Úvod: U 60 % nemocných s kolorektálním karcinomem jsou diagnostikovány jaterní metastázy (JMKRK), ať už v době stanovení diagnózy, nebo v průběhu léčby. Chirurgická léčba je jedinou potenciálně kurabilní metodou s pětiletým přežíváním dosahujícím cca 50 %. Jaterní resekci však podstoupí méně než 20 % pacientů s JMKRK. Společnou snahou onkologů a chirurgů je navýšit počet resekovaných pacientů. Jedním z postupů, jak navýšit resekabilitu JMKRK, je „liver first approach“ (LFA). Autoři prezentují vlastní předběžné výsledky této metody. Metody: Do multicentrického grantu IGA MZ NT 13660 – Hodnocení kvality multimodální péče u nemocných s jaterními metastázami kolorektálního karcinomu – bylo od září 2012 do ledna 2015 zařazeno v ÚVN 102 pacientů. LFA jsme použili u 12 pacientů (12 %). Po příznivé léčebné odpovědi neoadjuvantní systémové léčby jsme pacienty indikovali k resekci jater. U 11 pacientů se jednalo o mnohočetné (>4 meta) bilobární postižení. V jednom případě se jednalo o objemnou solitární metastázu pravého jaterního laloku. Primární tumor byl u 9 nemocných v rektu, u 3 v oblasti rektosigmatu, 3 pacienti měli derivační kolostomii, ostatní byli bez poruchy pasáže. Výsledky: U 11 pacientů jsme provedli R0 resekci jaterních metastáz, 3x jsme indikovali dvoufázovou hepatektomii s portální embolizací (PVE), 1x jsme pro progresi po PVE ustoupili od 2. fáze). Velkou resekci jsme provedli 7x, neanatomickou resekci včetně RFA 11x (6x v kombinaci s velkou resekcí). Perioperační mortalita byla nulová, morbidita 33 % (Dindo-Clavien >2). 10 pacientů absolvovalo adjuvantní chemoterapii, 7 z nich včetně radioterapie malé pánve pro lokálně pokročilý tumor rekta. U 7 pacientů (58 %) jsme doplnili resekci primárního tumoru. V současné době zemřeli na progresi onemocnění 2 pacienti (17 %), progresi onemocnění jsme zaznamenali u 6 pacientů (50 %). Závěr: LFA považujeme za vhodný především u pacientů s generalizovaným tumorem rekta, kdy lze po resekci jater adjuvantní systémovou léčbu kombinovat s radioterapií malé pánve. Otázkou zůstává načasování operace primárního tumoru, kdy je nutné vyloučit recidivu jaterních metastáz, která se objevuje u více než 50 % pacientů. Profit LFA musí být prokázán randomizovanými studiemi., Introduction: Liver metastases are diagnosed in 60% of patients with colorectal cancer, both at the time of diagnosis or later in the course of their management. Surgical treatment is the sole potentially curable method with 5–year overall survival of approximately 50%. However, only less than 20% of patients underwent liver resection. A joint effort of medical oncologists and surgeons is to increase the numbers of resected patients. The “liver first approach” (LFA) is one of approaches aimed at increasing resecability. The authors present their preliminary results using this method. Methods: 102 patients were included in the multicentre study supported by the grant IGA NT 13660 − Evaluation of quality of multimodal treatment for patients with colorectal cancer liver metastases – conducted at the Central Military Hospital between September 2012 and January 2015. We used LFA in 12 patients (12%). Patients were indicated for liver resection based on good response to neoadjuvant systemic therapy. Multiple bilobar liver involvement (>4 metastases) was present in 11 cases and one large solitary metastasis in the right liver lobe in one case. The primary tumor was located in the rectum in 9 patients, in the rectosigmoid in 3 patients; 3 patients had a colostomy. Others showed no signs of bowel obstruction. Results: We have performed R0 resections in 11 cases, and two-stage hepatectomy with portal vein embolisation was indicated 3 times (in one case we did not finish the second stage due to quick progression after PVE). We performed major resections 7 times, along with sever extraanatomic resections, incl. 11 RFA (6 times in combination with major resections). Perioperative mortality was 0%, morbidity 33% (Dindo-Clavien >2). Ten patients underwent adjuvant chemotherapy, in 7 cases including radiotherapy of the small pelvis due to a local