BACKGROUND: Radio frequency (RF) and chemical peels have been used for nonablative skin rejuvenation. Both of these cause collagen remodeling in the dermis and neo-collagen formation resulting in facial rejuvenation. There is limited literature on the evaluation of collagen remodeling by objective methods. OBJECTIVE: To compare the benefits of monopolar radiofrequency and glycolic acid peels in facial rejuvenation with regards to histopathology and Ultrabiomicroscopic sonography (UBM). METHODOLOGY: In this study, forty patients with mild to moderate photoaging received four treatments with 3 weeks interval of monopolar radiofrequency on one side of face and glycolic acid peels in increasing concentrations (NeostrataR) on the other side. Pre and post treatment, 2 mm biopsies were taken from both preauricular areas and Ultrasonography using a 35 MHz probe was done from outer canthus of eye and nasolabial folds from both sides of face. A blinded assessment was done to measure the increase in the grenz zone and dermal thickness. RESULTS: In 35/40 patients there was a significant increase in the grenz zone on histopathology and decrease in subepidermal low-echogenic band (SLEB) on UBM of the nasolabial folds on both sides of the face (p < 0.05). Východisko a cíl: Prognóza inoperabilního karcinomu jícnu je velmi závažná, léčba těchto pacientů je paliativní. Cílem sdělení je retrospektivní hodnocení intraluminální brachyterapie vysokým dávkovým příkonem nádorové stenózy inoperabilního karcinomu jícnu. Pacienti a metody: Od února 1996 do června 2011 bylo intraluminální brachyterapií léčeno 41 pacientů s karcinomem jícnu, z toho v 19 případech se jednalo o dlaždicobuněčný karcinom a ve 22 případech o adenokarcinom. U všech pacientů byla před zahájením brachyterapie přítomna dysfagie. Výsledky: U většiny pacientů došlo po brachyterapii k úlevě polykacích obtíží. Medián doby přežití činil 396 dní (95 % CI: 270–492 dní). Nebyla pozorována mechanická komplikace zavedení brachyterapeutického aplikátoru. Závěr: Intraluminální brachyterapie je účinná a bezpečná paliativní metoda léčby dysfagie způsobené nádorovou stenózou jícnu. Cíl práce: Upozornit na možnosti laparoskopického chirurgického řešení náhlé příhody břišní v graviditě. Typ práce: Kazuistika. Vlastní pozorování: Formou kazuistiky prezentujeme případ těhotné ve 23. týdnu gravidity s náhlou příhodou břišní neznámé etiologie a její laparoskopické řešení. Závěr: Laparoskopie v rukou zkušeného operatéra se jeví jako velmi bezpečná metoda v případech nejasné bolesti podbříšku, a to i v graviditě v závislosti na velikosti dělohy, tedy stáří gravidity. V tomto případě byla příčinou torze fibromu tuby, která byla řešena salpingektomií., The aim of the study: Draw attention to possibilities of LPSC surgical solutions of acute abdominal evens in pregnancy. Type of paper: Case study. Own observation: We are presenting a case of a pregnant woman at 23 weeks who suffered an acute abdominal event of unknown etiology and its laparoscopic solution. Conclusion: Laparoscopy in the hands of an experienced surgeon appears as a very safe method in cases of lower abdominal pain of unclear origin also in pregnancy, depending on the size of the uterus, i.e. the stage of pregnancy. Úvod: Jedním z onemocnění, které je potenciálním rizikovým stavem pro vznik chronické pankreatitidy, je pankreatitida akutní. Materiál: Z celkového počtu 231 osob, které bylo sledovaných s diagnózou chronické pankreatitidy, 56 nemocných bylo iniciálně léčeno s akutní pankreatitidou (24,2 %). V intervalu 12–24 měsíců od prvé ataky akutní pankreatitidy jejich stav postupně progredoval do obrazu pankreatitidy chronické. Osoby zařazené do studie po 1. atace akutní pankreatitidy abstinovaly, během období jejich sledování nebyla recidiva akutní pankreatitidy prokázána. Výsledky: V etiologii akutní pankreatitidy převládal alkohol (55,3 %), biliární etiologie byl prokázána v 35,7 %. Dle revidované Atlantské klasifikace byla u 69,6 % nemocných stanovena pankreatitida těžká, ostatní nemocní splnili kritérium formy intermediární, nemocní s lehkou formou nebyli zařazeni. Závěr: Významnými přítomnými rizikovými faktory bylo kouření a obezita a u 18 %, resp. u 25,8 % nemocných byl zjištěn pankreatogenní diabetes mellitus. Přitom 88,1 % nemocných trpících akutní pankreatitidou byli kuřáci. Největší počet osob s chronickou pankreatitidou po atace pankreatitidy akutní byl v produktivním věku, tj. 25–50 let. Do chronické pankreatitidy nepřechází pouze akutní pankreatitida alkoholická, ale nalezli jsme tento přechod i u pankreatitidy biliární etiologie., Introduction: One of the diseases involving a potential risk of developing chronic pancreatitis is acute pancreatitis. Material: Of the overall number of 231 individuals followed with a diagnosis of chronic pancreatitis, 56 patients were initially treated for acute pancreatitis (24.2 %). Within an interval of 12- 24 months from the first attack of acute pancreatitis, their condition gradually progressed to reached the picture of chronic pancreatitis. The individuals included in the study abstained (from alcohol) following the first attack of acute pancreatitis and no relapse of acute pancreatitis was proven during the period of their monitoring. Results: The etiology of acute pancreatitis identified alcohol as the predominant cause (55.3 %), biliary etiology was proven in 35.7 %. According to the revised Atlanta classification, severe pancreatitis was established in 69.6 % of the patients, the others met the criterion for intermediate form, those with the light form were not included. Conclusion: Significant risk factors present among the patients were smoking, obesity and 18 %, resp. 25.8 % had pancreatogenous diabetes mellitus identified. 88.1 % of the patients with acute pancreatitis were smokers. The majority of individuals with chronic pancreatitis following an attack of acute pancreatitis were of a productive age from 25 to 50 years. Úvod: Dynamické metody kvantitativního testování senzitivity (Quantitative Sensory Testing; QST) jsou relativně novou nadstavbovou aplikací QST umožňující objektivizovat funkci centrálních modulačních mechanizmů algické percepce, jejichž individuální nastavení může přispívat k rozvoji chronických bolestivých stavů. Testování těchto mechanizmů umožňuje např. predikci rozvoje pooperační bolesti. K nejčastěji hodnoceným dynamickým QST metodám patří tzv. podmíněná modulace bolesti (Conditioned Pain Modulation; CPM) a časová sumace percepce bolestivých podnětů (Temporal Summation; TS). Soubor a metodika: Vyšetření dQST (dynamic methods of Quantitative Sensory Testing) metod s využitím termoalgických podnětů a modulace bolestivým chladem bylo provedeno u 77 zdravých dobrovolníků (47 žen testovaných v ovulační fázi menstruačního cyklu, 30 mužů, věk 19–73 let). Hodnocen byl také vliv věku a pohlaví na výsledky obou dQST testů. Výsledky: Uspokojivá funkce podmíněné modulace bolesti byla prokázána u 84 % testovaných dobrovolníků. Časová sumace bolestivých podnětů byla zachycena u 92, resp. 98 % jedinců v závislosti na použité teplotě podnětů. Nefunkčnost mechanizmů u zbylé části souboru byla ve většině případů podmíněna nesprávně nastavenými technickými parametry testu. U žen testovaných v ovulační fázi menstruačního cyklu jsme prokázali signifikantně vyšší efektivitu CPM mechanizmu ve srovnání s muži. Funkce TS se mezi oběma pohlavími významně nelišila. Významný vliv věku na efektivitu CPM ani TS mechanizmu nebyl ve sledovaném souboru prokázán. Závěr: Provedená studie ověřila použitelnost zvolených algoritmů dynamického testování senzitivity a prokázala antinociceptivní nastavení žen v ovulační fázi menstruačního cyklu ve srovnání s muži, zatímco významný vliv věku na funkci centrálních modulačních mechanizmů algické percepce nebyl zachycen., Background: So called “dynamic quantitative sensory methods” (dQST) represent relatively new QST application which enables to objectify certain