidepresiva první generace (tricykUcká antidepresiva, TCA). Méně výrazný analgetický účinek, ale s výhodou menších nežádoucích účinků a menšího rizika interakcí s léky užívanými v somatické medicíně má třeti generace antidepresiv, SSRI (selektivní inhibitory zpětného vychytávání serotoninu). Perspektivní je užiti čtvrté generace antidepresiv, která mají zase jak účinek serotoninergní, tak noradrenergní. Jak v patogeneze depresivní poruchy, tak v patogeneze chronické bolesti se uplatňují stresové mechanizmy. Při zvládání stresu má vedle psychofarmak značný význam psychoterapie., Pain is a subjective phenomenon. Stimuli leading to the pain can be psychogenic or somatogenic. Mechanisms similar to those in depressive disorder are implicated in the pathogenesis of pain. Acute pain has a signaling function which usually helps a doctor to find a cause and a causal therapy. On the contrary, chronic pain lacks such a function and we often treat it besides its pathogenesis management (e.g. in rheumatic disorders) symp tomatically. Psychopharmacological agents are implicated or as an adjuvant therapy (antipsychotics, hypnotics, anxiolytics) or as agents with its own analgesic effect. These drugs include antidepressants which by strengthening noradrenergic and serotoninergic neurotransmission in descendent spinal paths dampen the pain already in the level of the spinal cord. Among antidepressants, the first generation (tricyclic antidepressants, TCA) has an infallible effect. The third generation antidepressants - SSRI (selective serotonine reuptake inhibitors) have less marked analgesic effect but an advantage of smaller adverse effects and lower risk of interactions with drugs used in somatic medicine. The use of the forth generation antidepressants which have again both serotoninergic and noradrenergic effects is perspective. Stress mechanisms are involved both in the pathogenesis of depressivé disorder and in the pathogenesis of chronic pain. - Cíl: Cílem předložené práce je detailní popis klinického nálezu u čtyř dětí s náhle vzniklým poklesem zrakových funkcí psychogenní příčiny. Metodika: Retrospektivně byla vyhodnocena zdravotnická dokumentace čtyř dívek s průměrným věkem 13 let (medián 14 let, interval 9–17 let), které byly sledovány na Oční klinice LF OU a FN Ostrava v době od 1. 1. 2014 do 31. 12. 2014. U všech dívek bylo provedeno komplexní oční a neurologické vyšetření včetně magnetické rezonance (MR) mozku, vizuálních evokovaných potenciálů (VEP), elektroretinografie (ERG) a v jednom případě lumbální punkce. Všechny dívky absolvovaly vyšetření dětským psychologem. Výsledky: U žádné z dívek v našem souboru případů jsme neprokázali organickou příčinu poklesu zrakové ostrosti. Výsledky oftalmologického, neurologického i doprovodných vyšetření (VEP, ERG, MR, lumbální punkce) byly ve všech případech negativní. Psychologické vyšetření prokázalo u všech dívek převahu psychosomatického původu obtíží. Po absolvování třech psychoterapeutických sezení u dětského psychologa došlo k plné úpravě zrakové ostrosti u všech dívek v našem souboru případů. Závěr: Neorganická ztráta zraku je klinická diagnóza vyhrazená pro stavy jedno- nebo oboustranného nevysvětlitelného poklesu zrakové ostrosti při absenci organického očního nebo celkového onemocnění. Diagnostický proces tohoto onemocnění je zdlouhavý, vyžadující multioborovou spolupráci. Základním léčebným přístupem je psychoterapie, která by měla probíhat pod dohledem zkušeného dětského psychologa., Purpose: The objective of the present paper is a description of clinical findings in four children with non-organic visual loss (NOVL). Methods: A retrospective case series analysis was performed on four girls who were followed up at the Ophthalmology Department of the University Hospital Ostrava from 1st January 2014 to 31st December 2014. The mean age of these patients was 13 years (median 14 years, range 9–17 years). Ophthalmological and neurological examinations were performed, including brain MRI, visual evocated potentials, electroretinography as well as lumbar puncture in one case. All girls were examined by a children psychologist. Results: An organic cause of reduced visual acuity was not identified in any of the four cases. The results of ophthalmological, neurological and other examinations (VEP, ERG, MRI, lumbar puncture) were negative. Psychological examination confirmed predominant psychosomatic background of the disorder in all cases. Visual acuity resolved completely in all patients after three psychotherapy sessions led by a child psychologist. Conclusion: Non-organic visual loss is a clinical diagnosis of inexplicable monocular or binocular visual acuity impairment without any organic eye disorder or general disease. Diagnostic process is lengthy and requires cooperation of various medical specialities. Psychotherapy is the primary therapeutic approach and should be supervised by an experienced child psychologist.