advanced primary tumor. Resection of the primary tumor was done in 7 patients (58%). Two patients died recently because of disease progression (17%); progression was observed in 6 patients (50%). Conclusion: We deem the LFA suitable especially for patients with metastatic rectal tumors where adjuvant systemic therapy can be combined with radiotherapy. The timing of the resection of the primary tumor still remains a question: it is necessary to rule out potential recurrence of liver metastases, which affected more than 50% of the patients. Cíl: Průzkumné šetření zjišťovalo schopnosti spolupráce nemocných při léčbě Warfarinem a dodrţování léčebného reţimu v průběhu dlouhodobé antikoagulační terapie Warfarinem. Konkrétně byla mapována a vyhodnocena úroveň edukace sestrou, informovanost a adherence k léčbě u pacientů léčených Warfarinem. Metodika: Průzkumný soubor tvořilo 108 pacientů evidovaných v ambulantním léčení interní a kardiologické ambulance při interním oddělení Nemocnice ve Frýdku-Místku. Průzkumné šetření probíhalo v prosinci roku 2010 a lednu roku 2011. Pro sběr dat byla vyuţita metoda dotazníkového šetření. Výsledky: Výsledky průzkumného šetření ukazují, ţe naprostá většina pacientů léčených Warfarinem se aktivně podílí na léčebném procesu. Nemocným se od zdravotníků dostává poučení o léčbě, sestry se na edukaci podílejí v 75 %. Většina dotázaných dochází na pravidelné kontroly a zná zpaměti uţívanou dávku léku. Čtvrtina nemocných však není schopna uvést krvácení jako hlavní riziko léčby. Převáţná většina respondentů si je vědoma skutečnosti, ţe účinek léčby můţe být ovlivněn sloţením přijímané potravy, méně neţ polovina respondentů zná interakci Warfarinu s jinými léky či doplňky stravy. Nemocní mají na zřeteli, ţe je nutno informovat kaţdého lékaře o této léčbě, jen polovina respondentů však informuje i lékárníka. Závěr: Adherence k léčbě ze strany pacienta je základem úspěšné a bezpečné terapie Warfarinem. Cílem všech zdravotníků by měla být snaha o kvalitní a efektivní komunikaci s nemocnými, snaha o dosaţení co nejvyšší úrovně spolupráce, ochoty a schopnosti pacientů řídit se pokyny zdravotníků. Tím se výrazně zvýší úspěšnost léčby a eliminuje se riziko moţných komplikací léčby., Aim: Exploratory investigation ascertained capabilities of cooperation of patients when treated with warfarin and adherence to treatment regimen during long term anticoagulant therapy by warfarin. Specifically was mapped and assessed the level of education by nurse, foreknowledge and adherence to treatment in patients treated with warfarin. Methods: The exploratory set consisted of 108 patients, who were registered in ambulant treatment of internal and cardiac ambulance at internal department of Hospital in Frydek-Mistek. Exploratory investigation took place in December 2010 and January 2011. For data collection was used method of questionnaire survey. Results: The results of questionnaire survey show, that absolute majority of patients treated with warfarin is actively participating in the treatment process. The patients receive from the health care providers education about the treatment, nurses are participating in education in 75%. Majority of respondents have regular check ups and knows by heart the dose of the drug. A quarter of patients is not able to state bleeding as the main risk of treatment. The vast majority of respondents are aware of the fact, that the treatment effect may be influenced by the composition of the diet, less than half of the respondents know the warfarin interactions with other medication or supplements. Patients have in mind that every doctor should be informed about this treatment, only half of the respondents, however, informs as well the pharmacist. Conclusion: Adherence to treatment by the patient is the foundation of a successful and safe warfarin therapy. The goal of all health professionals should be to aim for quality and effective communication with patients, effort to achieve the highest level of cooperation, willingness and ability of patients to follow medical instructions. Úvod: Existuje mnoho studií