mechanisms of pain processing, e.g. central integration or descending control. Their dysfunction may contribute to the development of chronic painful conditions. dQST methods proved the potential to predict the development of chronic postoperative pain. Conditioned pain modulation (CPM) and temporal summation (TS) are the most commonly used dQST tests. Patients and methods: The CPM and TS testing was performed in a group of 77 healthy volunteers (47 women tested in the ovulatory phase of the menstrual cycle, 30 men, age range 19–73) using thermal pain stimuli. Efficiency of CPM and TS mechanism including age and gender influence was evaluated. Results: Efficient CPM was found in 84% of healthy volunteers. In remaining 16%, the inefficiency was mainly due to low intensity of testing or modulatory stimuli. Based on the temperature used, efficient TS mechanism was found in 92% or 98% of healthy individuals. Women in the ovulatory phase of the menstrual cycle showed significantly more efficient CPM comparing to men. No sex differences of TS testing were found. Age had no significant effect on the CPM or TS mechanism in our group of healthy voluteers. Conclusions: Our study has verified the applicability of CPM and TS algorithms used. Women in ovulatory phase of the menstrual cycle show increased anti-nociceptive setting comparing to men. No significant effect of age on central modulation of pain perception has been found in pain-free healthy individuals. Still's disease is an inflammatory disorder of unknown etiology. First-line therapy is based on corticosteroids and non-steroidal anti-inflammatory drugs (NSAIDs) but the frequency of relapses and corticosteroid-induced adverse events are limiting factors. The efficacy of intravenous tocilizumab (TCZ) has been shown at a dose of 8 mg/kg but the corticosteroid-sparing effect of intravenous low-dose TCZ followed by subcutaneous (SC) injection in the course of the disease has been poorly investigated. We report the case of a 28-year old Caucasian woman presenting a relapse of Still's disease eleven months after diagnosis under treatment with 6 mg of methylprednisolone. TCZ at a dose of 4 mg/kg every 2 weeks was combined with 32 mg of methylprednisolone, followed by 162 mg SC every 3 weeks. Evolution was rapidly favourable with a decrease in corticosteroid doses. Incidence karcinomu endometria ve věkové skupině žen mladších 40 let narůstá, a proto roste i zájem o fertilitu šetřící léčbu. Nejčastěji používaná konzervativní léčba progestiny může být zvážena u pečlivě selektovaných pacientek s grade 1, stage IA karcinomem endometria bez myometriální invaze, které si přejí zachovat reprodukční funkce, a jsou ochotny podstoupit intenzivní léčbu a sledování a také určité riziko chirurgicky nestážovaného onemocnění. Před léčbou je potřeba co nejpřesněji zhodnotit biologický charakter a rozsah onemocnění. Hormonální terapie je aplikována obvykle 6 měsíců. Response rate se pohybuje přibližně v hodnotách 60–80 %, recurrence rate 25–40 %. Live birth rate je uváděn okolo 30 %. Je potřeba informovat pacientky o reálné možnosti recidivy, obvykle dobře léčitelné, i o výhodě použít metody asistované reprodukce, o potřebě intenzivního follow-up a doporučení definitivní léčby po dokončení reprodukčních plánů., The diagnosis of endometrial carcinoma in young women of childbearing age is rare, but the incidence in growing. Fertility-preserving therapy of endometrial carcinoma could be considered in patients with a histological diagnosis of grade 1 stage IA endometrial carcinoma without myometrial invasion who wish to preserve their fertility, are willing to accept intensive treatment, close follow-up and risk of unstaged disease. Conservative treatment is based on 6 months medical treatment with high-dose oral progestins. Response rate is referred between 60 and 80 %, recurrence rate 25–40 %; live birth rate around 30 %. Gliomy limbického a paralimbického systému představují anatomicky a tím pádem i