věnujících se problematice biliárních komplikací po jaterních resekcích. Přes zlepšení operační techniky a perioperační péče má incidence této komplikace spíše vzrůstající charakter. Byly analyzovány četné prediktivní faktory. Na jejich vliv na vznik biliárního leaku doposud nepanuje shoda. Za cíl analýzy jsme si stanovili zhodnotit incidenci biliárního leaku, jeho vliv na mortalitu a na dobu hospitalizace na našem pracovišti. Současně jsme provedli rozbor známých prediktivních faktorů. Metoda: Autoři retrospektivně analyzují soubor 146 nemocných, u kterých byla na Chirurgické klinice 2. LF Univerzity Karlovy a ÚVN, Praha v období 2010–2013 provedena jaterní resekce. K vyhodnocení biliárního leaku jsme užili současnou uznávanou klasifikaci ISGLS (International Study Group of Liver Surgery). Závažnost komplikace byla stanovena dle Clavien-Dindo. Statistickou významnost prediktivních faktorů jsme určili pomocí Fisherova exaktního testu a Studentova t-testu. Výsledky: Incidence biliárního leaku byla 21 %. Dle ISGLS bylo 6,5 % pacientů zařazeno do skupiny typu A, v 61,2 % se jednalo o leak B a v 32,3 % o leak typu C. Stupeň závažnosti dle Clavien-Dindo I-II, IIIa, IIIb, IV a V byl v poměru 19,3 %, 42 %, 9,7 %, 9,7 %, resp. 19,3 %. Jako statisticky významné jsme stanovili tyto faktory: operaci pro malignitu (p<0,001), velkou jaterní resekci (p=0,001), dobu operace (p<0,001), vyšší peroperační krevní ztrátu (p=0,02), konstrukci HJA (p=0,005), portální venózní embolizaci/two-stage chirurgii (p=0,009) a ASA skóre (p=0,02). Biliární leak významně prodloužil dobu hospitalizace (p<0,001). V souboru pacientů s biliárním leakem byla 23krát vyšší perioperační mortalita (p<0,001) než v souboru bez leaku. Závěr: Biliární leak je jednou z nejzávažnějších komplikací jaterních resekcí. Většina rizikových faktorů je těžko ovlivnitelných a na jejich vliv nepanuje jednoznačná shoda. Incidenci by mohly redukovat diskutované peroperační leak testy. V budoucnu bude třeba dále pracovat na zlepšení perioperačního managementu a techniky k prevenci vzniku této závažné komplikace. V léčbě se uplatňuje multidisciplinární přístup., Introduction: Many previous reports have focused on bile leakage after liver resection. Despite the improvements in surgical techniques and perioperative care the incidence of this complication rather keeps increasing. A number of predictive factors have been analyzed. There is still no consensus regarding their influence on the formation of bile leakage. The objective of our analysis was to evaluate the incidence of bile leakage, its impact on mortality and duration of hospitalization at our department. At the same time, we conducted an analysis of known predictive factors. Method: The authors present a retrospective review of the set of 146 patients who underwent liver resection at the Department of Surgery of the 2nd Faculty of Medicine of the Charles University and Central Military Hospital Prague, performed between 2010−2013. We used the current ISGLS (International Study Group of Liver Surgery) classification to evaluate the bile leakage. The severity of this complication was determined according to the Clavien-Dindo classification system. Statistical significance of the predictive factors was determined using Fisher‘s exact test and Student‘s t-test. Results: The incidence of bile leakage was 21%. According to ISGLS classification the A, B, and C rates were 6.5%, 61.2%, and 32.3%, respectively. The severity of bile leakage according to the Clavien-Dindo classification system – I-II, IIIa, IIIb, IV and V rates were 19.3%, 42%, 9.7%, 9.7%, and 19.3%, respectively. We determined the following predictive factors as statistically significant: surgery for malignancy (p<0.001), major hepatic resection (p=0.001), operative time (p<0.001), high intraoperative blood loss (p=0.02), construction of HJA (p=0.005), portal venous embolization/two-stage surgery (p=0.009) and ASA score (p=0.02). Bile leakage significantly prolonged hospitalization time (p<0.001). In the group of patients with bile leakage the perioperative mortality was 23 times higher (p<0.001) than in the group with no leakage. Conclusion: Bile leakage is one of the most serious complications of liver surgery. Most of the risk factors are not easily controllable and there is no clear consensus on their influence. Intraoperative leak tests could probably reduce the incidence of bile leakage. In the future, further studies will be required to improve the perioperative management and techniques to prevent such serious complications. Cíl: Cílem práce bylo shromáždit dostupná klinická, radiologická a patologická data o kalcifikujícím pseudotumoru nervové osy (CAPNON). Soubor: Soubor je tvořen kazuistikou a současně přehledem všech pacientů publikovaných v anglické literatuře, vyhledaných v databázi PubMed pomocí klíčových slov: calcifying pseudoneoplasm, calcifying pseudotumour, brain stone, cerebral calculi a fibro‑osseous lesion. Zahrnuty jsou taktéž všechny citace případů v nalezených publikacích. Výsledky: Včetně naší pacientky jsme v literatuře nalezli celkem 72 pacientů. Intrakraniálně bylo lokalizováno 62 % těchto lézí, v páteřním kanále 31 % a v kranio‑cervikálním přechodu zbylých 7 %. Průměrný věk výskytu je 45,6 (rozmezí 2–83) let. U intrakraniální lokalizace byla tato léze nalezena supratentoriálně v 74 % a infratentoriálně v 26 %. Z nejčastějším projevů intrakraniálních CAPNON jsme nalezli epilepsii v 33,3 %, bolesti hlavy v 24 %, nebo některý z příznaků zadní jámy v 16,7 %. U pacientů s epilepsii byl CAPNON nalezen v temporálním laloku v 50 %. U CAPNON nalezených v páteřním kanále byla dominantním projevem bolest v 82 %, následovaná poruchou chůze v 27 %. V kranio‑cervikálním přechodu byla dominantním příznakem bolest v 80 %. Závěr: CAPNON je velmi vzácnou, benigní, pomalu rostoucí lézí centrální nervové soustavy. Ve většině případů ji nalezneme intrakraniálně s lehkou převahou u mužského pohlaví. Původ této léze není zcela jasný. Diagnóza CAPNON by měla být zvažována setkáme‑li se s nálezem solidně kalcifikované léze při vyšetření CT mozku a hypodenzní léze v T1 a T2 obraze při vyšetření MR. Léčebnou modalitou je radikální odstranění této léze. Incidentální léze musí být monitorovány, jelikož mohou růst a stát se symptomatickými. Redakční rada potvrzuje, že rukopis práce splnil ICMJE kritéria pro publikace zasílané do biomedicínských časopisů., Aim: The aim of this study was to summarize clinical, radiological and pathological data on calcifying pseudoneoplasm of the neural axis (CAPNON). Methods: A case report of a female patient with CAPNON is presented. In addition, all CAPNON patient case histories published in English language are included. Articles were identified via PubMed searches using the following key words: calcifying pseudoneoplasm, calcifying pseudotumour, brain stone, cerebral calculi and fibro‑osseous lesion. All cases referenced within the identified publications are also included in this review. Results: Seventy two cases were identified, including our case report. Of these, 62% were intracranial, 31% spinal and 7% were located in the cranio‑cervical junction (CCJ). The mean age of the CAPNON patients was 45.6 (range 2–83) years. 74% of intracranial CAPNON were supratentorial and 26% were infratentorial. Symptoms of intracranial CAPNON included epilepsy in 33.3%, headaches in 24% and posterior fossa symptoms in 16.7%. The majority of patients with epileptic seizures had lesions in the temporal lobe (50%). Pain was the dominant symptom in 82% and gait disorder in 27% of spinal CAPNON cases. Pain was the dominant symptom (80%) in CCJ CAPNON. Conclusion: CAPNON is a rare, benign, slowly‑growing lesion of the central nervous system. In the majority of cases, location is intracranial and there is a slightly higher prevalence in men. The origin of CAPNON remains unclear. CAPNON should be considered whenever a CT scan reveals a calcified lesion combined with hypointensity on T1 and T2-weighted MRI. Radical removal is the treatment of choice. Assaultive acts committed by people with a mental illness is a major public health issue that affects patients with their families, law enforcement