operačními přístupy unikátní skupinu mozkových nádorů. Mikrochirurgie v kombinaci s moderními perioperačními postupy zvyšuje šance na radikální odstranění těchto gliomů. Z hlediska lokalizace je dělíme do tří základních skupin: 1. gliomy inzuly, 2. amygdalohippokampálního komplexu a 3. cingula. V minimonografii se věnujeme anatomii i funkci limbického a paralimbického systému a v návaznosti na ni představujeme ucelený pohled na jednotlivé neurochirurgické přístupy k těmto oblastem. Maximální resekce za zachování funkční integrity mozku je předpokladem následné smysluplné onkologické léčby. Taktéž prezentujeme výsledky operací našeho pracoviště ve smyslu morbidity a radikality za posledních sedm let., Limbic and paralimbic gliomas represent a unique group of brain tumors both from an anatomic and surgical point of view. Microsurgery, together with modern perioperative procedures, enables a wider extent of resection of these gliomas. Based on localization, we divide gliomas into three basic groups: 1. insular, 2. amygdalohippocampal complex and 3. cingulum gliomas. In our review, we describe the anatomy and function of the limbic and paralimbic system and in relation to that, we present a description of the specific neurosurgical approaches to these regions. Maximal resection with brain function preservation represents the basis for subsequent oncologic therapy. We also present the surgical results of our department in the last seven years with regard to morbidity and radicality. OBJECTIVE: The aim of the present study was to investigate the effect of allergic rhinitis on the success of the operation in chronic otitis surgery by using score for allergic rhinitis (SFAR). MATERIALS AND METHODS: In the present study; 121 patients, who underwent type 1 tympanoplasty were examined retrospectively. SFAR of all patients were recorded. The graft success rates of 26 patients with allergic rhinitis (AR) and 95 patients with no allergic rhinitis group (NAR) were compared. RESULTS: While the graft success rate in NAR group was 89.5%, this rate was 80.8% in the AR group. However, the difference between groups was not statistically significant (p = 0.311). CONCLUSION: These findings suggest that allergic rhinitis decreases the graft success rate of the pathologies occurring in eustachian tube, middle ear and mastoid although statistically significant difference wasn't found. Poúrazový hydrocefalus způsobený poruchami cirkulace mozkomíšního moku často komplikuje klinický průběh a proces léčby po kraniocerebrálním poranění. Hydrocefalus po poranění míchy je popisován pouze výjimečně. Sdělení popisuje dvě kazuistiky nemocných s kompletní lézí krční míchy, u kterých v poúrazovém období došlo k rozvoji hydrocefalu. Rozbor obou případů a přehled literárních dat potvrzují rozhodující úlohu extraspinálních faktorů pro rozvoj hydrocefalu po míšním poranění. I když k rozvoji hydrocefalu po míšním poranění dochází výjimečně, tato možnost by měla být zvážena při pozdním zhoršení klinického stavu nemocného po poranění krční míchy, zvláště při přítomnosti anatomických anomálií mokového prostoru a poúrazovém subarachnoidálním krvácení., Posttraumatic hydrocephalus caused by cerebrospinal fluid circulation disturbances frequently complicates the clinical course and treatment after craniocerebral injury. Hydrocephalus complicating spinal cord injury is only exceptionally reported. The paper presents two cases of complete cervical spinal cord injury with subsequent development of hydrocephalus. The analysis of both cases and literature data confirmed the dominant role of non-spinal factors in the development of hydrocephalus after spinal cord injury. Despite the exceptional occurrence of hydrocephalus after spinal cord injury, this diagnosis should be considered in cases of delayed deterioration of a patient with cervical spinal cord injury, particularly if cerebrospinal fluid space abnormalities and posttraumatic subarachnoid haemorrhage are present., and J.