authorities, and the public at large. Failure to provide treatment is in fact a major predictor of assaultive acts in patients with schizophrenia living in the community. Considering that the indigenous ethnic groups of Central Asia have similar sociocultural characteristics, these factors may be reflected in individuals with schizophrenia who have committed serious assaultive acts in Uzbekistan. Objectives: The aim of the work was to identify the sociocultural and clinical characteristics of schizophrenic representatives of indigenous ethnic groups of Central Asia who have committed violent crimes in Uzbekistan and have been found insane in regard to their offence, and to compare these subjects to ones belonging to the other ethnic groups. Material and methods: The data were collected in 2010–2013 in the Tashkent High Security Psychiatric Hospital via face-to-face interviews and also from the patients’ charts and from forensic psychiatric examination statements. Results: The sample consisted of 201 individuals. The sample was 90.1 percent (n = 181) male, with a predominance of the paranoid schizophrenia subtype according to the ICD-10 criteria. Of the subjects, 174 ones (86.6%) were representatives of the indigenous ethnic groups of Central Asia, and 27 ones (13.4%) were representatives of other ethnic groups. The duration of illness among the subjects belonging to the indigenous ethnic groups of Central Asia was less than in the other group; the individuals were rarely referred to psychiatric care because of the popularity of alternative medicine and the stigma attached to mental illness. A positive correlation between violence and various psychotic symptoms, such as delusions, hallucinations, and thought disorder, has also been demonstrated in this group. Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. Spánek se významně podílí na celkovém zdraví a kvalitě života člověka, je důležitý nejen pro duševní a fyzickou regeneraci, ale také pro normální funkci metabolických pochodů, endokrinního a imunitního systému. V moderní společnosti dochází k postupnému zkracování délky spánku v důsledku celé řady faktorů. Epidemiologické studie v posledním desetiletí potvrdily, že spánek kratší než 7 hodin představuje rizikový faktor pro rozvoj obezity, diabetu mellitu 2. typu, hypertenze, kardiovaskulárních onemocnění, depresí a zvyšuje riziko mortality. Cílem této práce bylo zhodnotit délku spánku a posoudit vztah mezi délkou spánku a BMI u dospělé populace (n=478, 18–65 let). Metoda sběru dat zahrnovala 7denní záznam o délce spánku za den a antropometrická měření. Z výsledků vyplynulo, že průměrná délka spánku v souboru činila 7,45 (±0,91) hodin za den. 28 % dospělých osob přesto spalo méně než doporučených 7 hodin denně. Statisticky nejméně spali jedinci ve věku 40–49 let (průměr 7,2±0,80 hodin). Riziko obezity definované jako BMI ≥30 kg/m2 v souboru stoupalo se zkracující se délkou spánku: pro kategorie spánku [<6,3 h], [≥6,3 až <8,3 h] a [≥8,3 h] bylo riziko obezity na základě relativních četností vyjádřeno poměrem 2 : 1,5 : 1,2. Jedinci spící průměrně <6,3 hodiny vykazovali 1,3krát vyšší riziko obezity než ostatní jedinci spící déle i při zohlednění faktoru věku, tento závěr však není statisticky významný (OR=1,256; 95% IS: 0,631; 2,422). It is not only important for mental and physical regeneration, but also for the normal functioning of metabolic processes, for the endocrine and immune systems. In modern society there is a gradual shortening of the length of sleep due to a number of factors. In the last decade a number of epidemiological studies suggest that sleeping less than 7 hours a day increases the risk of developing obesity, type 2 diabetes mellitus, hypertension, cardiovascular diseases, and depression and is associated with premature mortality. The aim of this study was to evaluate sleep duration and to assess the relationship between sleep duration and BMI in the adult population (n=478, 18 to 65 years). The method of data collection consisted of a 7-day record of the length of the sleep per day and anthropometric measurements. The results showed that the average sleep duration was 7.45 (±0.91) hours per day. However, 28% of adults slept less than a recommended 7 hours a day. The individuals aged 40–49 years slept statistically the shortest time (mean 7.2±0.80 hours). The risk of obesity increased indirectly with duration of sleep in this population, for sleep categories [<6.3 h], [≥6.3 to <8.3 h] and [≥8.3 h] the risk of obesity defined as BMI ≥30 kg/m2 was 2 : 1.5 : 1.2 based on the relative frequencies of obese and non-obese participants. Individuals sleeping on average <6.3 hours were at 1.3 times higher risk of obesity than other individuals sleeping longer after adjusting for age, this conclusion was not statistically significant (OR=1.256, 95% CI: 0.631; 2.422). Práce prezentuje prenatální diagnostiku, možnosti terapie a následnou péči u plodů a novorozenců s kongenitálním chylothoraxem na podkladě zhodnocení 14 případů na naší klinice v letech 1993 až 2012. Povšechný hydrops plodu byl popsán ve 4 případech (29 %), v 10 případech (71 %) byla provedena odlehčovací amniodrenáž a ve 3 případech (21 %) prenatální odlehčovací punkce hrudníku. Spontánně bylo porozeno 11 plodů (78 %), hrudní drenáž byla nezbytná v průměru 9 dnů po porodu. Mortalita dosáhla v souboru 29 % (4 případy). V článku popisujeme vyšetřovací algoritmy, terapeutické prenatální i postnatální postupy a je zdůrazněna nutnost mezioborové spolupráce., The work presents prenatal diagnosis, treatment and care options in fetuses and neonates with congenital chylothorax based on a detailed evaluation of 14 cases recorded in our department between years 1993 and 2012. Generalized fetal hydrops was described in 29% of cases, therapeutic amnio drainage performed in 71% and prenatal thoracentesis in 21% of cases. 78% of fetuses were delivered spontaneously; thoracic drainage was necessary nine days after birth in average. Mortality reached 29%. Cíl. Retrospektivně zhodnotit efekt a dlouhodobou průchodnost transjugulární intrahepatální portosystémové spojky (TIPS) u nemocných s trombózou jaterních žil. Metoda. V letech 1992-2011 jsme vytvořili TIPS celkem u 38 pacientů s trombózou jaterních žil nekontrolovatelnou medikamentózní léčbou. Léčeno bylo devět mužů a 29 žen ve věku 13-76 let (medián 33 let), v šesti případech se jednalo o děti. Predisponující trombofilní stav byl prokázán u 84,2 % osob. Klinická forma Buddova-Chiariho syndromu byla v šesti případech akutní, v 18 případech subakutní a ve 14 případech chronická. Indikací k výkonu byl u 31 nemocných ascites, u pěti pacientů jaterní selhání a u dvou krvácení do zažívacího traktu. Zákroky byly prováděny standardní technikou v analgosedaci nebo v celkové anestezii. U 17 pacientů jsme do kanálu TIPS implantovali nepotažený stent nebo kombinaci potaženého a nepotaženého stentu, u 21 nemocných jsme zkrat vyztužili pouze pro TIPS dedikovaným potaženým stentem (stentgraftem). Sledování průchodnosti zkratu po výkonu bylo založeno na pravidelných ultrasonografických, klinických a laboratorních kontrolách, při prokázané poruše průchodnosti byla provedena reintervence. Výsledky. Celková doba sledování souboru byla 8 dní - 207 měsíců (medián 52 měsíců). Technického úspěchu jsme dosáhli u 100% nemocných, hemodynamická úspěšnost výkonu byla 86,8 %, s poklesem mediánu portosystémového gradientu z 23 mm Hg na 8 mm Hg, klinicky bylo dosaženo ústupu obtíží u 89,5% nemocných. Významné komplikace jsme zaznamenali celkem u sedmi osob, žádný pacient nezemřel v přímé souvislosti se zákrokem. V průběhu sledování zemřelo celkem jedenáct nemocných (29 %), přežití souboru za 1 rok a za 5 let od TIPS bylo 85,3 % a 65,4 %. Během doby sledování jsme provedli celkem 50 reintervencí pro dysfunkci spojky, průměrný počet reintervencí přepočtený na jednoho nemocného za dobu 5 let byl 1,65 výkonu u pacientů se zkratem vytvořeným stentem a 0,67 výkonu u nemocných, u kterých byl primárně použit stentgraft. Ve skupině se zavedeným nekrytým stentem byla primární průchodnost 52,9% za 1 rok a 20 % za 5 let od výkonu, u skupiny s implantovaným potaženým stentem byla jednoletá primární průchodnost 80 % a pětiletá primární průchodnost 33,3%. V průběhu sledování jsme zároveň u osmi pacientů provedli angioplastiku dolní duté žíly pro její symptomatickou stenózu, u šesti z těchto nemocných byl do dolní duté žíly zaveden stent. Závěr. Vytvoření TIPS je velmi efektivní metodou léčby pacientů s trombózou jaterních žil. Použití pro TIPS dedikovaných potažených stentů vede knižšímu počtu dysfunkcí zkratu s nižším počtem reintervencí nezbytných k obnově průchodnosti, nutností je však současná důsledná antikoagulační léčba a léčba základního hematologického onemocnění., Aim. To retrospectively evaluate the efficacy and long-term patency of transjugular intrahepatic portosystemic shunt (TIPS) in patients with hepatic vein thrombosis. Method. In years 1992-2011, totally 38 patients with hepatic vein thrombosis resistant to medical therapy underwent TIPS creation in our department. We treated 9 males and 29 females, age 13-76 years (median 33 years), 6 patients were children. The underlying thrombophilic state was confirmed in 84.2% of treated patients. The Budd-Chiari syndrome was acute in 6, subacute in 18 and chronic in 14 cases. Ascites was indication for TIPS in 31 patients, liver failure in 5 and gastrointestinal bleeding in 2 persons. All the procedures were performed in anal-gosedation or under general anestesia using standard technique. In 17 patients, we implanted non-covered stent or combination of non-covered and covered stents during TIPS creation, in the other 21 patients only the TIPS dedicated covered stents had been used. TIPS patency during follow-up after the procedure was based on regular ultra-sonographic, clinical and laboratory examinations. In case of shunt dysfunction, a rein-tervention was performed. Results. The total follow-up period was 8 days - 207 months (median 52 months). The technical success rate was 100%, hemo-dynamical success rate 86.8% with median portosystemic gradient decrease from 23 mm Hg to 8 mm Hg, and clinically we were succesfull in 89.5% of patients. In 7 patients, severe complications occurred during shunt creation, but none of the patients died in direct connection with the procedure. Totally 11 patients (29%) died during follow-up period, the 1-year and 5-years survival rates were 85.3% and 65.4%, respectively. Due to TIPS dysfunction, totally 50 reinterventions were needed, the average 5-year reintervention rate per patient was 1.65 procedures in the bare stent group and 0.67 porcedures in the covered stent group. In the non-covered stent group, we achieved primary patency rates 52.9% 1 year and 20% 5 years after TIPS creation, in the covered stent group the 1-year primary patency rate was 80% and 5-year primary patency rate 33.3%. Angioplasty of symptomatic stenosis of the inferior vena cava was performed in 8 patients during follow-up, in 6 of these patients also stent implantation was necessary. Conclusion. TIPS creation is a very effective treatment of patients with hepatic vein thrombosis. Aldosterone blocker Spironolactone has antiinflammatory, antiproliferative and antioxidative effects, that is why pathogenetically it is expedient to use it in complex therapy of rheumatoid arthritis. Material and methods: 46 patients with RA took 2550 mg/day of Spironolactone during 12 months as an addition to standard therapy, the comparison group consisted of 47 patients that got only standard therapy, all the patients were fully examined prior and post the treatment. Results: complex RA therapy leads to improved VAS, HAQ, the antioxidative potential index F; decreased blood concentrations of TNFα, ICAM1, FGF and VEGF in contrast to standard therapy. Complex therapy made reduced the DAS 28 more > 0.6. Conclusions: applying of Spironolactone in complex therapy of rheumatoid arthritis contributes to more pronounced improvement in indices of articular syndrome and patients life quality, reduce of antiinflammatory, antiproliferative and angiogenic cytokines, and more effectively reduces the activity of the disease comparing to standard therapy., Elena Komarova, Borys Rebrov, and Literatura
